Nationwide trends and predictors of inpatient mortality in 83884 transjugular intrahepatic portosystemic shunt

AIM: to evaluate and validate the national trends and predictors of in-patient mortality of transjugular intrahepatic portosystemic shunt (tIPS) in 15 ***: Using the National Inpatient Sample which is a part of Health Cost and Utilization Project, we identified a discharge-weighted national estimate of 83884 tIPS procedures performed in the United States from 1998 to 2012 using international classification of diseases-9 procedural code 39.1. the demographic, hospital and co-morbility data were analyzed using a multivariant analysis. Using multi-nominal logistic regression analysis, we determined predictive factors related to increases in-hospital mortality. Comorbidity measures are in accordance to the Comorbidity Software designed by the Agency for Healthcare Research and ***: Overall, 12.3% of patients died during hospitalization with downward trend in-hospital mortality with the mean length of stay of 10.8 ± 13.1 d. Notable, African American patients (OR = 1.809 vs Caucasian patients, P transferred patients (OR = 1.347 vs non-transferred, P tive cases, P tients in the Northeast region (OR = 1.449 vs West, P tly higher odds of in-hospital mortality. Number of diagnoses and number of procedures showed positive correlations with in-hospital death (OR = 1.249 per one increase in number of procedures). Patients diagnosed with acute respiratory failure (OR = 8.246), acute kidney failure (OR = 4.359), hepatic encephalopathy (OR = 2.217) and esophageal variceal bleeding (OR = 2.187) were at considerably higher odds of in-hospital death compared with ascites (OR = 0.136, P ty measures with the highest odds of in-hospital death were fluid and electrolyte disorders (OR = 2.823), coagulopathy (OR = 2.016), and lymphoma (OR = 1.842).CONCLUSION: the overall mortality of the tIPS procedure is stea

Simulating advance distance in border irrigation systems based on the improved method of characteristics

Improving the simulation accuracy of the advance distance based on the method of characteristics is essential to develop numerical solutions for simulating surface *** of volume balance in the traditional method of characteristics(t-MC),the position of critical flow is determined to simulate the advance distance in the improved method of characteristics(I-MC),which is used in border irrigation systems with rapid variation in inflow discharge in the current ***,the zones of both subcritical and supercritical flow were firstly distinguished to determine the position of the critical flow point,which was also the upstream boundary of the wetting front region,and then the advance distance was calculated by applying the diffusive wave equation in the wetting front *** results showed that the I-MC accurately simulated the advance distance with high determination coefficients(0.984-0.998)and low errors(root mean square error of 0.35-1.56 min and coefficient of residual mass of 0.01-0.06),which performed much better than the *** I-MC provided a suitable and simple numerical simulation tool to improve the establishment of numerical surface irrigation models.

Short-term outcomes after the loading phase of intravitreal bevacizumab and subthreshold macular laser in non-center involved diabetic macular edema

AIM： to compare the effectiveness of intravitreal bevacizumab and subthreshold macular photocoagulation （SMP）, for the treatment of non-center involved diabetic macular edema （non-CI DME）. MEtHODS： Prospective, randomized, controlled clinical study included patients with type 2 diabetes, non-CI DME and best-corrected visual acuity （BCVA） of 0.30 logMAR or better. Each eye was randomized into three groups： group 1, monthly intravitreal bevacizumab; group 2, single SMP; group 3, single SMP and monthly bevacizumab. Main outcome measures were BCVA, and macular thickness measured with optical coherence tomography as macular central subfield thickness （CSt）, macular area of greater thickness （MAGt） and total macular volume （tMV）. Results were analyzed after 3mo. RESULtS： A total of 32 eyes were included. Group 3 improved in BCVA （0.19±0.16 to 0.12±0.14 logMAR; P=0.041） and in tMV （7.90±0.57 to 7.65±0.73 mm3; P=0.025）. Group 1 improved in MAGt （325±26.26 to 298.20±44.85 μm; P=0.022） and tMV （7.79±0.57 to 7.50±0.56 mm3, P=0.047）. Group 2 didn’t show significant improvement of any variable. CONCLUSION： the loading phase of bevacizumab as monotherapy or combined with SMP is superior to SMP as monotherapy in providing short-term visual and anatomical improvement in non-CI DME.

