Predictors of long-term survival after pancreaticoduodenectomy for peri-ampullary adenocarcinoma: A retrospective study of 5-year survivors

Background: Pancreaticoduodenectomy(PD) is the standard curative treatment for periampullary tumors. The aim of this study is to report the incidence and predictors of long-term survival( ≥ 5 years) after PD. Methods: This study included patients who underwent PD for pathologically proven periampullary adenocarcinomas. Patients were divided into 2 groups: group(I) patients who survived less than 5 years and group(II) patients who survived ≥ 5 years. Results: There were 47(20.6%) long-term survivors( ≥ 5 years) among 228 patients underwent PD for periampullary adenocarcinoma. Patients with ampullary adenocarcinoma represented 31(66.0%) of the long-term survivors. Primary analysis showed that favourable factors for long-term survival include age < 60 years old, serum CEA < 5 ng/mL, serum CA 19-9 < 37 U/mL, non-cirrhotic liver, tumor size < 2 cm, site of primary tumor, postoperative pancreatic fistula, R0 resection, postoperative chemotherapy, and no recurrence. Multivariate analysis demonstrated that CA 19-9 < 37 U/mL [OR(95% CI) = 1.712(1.24 8–2.34 8), P = 0.001], smaller tumor size [OR(95% CI) = 1.335(1.032–1.726), P = 0.028] and R0 resection [OR(95% CI) = 3.098(2.095–4.582), P < 0.001] were independent factors for survival ≥ 5 years. The prognosis was best for ampullary adenocarcinoma, for which the median survival was 54 months and 5-year survival rate was 39.0%, and the poorest was pancreatic head adenocarcinoma, for which the median survival was 27 months and 5-year survival rate was 7%. Conclusions: The majority of long-term survivors after PD for periampullary adenocarcinoma are patients with ampullary tumor. CA 19-9 < 37 U/mL, smaller tumor size, and R0 resection were found to be independent factors for long-term survival ≥ 5 years.

Biliary leakage following pancreaticoduodenectomy:Prevalence,risk factors and management

Background: Few studies investigated biliary leakage after pancreaticoduodenectomy(PD) especially when compared to postoperative pancreatic fistula(POPF). This study was to determine the incidence of biliary leakage after PD, predisposing factors of biliary leakage, and its management. Methods: We retrospectively studied all patients who underwent PD from January 2008 to December 2017 at Gastrointestinal Surgery Center, Mansoura University, Egypt. According to occurrence of postoperative biliary leakage, patients were divided into two groups. Group(1) included patients who developed biliary leakage and group(2) included patients without identified biliary leakage. The preoperative data, operative details, and postoperative morbidity and mortality were analyzed. Results: The study included 555 patients. Forty-four patients(7.9%) developed biliary leakage. Ten patients(1.8%) had concomitant POPF. Multivariate analysis identified obesity and time needed for hepaticojejunostomy reconstruction as independent risk factors of biliary leakage, and no history of preoperative endoscopic retrograde cholangiopancreatiography(ERCP) as protective factor. Biliary leakage from hepaticojejunostomy after PD leads to a significant increase in development of delayed gastric emptying, and wound infection. The median hospital stay and time to resume oral intake were significantly greater in the biliary leakage group. Non-surgical management was needed in 40 patients(90.9%). Only 4 patients(9.1%) required re-exploration due to biliary peritonitis and associated POPF. The mortality rate in the biliary leakage group was significantly higher than that of the non-biliary leakage group(6.8% vs 3.9%, P = 0.05). Conclusions: Obesity and time needed for hepaticojejunostomy reconstruction are independent risk factors of biliary leakage, and no history of preoperative ERCP is protective factor. AIM To evaluate the evolution, trends in surgical approaches a n d r e c o n s t r u c t i o n t e c h n i q u e s, a n d i m p o r t a n t lessons learned from performing 1000 consecutive pancreaticoduodenectomies(PDs) for periampullary *** This is a retrospective review of the data of all patients who underwent PD for periampullary tumor during the period from January 1993 to April 2017. The data were categorized into three periods, including early period(1993-2002), middle period(2003-2012), and late period(2013-2017).RESULTS The frequency showed PD was increasingly performed after the year 2000. With time, elderly, cirrhotic and obese patients, as well as patients with uncinate process carcinoma and borderline tumor were increasingly selected for PD. The median operative time and postoperative hospital stay decreased significantly over the periods. Hospital mortality declined significantly, from 6.6% to 3.1%. Postoperative complications significantly decreased, from 40% to 27.9%. There was significant decrease in postoperative pancreatic fistula in the second 10 years, from 15% to 12.7%. There was a significant improvement in median survival and overall survival among the *** Surgical results of PD significantly improved, with mortality rate nearly reaching 3%. Pancreatic reconstruction following PD is still debatable. The survival rate was also improved but the rate of recurrence is still high, at 36.9%.

