As being at the head of the establishment, the hospital manager＇s major concern is food safety in his hospital. Unfortunately, because of their health weaknesses, patients are more susceptible to foodborne illness than any other population group As match as food prepared according to less stringent hygiene＇s rules can infect or intoxicate more than it would be in a healthy population, as hygiene＇s rules must be observed in a particularly strict way, at the kitchen where meals are prepared for health care facilities. In fact, hospitals＇ food hygiene＇s rules are those who defined catering. They are made to avoid food contamination and microbial growth throughout the food chain from raw materials＇ delivery to the consumer. The main objective of taking the meal as a patients＇ tracer is to detect and study the infection＇s risk associated with feeding at hospital, and to provide practical tools to better manage this risk. Methodology was based on an analysis of the last six years＇ work concerning hygiene＇s audits and bacteriological analyses. The results showed that the food infectious risk has an important frequency, on one side; on another, they were used as a standard model of such risk＇s management.

Antioxidant and antimicrobial activities of Padina pavonica and Enteromorpha sp. from the Tunisian Mediterranean coast

Objective: To examine the antioxidant and the antimicrobial activities of the marine seaweeds Padina pavonica (P. pavonica) and Enteromorpha sp. from the Tunisian Mediterranean coast. Methods: The acetone and water were used for algae extraction to envisage the antimicrobial activity versus Gram-negative (Escherichia coli and Pseudomonas aeruginosa), Gram-positive bacteria (Staphylococcus aureus and Enterococcus faecalis) and against four Candida. The microdilution method was used to evaluate this activity. In vitro, total phenolic content and the antioxidant activity including 1,1-diphenyl-2-picrylhydrazyl (DPPH) radical scavenging and 2,2'-azino-bis-(3-ethylbenthiazoline)-6-sulfonic acid (ABTS) were also studied. Results: The highest amount of phenolic compound was found in the P. pavonica acetonic extract ((90.61 ± 0.11) mg catechin equivalent/g extract)This brown algae sample demonstrated greater DPPH and ABTS radical scavenging ability potential in comparison to other green seaweed, Enteromorpha sp. The maximum antimicrobial activity was shown by the P. pavonica acetonic extract against all the pathogenic strains tested (minimum inhibitory concentrations = minimum inhibitory bactericidal = minimum inhibitory fungicidal concentrations = 0.04 mg/mL). Those activities might be due to phenolic substances present in this fraction. Conclusions: The present results highlight the possible use of P. pavonica as source of antioxidant and antimicrobial compounds.

At hospital, nutrition represents an important value of care, particularly for patients at risk. However, it is observed that for various reasons, the restoration of the hospitalized patients is often neglected in the privileges of the medical care. The establishment of a Liaison Committee of Food and Nutrition （LCFN） within a health establishment has shown according to several works, its positive impact in improving the hygienic quality and nutritional dishes served to patients. In this framework, we conducted a quasi-experimental study into CHU F hached Sousse of Tunisia type （before/after, creation LCFN） in order to assess the role of such structure （LCFN） in the qualification and the improvement of patients＇ food. Our study was based during the two phases （2007/2010） on the same methodology. Thus we have conducted an audit of observation of hygiene practices along the distribution chain of patient＇s meals and the samples for microbiological analyzes from food, surfaces, equipment and personnel＇s *** results obtained have shown, in one hand, a degradation of the average rate of contamination for the bacteriological analyzes, and in the other hand, an evolution of the rate of hygiene standards respected. According to this study, the role played by the LCFN is becoming more and clearer in the improvement of the hygienic quality of patients＇ dishes without forgetting the impact of improving the nutritional quality and Hedonics.

