Evaluation of the Neurodevelopmental Outcome of Toddlers with Spinal Dysraphism after Surgical Repair Using ASQ-3^™ Scores

Background: Spinal dysraphism represents a wide spectrum of congenital abnormalities of the spine. Myelomeningocele is considered the most common malformation and the most common we saw in our community, with its morbidity problems seen commonly in the postoperative period. ASQ-3TM Scores are the ages and stages questionnaire, third edition, and represent a tool to assess the development progress, especially in toddlers. Objectives: Evaluation of neurodevelopmental outcome among Sudanese toddlers with spinal dysraphism after surgical closure with or without a VP shunt using ASQ-3TM Scores. Methodology: This is a retrospective hospital-based study of 84 patients who underwent myelomeningocele repair at the National Center for Neurological Sciences (NCNS) during the period from 2017 up to 2019. Data were collected through a constructed questionnaire, including ASQ-3TM Scores. Data were processed and analyzed using the Statistical Package for Social Science (SPSS) computer program. Version 25. Results: 84 patients were included in this study;all patients were diagnosed with spinal dysraphism. Out of them, 51 (60.7%) were 2 years old, 33 (39.3%) were 3 years old, 45 (53.6%) were male, 45 (53.6%) of patients mothers attended ANC irregularly, and 54 (64.3%) their mothers didn’t receive folate supplements. 44 (52.3%) of patients underwent MMC repair only, while 40 (47.7%) underwent MMC repair and VP shunt. The commonest postoperative complication was infection, reported in 12 (14.3%) of patients, followed by VP shunt revision in 9 (10.7%) of patients. Neurological assessment showed that the majority of patients need further assessment with a professional, 57 (67.9%) of children don’t walk, run, or climb like other toddlers as their parent’s state;also, half of patients (42, 50%) had medical problems, and 27 (32.1%) of their parent’s state that they do not talk like other toddlers their age. The conversion of the cartesian coordinates of a point to its geodetic equivalent coordinates in reference to the geodetic ellipsoid is one of the main challenges in *** ellipse equation in the meridian plane significantly influences the value of the geodetic *** research analyzes this influence and how it can contribute to their *** study investigates the mathematical relation between them and presents an exact formula relating to the geodetic height and the ellipse *** addition,a heuristic formula for the relation between the geodetic height and the ellipse equation is proposed,which is independent of the geodetic latitude and has a relative accuracy better than 99.9 %.The calculation is stable,and the cost is low.

In the south Eastern Desert of Egypt,two contrasting types of magmatism(mafic and felsic) are recorded in the Wadi Kalalat area,and form the Gabal El Motaghiarat and Gabal Batuga intrusions,*** two intrusions post-dates ophiolitic and arc associations represented by serpentinite and metagabbro-diorite,*** mafic intrusion has a basal ultramafic member represented by fresh peridotite,which is followed upward by olivine gabbro and anorthositic or *** mafic intrusion pertains to the Alaskan-type mafic-ultramafic intrusions in the Arabian-Nubian Shield(ANS)being of tholeiitic nature and emplaced in a typical arc *** the other hand,the Gabal Batuga intrusion comprises three varieties of fresh A-type granites of high K-calc alkaline nature,which is peraluminous and garnetbearing in parts.A narrow thermal aureole in the olivine gabbro of the mafic intrusion was developed due to the intrusion of the Batuga *** results in the development of a hornfelsic melagabbro variety in which the composition changed from tholeiitic to a calc-alkaline composition due to the addition of S_(i)O_(2),Al_(2)O_(3),alkalis,lithosphile elements(LILEs) such as Rb(70 ppm) and Y(28 ppm) from the felsic *** the thermal aureole,Rb amounts 2-8 ppm and Y lies in the range <*** is believed that the Gabal Batuga felsic intrusion started to emplace during the waning stage of an arc system,with transition from the pre-collisional(i.e.,arc setting) to post-collisional and within plate *** from which the Gabal Batuga granites were fractionated is high-K calc-alkaline giving rise to a typical post-collisional A-type granite(A_(2)-subtype) indicating an origin from an underplating crustal ***,it is stressed here that the younger granites in the ANS are not exclusively post-collisional and within-plate but most likely they started to develop before closure of the arc *** possible source(s) of mafic magmas

Peripheral artery disease(PAD)is a common condition characterized by atherosclerosis in the peripheral arteries,associated with concomitant coronary and cerebrovascular *** convertase subtilisin/kexin type 9(PCSK9)inhibitors are a class of drugs that have shown potential in hypercholesterolemic *** review focuses on the efficacy,safety,and clinical outcomes of PCSK9 inhibitors in PAD based on the literature indexed by *** such as FOURIER and ODYSSEY demonstrate the efficacy of evolocumab and alirocumab in reducing cardiovascular events,offering a potential treatment option for PAD *** evaluations from trials show few adverse events,most of which are injection-site reactions,indicating the overall safety profile of PCSK9 *** outcomes show a reduction in cardiovascular events,ischemic strokes,and major adverse limb ***,despite these positive findings,PCSK9 inhibitors are still underutilized in clinical practice,possibly due to a lack of awareness among care providers and cost *** research is needed to establish the long-term effects and cost-effectiveness of PCSK9 inhibitors in PAD patients.

