AIM:To characterize the relationship between depression and epilepsy-related seizures,treatment,hormonal and biological variables.METHODS:Included were 200 Egyptian adults(male=100,female=100)with epilepsy(mean age:30.87±7.88 years;duration of illness:13.89±7.64 years)and 100 healthy matched subjects for comparison.Psychiatric interview,Beck Depression Inventory(BDIII)and Hamilton Anxiety Rating Scale(HAM-A)were used to assess depression and anxiety.Blood levels of free testosterone,sex hormone binding globulin,prolactin,free thyroxin and thyroid stimulating hormone,serotonin,noradrenaline and adrenaline neurotransmitters were measured to assess endocrine and biological states.RESULTS:Patients had higher rates of depressive disorder(25.5%or 51/200),mostly intermixed with anxiety(47.06%),psychotic features(19.61%),aggression(40%)and suicide(55%).Compared to controls,higher scores on the BDI-II were observed with right-sided epileptic foci(P=0.011),polytherapy(P=0.001)and lack of control on antiepileptic drugs(AEDs)(P=0.0001).Patients had lower levels of serotonin(P=0.001)[marked with depression(P=0.012)]and adrenaline(P=0.0001),while noradrenaline was lower with temporal lobe epilepsy(P=0.039),left-sided foci(P=0.047)and lack of control on AEDs(P=0.017).Negative correlations were observed between levels of serotonin and BDI-II(P=0.048)and HAM-A(P=0.009)scores,but not with AEDs dose or drug level.CONCLUSION:Comorbid depressive disorder with epilepsy appears to be closely related to seizure type,focus,side,intractability to medications and neurotransmitter changes.Thus,optimizing seizure control and early recognition and management of depression is necessary to improve patients’quality of life.

Problem of living liver donation in the absence of deceased liver transplantation program:Mansoura experience

We report our experience with potential donors for living donor liver transplantation(LDLT), which is the first report from an area where there is no legalized deceased donation program. This is a single center retrospective analysis of potential living donors(n = 1004) between May 2004 and December 2012. This report focuses on the analysis of causes, duration, cost, and various implications of donor exclusion(n = 792).Most of the transplant candidates(82.3%) had an experience with more than one excluded donor(median = 3). Some recipients travelled abroad for a deceased donor transplant(n = 12) and some died before finding a suitable donor(n = 14). The evaluation of an excluded donor is a time-consuming process(median = 3 d, range 1 d to 47 d). It is also a costly process with a median cost of approximately 70 USD(range 35 USD to 885 USD). From these results, living donor exclusion has negative implications on the patients and transplant program with ethical dilemmas and an economic impact. Many strategies are adopted by other centers to expand the donor pool; however, they are not all applicable in our locality. We conclude that an active legalized deceased donor transplantation program is necessary to overcome the shortage of available liver grafts in Egypt.

Early detection of hepatocellular carcinoma co-occurring with hepatitis C virus infection: A mathematical model

AIM: To develop a mathematical model for the early detection of hepatocellular carcinoma(HCC) with a panel of serum proteins in combination with α-fetoprotein(AFP).METHODS: Serum levels of interleukin(IL)-8, soluble intercellular adhesion molecule-1(s ICAM-1), soluble tumor necrosis factor receptor Ⅱ(s TNF-RⅡ), proteasome, and β-catenin were measured in 479 subjects categorized into four groups:(1) HCC concurrent with hepatitis C virus(HCV) infection(n = 192);(2) HCV related liver cirrhosis(LC)(n = 96);(3) Chronic hepatitis C(CHC)(n = 96); and(4) Healthy controls(n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different groups. RESULTS: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model(-11.3 + 7.38 × Prot + 0.00108 × s ICAM + 0.2574 ×β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × s ICAM-1 +(4.251 × 10-4) × s TNF + 0.02567 ×β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. Additionally, we derived an algorithm, based on a binary classifier, for resolving the multi-class classification problem by using three successive mathematical model predictions of liver disease status. Environmental Studies on the Aqaba Gulf Coastal Waters during 2011-2013

