Introduction: Our study focused on the evaluation of renal function in children with sickle cell disease compared to children without sickle cell disease at the pediatric emergency unit of the Donka National Hospital and SOS Drepano-Guinea center. Patients and Methods: This was a cross-sectional descriptive and analytical study lasting 3 months (October 1 to December 31, 2020). Were included, all sickle cell and non-sickle cell children aged 0 to 15 received on an outpatient basis and had undergone an exploration of renal function (serum creatinine and urine dipstick). Results: We performed the urine dipstick and serum creatinine in 75 children, 45 of whom were sickle cell and 30 were not sickle cell. 27 of our patients or 36% had a reduction in GFR, among them 9 or 20% were sickle cell and 18 or 60% were not sickle cell. The most affected age group was 5 to 9 years in sickle cell (66.7%) and non-sickle cell (38.9%). In sickle cell patients, 9 cases (100%) had mild renal failure (IRL). Non-sickle cell patients, had 14 cases or 77.8% of IRL and 4 cases (22.2%) of moderate IR. Sickle cell disease and antibiotics which had the respective p-value (0.01);(0.02), were statistically significant with the onset of renal failure. Conclusion: Several factors including sickle cell anemia and antibiotics are believed to be involved in lowering GFR. It would be essential to detect early the children received in consultation.

SAPHO Syndrome: About Three Cases and Review of the Literature

Background: SAPHO syndrome (synovitis, acne, pustulosis, hyperostosis, osteitis) is both a dermatological and a rheumatological entity, with the common denominator of an aseptic inflammatory process, the bone manifestations of which are the essential element of the diagnosis. Despite its chronic and recurrent nature, SAPHO syndrome is very rarely the cause of major disability, even after several decades of evolution. Objective: We want to illustrate through three clinical cases, the importance of the anamnesis, the clinic, the diagnostic criteria in the diagnosis of SAPHO syndrome. Presentation of the cases: We studied the clinical file of three patients consulted in the Rheumatology Department of the CHU Aristide LeDantec in Dakar, Senegal, between January 2012 and December 2020, suffering from SAPHO syndrome of diagnosis established in accordance with the diagnostic criteria of SAPHO syndrome proposed by Khan et al. of 1994. Conclusion: SAPHO syndrome is a rare pathological entity, over a period of 8 years we report 3 cases.

Genetic and Metabolic Determinants of Plasminogen Activator Inhibitor 1 (PAI-1) in Tunisian Type 2 Diabetes Patients

Background: PAI-1 (plasminogen activator inhibitor-1) is a powerful regulator of fibrinolysis and plasma level is high in type 2 diabetes and cardio-vascular disease, which is determined by genetic polymorphisms in PAI-1 gene and environmental factors. The aim of the study was to examine the determinants of plasma PAI-1 Ag level among type 2 diabetes patients. Methods: 491 Tunisian type 2 diabetes patients had clinical evaluation (weight, high, BMI, Waist Circumference), laboratory investigations including FBG Hb1Ac, cholesterol, triglyceride;HDL-cholesterol was done;plasma PAI-1 antigen level was done with ELISA;−675 4G/5G and −844 G/A polymorphisms of PAI-1 gene was done by PCR-ASA and PCR-RFLP respectively. Results: The mean age for our patients was 58.3 ± 10.5 years;sex-ratio = 0.92;mean PAI-1 level was 34.6 ± 21.3 ng/ml. We didn’t find correlation between PAI-1 level and BMI, but we have found significant correlation between PAI-1 and waist circumference (p = 0.032), most enhanced in men (P = 0.002), T2D patients who have FBG > 11 mmol/l had PAI-1 Ag level higher than those who have FBG P = 0.034), but no difference found between T2D with high Hb1Ac > 8% and those with Hb1Ac < 8%, significant correlation was seen between PAI-1 level and LDL-cholesterol (P = 0.05), high correlation between PAI-1 Ag level and −675 4G/5G polymorphism genotype was seen, 4G/4G carriers had the highest PAI-1 level, 4G/5G had intermediary level and 5G/5G had the lowest level (P −844G/A polymorphism genotypes. Using multiple variable linear regression analysis, the independent factor associated with plasma PAI-1 level was −675 4G/5G polymorphism (regression coefficient β = 4.6, P Conclusion: the present study identifies −675 4G/5G not −844 G/A polymorphism of PAI gene as the principal determinant of plasma PAI-1 level in Tunisian T2D patients, the android fat distribution, dyslipidemia and hyperglycemia play a modest role in this variation.

