Cerebral Malaria: Epidemiological, Clinical and Prognosis Aspects in the Anesthesia-Resuscitation Department CHU Ignace Deen

Objective: To describe the epidemiological, clinical, paraclinical and prognostic aspects of cerebral malaria received in the anesthesia-resuscitation department. Methodology: This was a prospective descriptive type study carried out in the anesthesia-resuscitation department over a period of three (03) months (June 01 to August 31, 2022). All patients admitted for cerebral malaria were included. Sociodemographic, clinical and prognostic parameters were studied. Results: We collected 25 cases out of 105 admitted patients (23.8%). The average age was 27.6 ± 9.5 years with extremes of 11 and 50 years. The sex ratio was 0.7 (M/F). Students and housewives were in the majority, i.e. 52% and 24%. Neurological disorders were found on admission in all patients, dominated by impaired consciousness at 100% followed by convulsion (60%), prostration (44%), confusion (36%) associated with deep coma in (68%). Gross hemoglobinuria was present in (84%) of cases. On the blood count, anemia was present in (70%) of the patients followed by thrombocytopenia in more than half of the cases (60%) and transfusion was necessary in 19 cases. P Falciparum malaria was found in all patients (100%), the average parasite density was 60342.8 ± 30425.6 trophozoites/μl with extremes of 9000 to 100000 trophozoites/μl. All our patients were treated with intravenous injectable artesunate. Transfusion was performed in 76% of our patients. Eighty percent of the patients had benefited from dialysis. High oxygen therapy was performed in (92%) of cases. The average duration of hospitalization was 5.74 ± days with extremes of 1 to 17 days. Mortality was 48%. Conclusion: Cerebral malaria can take different clinical forms, the most important of which is cerebral involvement. Prompt initiation of appropriate resuscitation can reduce mortality.

The Impact of Human Parvovirus B19 Co-Infection on Liver Function of HCV Infected Patients

Background: Human Parvovirus B19 is most known for causing disease in the pediatric population but can also affect adults. Human co-infection with Parvovirus B19 could deteriorate the prognosis of patient with chronic ill-ness. Objectives: This paper attempts to determine the prevalence of Parvovirus B19 in HCV infected patients and to evaluate the impact of Parvovirus B19 on liver enzymes activity of Hepatitis C patients. Study Design: The study population includes 74 chronic HCV (patient group) and 49 cases without viral hepatitis (control group). Nucleic acid of Parvovirus B19 was detected in Serum samples by nested polymerase chain reaction (nested-PCR) method. Results: Parvovirus B19-DNA infection was detected in 28.0% of chronic HCV patients. Parvovirus B19-HCV co-infection caused increasing in Alanine aminotransferase (ALT) and Aspartate aminotransferase (AST) activity than in B19-negative HCV patients. Conclusion: We conclude that Parvovirus B19 acted synergistically with HCV by increasing the levels alanine ami-notransferase (ALT) and Aspartate aminotransferase (AST).

Production and Characterization of Pozzolan with Raw Clay from Burkina Faso

Four raw deposits clayey materials in Burkina Faso have been characterized using X-ray diffraction, infrared spectroscopy and ICP-AES to evaluate their suitability to be used as pozzolan in cement. The samples have been activated by thermal treatment at 730°C during 5 hours. The obtained metakaolin was investigated by means of complementary techniques for their use as pozzolan in building materials. Fineness, surface area and amorphousness of metakaolins were the dominant factors which influenced their pozzolan reactivity. The compressive strengths of blended specimens were slightly increased with metakaolins named MK1 and MK2 than the reference for the substitution of 20 - 25 wt%. This was due to the metakaolins’ pozzolan reactivity and their filler effects which promoted CSH formation and reduced pores in the specimens. MK1 and MK2 were suitable for replacement of Portland cement in the field of building materials.

The Antiviral Efficacy of Withania somnifera(Ashwagandha) against Hepatitis C Virus Activity: In Vitroand in SilicoStudy

Objective: Evaluation antiviral effects of Withania somnifera (Ashwagandha) leaf extract against HCV. Methods: cell proliferation was assessed using MTT assay after isolation of lymphocyte cells and treated with Ashwagandha water extract (ASH-WX) (6.25 mg/ml - 100 mg/ml). Assessment of quantitative Real-time PCR, Colony forming assay, TNF-α and molecular docking studies after infection of normal lymphocyte cells with 1 ml (1.5 × 106 HCV) serum then incubated with ASH-WX at concentration 25 mg/ml & 50 mg/ml. Results: MTT assay revealed a significant increase (p α significantly decreased following ASH-WX treatment compared with control untreated infected cells (p 3 IU/mL. Colony formation assay test revealed colony formation reduction compared to positive untreated control. Molecular docking analysis revealed good prediction of binding between Ashwagandha and NS5B and PKN2 compared to Sovaldi. Conclusion: ASH-WX may be a powerful antiviral against HCV infection.

Zn(1-x)CdxO films have been grown on (a-plane) and (r-plane) sapphire substrate by metal organic chemical vapor deposition. A maximum cadmium incorporation of 8.5% and 11.2% has been respectively determined for films deposited on a- and r-plane sapphire. The optical transmission spectra and energy band-gap equation established by Makino et al. were used to estimate the cadmium mole fraction of the solid solutions. Structural, morphological and optical properties of these films were examined using high resolution X-ray diffraction (HRXRD), atomic force microscopy (AFM) and room and low temperature photoluminescence (Pl) as Cd incorporation and employed substrate. X-ray diffraction study revealed that all films had wurtzite phase but solid solution grown on a-plane sapphire are polycrystalline with a preferred orientation along the [0001] direction and a-plane film are epitaxially grown on r-plane sapphire. AFM image show significant differences between morphologies depending on orientation sapphire substrate but no significant differences on surface roughness have been found. The near band-edge photoluminescence emission shifts gradually to lower energies as Cd is incorporated and reaches 2.916 eV for the highest Cd content (11.2%) at low temperature (20 K). The room temperature hall mobility decreases with the Cd incorporation but it is larger for Zn(1-x)CdxO grown on r-plane sapphire.