Anaemia in Waldmann's disease: A rare presentation of a rare disease

A 32-year-old female presented with 5-year history of iron deficiency anemia, marked pallor and edema of both lower limbs. Laboratory investigations including complete blood count, blood film, iron studies, lipid profile, ascitic fluid analysis, test of stool for occult blood and alpha 1 anti-trypsin. Upper, lower gastrointestinal(GIT) endoscopies, and enteroscopy were performed. Imaging techniques as abdominal ultrasonography and computed tomography were done. Echocardiography, lymph node biopsy and bone marrow examination were normal. The case was diagnosed as Waldmann's disease with protein losing enteropathy and recurrent GIT bleeding. Management started with low fat diet with medium chain triglyceride, octreotide 200 μg twice a day, tranexamic acid and blood transfusion. Then, exploratory laparotomy with pathological examination of resected segment was done when recurrent GIT bleeding occurred and to excluded malignant transformation.

Paleoecology of Benthic Foraminifera in Coral Reefs Recorded in the Jurassic Tuwaiq Mountain Formation of the Khashm Al-Qaddiyah Area, Central Saudi Arabia

Thirty three benthic foraminiferal species belong to 23 genera and 16 families have been recorded from the coral reefs of the Callovian Tuwaiq Formation, Khashm AI-Qaddiyah area, Central Saudi Arabia. Three species： Astacolus qaddiyahensis, Nodosaria riyadhensis, Siderolites jurassica are believed to be new. Nearly all identified foraminifera are of Atlantie-Miditeranean affinity. The foraminiferal assemblage recorded in the present work is mixed of open marine, moderately deep marine conditions associations and shallow to deep lagoon. The reefal part of upper Twiaq Formation may have been deposited in shallow water of lower to middle shelf depth （20-50 m） as indicated by abundant corals and benthic foraminifera. The coral fauna and bearing benthic foraminifera indicated moderate water energy.

Facies Associations and Sequence Stratigraphy of the Toarcian Marrat Formation(Saudi Arabia) and Their Equivalents in Some Gondwanaland Regions

The Toarcian Marrat Formation is exposed in outcrops in central Saudi Arabia and displays a variety of clastic and carbonate facies associated with well-preserved depositional geometries. It is unconformably overlies the Triassic Minjur Formation and it in turn is overlaid by the Middle Jurassic Dhruma Formation. Thirteen lithofacies types can be identified that permit the recognition of five lithofacies associations in a mixed clastic/carbonate platform. These lithofacies range from low-energy peritidal, intertidal, and back-shoal to moderate-and high-energy shoal and foreshoal lithofacies associations. The Marrat Formation exhibits three depositional sequences, each sequence is grouped into a transgressive systems tract(TST) and a highstand systems tract(HST) and then bounded by sequence boundary surfaces(SBSs). The TSTs are generally identified in clastic tidal-flat beds and back-shoal wackestones, while the HST is generally recorded in the carbonate tidal-flat and shoal. The vertical succession of facies associations from peritidal to foreshoal depositional environments is indicative of a deepening upward and retrogradational systems tract, from Lower to Upper Toarcian. The correlation between the studied sections reveals a general shallowing towards the south and the similarities between the studied sequences and others in the Arabian Gulf, the northern Neo-Tethys Plate, and Gondwanaland countries.

Facies Analysis and Biostratigraphy of the Miocene Sequence,Cairo-Suez District,Egypt

Miocene siliciclastic-carbonate deposits are widely exposed in Cairo-Suez District, Egypt. These deposits are underlain and overlain by continental sediments of Oligocene and post Miocene, respectively. Three stratigraphic sections were investigated at Gabals Geneife, Homeira and Gharra. Lithostrtigraphically, the Miocene sequence could be differentiated into two main rock units representing shallow deposits with relatively intermittent deep marine incursions. These are from base to top, Gharra Formation and Genefe Formation. Detailed macrofossils investigations led to the recognition of four macrofossil zones, namely Alectryonella plicatula-Crassostrea frondosa Range Zone, Echinolampas amplus-Scutella ammonis Range Zone, Chlamys（Macrochlamys） sardoa-Chlamys（Argopecten） submalvinae Range Zone, and Chlamys gentoni-Pecten（Oppenheimopecten） benedictus-Pecten（P.） ziziniae Assemblage Zone. Microfacies analysis and identified taxa indicated that the Miocene sequence was deposited in transgressive-regressive cycles ranged from near shore, warm shallow inner to middle shelf marine environments with storm influence during the accumulation of the oyster banks.
