Bioaccumulation of cadmium in gills and muscles of shellifsh from Pulicat lake, Tamil Nadu, India

Objective:To evaluate the presence of heavy metal cadmium in six species of shellfish at Pulicat Lake, Tamil Nadu, India. Methods: Six species of shellfish,Fenneropenaeus indicus,Fenneropenaeus monodon, Fenneropenaeus semisulcatus,Scylla serrata,Clibanarius longitarsus and Meretrix casta (M. casta) in Pulicat lake, Tamil Nadu, India were analysed for the presence of cadmium in the gills and muscles from January 2011 to December 2012. Results:The results showed seasonal variations in the uptake of cadmium. Very high accumulation of cadmium was found in the gills and muscles ofM. casta during post monsoon, summer, premonsoon and monsoon. The corresponding values of cadmium present in the gills of M. casta were 1.59, 1.56, 1.48 and 1.46μg/g in 2011 and 1.16, 1.25, 1.15 and 1.14μg/g in 2012. Whereas for muscles, they were 1.14, 0.11, 0.96 and 0.80μg/g in 2011 and 0.49, 0.34, 1.05 and 1.20μg/g in 2012. Conclusion:The results of the present study has shown that the accumulation of cadmium found in the gills and muscles were high in M. casta when compared to other species of shellfish. Thus, the consumption of the shellfish is safe, but does not exclude bioaccumulation risk in their meat. This present study has highlighted the need for estuarine biomonitoring to avoid possible contamination of shellfish and its consumers. The overall scenario of the shellfish accumulating high levels of cadmium indicates that the Pulicat lake is polluted with undesirable elements and the risk of consuming the meat of shellfish by man and other carnivores may lead to their toxicity. Stringent control measures are necessary to control the pollution of this precious lake to reduce the bioaccumulation of toxic metals in organisms.

Acute pancreatitis revealing ulcerative colitis—A case report

The association of acute pancreatitis with ulcerative colitis [UC] has been described in the literature. It is usually induced by drugs, but sometimes it may be idiopathic. This association remains rare. We report the case of a patient who was treated in our department. Medical observation: Mr. KA, a 60-year-old man, was admitted in our unit for management of acute epigastric. The clinical examination at the admission was normal. After eliminating a cardiac or surgical cause, the diagnosis of acute pancreatitis was made on the basis of the presence of a serum lipase up to 5 times the normal level and pain intensity. An abdominal scanner tomography was performed for the assessment of the pancreatitis. It has shown a pancreatitis stage C associated with a thick rectosigmoidien that was discovered incidentally. Symptomatology was enriched 10 days after by the occurrence of rectal bleeding. A lower endoscopy was performed after the improvement of the pancreatitis and had shown an ulcerative colitis on pancolitis which was confirmed by biopsy. In order to search other causes of this pancreatitis, other tests were made [a biliary IRM, endoscopic ultrasonography, autoimmune tests] and the results were negative. We concluded an idiopathic pancreatitis. The ulcerative colitis was classified as moderate and the patient was put on oral corticosteroids with degression. The evolution was marked by a clinical and biological improvement of pancreatitis and colitis. Conclusion: The association of idiopathic pancreatitis with ulcerative colitis is rare. The case of our patient is the first case reported in our series with 400 cases of ulcerative colitis diagnosed in our service.

Managing the Mass Effect: A Rare Case of Giant Ethmoid Sinus Osteoma with Orbital Extension—A Case Report

Background: Sinonasalosteomas are rare benign tumors that can cause clinical symptoms depending on their size and location. Osteomas are classified as giant osteomas when their size gets larger than three centimeters. In giant osteomas, severe symptoms and complications can occur if they expand into adjacent structures. Aim: In this case report, we highlight the diagnostic importance of CT scans of paranasal sinuses in diagnosing osteomas and the superiority of endoscopic surgery over an open approach in removing these benign tumors. Case Presentation: We report a rare case of giant ethmoidosteoma with extension to intraorbital facilities in 19 years old female which was causing a mass effect on the orbital space and discuss the appropriate management. Conclusion: Ethmoid sinus osteoma is asymptomatic until it increases in size producing a pressure effect on the eyes and sinusitis. CT imaging is the gold standard diagnostic modality. Surgical excision is the treatment of choice for ethmoid sinus osteoma. Due to the size of the lesion, endoscopic surgical excision is a less commonly employed modality, but it has advantages like earlier recovery and cosmetic benefits due to which it was the treatment employed in this case.