Efficient ECG classification based on Chi-square distance for arrhythmia detection

This study introduces a new classifier tailored to address the limitations inherent in conventional classifiers such as K-nearest neighbor(KNN),random forest(RF),decision tree(DT),and support vector machine(SVM)for arrhythmia *** proposed classifier leverages the Chi-square distance as a primary metric,providing a specialized and original approach for precise arrhythmia *** optimize feature selection and refine the classifier’s performance,particle swarm optimization(PSO)is integrated with the Chi-square distance as a fitness *** synergistic integration enhances the classifier’s capabilities,resulting in a substantial improvement in accuracy for arrhythmia *** results demonstrate the efficacy of the proposed method,achieving a noteworthy accuracy rate of 98% with PSO,higher than 89% achieved without any previous *** classifier outperforms machine learning(ML)and deep learning(DL)techniques,underscoring its reliability and superiority in the realm of arrhythmia *** promising results render it an effective method to support both academic and medical communities,offering an advanced and precise solution for arrhythmia detection in electrocardiogram(ECG)data.

Comparative study between three carbonaceous nanoblades and nanodarts for antimicrobial applications

The design of nanostructured materials occupies a privileged position in the development and management of affordable and effective technology in the antibacterial ***,we discuss the antimicrobial properties of three carbonaceous nanoblades and nanodarts materials of graphene oxide(GO),reduced graphene oxide(RGO),and single-wall carbon nanotubes(SWCNTs)that have a mechano-bactericidal effect,and the ability to piercing or slicing bacterial *** demonstrate the significance of size,morphology and composition on the antibacterial activity mechanism,the designed nanomaterials have been *** minimum inhibitory concentration(MIC),standard agar well diffusion,and transmission electron microscopy were utilized to evaluate the antibacterial activity of GO,RGO,and *** on the evidence obtained,the three carbonaceous materials exhibit activity against all microbial strains tested by completely encapsulating bacterial cells and causing morphological disruption by degrading the microbial cell membrane in the order of RGO>GO>*** of the external cell wall structure and outer membrane proteins,the synthesized carbonaceous nanomaterials exhibited higher antibacterial activity against Gram-positive bacterial strains than Gram-negative and fungal *** had the lowest MIC values(0.062,0.125,and 0.25 mg/mL against ***,***,and ***,respectively),as well as minimum fungal concentrations(0.5 mg/mL for both *** and ***).At 12 hr,the cell viability values against tested microbial strains were completely *** lysis and death occurred as a result of severe membrane damage caused by microorganisms perched on RGO *** work gives an insight into the design of effective graphene-based antimicrobial materials for water treatment and remediation.

Evaluating mechanism of banana pseudo-stem retting using seawater:A cost-effective surface pre-treatment approach

Retting has been employed to extract natural fibers from agricultural wastes as a biological and cost-effective approach for *** its global abundance,banana pseudo-stem is a promising agro-waste for lignocellulosic fiber *** this study,fibers were extracted from the pseudo-stems after being pre-treated under four conditions using seawater at room temperature for up to 35 d Bacterial isolation from the fresh seawater sample and screening for ligninolytic ability were *** load as well as laccase and manganese peroxidase enzyme activity profile assay during the retting duration were *** transform infrared(FT-IR)and X-day diffraction(XRD)analyses were also examined for both pre-treated and untreated extracted *** results shows that six out of the eight bacterial isolates had the ability to degrade *** treatments(Raw stem+Raw seawater)and(Autoclaved stem+Raw seawater)recorded the highest viable bacterial load of 9.24×102 and 4.46×102 CFU,respectively,on the 14th day of the retting ***,the highest laccase and manganese peroxidase enzymes activity was recorded for(Raw stem+Raw seawater)and(Autoclaved stem+Raw seawater)treatments in the second to the third *** FT-IR spectra of the pre-treated fibers revealed relative reductions in peaks attributed to polysaccharides and other amorphous substances for all retting *** XRD diffractogram revealed that the crystallinity index(CI)of pre-treated fibers increased in all seawater retting treatment ***,the CI for fibers pre-treated under enzymatic conditions were enhanced even after five *** analysis for selected bacterial isolates showed homology to sequences of Bacillus velezensis,Shewanella sp.L8–5,and Citrobacter amalonaticus and Bacillus subtilis j8 *** these findings,it was suggested that physical,biological,and chemical actions were collectively involved in the seawater retting process of banana ps

In this editorial,we respond to a review article by Nabi et al,in which the authors discussed gastroesophageal reflux(GER)following peroral endoscopic myotomy(POEM).POEM is presently the primary therapeutic option for achalasia,which is both safe and effective.A few adverse effects were documented after POEM,including *** diagnostic criteria were not clear enough because approximately 60%of patients have a long acid exposure time,while only 10%experience reflux *** predictors of high disease incidence have been identified,including old age,female sex,obesity,and a baseline lower esophageal sphincter pressure of less than 45 *** technical steps during the procedure,such as a lengthy or full-thickness myotomy,may further enhance the *** pump inhibitors are currently the first line of *** voices are increasingly advocating for the routine combining of POEM with an endoscopic fundoplication method,such as peroral endoscopic fundoplication or transoral incisionless ***,more research is necessary to determine the safety and effectiveness of these procedures in the long term for patients who have undergone them.
