Current Advances in Transcatheter Intervention for Children Born with Congenital Heart Defects: A Review of Literature

This review aims to sum up the improvements witnessed in the field of interventional cardiology during recent times. The last decade has witnessed significant technical advances in the field of radiological imaging and also in interventional cardiology which has helped to offer more non-invasive solutions for the management of congenital heart defects. This has resulted from the use of advanced 3-dimensional fusion imaging instead of conventional 2-dimensional angiography, applying interactive real-time enhancement and using computed tomography and Magnetic Resonance Imaging for interventional procedures. Similarly the introduction of next generation devices, have not only improved the final outcome of the procedure but also has helped in reducing the challenges that were faced before and with the initial generation of devices. These advances have helped not only in reducing the radiation exposure, the use of contrast medium dose but also have resulted in improved early survival. The field of neonatal cardiology has advanced at an unprecedented pace. The transcatheter closure of patent ductus arteriosus has evolved over time and now it has been made possible at much lower body weight. Similarly, early use of stents for aortic coarctation has been found effective in some patients, especially when surgical intervention had been denied. The application of the hybrid approach for the management of complex congenital heart defects has also been effectively applied. More recently transcatheter placement of the pulmonary valve has been introduced for severely stenotic and/or regurgitant pulmonary valve in adolescents and adults. It is anticipated that in near future, this procedure would be available for relatively younger patients. In conclusion: last 2 decades have improvised pediatric interventional cardiology to incorporate less invasive solutions for CHD. Diagnostic and Therapeutic Challenges of De Quervain Thyroiditis in Sub-Saharan Africa: Illustration with Two New Cases in Dakar

Introduction: De Quervain’s subacute thyroiditis is an inflammatory granulomatosis with clinical presentation some time atypical leading eventually to misdiagnosis or inadequate treatment. In our area, physician should be aware of this possible etiology in presence of recent and acute features of thyrotoxicosis with or without painful firm goiter. We illustrate this fact with two new observations from Sub-Saharan Africa. Observations: Our first patient is a 70-year-old Guinean woman presenting a painful nodular and hard goiter at palpation. Her history was recent in a context of deterioration of the general state with a sub clinical thyrotoxicosis syndrome. The diagnosis of probable malignant thyroid tumor was evoked initially and a thyroidectomy considered. She presented an important biological inflammatory syndrome and biological peripheral hyperthyroidism. Neck ultrasound examination showed heterogeneous patchy decreased echogenicity. The diagnosis of subacute De Quervain’s thyroiditis was set up. A corticosteroid therapy at a rate of 20 mg/day of prednisone was begun with a fast degression of dose. After 3 weeks of treatment, we noticed total clinical recovery normalization of biological parameters. The second observation was about a 52-year-old Senegalese women living in Bamako. She presented cervical pains evolving for 2 months in a context of flu-like syndrome associated with thyrotoxicosis and marked biological inflammatory syndrome. The thyroid ultrasound showed global heterogeneity with characteristic aspect of “geography map” leading to the diagnosis of subacute thyroiditis. A combination therapy with low dose of prednisone, ß-blockers and tranquillizers. We noticed fast disappearance of clinical and biological signs. At 5 mg per day of prednisone, there were moderate signs of relapse with rapid favorable evolution after few days under increased dose of prednisone up to 10 mg/day. Management of Hepatocellular Carcinoma Patients with Portal Vein Thrombosis Using Three-Dimensional Conformal Radiotherapy

Objective: To determine the possible therapeutic gain of using three-dimensional conformal radiotherapy (3D-CRT) as a treatment option for portal vein tumor thrombus (PVTT) in patients with hepatocellular carcinoma (HCC) and to evaluate the tolerance and toxicity of using such treatment. Materials and methods: Sixty two patients were enrolled in this prospective study between June 2013 and August 2015. The clinical target volume (CTV) was the PVTT and the prescribed dose was 50 Gy/25 fractions. The median follow-up time was 7.4 months. Results: The thrombus crude response rate was 40.4% and the only significant prognostic factor for response was the thrombus size. Responders had significant better survival compared to non-responders with a median survival of 12.5 and 8 months respectively (P Conclusions: The results of this study suggest that radiotherapy should be considered as a safe treatment option for HCC patients with PVTT. It is effective not only for PVTT local control but also for survival, although prospective randomized trials are needed to confirm these results.

