BACKGROUND Type 2 diabetes is one of the most prevalent chronic diseases worldwide,significantly impacting patients'quality of *** treatment options like metformin(MET)effectively counteract hyperglycemia but fail to alleviate diabetes-associated complications such as retinopathy,neuropathy,nephropathy,hepatopathy,and cardiovascular *** To propose the supplementation of cholecalciferol(CHO)and taurine(TAU)to enhance MET efficacy in controlling diabetes while minimizing the risk of associated *** The study involved sixty rats,including ten non-diabetic control rats and fifty experimental rats with type 2 diabetes induced by *** experimental rats were further subdivided into positive control and treatment *** four treatment groups were randomly allocated to a single MET treatment or MET combined with supplements either CHO,TAU,or *** Diabetic rats exhibited elevated levels of glucose,insulin,Homeostasis Model Assessment of Insulin Resistance(HOMA-IR),glycated hemoglobin percentage,lipid markers,aspartate aminotransferase,and malondialdehyde,along with reduced levels of antioxidant enzymes(catalase and superoxide dismutase).The administration of CHO and TAU supplements alongside MET in diabetic rats led to a noticeable recovery of islet *** antioxidative,anti-inflammatory,and anti-apoptotic properties of the proposed combination therapy significantly ameliorated the aforementioned *** The supplementation of CHO and TAU with MET showed the potential to significantly improve metabolic parameters and protect against diabetic complications through its antioxidative,anti-inflammatory,and antiapoptotic effects.

Genetic and morphological divergence in the purple sea urchin Paracentrotus lividus(Echinodermata,Echinoidea)across the African Mediterranean coast

The present investigation focuses on population structure analysis of the purple sea urchin Paracentrotus lividus across the African Mediterranean coast, with the main aim of assessing the influence of the Siculo-Tunisian Strait on gene flow disruption in this highly dispersive echinoid species. For this purpose, patterns of morphological and genetic variation were assessed among its populations from the western and eastern Mediterranean coasts. A total of 302 specimens from seven Tunisian sites were collected and examined for morphometric variability at twelve morphometric traits. Concordant results, inferred from CDA（canonical discriminant analyses）, pairwise NPMANOVA（non parametric multivariate analysis of variance） comparisons and MDS（multidimensional scaling） plot, unveiled significant inter-population differences in the measured traits among the studied populations. Furthermore, the combined use of the one way ANOSIM（analysis of similarities） and the Discriminant/Hotelling analysis allowed unravelling two morphologically differentiated groups assigned to both western and eastern Mediterranean basins. The SIMPER（similarity percentages） routine analysis showed that total dry weight, test diameter and spine length were major contributors to the morphometric separation between locations and between groups. Pattern of phenotypic divergence discerned in P. lividus across the Siculo-Tunisian Strait is interestingly in congruence with that inferred from the genetic investigation of the purple sea urchin populations from the same region based on the analysis of the mt DNA COI（cytochrome oxidase I） gene in 314 specimens from nineteen locations covering a wider geographic transect, streching westward to the Algerian coast and eastward to the Libyan littoral. 畜牧业产生的甲烷是引起全球变暖的主要因素。地球气候变暖成为全球重要问题之一。本研究中在体外条件下研究不同浓度的桉叶油对瘤胃中生产甲烷的抑制作用。试验设计为:不同精粗比的饲粮,分别为70:30和60:40。饲粮干物质中桉叶油的浓度分别为0 mL/kg、2 mL/kg、4 mL/kg、6 mL/kg、8 mL/kg和10 mL/kg。试验采用的瘤胃原液从两只健康瘘管绵羊获取,分别在3 h、6 h、9 h、12 h、24 h、36 h、48 h和72 h,检测气压(GP)、甲烷产量(CH4)。发酵(72 h)结束后,检测挥发性脂肪酸(VFA)、氨态氮(NH3)、干物质降解率(DMD)及有机物降解率(OMD)、pH值和原生动物数等。结果表明,不同浓度的桉叶油处理条件下,甲烷产量有所降低(P<0.05)。在精粗比30:70的处理中,2 mL/kg组的甲烷产量较对照组减少32%,10 mL/kg组的甲烷产量较对照组减少38%。在精粗比60:40的处理中,2 mL/kg组的甲烷产量较对照组减少42%,10 mL/kg组的甲烷产量较0 mL/kg组减少46%。在所有精粗比处理中,通过提高桉叶油的浓度,与0 mL/kg组相比,原虫数量显著下降(P<0.05)。NH3浓度较0 mL/kg组下降,在精粗比为60:40组中不显著,在70:30中显著(P<0.05)。在所有精粗比处理中,随着桉叶油浓度的提高,A/P值、DMD和OMD都显著下降(P<0.05)。除了精粗比70:30处理中的10 mL/kg组,其他各组的pH值没有显著差异。综上,2 mL/kg组为效率最高组。该浓度的桉叶油降低甲烷气体为最佳浓度。同时成本也是最低的。

