Epidemiology of Sleep Disorders among Chronic Hemodialysis Patients in Senegal: A Multicentric Study

Introduction: Sleep disorders (SD) are common dialysis patients and can impact their quality of life. In previous studies, black ethnicity was associated with higher incidence of SD but a few data are available in African patients. This study aimed to describe prevalence and risk factors of SD among Senegalese dialysis patients. Methods: We performed a cross-sectional study between February 15th and April 30th 2012 including 127 patients (75 males and 52 females) aged 46.8 ± 16.9 (16 - 85 years) and dialysed since >6 months in three dialysis centres. For each patient, we assessed insomnia according to international definition, obstructive sleep apnea syndrome (OSAS) with the Berlin questionnaire, restless leg syndrome (RLS) using abridged version of Cambridge-Hopkins RLS questionnaire, and excessive daytime sleepiness (EDS) with Epworth sleepiness scale. Logistic multivariate regression was used to identify factors associated with different SD. Results: Overall prevalence of SD was 88% comprising: insomnia (64.3%), OSAS (49.1%), RLS (24.1%) and EDS (20.5%). Forty-two patients presented at least two disorders. No difference was noticed in prevalence of SD between genders (p = 0.14). Level of blood pressure were not different across patients with and without SD. Insomnia correlated with anemia, inflammation and EDS. OSAS was associated with age ≥50 years, EDS and neck circumference ≥25 cm. RLS correlated with anemia and EDS. Other parameters such as gender, dialysis vintage, KT/V, obesity, diabetes status and hypoalbuminemia were not associated with the different SD. The majority of patients had not been diagnosed before the survey and none of them was under treatment. Conclusions: Our findings are compatible with high prevalence of sleep disorders reported in other populations. Insomnia and OSAS are the most frequent SD but some patients combined many disorders. Tako-tsubo cardiomyopathy in a male patient after administration of isoproterenol and implantation of a pacemaker

Background: Tako-tsubo cardiomyopathy occurs typicallly after an intrinsic adrenergic hyperstimulation triggered by a psychological or physically stressful event. Exceptionally, it may be caused by an exogenous hyperadrenergism. We report the case of an 85 year old man, hypertensive, with a history of ischemic stroke. He consulted for signs of heart failure with recurrent dizziness two weeks which was explained by an atrioventricular block. Initial echocardiography showed left ventricular ejection fraction to 60% with no wall motion abnormalities. The patient received iso-prenaline (0.02 mcg/kg/mn) for 20 hours before the implantation of a single chamber pacemaker. At 24 hours of admission an acute pulmonary edema occurred. The control echocardiography showed impaired left ventricular ejection fraction of 25% with apical ballooning and akinesis was also found on ventricu-lography. Coronary angiography showed no significant coronary lesions. Troponin level was elevated to 2 ng/ml. The pulmonary edema was then controlled. Subsequent clinical and echocardiographic were favo- rable after two weeks which was consistent with the diagnosis of tako-tsubo cardiomyopathy. Conclusion: This case illustrates an example of tako-tsubo syndrome induced by an exogenous catecholergic stimulation (isoprenaline) combined with an endogenous release after a pacemaker implantation which confirmed the physiopathological hypothesis of a catecholamine-mediated stunning in tako-tsubo cardiomyopathy.

Quality of Life of Patients on Peritoneal dialysis in Dakar: A Senegalese Single Centre Experience

Introduction: Measuring the quality of life (QOL) in recent years has become an indispensable tool in monitoring patients suffering from chronic diseases. We conducted this study to assess QOL of patients undergoing peritoneal dialysis in Dakar, and to identify associated factors. Patients and Methods: This is a cross-sectional study which was carried out from 10 to 30 June, 2011 in the peritoneal dialysis unit at university hospital in Dakar. We included all patients with end-stage renal disease (ESRD) of any age, who were on PD since at least six months and who gave their consent. The QOL was assessed using the Kidney Disease Quality of Life Short-Form 1.2 (KDQoL-SF). Results: Sixteen patients were included with a mean age of 50.25 ± 13.48 years and a sex-ratio of 1.27. Considering SF-36, the overall mean score (SMG) was 60.11 ± 15.96 with a Mean Physical Component Summary Scale of 53.66 ± 16.98 and a Mental Component Summary Scale of 70.85 ± 6.14. Concerning the KDQoL-SF, the global mean score was 61.83 ± 19.35 with a mean physical score of 50.55 ± 16.52 and a mean mental score of 62.52 ± 21.53. The mean dialysis specific dimension score was 62.52 ± 21.53 and the mean mental health score was 85.93 ± 12.06. Age, weight, level of instruction and social support were correlated with a worse QOL. Conclusion: This study showed an alteration of our PD patients’ QOL, particularly in their physical health. However, the number of patients included in the study is not enough to permit a formal conclusion.