Hybrid Genetic Algorithm with K-Means for Clustering Problems

The K-means method is one of the most widely used clustering methods and has been implemented in many fields of science and technology. One of the major problems of the k-means algorithm is that it may produce empty clusters depending on initial center vectors. Genetic Algorithms (GAs) are adaptive heuristic search algorithm based on the evolutionary principles of natural selection and genetics. This paper presents a hybrid version of the k-means algorithm with GAs that efficiently eliminates this empty cluster problem. Results of simulation experiments using several data sets prove our claim.

Recent Foraminifers from Oman Coast

This study presents the first documentation work on recent benthic foraminifers of Oman coasts, especially of the northern part of Sultanate at Sawadi and the southern part at Janoof Tidal Creek near Taqah-Salalah beach. Eleven samples were collected from both localities. Twenty-nine species of benthonic Foraminifera belonging to 20 genera are retrieved, identified and illustrated herein. The study areas show the interaction in land and sea. Lagoons with normal marine hypersaline inhabited with sea grass and Mangrove-swamps. Mainly calcareous porcellaneous and hyaline walled foraminifers are abundant. Environmentally, the Janoof Tidal Creek shows better condition than Sawadi Tidal Creek as reflected by the more diverse and high abundance of foraminiferal taxa at the former locality. Both water bodies are free of toxic-heavy elements as evidenced by the lack of any abnormal foraminiferal test.

Objective:The objective of this study was to assess the prevalence and level of social media addiction among nursing students in the Faculty of Nursing,Cairo *** and Methods:A descriptive cross‑sectional research design was conducted on samples of 340 students at the Faculty of Nursing,Cairo *** were collected through demographic background information sheet and Social Networking Addiction ***:All the students were addicted as 6.76%were severely addicted and 60.59%and 32.65%were moderately and mildly addicted,*** relations were found between social media addiction and students’age(χ^(2)=11.331,P=0.003),educational level(χ^(2)=20.239,P=0.003),and grade point average(χ^(2)=19.378,P=0.013).Conclusion:Internet addiction was prevalent among all students but at different levels,so early screening of students for Internet addiction using the Internet Addiction Scale is important to provide early treatment and prevent hazards to health.

Otological Injuries Observed in Head and Neck Trauma in a Second-Line Hospital

Otological injuries are common during cervicofacial trauma. The aim of this study is to describe the epidemiological characteristics and to map otological injuries during head and neck trauma. This was an observational, descriptive, prospective study conducted at the ENT unit of the Referral Health Center Hospital in Mali from January 2017 to September 2018. Patients with middle and/or inner ear injuries due to trauma were included. The prevalence of otologic injuries was 34.54% of cases. The mean age was 25 years and the sex ratio was 1.11. Students accounted for 42.10%. The mechanism of occurrence was an intentional assault in 68.42% of the cases followed by road traffic accidents (15.79%). The most common functional signs were hearing loss (68.42%), tinnitus (57.89%), and facial paralysis in 5.26%. Otoscopy showed tympanic perforation (47.83%), otorrhagia (21.05%) and otoliquorrhea (5.26%). Rock fractures with osteodural breach (10.53%) and pneumencephaly were found on a CT scan of the rock. The evolution was marked by the regression of the facial paralysis and the healing of the tympanum in 45.45% of the cases. Sequelae persisted in the form of sensorineural deafness (17%). The otological lesions observed during cervicofacial trauma are varied. The challenges in our context are felt at three levels: the improvement of the technical platform, the long-term follow-up of patients, and the cost of care in terms of equipment. These lesions are responsible for disabling deafness, a source of difficulty in social and school integration.

Post Tonsillectomy Bleeding among Children in a Tertiary Hospital in the Kingdom of Bahrain: Two Years Experience

Background: Palatine tonsils are part of the immune system located within the Waldeyer’s ring, they are prone to infections, hypertrophy or both. These conditions are known as tonsil diseases that usually require surgical removal through tonsillectomy. Tonsillectomy is one of the most common procedures done for children but it is challenged by the occurrence of complications, especially post tonsillectomy bleeding (PTB). In the current study, the incidence of post tonsillectomy bleeding in children younger than 14 years undergoing tonsillectomy in Salmaniya Medical Complex is evaluated and their demographic data and risk factors are assessed. Method: Pediatric patients who are younger than 14 years with postoperative tonsillectomy bleeding that were treated in Salmaniya Medical Complex between the period of January 2018 and December 2019 were retrospectively studied for risk factors. Results: A total of 1161 patients within the age range of 2 - 13 years old underwent tonsillectomy in Salmaniya Medical Complex from the period of January 2018 to December 2019. Twenty-eight pediatric patients had post-tonsillectomy bleeding (PTB) during the study period with an incidence of 2.4% of the total number of pediatric tonsillectomies done in our institute. The majority were male patients and the mean age was 6.2 years. The main indication of tonsillectomy for those with bleeding was recurrent tonsillitis. A second surgery was needed in 64.5% to control the bleeding. The size of the tonsil, the level of the surgeon and the season at which the tonsillectomy was done did not seem to have any effect on the incidence of post-tonsillectomy bleeding (PTB). Conclusion: The incidence of post tonsillectomy bleeding (PTB) in Salmaniya Medical Complex is 2.4%, which makes it a common complication for a common surgery irrelevant to any patient, surgeon or climate related factors.