Urinary Continence after Orthotopic Detubularized Ileocaecal Pouch Versus Ileal Neobladder:Does it Differ?

Objective:To compare both techniques of detubularized ileocaecal and ileal neobladder post radical cystectomy with regards to functional results in the form of continence(day and night)with the use of urodynamics(for assessment of voiding volume and pouch capacity).Methods:This is a prospective randomized clinical trial that was done at the National Cancer Institute,Cairo University from August 2016 and January ***-two patients presented with muscle invasive urinary bladder cancer and no history of urinary incontinence or urethral stricture were included in this *** patients underwent preoperative cystoscopic *** cystectomy was done,and patients were divided into two groups according to the type of *** A included 31 patients with orthotopoic detubularized ileal neobladder and group B included 31 patients with orthotopic detubularized ileocaecal *** studies were done to 27 patients in group A and 27 patients in group B at just 6 and 12 months ***:The mean age in group A was 59.2±8.0 while in group B was 57.8±*** each group,we compared day and night continence in a period of 3 and 6 *** in group A,day continence after 3 months was 81.5% which improved to 96.3% after 6 months postoperatively with significant P-value(0.025).Night continence after 3 months was fair in 66.7%,unsatisfactory in 33.3% and 0% for good state which improved to 70% for fair state,18% unsatisfactory and 11% for good state after 6 months with significant P-value(0.035)while in group B,day continence after 3 months and 6 months postoperatively was 100% and night continence after 3 months was good in 0.0%,fair in 78.6% and unsatisfactory in 21.4% which improved to 10.7% for good state,85.7% for fair state and 3.6% for unsatisfactory after 6 months with significant P-value(0.005).Conclusion:There are no significant differences between detubularized ileocaecal and ileal neobladder in relation to contin

Histopathological characteristics and causes of kidney graft failure in the current era of immunosuppression

In this study, we present our experience working with kidney transplant recipients with graft failure within one year of the *** To report the histopathological characteristics of failed kidney allografts in the current era of immunosuppression based on the time after transplant, cause of the end-stage renal disease and induction immunosuppressive *** In a single-center observational study, we characterized the histopathological findings of allograft biopsies in kidney transplant recipients with graft failure within one year after the *** We identified 329 patients with graft failure that met the selection criteria between January 1, 2006 and December 31, 2016. The three most common biopsy findings were interstitial fibrosis and tubular atrophy(IFTA, 53%), acute rejection (AR, 43%) and transplant glomerulopathy(TG, 33%). Similarly, the three most common causes of graft failure based on the primary diagnosis were AR(40%),TG(17%), and IFTA(13%). Most grafts failed within two years of post-transplant(36%). Subsequently, approximately 10%-15% of grafts failed every two years: >2-4 years(16%), > 4-6 years(13%), > 6-8 years(11%), > 8-10 years(9%) and > 10 years(16%). AR was the most common cause of graft failure in the first six years(48%), whereas TG was the most prevalent cause of graft failure after 6 years(32%) of *** In the current era of immunosuppression, AR is still the most common cause of early graft failure, while TG is the most prevalent cause of late graft failure.

Liponeurocytoma of the Cerebellum with Myoid Features

We report an extremely rare tumor presenting with myoid features in the left cerebellum lobe in a 62-year-old man. This tumor consisted of medium to large round cells with focal lipomatous and myoid differentiation. Immunohistochemically, the tumor cells expressed synaptophysin, GFAP (glial fibrillary acidic protein) and focally desmin. From these findings, we concluded that this tumor was a liponeurocytoma with myoid features. To our knowledge, this is the second report of liponeurocytoma with myoid differentiation in the cerebellum.
