Analgesia and Safety of Perioperative Bilateral Erector Spinae Infusion versus Thoracic Epidural Infusion in Upper Abdominal Oncological Surgeries: A Randomized Clinical Trial

Objective: Erector spinae plane block is a novel regional anaesthetic intervention that is said to have an effective analgesic profile in the upper abdominal region. We compared its analgesic efficacy with thoracic epidural block for upper abdominal cancer surgeries. Methods: This prospective study included 60 patients, 18 to 65 years old with ASA class II who underwent gastrectomy and Whipple’s procedures under general anesthesia. Patients were assigned into two groups: Thoracic epidural group, with injection of 0.1 ml/kg of bupivacaine 0.25% in epidural catheter followed by 0.1 ml/kg/h of 0.125% bupivacaine infusion for 48 hrs., and Erector spinae group, with insertion of bilateral erector spinae catheters to inject 0.1 ml/kg of bupivacaine 0.25% followed by bilateral infusion of 0.1 ml/kg/h of bupivacaine 0.125% for 48 hrs. The primary endpoint was postoperative VAS scores at rest and movement. Secondary endpoint included postoperative total opioid consumption, 1st request for analgesia, hemodynamic changes and satisfaction scores. Results: Pain scores and needed rescue analgesia in both groups were comparable. However, 1st request for opioid was significantly longer in TEPI compared to ESI group. Hemodynamics were significantly lower in TEPI group with no differences in the incidence of postoperative complications except for hypotension. Patient satisfaction scores were arbitrarily higher in ESI group without significance. Conclusion: Erector spinae infusion is a highly promising regional technique with comparable effects to thoracic epidural blockade in reducing pain and opioid needs while causing minimal hemodynamic consequences.

This study presents a new systematic algorithm to optimize the durability of reinforced recycled aggregate *** proposed algorithm integrates machine learning with a new version of the firefly algorithm called chaotic based firefly algorithm(CFA)to evolve a rational and efficient predictive *** CFA optimizer is augmented with chaotic maps and Levy flight to improve the firefly performance in forecasting the chloride penetrability of strengthened recycled aggregate concrete(RAC).A comprehensive and credible database of distinctive chloride migration coefficient results is used to establish the developed algorithm.A dataset composite of nine effective parameters,including concrete components and fundamental characteristics of recycled aggregate(RA),is used as input to predict the migration coefficient of strengthened RAC as output,k-fold cross validation algorithm is utilized to validate the hybrid *** numerical benchmark analyses are applied to prove the superiority and applicability of the CFA algorithm in predicting chloride *** show that the developed CFA approach significantly outperforms the firefly algorithm on almost tested functions and demonstrates powerful *** addition,the proposed strategy can be an active tool to recognize the contradictions in the experimental results and can be especially beneficial for assessing the chloride resistance of RAC.

TFN-FICFM:sEMG-Based Gesture Recognition Using Temporal Fusion Network and Fuzzy Integral-based Classifier Fusion

Surface electromyography(sEMG)-based gesture recognition is a key technology in the field of human–computer ***,existing gesture recognition methods face challenges in effectively integrating discriminative temporal feature representations from sEMG *** this paper,we propose a deep learning framework named TFN-FICFM comprises a Temporal Fusion Network(TFN)and Fuzzy Integral-Based Classifier Fusion method(FICFM)to improve the accuracy and robustness of gesture ***,we design a TFN module,which utilizes an attention-based recurrent multi-scale convolutional module to acquire multi-level temporal feature representations and achieves deep fusion of temporal features through a feature pyramid ***,the deep-fused temporal features are utilized to generate multiple sets of gesture category prediction confidences through a feedback ***,we employ FICFM to perform fuzzy fusion on prediction confidences,resulting in the ultimate *** study conducts extensive comparisons and ablation studies using the publicly available datasets Ninapro DB2 and *** demonstrate that the TFN-FICFM model outperforms state-of-the-art methods in classification *** research can serve as a benchmark for sEMG-based gesture recognition and related deep learning modeling.

Numerical simulation and analysis of confined turbulent buoyant jet with variable source

In this work, experimental and numerical investigations are undertaken for confined buoyant turbulent jet with varying inlet temperatures. Results of the experimental work and numerical simulations for the problem under consideration are presented. Four cases of different variable inlet temperatures and different flow rates are considered. The realizable k-ε turbulence model is used to model the turbulent flow. Comparisons show good agreements between simulated and measured results. The average devia- tion of the simulated temperature by realizable k-ε turbulent model and the measured temperature is within 2%. The results indicate that temperatures along the vertical axis vary, generally, in nonlinear fashion as opposed to the approximately linear varia- tion that was observed for the constant inlet temperature that was done in a previous work. Furthermore, thermal stratification exits, particularly closer to the entrance region. Further away from the entrance region the variation in temperatures becomes relatively smaller. The stratification is observed since the start of the experiment and continues during the whole course. Numerical experime- nts for constant, monotone increasing and monotone decreasing of inlet temperature are done to show its effect on the buoyancy force in terms of Richardson number.

