BACKGROUND The concept of macroscopic on-site evaluation(MOSE)was introduced in 2015 when the endoscopist observed better diagnostic yield when the macroscopically visible core on MOSE was superior to 4 *** studies suggest that MOSE by the endoscopist may be an excellent alternative to rapid on-site evaluation,and some classi-fications have been *** studies have assessed the adequacy of histologic cores in MOSE during endoscopic ultrasound-guided fine-needle aspiration/biopsy(EUS-FNA/FNB).AIM To evaluate the performance of MOSE during EUS-FNA/*** This multicentric prospective study was conducted in 16 centers in 3 countries(Egypt,Iraq,and Morocco)and included 1108 patients with pancreatic,biliary,or gastrointestinal pathology who were referred for EUS *** prospectively analyzed the MOSE in 1008 patients with available histopathological reports according to 2 classifications to determine the adequacy of the histological core *** management and analysis were performed using a Statistical Package for Social Sciences(SPSS)version *** A total of 1074 solid lesions were biopsied in 1008 patients with available cytopathological *** age was 59 years,and 509 patients(50.5%)were *** mean lesion size was 38 *** most frequently utilized needles were FNB-Franseen(74.5%)and 22 G(93.4%),with a median of 2 *** to 2 classifications,618 non-bloody cores(61.3%)and 964 good samples(95.6%)were adequate for histological *** overall diagnostic yield of cytopathology was 95.5%.The cytological examination confirmed the diagnosis of malignancy in 861 patients(85.4%),while 45 samples(4.5%)were ***-procedural adverse events occurred in 33 patients(3.3%).Statistical analysis showed a difference between needle types(P=0.035)with a high sensitivity of FNB(97%).The analysis of the relationship between the MOSE-score and the final diagnosis showed a significant difference between the different

Beryl is the commercial source of beryllium and several varieties of it are valued as a *** contribute to understanding the mechanism of beryl formation,we carried out detailed geological,petrographical,and geochemical investigations on beryl mineralization occurrences in the Zabara-Wadi El Gemal(Z-WG)*** region is an NW-SE trending tract that includes six berylhosting *** green gem variety of beryl(emerald)is restricted to phlogopite schist,pegmatite,and quartz *** hexagonal emerald crystals are well-developed in phlogopite schist and *** gem variety emerald examined is sodic and *** contains high concentrations of chromophore transition elements ordering Cr(up to 1511 ppm)>V(up to 242 ppm)>Sc(up to 245 ppm),giving rise to its vivid green color,refl ecting mafic-ultramafic source *** the investigated emeralds,the Sikait area contains the highest BeO(av.10.76wt.%)*** compositional variability of emeralds is most likely attributed to the contribution from the host *** is revealed by the examined emerald mineralization,for instance;the Abu Rusheid area(one of the best areas exposing rare metal-bearing granitoids)possesses the highest average of trace and REEs *** contrast,Um Kabu emerald has the highest contents of Co(av.20 ppm),Ni(av.299 ppm),MgO(av.8.2wt.%),Fe_(2)O_(3)(av.3.12wt.%),and CaO(avg.3.4wt.%)relative to other areas,which may be linked to contribution of ultramafic rocks exposed *** proposed mechanism we suggest for emerald genesis is metasomatic interaction between felsic intrusions,that are enriched with K,Na,Be,Li,and B,with mafic-ultramafic rocks that are enriched in Cr,V,Mg,Fe,and *** interaction is marked by the formation of phlogopite schist,the growth of emerald crystals,and desilicated pegmatite.

Removal of Alcyon II, CGR, MeV <sup>60</sup>Co Teletherapy Head and Evaluation of Exposure Dose

Teletherapy units containing 60Co source are used in Egypt and worldwide for treatment of cancer diseases. One of the widely used in Egypt is Alcyon CGR MeV. Different radiotherapy facilities need to unload this 60Co unit after being disused. The head containing the source was removed and put into approved package for transportation, after the source kept in safe position. This study describes the planning and removing process of the disused sources and, transports it into its head. The personal doses measured during the process. This work pre-estimates the doses received to workers during the removing process. The workers received doses of approximately 55 μSv during the whole process. This study shows that the original lead head can be used as the source container of this 60Co unit but not as a long term storage option. The 60Co machine was smoothly dismantled and transported by trained workers and driver, using planned and controlled radiation protection, followed by ALARA (as low as reasonably achievable) rule.

Bipartite Laparoscopic Cholecystectomy: New Technique for Avoiding Bile Duct Injury in Difficult Cases

The incidence of bile duct injury in laparoscopic cholecystectomy (LC) is still two times greater compared to classic open surgery. This study offers new procedure to avoid this complication during LC. The gall bladder was divided into two parts above the Hartmann pouch and all contents were aspirated. Then, the distal part was dissected for short distance. The proximal part was dissected dome down until reaching to cystic duct which was tied or clipped and cut. J-vac drain was put in peritoneal cavity. Between September 2012 and October 2013, overall 77 patients (53 females and 24 males) with mean age of 49 years (between 23 and 67 years) underwent bipartite laparoscopic cholecystectomy. The mean operative time was 60 minutes (between 40 and 90 minutes). The dissection of both parts of gall bladder was safe and easy as close as possible from its wall. No biliary tract injuries were recorded during or after procedure and also at follow-up period (20 months). Bipartite laparoscopic cholecystectomy is safe, easy to do, and can avoid all complications especially bile duct injuries in difficult cases.

Laparoscopic Inguinal Hernia Repair with Closure of Hernial Defect and Central Mesh Fixation Using Glubran 2

Background: Laparoscopic tension-free repair of inguinal hernia was presented in 1990s, promising less pain and short recovery period, but carrying the risk mesh bulging and migration. Objective: We have presented our technique in which central zone of mesh is fixed only after closure of hernial defect. Patients and Methods: This study included 27 males patients (14 indirect inguinal hernias, 9 direct inguinal hernias, 4 both direct and indirect inguinal hernias on the same side). These cases are undergoing tension-free mesh repair after closure of hernial defect, and the mesh is fixed only at its central zone using Gulbran 2, between April 2011 and March 2013. The follow-up period ranged from 6 to 30 months. The intra and postoperative complications were recorded. Results: Mean hospital stay was 1 day. The age of this group of patients ranged from 23 to 63 years (mean, 47 years). The operative time ranged from 30 to 100 minutes (mean, 45 minutes). The intraoperative complications were in form of mild bleeding in 7 patients (25.9%) during hernial sac dissection. Postoperative complications were mild inguinal pain in 4 patients (14.8%) for three weeks. Mild hydrocele in 3 patients (11%) was recorded. No recurrence or bulging at hernia site was noticed during the period of follow-up. Conclusion: Laparoscopic inguinal hernia repair with central mesh fixation after closure of hernial defect is effective, easy and free of complications.