Effect of Laser Cladding Processing Parameters on Nitinol’s Clad Dimensions, Microstructure, and Hardness

Nickel Titanium alloy (Nitinol) is characterized by its good mechanical properties, good damping properties in addition to its distinctive shape-memory effect and superelasticity effect besides its great bio-mechanical compatibility and corrosion resistance. These properties have empowered its applications, particularly within the bio-medical and aerospace industry. Despite these exceptional properties, the manufacturing of Nitinol by conventional methods is exceptionally troublesome and costly and consequently must be inspected. Therefore, additive manufacturing specifically laser-based ones were used recently. In this research, the effect of processing parameters of laser cladding/laser direct deposition on Nitinol’s Microstructure, Hardness and Clad Dimensions was evaluated. Systematic characterization of Nitinol samples was done utilizing Optical Microscopy and Vickers hardness tester. Samples of Nitinol were synthesized with different processing parameters using laser cladding and its properties were investigated and compared to one another to get the optimum processing parameters to synthesize a near net shape, fully dense Nitinol component with reliable properties. The results showed that there’s a processing parameter window at which the alloy possesses its best mechanical and functional properties which were of Laser power of value 1.25 Kw, Scan speed of 1.5 m/min and powder deposition rate of 1.5/1.5 RPM, these conditions resulted in the formation of martensite phase which is responsible for its functional properties with 40% volume fraction and a hardness value of 598 HV.

Physiological Studies on the Effect of Fenugreek on Productive Performance of White New-Zealand Rabbit Does

The objective of the current work is to investigate the possible beneficial effects of forced feeding of fenugreek to rabbit does on their milk composition, milk yield, anemia screening parameters, immunity and the growth performance of bunnies. Twenty white New Zealand female rabbits were randomly assigned into 5 groups: normal control and 4 treated groups (vehicle (molasses), fenugreek powdered seeds + molasses, sprouted fenugreek + molasses, and fenugreek oil + molasses). All treated groups took the given doses orally for two days before parturition and 13 days after parturition for 2 successive pregnancies. Blood samples were collected from each doe on the 13th day of lactation for determination of hematological, biochemical and immunological parameters. Milk samples were taken for evaluation of milk composition. All rabbit does and their bunnies were weighed for calculation of the milk yield. The fenugreek seeds and oil administered groups (G3 & G5) recorded the highest percent values of fat, protein, lactose, total solids and solid not fat contents of the rabbits’ milk in both 1st and 2nd lactation periods relative to other tested groups. At the beginning and mid of lactation periods, the milk yields recorded non-significant differences either between tested groups or between 1st and 2nd pregnancy in the same group. Whereas, at the end of lactation, germinated seeds (G4) and oil (G5) treated groups recorded significantly higher rates of milk yield relative to control and fenugreek seeds groups. Bunnies’ body weights suckling from dams of group III (G3) recorded the highest weights relative to other tested groups in both 1st and 2nd pregnancy. Cystic Lymphangioma on the Floor of the Oral Cavity Extending to the Submandibular Region in Adult Patients

Lymphatic malformation (ML), formerly called cystic lymphangioma is a benign hamartomatous tumor of lymphatic vessels. The onset of lymphangiomas is either at birth (60% to 70%) or up to two years of age (90%) and rare in adults. These malformations can occur in any region of the body, Lymphangiomas have marked predilection for the head and neck region (50% - 70%). The most common location in the mouth is the dorsum of tongue. If the positive diagnosis is generally easy, the therapeutic management remains controversial. We report the clinical case of a 23-year-old adult patient with cystic lymphangioma on the floor of the oral cavity extending gradually to the right submandibular region. The clinically suspected tumor was thoroughly explored using tomodensitometry. The treatment consisted of complete resection by a cervicotomy with histological examination confirming the macroscopic diagnosis of cystic lymphangioma. Follow-up at 3 years showed no recurrence.