Combinatory effect of hesperetin and mesenchymal stem cells on the deteriorated lipid profile,heart and kidney functions and antioxidant activity in StZ-induced diabetic rats

this study aimed to assess the effect of hesperetin and/or bone marrow-derived mesenchymal stem cells(BM-MSCs)on disturbed lipid profile,heart and kidney functions,oxidative stress and antioxidant defense system in streptozotocin(StZ)-induced diabetic *** 1 diabetes mellitus(t1DM)was induced in male Wistar rats by injecting 40 mg/kg body weight(b.w.)StZ dissolved in citrate buffer(pH 4.5).the diabetic rats were treated with hesperetin orally administered at dose 20 mg/kg b.w.,BM-MSCs intravenously injected at a dose of 1 x 106 cells/rat/week and their combination for 6 *** diabetic rats exhibited lipid abnormalities manifested by elevated serum levels of total cholesterol,triglycerides,LDL-cholesterol and VLDL-cholesterol and lowered HDL-cholesterol as well as elevated liver cholesterol and triglycerides content in association with the resultant fasting and postprandial hyperglycemia and insulin *** heart function biomarkers including CK-MB,ASt and LDH activities as well as levels of kidney function parameters,creatinine,and urea,were significantly raised in the serum of diabetic *** changes were concomitant with abnormal redox balance represented by elevated lipid peroxidation,decreased glutathione content,and suppressed antioxidant enzyme activities in both heart and kidney of diabetic *** previous deleterious alterations were significantly ameliorated after the treatment of diabetic rats with hesperetin and BM-MSCs singly or in combination;the treatment with hesperetin together with BM-MSCs was the most *** on these findings,it can be concluded that the use of hesperetin with BM-MSCs may have more additive therapeutic value than their uses singly in *** addition,the ameliorative effects of hesperetin and BM-MSCs on lipid profile and heart and kidney functions in diabetic rats may be mediated,at least in part,via their suppressive effects on oxidative stress and ameliorative effects on the antioxidant defense system

Dolomite apatite separation by amphoteric collector in presence of bacteria

Bioflotation represents one of the growing trends to enhance the selectivity of conventional flotation processes. It utilizes the micro-organisms to replace or to interact with the chemical reagents to increase the gap between surface properties of similar minerals and to enhance the separation selectivity. In this work, dolomite-phosphate separation was investigated using amphoteric collector （dodecyl-N-carboxyethyl-N-hyroxyethyl-imidazoline） in presence of bacteria. two types of bacteria, Corynebacterium- diphtheriae-intermedius （CDI）, and Pseudomonas aeruginosa （PA）, were used. the collector-bacteria interaction was characterized by Fourier transform infra-red （FtIR）, frothing height and Zeta potential. the results show that the collector-bacteria interaction improves the flotation selectivity. Although, the PA positively affects the separation results, the CDI cannot lower the MgO to less than 1%. A phosphate content of 0.7% MgO and 31.77% P205 with a recovery of 68% at pH 11, 3.0 kg/t amphoteric collector, 4× 10^7 cells of PA is obtained.

Predicting the frequency of insurance claims has become a significant challenge due to the imbalanced datasets since the number of occurring claims is usually Significantly lower than the number of non-occurring *** a result,classification models tend to have a limited ability to predict the occurrence of ***,in this thesis,we’ll use various Resampling Methods to try to solve the imbalanced data problem in the insurance *** developed 84 machine learning models for predicting insurance claims occurrence（（under-sampling,over-sampling,the combination of over-and under-sampling（hybrid）,and SMOtE）×（logistic regression,K-nearest neighbors,na（?）ve bayes,three Decision tree models,three boosting models,two bagging models,SVM,4 neural network,and 5 deep neural network）= 84 models）,and we compared the models’ accuracies,sensitivities,and specificities to comprehend the prediction performance of the built *** dataset contains 81628 claims,each of which is a car insurance *** were5714 claims that occurred and 75914 claims that didn’t *** to the findings,the Ada Boost classifier with oversampling and the hybrid method had the most accurate predictions,with a sensitivity of 92.94%,a specificity of 99.82%,and an accuracy of 99.4%.And with a sensitivity of 92.48%,a specificity of 99.63%,and an accuracy of 99.1%,*** thesis confirmed that when analyzing imbalanced data,the Ada Boost classifier,whether using oversampling or the hybrid process,could generate more accurate models than other models that used in this *** in general,when we applied machine learning models on the different balanced data created by different resampling methods,we can notice that all ML classifiers’ results have improved,and all classes can be predicted,indicating that the classifiers’ performance is satisfactory.

