Delorme's operation plus sphincteroplasty for complete rectal prolapse associated with traumatic fecal incontinence

Rectal prolapse associated with traumatic fecal incontinence is a rare clinical combination. This study was designed to assess Delorme＇s operation with sphincteroplasty as a surgical management of this combination in terms of recurrence and improvement of fecal incontinence. In this prospective study, we enrolled patients suffering from short, full-thickness rectal prolapse associated with traumatic fecal incontinence who had been admitted to Alexandria main University Hospital during the period of may 2010-January 2013. Preoperative data including cause of trauma, duration of symptoms, results of anal manometry, and degree of fecal incontinence using Wexner score were collected from all patients. Delorme＇s procedure with overlap sphincteroplasty was done in all patients. Recurrence of prolapse and improvement of fecal incontinence were assessed after 1, 3, 6 and 12 months. The study included 13 patients aged （32±8.7） years, 9 females and 4 males. Cause of sphincteric injury included previous anal surgery in 7 patients and normal labor in 6 patients. Duration between sphinctefic injury and operation was （8.08±2.47） months. Preoperative Wexner＇s mean score was 16.07±3.4. Early postoperative complications included superficial wound infection （69.2%）, minor wound dehiscence （61.5%）, and postoperative bleeding （7.6%）. Recurrence was detected in 1 patient at 6 month follow-up. Wexner＇s score showed significant improvement for all patients after 6 months （4.00±2.04）. In conclusion, combination of Delorme＇s procedure and sphincteroplasty for treatment of patients with short complete rectal prolapse associated with traumatic fecal incontinence is a safe, effective surgical management with satisfactory results regarding anatomical and functional outcomes.

Comparative study between three carbonaceous nanoblades and nanodarts for antimicrobial applications

The design of nanostructured materials occupies a privileged position in the development and management of affordable and effective technology in the antibacterial sector.Here,we discuss the antimicrobial properties of three carbonaceous nanoblades and nanodarts materials of graphene oxide(GO),reduced graphene oxide(RGO),and single-wall carbon nanotubes(SWCNTs)that have a mechano-bactericidal effect,and the ability to piercing or slicing bacterial membranes.To demonstrate the significance of size,morphology and composition on the antibacterial activity mechanism,the designed nanomaterials have been characterized.The minimum inhibitory concentration(mIC),standard agar well diffusion,and transmission electron microscopy were utilized to evaluate the antibacterial activity of GO,RGO,and SWCNTs.Based on the evidence obtained,the three carbonaceous materials exhibit activity against all microbial strains tested by completely encapsulating bacterial cells and causing morphological disruption by degrading the microbial cell membrane in the order of RGO>GO>SWCNTs.Because of the external cell wall structure and outer membrane proteins,the synthesized carbonaceous nanomaterials exhibited higher antibacterial activity against Gram-positive bacterial strains than Gram-negative and fungal microorganisms.RGO had the lowest mIC values(0.062,0.125,and 0.25 mg/mL against B.subtilis,S.aureus,and E.coli,respectively),as well as minimum fungal concentrations(0.5 mg/mL for both A.fumigatus and C.albicans).At 12 hr,the cell viability values against tested microbial strains were completely suppressed.Cell lysis and death occurred as a result of severe membrane damage caused by microorganisms perched on RGO nanoblades.Our work gives an insight into the design of effective graphene-based antimicrobial materials for water treatment and remediation.

Robust malicious Executable Detection Using Host-Based machine Learning Classifier

The continuous development of cyberattacks is threatening digital transformation endeavors worldwide and leadsto wide losses for various organizations. These dangers have proven that signature-based approaches are insufficientto prevent emerging and polymorphic attacks. Therefore, this paper is proposing a Robust malicious ExecutableDetection (RmED) using Host-based machine Learning Classifier to discover malicious Portable Executable (PE)files in hosts using Windows operating systems through collecting PE headers and applying machine learningmechanisms to detect unknown infected files. The authors have collected a novel reliable dataset containing 116,031benign files and 179,071 malware samples from diverse sources to ensure the efficiency of RmED approach.The most effective PE headers that can highly differentiate between benign and malware files were selected totrain the model on 15 PE features to speed up the classification process and achieve real-time detection formalicious executables. The evaluation results showed that RmED succeeded in shrinking the classification timeto 91 milliseconds for each file while reaching an accuracy of 98.42% with a false positive rate equal to 1.58. Inconclusion, this paper contributes to the field of cybersecurity by presenting a comprehensive framework thatleverages Artificial Intelligence (AI) methods to proactively detect and prevent cyber-attacks.