−675 4G/5G and −844 G/A of Plasminogne Activator Inhibitor-1 (Pai-1) Gene Polymorphisms and Type 2 DiabetesMellitus in Tunisia: Case-Control Study

Background: The plasminogen activator inhibitor-1 (PAI-1) is a puissant antifibrinolytic factor;plasma PAI-1 level is high in type 2 diabetes. 4G/5G polymorphism of PAI-1 gene is a major genetic determinant of plasma PAI-1 levels, with 4G carriers having high PAI-1 level than 5G, theses pose the question about relation T2 patients and those polymorphisms. The aim of this study was to determine the relationship between the polymorphisms −675 4G/5G and −844 G/A of PAI-1 gene and type 2 diabetes mellitus. Methods: A case control study of 491 diabetic and 400 healthy controls. Genotyping of the polymorphism −675 4G/5G was done by PCR-ASA (polymerase chain reaction, allele specific amplification), and the polymorphism −844 G/A was done with PCR-RFLP (restriction fragment length polymorphism), the allelic frequency is calculated with hardy-Weinberg law, the statistic analysis was done by SPSS version 10. Results: Higher frequencies of The genotypes 4G/4G (p = 0.01) and 4G/5G of polymorphism −675 4G/5G were seen in diabetic (p = 0.05) and higher frequencies of 5G/5G was seen in controls (p −844 G/A was seen in diabetics and G/G was seen in controls (p = 0.01). Conclusion: Our study found association between 4G allele of −675 4G/5G and A allele of −844 G/A of PAI-1 gene and having type 2 diabetes mellitus in Tunisian population.

Study of the Mechanical Behaviour of Mortars Modified with Rice Husk Ash

The purpose of this work is to study the influence of Rice Husk Ash (RHA) on the mechanical strength of mortars. For this purpose, ash was produced by calcining rice husk at 680°C for 5 hours to produce reactive pozzolan. The chemical and mineralogical composition studied by Inductively Coupled Plasma-Atomic Emission Spectrometry (ICP-AES) and X-Ray Diffraction (XRD) showed that this ash is rich in amorphous silica. The ash presents a good pozzolanic activity with a lime fixation rate of 100% after only 7 days of treatment. The addition of RHA to cement contributes to the formation of calcium silicate hydrate (CSH) and calcium aluminate hydrate of C3ASH6 type. The presence of RHA in the mortars improves their mechanical strength. This improvement is due to the filler effect of RHA and to formation of the CSH resulting from the pozzolanic reactivity between the amorphous silica of the RHA and the portlandite released by hydration of the cement.

Synthesis, crystal structure and characterization of a new octane-1,8-diammonium bis(dihydrogenmonophosphate)

The synthesis and the structure of a new [H3N-(CH2)8-NH3][H2PO4]2 is described. This compound crystallizes in the orthorhombic system, with the centric space group Pbcn and the following unit cell parameters: a = 7.2887 (4), b = 9.1728 (6), c = 23.2169 (19) ?, Z = 4 and V = 1552.23 (18) ?3. The crystal structure has been solved and refined to R = 0.074 and R(w) = 0.173. The [H2PO4]- entities are associated by O---H...O strong hydrogen bonds to form layers. These latters are interconnected with the organic entities via (N)-H...O hydrogen bonds. In this atomic arrangement, H bonds between the different species play an important role in the three-dimensional network. This compound has also been characterized by infrared spectroscopy, solid state MAS-NMR and thermal analysis.