An Efficient HW/SW Design for Text Extraction from Complex Color Image

In the context of constructing an embedded system to help visually impaired people to interpret text,in this paper,an efficient High-level synthesis(HLS)Hardware/Software(HW/SW)design for text extraction using the Gamma Correction Method(GCM)is ***,the GCM is a common method used to extract text from a complex color image and *** purpose of this work is to study the complexity of the GCM method on Xilinx ZCU102 FPGA board and to propose a HW implementation as Intellectual Property(IP)block of the critical blocks in this method using HLS flow with taking account the quality of the text *** IP is integrated and connected to the ARM Cortex-A53 as coprocessor in HW/SW codesign *** experimental results show that theHLS HW/SW implementation of the GCM method on ZCU102 FPGA board allows a reduction in processing time by about 89%compared to the SW *** result is given for the same potency and strength of SW implementation for the text extraction.

Estimation of the novel antipyretic, anti-inflammatory, antinociceptive and antihyperlipidemic effects of silymarin in Albino rats and mice

Objective: To evaluate the other pharmacological actions of silymarin in Albino rats and mice such as antipyretic, anti-inflammatory, antinociceptive and antihyperlipidemic effects. Methods: Rats were injected intramuscularly with pyrogenic dose of brewer's yeast for the antipyretic test of silymarin. Another group of rats injected with 0.1 mL of 1% carrageenan solution in saline at the subplanter area of the right hind paw for the anti-inflammatory test of silymarin. Another group of mice tested by hot plate method for determination of antinociceptive effect of silymarin. Hyperlipidemia was induced using high fat diet for 2 months to estimate the antihyperlipidemic activity of silymarin. Results: Silymarin showed a significant antipyretic effect of both doses(50 and 100 mg/kg) compared with control untreated group. Moreover, silymarin elucidated a significant anti-inflammatory effect of both doses reflected on the decrease of the rat paw edema every hour interval for 4 h after administration in comparison with control positive group. By the same taken, both doses of silymarine revealed a significant antinociceptive action in hot plate method at 30 and 60 min post administration. Besides, it lowered significantly the serum levels of prostaglandin E2, tumor necrosis factor alpha and interleukin 1 beta after 2 h of silymarin administration in carrageenan induced rat paw edema besides the significant decrease of total cholesterol, triglycerides, low density lipoprotein and significantly elevated high density lipoprotein after 2 weeks of silymarin administration. Conclusions: These outcomes delivered a new vision into the possible pharmacological mechanisms by which silymarin advances antipyretic, anti-inflammatory, antinociceptive and antihyperlipidemic effects.

The challenge faced by the visually impaired persons in their day-today lives is to interpret text from *** this context,to help these people,the objective of this work is to develop an efficient text recognition system that allows the isolation,the extraction,and the recognition of text in the case of documents having a textured background,a degraded aspect of colors,and of poor quality,and to synthesize it into *** system basically consists of three algorithms:a text localization and detection algorithm based on mathematical morphology method(MMM);a text extraction algorithm based on the gamma correction method(GCM);and an optical character recognition(OCR)algorithm for text recognition.A detailed complexity study of the different blocks of this text recognition system has been *** this study,an acceleration of the GCM algorithm(AGCM)is *** AGCM algorithm has reduced the complexity in the text recognition system by 70%and kept the same quality of text recognition as that of the original *** assist visually impaired persons,a graphical interface of the entire text recognition chain has been developed,allowing the capture of images from a camera,rapid and intuitive visualization of the recognized text from this image,and text-to-speech *** text recognition system provides an improvement of 6.8%for the recognition rate and 7.6%for the F-measure relative to GCM and AGCM algorithms.

Role of zinc as an antioxidant and anti-inflammatory to relieve cadmium oxidative stress induced testicular damage in rats

Objective:To investigate the role of zinc in reducing the deleterious effects of cadmium on male ***:Rats were injected subcutaneously with CdCl_2 and ZnCl_2 at dose level of2.2 mg/kg(1/40 of LD50 of cadmium per day).Results:The rats treated with cadmium exhibited a significant increase in levels of testicular malondialdehyde,tumor necrosis factor-alpha,nitrogen oxide and inducible nitrogen oxide synthase immunostaining reaction,as well as an elevation of blood hydroperoxide and follicle stimulating *** addition,a significant decrease in testicular ascorbic acid,zinc,reduced glutathione,catalase,superoxide dismutase,sex organ weight,plasma testosterone and luteinizing hormone were observed in the cadmium *** motility and count were decreased with cadmium treatment,while sperm abnormalities elevated *** treatment was found to mitigate the toxic effects of cadmium on oxidative stress,spermatogenesis,sex hormones,and inflammatory *** injected with cadmium showed intense histopathological *** manifested protective role and markedly reduced tissues damage induced by ***:The protective effect of zinc can be attributed to its antioxidant and antiinflammatory properties.