In order to assess the quality of the Aqaba Gulf coastal waters for the sustainable use and development, and consequently for the national income, four field campaigns were annually carried out during the period from 2011-2013 to monitor hydrographic variables, i.e. temperature, salinity, pH, etc., nutrients, some heavy metals, i.e. iron, lead, copper, cadmium, chromium, etc. in addition to petroleum hydrocarbons. Except for the area of Sharm El-Sheikh Harbor, the results for the density, composition, and distributions of beach litter cleared out that the Egyptian shoreline of the Aqaba Gulf is relatively not affected by man-made litter. The area of Sharm El-Sheikh Harbor was subjected to many factors which undoubtedly affected the rate of man-made litter ac-cumulation, especially, oil contamination (oil and old or new tar) which is a good es-timator of levels of oil contamination and an effective means of evaluating the poten-tial threat of oil on coastal resources. The results of the present study cleared out also that water temperature followed seasonal changes in air temperature. Aqaba Gulf water is characterized by its high salinity and the presence of well oxygenated waters. Minor changes in the distribution of pH, BOD, DOM and COD, revealed limited effects of human impacts. Low levels of Chl-a and TSM concentrations and high transparency revealed also negligible effects for human impacts. Significantly higher sea water temperatures, TSM, DO, BOD, DOM, COD, NH4, NO2, PO4 and TP were observed in summer season compared to their corresponding values in winter season. On contrast, higher values of chlorophyll-a, NO2, TN, and SiO4 were observed in winter compared to summer season. Based on the mean annual values, Aqaba Gulf coastal waters are classified as oligotrophic to mesotrophic state. Assessment of the Hydrochemical Characteristics of the Suez Gulf Coastal Waters during 2011-2013

The coastal region of the Suez Gulf, is one of the most densely industrialized zones in Egypt. In order to assess the quality of Suez Gulf coastal waters for the sustainable use and development, and consequently for the national income, 12 field campaigns were carried out in 3 years (2011-2013). The results of beach litter cleared out that the shoreline of the Suez Gulf, especially Ras Gharib area was subjected to many factors which undoubtedly affected the rate of man-made litter accumulation. The most abundant visible items were general litter as they represented 30.64% of the total items encountered. Oil, seaweeds as well as old and new tar constituted 15.41%, 14.85% and 11.84%, respectively. The increase on the levels of water temperature, Chl-a, TSM, DO, BOD, DOM, COD, ammonium ion, nitrite, nitrate, total phosphorus, silicate and total nitrogen developed a eutrophic region. In general, dissolved inorganic nitrogen (DIN) species were almost 9 times higher in the northern parts compared to the south-ern ones revealing that the northern parts of the Gulf were more affected by human impacts compared to the southern area. Moreover, concentrations of most studied metals such as Zn, Mn, Ni, Cu, Cr, Cd, Pb, and Hg were higher in Northern parts com-pared to southern parts of the Gulf. The concentrations of dissolved metals in the northern and southern Suez Gulf followed the following order: Fe ≥ Zn ≥ Cu > Pb > Mn > Ni > Cr > Cd ≥ Hg. Except for Hg, concentrations of metals were in the accepta-ble ranges recommended internationally. Hg concentrations especially in the northern Suez Gulf were slightly higher than the permissible levels. As the northern parts of the Gulf were more suffered from oil pollution, total petroleum hydrocarbons revealed higher concentrations in the northern parts compared to the southern parts of the Gulf. Seasonal variations, on the other hand, indicated higher levels of DO, BOD, DOM and COD, chloropyll-a, TSM, ammonia, nitrate, total nitrogen in winter compared to their corresponding values

Evaluation of the Quality for the Egyptian Red Sea Coastal Waters during 2011-2013