Mass concentrations of PM10 and PM2.5 for January 2012 up to October 2013 were measured at two sites;a suburban area, Riyadh airport (AP) and an urban area, King Fahd road district (KF) (densely trafficked site). Daily concentrations of both PM10 and PM2.5 measured at these two sites were highly variable. The temporal variation observed either for PM10 or PM2.5 was associated mainly with the dust events. The annual pattern of PM10 and PM2.5 at the AP station shows a significant increase in spring and summer (dusty seasons) where monthly mean concentrations of up to 696 μg·m-3 PM10 were recorded in March. PM10 concentrations during winter months are significantly lower, with 65 μg·m-3 measured in November and 78 μg·m-3 measured in December. January exhibits PM10 concentrations little above 100 μg·m-3. Both PM10 and PM2.5 showed substantial variations at these two monitoring stations during the study period. In the ur-ban station (King Fahd), average annual PM10 varied by up to roughly a factor of two. This variation implies that local emission sources in this site often contribute much (i.e. greater than half) of the PM10 mass on annual average. The variation of monthly (temporal) mean PM10 at KF station showed more or less similar results to the AP station. Higher concentrations were observed during spring and summer (12%, 17% and 13% in Feb., March and April respectively and 17, 15% during June and July). Starting from September and October, PM10 concentrations started to drop to 7% and 8% respectively. The relationships between the mean values of suspended particle concentrations and meteorological variables (relative humidity ambient temperature and wind speed) were analyzed. The results demonstrate that the influence of weather factors upon dust particulates is largely inconclusive. However, At KF station a moderate positive correlation was observed between wind speed and PM.

Concentrations of airborne particulate matter (PM10 & PM2.5) were measured over a period extended from January 2012 to October 2013 at two sites: a suburban area, Riyadh airport (AP) and an urban area, King Fahd road district (KF) (densely trafficked site) in Riyadh city, Saudi Arabia. The quality of ambient air using the U.S. Environmental Protection Agency (USEPA) Air Quality Index (AQI) was evaluated during this period. The average AQI values exceeded 100 on 6 months out of 11 in AP district. At KF district the condition was worse, and AQI values exceeded 100 on all months. This emphasizes the fact that PM concentrations over Riyadh can be regarded as a real environmental problem that could be a severe risk to quality of life and threaten human health. A higher-volume air sampler at the two sites was used to collect particulate matter (PM10 and PM2.5) for chemical analysis of the PM for PAHs and total organic carbon. At KF station the mean concentrations’ values of total PAHs in PM2.5 and PM10 were 4.89 ng/m3 and 12.96 ng/m3 respectively. At the AP station the corresponding values of total PAHs in PM2.5 and PM10 were 8.07 ng/m3 and 11.91 ng/m3 respectively. The PAHs did not exceed the air criteria of World Health Organization indicating the air pollution of PAHs in Riyadh District was not serious. As for the total organic carbon the results demonstrated that the concentration ranged from 0.211 to 0.506 g/kg at KF station and from 0.054 to 0.342 g/kg at AP station. On an annual basis, OC concentrations were two times higher for the city center site (KF) compared to the rural AP site. This can most likely be explained by increased emissions from higher traffic at KF site. The diurnal variability of PM was mainly associated with the severity of pollution.

Background: The liver is the most commonly damaged organ in abdominal trauma. The management of liver trauma has experienced many changes over the last two decades. Currently there is a trend toward a non-operative treatment warranted by the successful pediatric experience and better results recorded in many trauma centers worldwide. This study aimed to evaluate outcomes of operative and non-operative management of liver trauma in our institution over the last five ***: The patients with a diagnosis of blunt or penetrating liver injuries, admitted and managed in our hospital from January 2012 to December 2016 were retrospectively studied. The patients were divided into 2 groups, operated and non-operated groups, according to the initial management considered appropriate at the time of patient admission. Clinical features and outcomes were ***: The study involved 83 patients, with a mean age of 33 years and a marked male predominance(85.5%). The most common type of lesions was blunt trauma and the main cause was road traffic accidents. Sixty-eight liver injuries(81.9%) were of low severity(grades Ⅰ,Ⅱ,Ⅲ), while 15(18.1%) were of high severity(grade Ⅳ or greater). Fifty-six patients(67.5%) had multiple injuries. Surgical treatment was performed in 26(31.3%) patients. Non-operative management was undertaken in 57 cases(68.7%). The morbidity and mortality rates were clearly lower in non-operative patients compared to those in the operated ***: Careful non-operative management is an adequate therapeutic strategy for the patients suffering from liver trauma with stable hemodynamics. Patients with complex hepatic trauma and especially those with other organ injuries continue to have significantly higher mortality.