Condition monitoring of the insulating system within power transformers has a massive importance according to the electrical utilities.Dissolved gas analysis(DGA)is frequently used for this purpose.However,DGA lacks the necessary level of accuracy to identify all equipment faults,particularly in their initial stages of degradation.Also,it does not have the capability for real-time monitoring and relies on manual sampling and laboratory testing,causing potential delays in fault identification.Additionally,the interpretation of DGA data necessitates specialised expertise,which may pose difficulties for smaller entities that have limited access to resources.Therefore,the contribution of this research is to use infrared spectroscopy measurements as a new effective technique substituting the DGA method for fault diagnosis in insulating oil.The inception faults that were considered in this study were the electrical fault(discharges of high energy)and the thermal fault(300°Cmperaturemulation processes inside the laboratory for both types of inception faults.Subsequently,six samples of pure paraffinic mineral oil were taken to be degraded in the laboratory.Following that,all of them besides another sample that were not subjected to any kind of faults were taken to be examined by Fourier transform infrared(FTIR)spectroscopy to obtain an overview of the oil's behaviour in each fault case.After that,the FTIR analysis was initially verified utilising the DGA method.Then,for further affirmation,the dielectric dissipation factor(DDF)for all samples was measured.In the final analysis,the verification tests provide experimental evidence about the outperformance of this new optical technique in detecting the transformer's inception faults in addition to proving its potential for being a superior alternative to the well-known traditional diagnostic techniques.

Developing a biomaterial for wound healing applications is still a challenge.Herein,dysprosium oxide（Dy2O3）,aluminum oxide（Al2O3）,and graphene oxide（GO） were embedded in cast films based on polycaprolactone（PCL） to be examined for wound dressing usage.Different techniques were used to characterize the fabricated films including X-ray diffraction（XRD）,Fourier transform infrared spectroscopy（FTIR）,Raman,and scanning electron microscopy（SEm）,besides their biological activity.The thermogravimetric analysis（TGA） exhibits high thermal stability as the scaffold weight decreases slightly to 98.6% after raising the temperature from room temperature to 280℃.The cell viability was investigated and it is shown that the viable cells grow up to approximately 93% at 75 μg/mL.meanwhile,the cell attachment shows excellent behavior as the cells attach on most of the surface of the modified PCL which shows high biocompatibility.

BACKGROUND Liver transplantation(LT)is a life-saving procedure for patients with end-stage liver disease and has become the standard and most effective treatment method for these patients.There are many indications for LT that vary between countries and settings.The outcome of LT depends on the available facilities and surgical expertise,as well as the types of liver graft donors available.AIm To assess the clinical characteristics of patients from Bahrain who underwent LT overseas,and analyze factors affecting their survival.mETHODS In this retrospective cohort study,we reviewed the medical records and overseas committee registry information of all pediatric and adult patients who were sent overseas to undergo LT by the Pediatric and medical Departments of Salmaniya medical Complex and Bahrain Defence Force Hospital via the Overseas Treatment Office,ministry of Health,Kingdom of Bahrain,between 1997 and 2023.Demographic data,LT indication,donor-recipient relationship,overseas LT center,graft type,post-LT medications,and LT complications,were collected.Outcomes measured included the overall and 5-year LT survival rate.Fisher’s exact,Pearsonχ2,and mann-Whitney U tests were used to compare the pediatric and the adults’group in terms of clinical characteristics,donor-recipient relationship,medication,complications,and outcome.Survival analysis was estimated via the Kaplan-meier’s method.Univariate and multivariate analyses were used to detect predictors of survival.RESULTS Of the 208 eligible patients,170(81.7%)were sent overseas to undergo LT while 38(18.3%)remained on the waiting list.Of the 170 patients,167(80.3%)underwent LT and were included in the study.The majority of the patients were Bahraini(91.0%),and most were males(57.5%).One-hundred-and-twenty(71.8%)were adults and 47(28.3%)were children.The median age at transplant was 50.0[interquartile range(IQR):14.9–58.4]years.The main indication for pediatric LT was biliary atresia(31.9%),while that of adult LT was hepatitis C-related cirrhosis(35.0%).Six(3.6%)pa

Aim: To investigate depression, anxiety, PTSD, and insomnia levels among Sudanese citizens after a year has passed since the start of Armed Forces conflict in Sudan. methods: An online survey was distributed. It is composed of five parts, covering the following areas: 1) sociodemographic data;2) depression assessment;3) generalized anxiety disorder assessment;4) post-traumatic stress disorder assessment;and 5) insomnia assessment. Statistical Package for Social Sciences version 27 was used for data analysis;frequency and percentage were used to describe the qualitative variables. Spearman’s correlation analysis and Chi-square test were used for correlation and association analysis;a P-value equal to or less than 0.05 was considered statistically significant. Results: The study included 283 participants, primarily female (76.0%), with a mean age of 35.26 ± 6.96 years. The majority of participants were married (50.5%) and had a university level of education (50.2%). At the start of the war, 70.3% of participants were inside the war zones, and at the time of data collection, 73.5% were outside Sudan. The length of stay in the war zone ranged from 1 to 300 days, with a mean of 48.59 ± 70.284 days. Additionally, 63.3% of participants stated that they did not receive any form of mental health intervention, and the remaining did not provide an answer. There was a high prevalence of moderate-severe depression (50.5%), moderate anxiety (35.3%), PTSD (56.5%), and sub-threshold insomnia (53.4%) among the participants. Correlation analyses revealed associations between demographic factors such as sex, age, marital status, and education and mental health issues. Notably, female participants showed a higher prevalence of PTSD (53.0%) compared to male participants (67.6%). Conclusions: There is a substantial impact of war-related trauma on mental health, revealing a high prevalence of moderate-severe depression, moderate anxiety, PTSD, and sub-threshold insomnia among the participants. Trauma to the solid abdominal organs:The missed dark box of colonoscopy