To assess the quality of the Egyptian Red Sea coastal waters for the sustainable use and development, due to its importance for the national income, four field campaigns were annually carried out during the period from 2011-2013 to investigate the hydrography, nutrient salts, heavy metals and petroleum hydrocarbons. Except for the area of Bir Shalatein, the results of beach litter cleared out that the shoreline of the studied area is not affected by man-made litter. No sewage could be observed. The results of the present study showed that water temperature followed seasonal changes in air temperature. Red Sea water is more saline than adjacent Arabian Sea. DO revealed high values and presence of well oxygenated waters. Minor changes in the distribution of pH, BOD, DOM and COD, revealed that limited effects of human impacts and depend mainly on the dynamics of its water as well as on the geographical location. Low Chl-a and TSM concentrations and high transparency revealed that also the effect of human impacts is almost negligible. Significantly higher sea water temperatures, TSM, pH, DO, BOD, DOM, and COD were observed in summer season compared to their corresponding values in winter season. Dissolved inorganic nitrogen concentrations were quite low because there is little nutrient input from soil, agriculture and pollution on land. Based on the annual mean values, the pattern concentrations of dissolved inorganic nitrogen forms followed the order: NO3 > NH4 > NO2. The Red Sea coastal waters are classified as oligotrophic to mesotrophic state. A remarkable increase of PO4 concentration was observed in the middle Red Sea stations due to huge amounts of effluents enriched with phosphate from the main shipping and industry of Phosphate Companies. An experimental program is conducted on model piled rafts in sand soil.The experimental program is aimed to investigate the behavior of raft on settlement reducing piles.The testing program includes tests on models of single pile,unpiled rafts and rafts on 1,4,9,or 16 piles.The model piles beneath the rafts are closed ended displacement piles installed by driving.Three lengths of piles are used in the experiments to represent slenderness ratio,L/D,of 20,30 and 50,respectively.The dimensions of the model rafts are 30 cm x 30cm with different thickness of 0.5 cm,1.0 cm or 1.5 cm.The raft-soil stiffness ratios of the model rafts ranging from 0.39 to 10.56 cover flexible to very stiff rafts.The improvement in the ultimate bearing capacity is represented by the load improvement ratio,LIR,and the reductions in average settlement and differential settlement are represented by the settlement ratio,SR,and the differential settlement ratio,DSR,respectively.The effects of the number of settlement reducing piles,raft relative stiffness,and the slenderness ratio of piles on the load improvement ratio,settlement ratio and differential settlement ratio are presented and discussed.The results of the tests show the effectiveness of using piles as settlement reduction measure with the rafts.As the number of settlement reducing piles increases,the load improvement ratio increases and the differential settlement ratio decreases.

Pancreatic injury in patients with septic shock: A literature review

Sepsis and septic shock are life threatening condition associated with high mortality rate in critically-ill patients. This high mortality is mainly related to the inadequacy between oxygen delivery and cellular demand leading to the onset of multiorgan dysfunction. Whether this multiorgan failure affect the pancreas is not fully investigated. In fact, pancreatic injury may occur because of ischemia, overwhelming inflammatory response, oxidative stress, cellular apoptosis and/or metabolic derangement. Increased serum amylase and/or lipase levels are common in patients with septic shock. However, imaging test rarely reveal significant pancreatic damage. Whether pancreatic dysfunction does affect the prognosis of patients with septic shock or not is still a matter of debate. In fact, only few studies with limited sample size assessed the clinical relevance of the pancreatic injury in this group of patients. In this review, we aimed to describe the epidemiology and the physiopathology of pancreatic injury in septic shock patients, to clarify whether it requires specific management and to assess its prognostic value. Our main finding is that pancreatic injury does not significantly affect the outcome in septic shock patients. Hence, increased serum pancreatic enzymes without clinical features of acute pancreatitis do not require further imaging investigations and specific therapeutic intervention.