Impact of referral pattern and timing of repair on surgical outcome after reconstruction of post-cholecystectomy bile duct injury:A multicenter study

Background:Bile duct injury(BDI)after cholecystectomy remains a significant surgical *** guideline exists to guide the timing of repair,while few studies compare early versus late repair *** study aimed to analyze the outcomes in patients undergoing immediate,intermediate,and delayed repair of ***:We retrospectively analyzed 412 patients with BDI from March 2015 to January *** patients were divided into three groups based on the time of BDI *** 1 underwent an immediate reconstruction(within the first 72 hours post-cholecystectomy,n=156);group 2 underwent an intermediate reconstruction(from 4 days to 6 weeks post-cholecystectomy,n=75),and group 3 underwent delayed reconstruction(after 6 weeks post-cholecystectomy,n=181).Results:Patients in group 2 had significantly more early complications including anastomotic leakage and intra-abdominal collection and late complications including anastomotic stricture and secondary liver cirrhosis compared with groups 1 and *** outcome was observed in 111(71.2%)patients in group 1,31(41.3%)patients in group 2,and 157(86.7%)patients in group 3(P=0.0001).Multivariate analysis identified that complete ligation of the bile duct,level E1 BDI and the use of external stent were independent factors of favorable outcome in group 1,the use of external stent was an independent factor of favorable outcome in group 2,and level E4 BDI was an independent factor of unfavorable outcome in group *** BDI and level E4 BDI were independent factors of unfavorable ***:Favorable outcomes were more frequently observed in the immediate and delayed reconstruction of post-cholecystectomy *** ligation of the bile duct,level E1 BDI and the use of external stent were independent factors of a favorable outcome.

Assessment of Commercial Specific IgE Assay for Detection of Allergens in Allergic Patients

Allergy is a serious health problem throughout the world, affecting people of all ages. Allergic diseases such as asthma, rhinitis and atopic dermatitis are becoming an epidemic in our country, the cost of investigating these diseases is increasing and becoming expensively high. There are many ways to explore allergenic antibodies to assess the presence the amount of specific IgE as: Skin test (Prick), Specific IgE (ELISA), RAST *** and Elimination Challenge method. Skin test produces pain, local and anaphylactic reaction and patient discomfort, other procedure with highly cost to patient. So, we have built up an evidence to use it in our allergy and immunology clinic has suitable cost for all patients based about for the other tests but it was very simple, accurate, cheap and does not produce any problems to patients. Our test depends on ELISA technique, it measures the quantitative amount of the following different allergens: As regard food allergens: Milk, Eggs, Banana, Maize, Fish, Chocolate, Wheat, Nuts, Strawberry, Shrimps, Spices, with drug as Aspirin. As regard inhalants as: House dust, Mite, Mixed Pollens, Mixed Moulds, Hay dust, Wool, Latex and Cat Hair. 150 allergic patient results of our test compared with specific IgE national kits (ELISA), *** (RAST), Skin test and elimination challenge test. The statistical evaluation results as regard senistivity respectively: 88.9%, 89.6%, 91.2%, 71.4%, 93.1%. As regard specificity the results: 93.1%, 94.7%, 95.3%, 65.5%, 91.6%, the latter results showed that our test was in line with all tests. Our test was probably in food, drug and inhalant allergens, it can be also noted that the cheapest and most commercial technique was used, so as to be available immediately in standard laboratory and its reagents, plate and other requirements were prepared locally in our laboratory. Now it was applying successfully in allergy and immunology unit Ain Shams university hospitals in Egypt.