Investigation of the volatile oils,lipid constituents and biological activity of Ballota andreuzziana,teucrium zanonii and Verbena tenuisecta in Libya

Objective:to determine the chemical composition of essential oils and lipid constituents of Ballola andreuzziana(***),teucrium zanonii(***) and Verbena tenuisecta(V. tenuisecta) growing in Libya,and to test the antibacterial activity of different extracts of teucrium ***:the volatile oils of all plants were extracted by hydrodistillation method and analyzed by GC/MS *** lipid constituents of plants were obtained by extraction with petroleum ether and fractionated into fatty alcohols,fatty acids and unsapoinfiable matters. Antibacterial activity of *** extracts and antioxidant activity of different extracts of t. zanonii were also ***:the volate oil of *** was found to consists of 20 compounds in which caryophyllin is the main one(63.1%),the volatile oil of *** consists of 74 compounds in which germacrene-D was the main compound,while the volatile oil of V. tenuisecta consists of 13 compounds with l-octen-3- ol as a major constituent(52.87%).the study of antimicrobial activity of different extracts of *** showed that,both methanol and butanol extracts exhibited the highest activity against Mycobacterium phlei(***) and Candida albicans(***) respectively,while petroleum ether,fatty alcohols and unsaponifiable fractions had no antimicrobial activity against all the tested microorganisms. Investigation of the antioxidant activity of different extracts of *** using DPPH method proved that,the ethyl acetate and butanol fractions showed the highest activity where the inhibition percentage(1%) are 93.6 and 92.1 ***:this is the first report about the volatile oils of these plants where *** have the highest content and the highest number of the identified *** study of antioxidant *** extracts proved that,the ethyl acetate,butanol and aqueous extracts have the highest antioxidant *** and butanol extracts of *** exhibi

Prevalence of retinopathy of prematurity in Brunei Darussalam

AIM: to identify the prevalence of retinopathy of prematurity (ROP) among preterm neonates admitted to Department of Neonatology, RIPAS hospital, Brunei Darussalam. ·MEtHODS: We studied 67 preterm infants fulfilling the eligibility criteria for ROP screening. All infants studied were admitted to the Department of Neonatology, RIPAS Hospital, within a period of one year. Birth weight (BW), gestational age (GA), corrected age at each review, initial and final diagnoses and number of reviews required was recorded for each infant. Infants were followed up two weekly until they reach a corrected age of 40 weeks or complete vascularization was noted. Prevalence of ROP was identified. Descriptive analysis, regression analysis and independent-sample t-test were used to statistically check for differences between ROP and non -ROP groups. ·RESULtS: A total number of 201 ROP screenings were carried out for 67 preterm infants. Males outnumbered females (56.7%). the mean number of reviews per child was (3.19 ±1.1) times (range: 1 -6 times), the mean GA among the preterm babies examined was (29.5 ±2.6) weeks (range: 23 -36 weeks), and the mean BW was 1300 ±500g (range: 660 -3600g). the prevalence of ROP among the examined infants was 34.8% . Prevalence of threshold disease that required laser treatment was 25.4% . Prevalence of ROP among those with extremely low BW was 86.7% compared to 27.8% in those with very low BW. Respiratory distress and congenital heart diseases were significantly associated with higher incidence of ROP. ·CONCLUSION: Lower BW, lower GA and female gender are associated with higher risk of developing ROP among preterm infants in Brunei Darussalam.