Colonoscopy is an integral part of the lower bowel care and is generally considered a potentially safe diagnostic and therapeutic procedure performed as a daycare outpatient procedure.Colonoscopy is associated with different complications that are not limited to adverse events related to the bowel preparation solutions used,the sedatives used,but to the procedure related as well including bleeding and perforation.Injuries to the extra-luminal abdominal organs during colonoscopy are uncommon,however,serious complications related to the procedure have been reported infrequently in the literature.Life threatening injuries to the spleen,liver,pancreas,mesentery,and urinary bladder have been reported as early as in mid-1970s.These injuries should not be overlooked by clinicians and endoscopists.Steadily increasing abdominal pain,abdominal distension,and hemodynamic instability in absence of rectal bleeding should raise the possibility of severe organ injury.Splenic and hepatic injury following colonoscopy are usually serious and may be life threatening.Although conservative management may help,yet they usually need interventional radiology or surgical intervention.Acute pancreatitis following colonoscopy is usually mild and is mostly managed conservatively.The mechanism of abdominal organ injuries during colonoscopy is not fully understood,however many risk factors have been identified,which can be classified as-organ related,procedure related,and local abdominal factors.Difficult colonoscopy and prior intra-abdominal adhesions are probably the most relevant risk factors for these injuries.Left lateral position,avoidance of looping and excessive force during the procedure would probably reduce the risk of such injuries.

Ballistic design and testing of a composite armour reinforced by CNTs suitable for armoured vehicles

This paper is investigating the use of composite armour reinforced by nanomaterials, for the protection of light armoured(LAV) and medium armoured military vehicles(mAV), and the interaction between the composite materials and high-performance ballistic projectiles. Four armour materials, consisted of front hybrid fibre reinforced polymer cover layer, ceramic strike-face, fibre reinforced polymer intermediate layer and the metal matrix composite reinforced backplate, were manufactured and assembled by adhesive technology. The proposed laminated protection system is suitable for armoured ground vehicles;however, it could be used as armour on ground, air and naval platforms. The design of the protection system, including material selection and thickness, was elaborated depending on the performance requirements of Level 4 + STANAG 4569 military standard(projectile 14.5 mm × 114 mm API B32) and especially on a design philosophy which is analysed with the specifications. The backplate of this new composite is a hybrid material of metal matrix Composite(mmC) reinforced with carbon nanotubes(CNTs), manufactured with the use of powder metallurgy technique. The composite backplate material was morphologically, mechanically and chemically analysed. Results show that all plates are presenting high mechanical properties and ballistic characteristics, compared to commonly used armour plates. Real military ballistic tests according to AEP-STANAG 4569 were carried out for the total composite armour systems. After the ballistic tests, AA2024-CNT3 showed the best protection results, compared with the other plates(AA2024-CNT1 and AA2024-CNT2), with the projectile being unable to fully penetrate the composite plate.

Geochemistry of island arc assemblage in the Eastern Desert of Egypt and the role of Pan-African magmatism in crustal growth of the Arabian–Nubian Shield:A review

Neoproterozoic island arc assemblage of the Arabian–Nubian Shield(ANS)in the Eastern Desert(ED)of Egypt comprises juvenile suites of metavolcanics(mV),large amounts of meta-sedimentary rocks(mS),and voluminous metagabbros-diorites(mGD)and syn-tectonic intrusions of older granitoids(OG).We report here the updates of these four rock units in terms of classification,distribution,chemical characteristics,geodynamic evolution,metamorphism,and ages.In addition,we discuss these integrated data to elucidate a reasonable and reliable model for crustal evolution in the ANS.The main features of these rock units indicate their relation to each other and the geodynamic environment dominated by early immature oceanic island arcs to primitive continental arcs.Integrated information of the island arc metavolcanic and plutonic rocks(gabbros,diorites,tonalites,and granodiorites)furnish evidence of the genetic relationships.These include proximity and a coeval nature in the field;all protolith magmas are subalkaline in nature following calc-alkaline series with minor tholeiitic affinities;common geochemical signature of the arc rocks and subduction-related magmatism;their similar enrichment in LREEs;and similar major element compositions with mafic melts derived from metasomatized mantle wedge.The volcano-sedimentary and the OG rocks underwent multiphase deformation events whereas the mGD complexes deformed slightly.Based on the magmatic,sedimentological,and metamorphic evolutions constrained by geochronological data as well as the progressive evolutionary trend from extensional to compressional regimes,a possible gradual decrease in the subducted slab dip angle is the most infl uential in any geodynamic model for arc assemblage in the ED of Egypt.