Map Aided Pedestrian Dead Reckoning Using Buildings Information for Indoor Navigation Applications

Navigation systems play an important role in many vital disciplines. Determining the location of a user relative to its physical environment is an important part of many indoor-based navigation services such as user navigation, enhanced 911 (E911), law enforcement, location-based and marketing services. Indoor navigation applications require a reliable, trustful and continuous navigation solution that overcomes the challenge of Global Navigation Satellite System (GNSS) signal unavailability. To compensate for this issue, other navigation systems such as Inertial Navigation System (INS) are introduced, however, over time there is a significant amount of drift especially in common with low-cost commercial sensors. In this paper, a map aided navigation solution is developed. This research develops an aiding system that utilizes geospatial data to assist the navigation solution by providing virtual boundaries for the navigation trajectories and limits its possibilities only when it is logical to locate the user on a map. The algorithm develops a Pedestrian Dead Reckoning (PDR) based on smart-phone accelerometer and magnetometer sensors to provide the navigation solution. Geospatial model for two indoor environments with a developed map matching algorithm was used to match and project navigation position estimates on the geospatial map. The developed algorithms were field tested in indoor environments and yielded accurate matching results as well as a significant enhancement to positional accuracy. The achieved results demonstrate that the contribution of the developed map aided system enhances the reliability, usability, and accuracy of navigation trajectories in indoor environments.

Beef Meat Allergy in Cow’s Milk Allergic Adults

Milk has been recognized as a leading cause of food allergy in children;however, studies on cow’s milk allergy (CMA) in adults are scanty. The known cross-reactivity of bovine serum albumin and bovine γ-globulin which are present in both milk and beef is recognized, so that, patients with allergy to cow’s milk are often instructed to avoid beef. Our Objective was to determine the prevalence of allergy to beef meat in adult patients with allergy to cow’s milk. Thirty adults with CMA were included in the study. The diagnosis of cow’s milk and beef allergy was based on a thorough history concerning type of allergic symptoms and its link to the ingestion of milk and beef supported by skin prick test (SPT) with fresh milk and raw beef extracts and by determination of their serum specific IgE by enzyme linked immuosorbant assay (ELISA). Finally, cross reactivity test between milk and meat was performed. Three of 30 patients (10%) evaluated for CMA were found to have symptomatic sensitivity to beef, 2 gave urticarial symptoms and one gave gastrointestinal symptoms. Six patients (20%) had a positive SPT response to beef. On the other hand, beef specific IgE were positive in 18 CMA patients (60%). Concordance of positive specific IgE and SPT to meat was found in only 5 patients (17%). Mean cross reactivity between milk and beef was 31.6 ± 13.1% among studied patients. Patients allergic to milk protein are not necessarily to be allergic to beef, so, elimination of beef from the diet of adults with CMA should not be done except after investigations so as not to lose its nutritional value without benefit.

Nutrient Fluxes and Sediments Composition in El Mex Bay and Surround Drains, Alexandria, Egypt

The regeneration of pore water (PW) nutrients was investigated and the contribution of benthic nutrient fluxes to the overlying bottom water (BW) was examined. Dissolved inorganic nutrients (NO-2, NO-3, PO3-4 and SIO4-4) were measured in PW and BW in El Mex Bay and surround drains during spring 2010. Nutrient concentrations gradiance in PW with overlying seawater were evaluated according to Fick’s Law. Average inorganic nutrient fluxes were estimated using the pore water gradient concentration across the sediment-water interface. Calculated nutrient fluxes had averages of ﹣7.24, ﹣1.36, ﹣7.86 and ﹣1.33 in El Mex Bay. Additionally, the fluxes in the drains were ﹣34.39, ﹣32.28, ﹣53.20 and ﹣117.6 mg·m﹣2·day﹣1 for NO-3, NO-2, PO3-4 and SIO4-4, respectively. Mineralogical studies of sediment samples by using IR, X-ray analysis were carried out to identify the chemical structure of sediments. The results revealed that calcite, aragonite and quartz are the dominant minerals. On the other hand, differential thermal analysis (DTA) was used to evaluate and discuss different kinetic parameters such as Ea#, ΔG#, ΔH#, ΔS#, Z and Tm support the view of the extra stability of these sediments.