Background: The plasminogen activator inhibitor-1 (PAI-1) is a puissant antifibrinolytic factor;plasma PAI-1 level is high in type 2 diabetes. 4G/5G polymorphism of PAI-1 gene is a major genetic determinant of plasma PAI-1 levels, with 4G carriers having high PAI-1 level than 5G, theses pose the question about relation T2 patients and those polymorphisms. The aim of this study was to determine the relationship between the polymorphisms −675 4G/5G and −844 G/A of PAI-1 gene and type 2 diabetes mellitus. Methods: A case control study of 491 diabetic and 400 healthy controls. Genotyping of the polymorphism −675 4G/5G was done by PCR-ASA (polymerase chain reaction, allele specific amplification), and the polymorphism −844 G/A was done with PCR-RFLP (restriction fragment length polymorphism), the allelic frequency is calculated with hardy-Weinberg law, the statistic analysis was done by SPSS version 10. Results: Higher frequencies of The genotypes 4G/4G (p = 0.01) and 4G/5G of polymorphism −675 4G/5G were seen in diabetic (p = 0.05) and higher frequencies of 5G/5G was seen in controls (p −844 G/A was seen in diabetics and G/G was seen in controls (p = 0.01). Conclusion: Our study found association between 4G allele of −675 4G/5G and A allele of −844 G/A of PAI-1 gene and having type 2 diabetes mellitus in Tunisian population.

Background: PAI-1 (plasminogen activator inhibitor-1) is a powerful regulator of fibrinolysis and plasma level is high in type 2 diabetes and cardio-vascular disease, which is determined by genetic polymorphisms in PAI-1 gene and environmental factors. The aim of the study was to examine the determinants of plasma PAI-1 Ag level among type 2 diabetes patients. Methods: 491 Tunisian type 2 diabetes patients had clinical evaluation (weight, high, BMI, Waist Circumference), laboratory investigations including FBG Hb1Ac, cholesterol, triglyceride;HDL-cholesterol was done;plasma PAI-1 antigen level was done with ELISA;−675 4G/5G and −844 G/A polymorphisms of PAI-1 gene was done by PCR-ASA and PCR-RFLP respectively. Results: The mean age for our patients was 58.3 ± 10.5 years;sex-ratio = 0.92;mean PAI-1 level was 34.6 ± 21.3 ng/ml. We didn’t find correlation between PAI-1 level and BMI, but we have found significant correlation between PAI-1 and waist circumference (p = 0.032), most enhanced in men (P = 0.002), T2D patients who have FBG > 11 mmol/l had PAI-1 Ag level higher than those who have FBG P = 0.034), but no difference found between T2D with high Hb1Ac > 8% and those with Hb1Ac < 8%, significant correlation was seen between PAI-1 level and LDL-cholesterol (P = 0.05), high correlation between PAI-1 Ag level and −675 4G/5G polymorphism genotype was seen, 4G/4G carriers had the highest PAI-1 level, 4G/5G had intermediary level and 5G/5G had the lowest level (P −844G/A polymorphism genotypes. Using multiple variable linear regression analysis, the independent factor associated with plasma PAI-1 level was −675 4G/5G polymorphism (regression coefficient β = 4.6, P Conclusion: the present study identifies −675 4G/5G not −844 G/A polymorphism of PAI gene as the principal determinant of plasma PAI-1 level in Tunisian T2D patients, the android fat distribution, dyslipidemia and hyperglycemia play a modest role in this variation.