Antisynthetase Syndrome in Senegalese Patients: Report of Three Cases

Introduction: Antisynthetase syndrome is an original entity and rare autoimmune myositis and systemic disease, characterized clinically by a wide spectrum of clinical manifestations and the presence of autoantibodies directed against aminoacyl RNAt synthetases. We describe this disease in 03 Senegalese patients. Observations: The first patient was a 49-years-old black woman who was referred in our department after 06-months of follow-up for a misdiagnosis of tuberculosis. The clinical examination revealed polyarthritis, muscle weakness, chronic cough with crackling rales at the pulmonary bases, Raynaud phenomenon and dry syndrome. The second patient, a 21-years-old black woman, had polyarthritis and a progressive muscle weakness. The clinical examination showed also cutaneous signs including an erythema on the dorsal part of the fingers and the presence of the heliotrope erythema on the eyes. The last patient was a 52 years-old black woman. His clinical examination showed polyarthritis, muscle weakness and an appearance of mechanics’ hands. The creatinine phosphokinase was at 6.26 × N, 40.3 × N and 33.64 × N respectively in our patients. The chest computer tomography revealed an interstitial lung disease with a pattern of non-specific interstitial pneumonia in all three patients. The autoantibodies anti-Jo1 was also positive in all patients. The diagnosis of antisynthetase syndrome was retained with an overlap of antisynthetase and Sj?gren’s syndrome in the first observation. The evolution was favourable in our 03 observations with a therapeutic combination including Prednisone-Azathioprine and Kinesitherapy. Conclusion: Antisynthetase syndrome has been exceptionally reported in sub-Saharan Africa. It must be particularly mentioned in front of the triad: myositis, arthritis and interstitial lung disease. Prevalence and the Risk Factors of Renal Insufficiency in the City of Saint Louis in Senegal

Background: The true scale of renal insufficiency (RI) in Sub-Saharan Africa remains unknown due to the lack of national registries. The aim of this study is to describe the epidemiological characteristics of renal insufficiency in urban areas in Saint Louis of Senegal. Materials and Methods: It is an observational, cross-sectional and descriptive study. The study was conducted during 27 days starting from 3 to 30 May 2010. All senegalese residents of Saint Louis (older than 15 years at the time of the study) in whom creatinine clearance was performed were included in the study. The sampling method used was a systematic random sampling, stratified cluster. The survey was designed by an expert comitee based on STEPS survey of the World Health Organization. RI was defined as a glomerular filtration rate (GFR) 2. Results: Among 1424 people initially selected a final selection of 1416 was made. The sex ratio was 0.45. The mean age was 43.4 ± 17.8 years. The overall prevalence of renal insufficiency according to MDRD (Modification of diet in renal disease) formula was 181 cases or 12.7%. The mean age of the people with renal insufficiency was 47.6 ± 17.4 years. Renal insufficiency was correlated to height blood pressure (p = 0.01) and Physical inactivity (p = 0.0001). The prevalence of renal insufficiency was higher in diabetics (71.4%) and obese people (66.6%) than in non-diabetics (64.9%) and non-obese people (56.5%), although the difference was not statistically significant. Dyslipidemia and smoking were not correlated to the risk of occurrence of IR. Conclusions: This study reports the increasing magnitude of RI and its risk factors in the city of Saint Louis in Senegal. It is imperative to establish à national prevention strategies to avoid the dizzying growth of this scourge.

Gravimetric Study of Geological Structures of Teboursouk Area, Northern Tunisia

Gravity data associated with surface geology in the Northern Tunisian Atlas offer better understand to the underlying structures in Teboursouk area and to highlight other deep or unknown structures in surface. The gravity study was based on qualitative and quantitative analysis including the construction of the gravity Bouguer anomaly, upward continuations, residual anomaly, and Horizontal gradient maxima maps. The main results display many positive and negative anomalies as the response of geological structures (J. Cheid Triassic structure, Khalled plain, El Aroussa plain). In addition, the horizontal gradient maxima integrated with geological and structural maps let the identification of major directions of gravimetric lineaments in the study area us NE-SW trending features at the boundaries of J. Cheid structure, NW-SE direction that limit Gaafour plain and Tabet Ech cherif syncline, and N-S trending that bordered El Aroussa basin. Major results allowed the construction of a new structural map of the study zone.

We report a case of torsion of an ovarian tumor in a 68-year-old woman with no medical and surgical history. The diagnosis was made in front of an intermittently painful pelvic mass. The treatment consisted of a left annexectomy;the anatomopathological examination revealed a fibro-inflammatory and hemorrhagic cyst. Postoperative results were simple with a 12-month follow-up.