Finger movement at birth in brachial plexus birth palsy

AIM: To investigate whether the finger movement at birth is a better predictor of the brachial plexus birth ***: We conducted a retrospective study reviewing pre-surgical records of 87 patients with residual obstetric brachial plexus palsy in study 1. Posterior subluxation of the humeral head(PHHA), and glenoid retroversion were measured from computed tomography or Magnetic resonance imaging, and correlated with the finger movement at birth. The study 2 consisted of 141 obstetric brachial plexus injury patients, who underwent primary surgeries and/or secondary surgery at the Texas Nerve and Paralysis Institute. Information regarding finger movement was obtained from the patient's parent or guardian during the initial ***: Among 87 patients, 9(10.3%) patients who lacked finger movement at birth had a PHHA > 40%, and glenoid retroversion 40%, and retroversion <-8° in study 1. The improvement in glenohumeral deformity(PHHA, 31.8% ± 14.3%; andglenoid retroversion 22.0°± 15.0°) was significantly higher in patients, who have not had any primary surgeries and had finger movement at birth(group 1), when compared to those patients, who had primary surgeries(nerve and muscle surgeries), and lacked finger movement at birth(group 2),(PHHA 10.7% ± 15.8%; Version-8.0°± 8.4°, P = 0.005 and P = 0.030, respectively) in study 2. No finger movement at birth was observed in 55% of the patients in this study ***: Posterior subluxation and glenoid retroversion measurements indicated significantly severe shoulder deformities in children with finger movement at birth, in comparison with those lacked finger movement. However, the improvement after triangle tilt surgery was higher in patients who had finger movement at birth.

Interest of the Padding in the Prevention of Lymphocele Production after Mastectomy with Axillary Dissection for Breast Cancer at Joliot Curie Cancer Center

Introduction: The principal postoperative complication of mastectomies with axillary dissection is the lymphocele that can last many months after surgery. The purpose of our study was to prevent its formation using the padding. Methods: Sixty-one patients have been included in our study. The follow-up was 6 months. The patients were divided in two groups through a random draw (simple drainage and drainage associated with padding). All patients had a mastectomy with axillary dissection following the Madden technique. All quantities of lymphoceles during postoperative hospitalization and ambulatory care have been noted. Results: Twenty-five patients had benefited from the padding and 36 of a simple axillary drainage. Six months after the surgery, the patients benefitting from the padding had a quantity of lymphocele equal to half that of the control group (761.83 mL against 1373.60 mL;p = 0.01). During the postoperative hospitalization, the quantities were of 362.80 mL for the padding group versus 630.83 mL;p Conclusion: The production of postoperative lymphocele is heterogenous, varying from one patient to another. Evaluation of Loss of Load Probability and Expected Energy Generation in Multi-area Interconnected Systems with Wind Generating Units

Due to randomness of wind generator＇s availability, power system planners have big concern on EEG （expected energy generation） and system reliability of power system with wind generators. This paper presents a methodology to evaluate the EEG as well as overall LOLP （loss of load probability）, which is an index for system reliability of multi-area interconnected systems with wind generators, as well as conventional fossil fuel based generating units. The proposed model is also capable of tracking the energy export incorporating the multi-state probability model for wind generator which output varies with time and season.

Acute or worsening heart failure is a common reason for hospitalization which carries a high mortality. Prompt diagnosis or exclusion of HF and its cause and precipitating factors can improve the quality and efficiency of care, shorten hospital stay, reduce readmission and improve prognosis. While the clinician remains central to reaching a clinical diagnosis of heart failure, the use of traditional and novel diagnostic technologies will improve the specificity and sensitivity of the diagnosis of heart failure diagnosis and provide insights into its pathophysiogical profile and help tailor therapy to individual patient need. Chest X-rays and electrocardiograms are generally available;echocardiograms less so. Novel technologies include both invasive and non-invasive methods to detect increases in intrathoracic fluid, pulmonary congestion, left ventricular filling pressures, cardiac output and vascular function. However, few of these technologies have been subjected to randomised controlled trials investigating their ability to improve patient management.