Introduction: Management of pheochromocytoma faces different challenges leading to misdiagnosis and therapeutic delay in developing countries. The aim of our study was to evaluate the diagnostic and therapeutic features as well as the evolution of our management strategies in hospital setting in recent years in Dakar. Patients and Method: We carried out a retrospective study over 12 years (January 2005 to June 2017), in 4 hospitals in Dakar. We only included patients with established diagnosis of pheochromocytoma upon clinical, biological, radiological and histological arguments. Results: During this period, 16 observations have been collected and were all about female patients. The mean age at the time of diagnosis was 36.81 years. The clinical signs were dominated by hypertension found for 14 patients. At biological examination, hypersecretion catecholamine was found in 13 patients, high urinary metanephrines in 7 patients, high plasmatic metanephrines in 2 patients and raised urinary Vanyl Mandelic Acid (VMA) in 4 patients. Dosage of chromogranin A showed high level for one patient. Abdominal CT scan allowed making the adrenal origin of tumors precise. One patient presented bilateral pheochromocytoma. Before being treated surgically, all patients had had medical preparation. The surgical treatment by median laparotomy consisted of an enucleation of the tumor in 2 patients, and adrenal resection in *** evolution after surgery was favorable in 12 patients out of 13. One patient died 2 days after surgery by hemodynamic instability. In 2 patients who presented metastases, multidrug therapy with cyclophosphamide, vincristine and dacarbazine was decided after a multidisciplinary meeting, but they died before treatment. Conclusion: Management of pheochromocytomas in developing countries is obviously improving, but the mortality rate remains high. Progression of Minimally Invasive Urological Surgery at Hôpital Général Idrissa Pouye in Dakar in 20 Years of Practice

Introduction: Open surgery is gradually being supplanted by minimally invasive surgical techniques worldwide. Our study aimed to describe the place of minimally invasive surgery at the Hôpital Général Idrissa Pouye (HOGIP) in Dakar. Materials and Methods: This is a descriptive cross-sectional study over a 20-years period from 1st June 2000 to 31st December 2021 in the urology department of HOGIP in Dakar. The list of all surgical procedures performed was computed. We evaluated the distribution of minimally invasive procedures (endoscopy, PCNL, laparoscopy) overall and over the years. We made calculations of proportions and statistical significance was considered for alpha = 0.05. Results: In 20 years, 14,855 surgical procedures were performed, of which 5344 (36%) were minimally invasive surgeries. The average age of men was 53.19 years (standard deviation: 21.77) vs 47.32 years (standard deviation: 18.43) for women. Minimally invasive procedures in the lower urinary tract accounted for 71.93% and involved 1033 cystoscopies (26.87%), 1020 Trans Urethral Resection of Prostate (TURP) (26.53%), 931 Direct Vision Internal Urethrotomy (DVIU) (24.21%, and 612 Trans Urethral Resection of the Bladder (TURB) (15.92%). In the upper urinary tract, 1461 (28.07%) minimally invasive procedures were performed, including PCNL in 193 cases (3.61%), laser endopyelotomy in 104 cases (1.95%), ureteroscopy in 486 cases (1.7%) and laparoscopy in 39 cases (0.46%). The proportion of minimally invasive surgery has gradually increased. Among the 5344 minimally invasive procedures, 333 (5.23%) were performed in 2000-2004 and 2332 (43.63%) in 2015-2019. Conclusion: Minimally invasive surgery represents an important part of the activity of the HOGIP urology department. Its development has improved over the years offering its advantages to the patients.

The Factors of the Failure in the Treatment of Congenital Equinus Clubfoot Varus by the Ponseti Method at the Reference Health Care Center (CSREF) of Commune III of Bamako

Clubfoot malformation is the most common serious congenital anomaly affecting the foot in children. Its treatment by the Ponseti method is simple, profitable and widely used in the world. Objective: The objective of this study was to identify the factors of the failure of its treatment by the Ponseti method. Material and Method: We carried out a retrospective and descriptive study of cases of congenital equinus clubfoot varus at the Reference Health Care Center of Commune III of Bamako over 26 months from September 2020 to November 2022. Data were treated with the utmost anonymity. Result: This study was performed on 44 children seen for clubfoot: male (68%) and female (32%), with a sex ratio of 2.1. We obtained 13 cases of recidivism including 7 boys and 6 girls. We found 21 cases of unilateral and 23 bilateral;among which 9 recurrences were found against 4 in the unilateral forms. 85% of recurrences did not have good adherence to the splint and 62% did not come regularly for follow-up consultation. We obtained 33 children with idiopathic clubfeet (75%) with a recurrence of 24%, and 7 children with secondary clubfeet with 71 % recurrence. There was no recurrence in the postural type. Among the recurrences, 38.5% started treatment between 1 and 6 months, 23.1% from 0 to 1 month and 15.4% at 2 years and more. 85% of recurrences had a Pirani score between 4.5 to 6 at the start of treatment and 15% at a score of 2.5 to 4. Conclusion: The factors of the failure of the Ponseti method are mainly non-compliance with treatment, secondary clubfeet, and a high Pirani score at the start of treatment.
