Pull Breech out versus Push i.pacted Head up i. Emergency Cesarean Secti.n: A Comparati.e Study

Objecti.e: To compare maternal and fetal outcome associ.ted wi.h two methods Reverse breech extracti.n versus Head pushi.g to deli.er the i.pacted fetal head i. advanced labor requi.i.g emergency Cesarean secti.n. Method: A prospecti.e comparati.e study was conducted on 80 pregnant women at term wi.h cephali. presentati.n i. advanced labour, requi.i.g emergency Cesarean Secti.n. Reverse breech extracti.n techni.ue (pull method) was used i. 40 cases (group i. and pushi.g the head up through the vagi.a (“push” method) was tri.d i. (group i.) 40 cases. The maternal outcome was assessed by extensi.n of the uteri.e i.ci.i.n, bladder i.jury, i.tra and postoperati.e blood transfusi.n, Postpartum hemorrhage, wound i.fecti.n and durati.n of hospi.al stay. Fetal outcome was Apgar score and admi.si.n to neonatal i.tensi.e care uni.. Results: Extensi.n of the uteri.e i.ci.i.n was si.ni.i.antly lower i. women undergoi.g reverse breech extracti.n compared to cephali. deli.ery (20% versus 50%;p = 0.001). The mean operati.e ti.e (pull group) was lower than that i. the (push group) 59.7 ± 4.2, versus 75.2 ± 6.1 p = 0.001 and blood loss was si.ni.i.antly lower i. the (pull group) than that i. the (push group) 878 ± 67 ml, versus 1321 ± 57 ml, p = 0.001. No si.ni.i.ant di.ference between groups regardi.g maternal and neonatal outcome. Conclusi.n: Reverse breech extracti.n (pull) i. safer than pushi.g head up through vagi.a (push) for deli.ery of a deeply i.pacted fetal head i. advanced labour sensi.i.i.g emergency Cesarean Secti.n and i. associ.ted wi.h the least maternal compli.ati.ns.

A novel reducti.n techni.ue has been developed to synthesi.e nano-si.ed tungsten heavy alloys powders and compared wi.h the same powders processed by mechani.al alloyi.g techni.ue. i. the fi.st method, nano-si.ed tungsten heavy alloys powders have been obtai.ed by reducti.n of precursors obtai.ed by spray dryi.g of several appropri.te aqueous soluti.ns, whi.h were made from salts contai.i.g tungsten, cobalt, and ni.kel. By adjusti.g the stoi.hi.metry of the component of the soluti.ns, i. i. possi.le to obtai. the desi.ed chemi.al composi.i.n of the tungsten heavy alloys powders. i. the second method, hi.hly pure elemental powders of tungsten heavy alloys have been mechani.ally alloyed i. a tumbler ball mi.l for di.ferent mi.li.g ti.e. The i.vesti.ated tungsten heavy alloy powders wi.h the composi.i.n (95%W-3.5%Ni.1.5%Fe), (93%W-4.5%Ni.1.0%Fe-1.5%Co), and (90%W-6%Ni.4%Cu) have been prepared usi.g both methods. The prepared powders have been compacted at 70 bar (200 MPa) and si.tered i. vacuum furnace at 1400℃. Vacuum si.teri.g was carri.d out to achi.ve full densi.i.ati.n of the produced tungsten heavy alloys. The i.vesti.ated materi.ls were goi.g to be evaluated the physi.al and mechani.al properti.s of the si.tered parts such as densi.y;electri.al conducti.i.y, hardness, and transverse rupture strength. The results reveal that, the grai. si.e of alloys fabri.ated by chemi.al reducti.n techni.ue (53.1 - 63.8 nm) i. smaller than that fabri.ated by mechani.al alloyi.g techni.ue (56.4 - 71.4 nm).

Objecti.es: To study the ambulatory measured blood pressure (ABPM) profi.e i. normotensi.e pati.nts wi.h mi.d cogni.i.e i.pai.ment (MCi.. Pati.nts and Methods: The study was desi.ned as a case control study i.cludi.g 50male pati.nts wi.h mi.d cogni.i.e i.pai.ment i. the age group of 30 - 50 years old. The control group i.cluded 30 volunteers wi.h no cogni.i.e i.pai.ment and i. the same age group (30 - 50 years old) and same gender. Mi.i.mental estate exami.ati.n, offi.e and ABP moni.ori.g (ABPM) and brai. MRi.scans were done for cases and controls. Results: Thi.ty pati.nts (60%) wi.h MCi.revealed a non-di.per blood pressure pattern. Sleepi.g systoli. blood pressure and sleepi.g systoli. load were si.ni.i.antly hi.her i. pati.nts wi.h MCi.than i. normal volunteers (p = 0.01). MRi.brai. showed more whi.e matter lesi.ns (WMLs) i. pati.nts wi.h MCi.than i. normal volunteers;however, thi. di.n’t reach si.ni.i.ance level (p = 0.056). Conclusi.n: MCi.i. normotensi.e young adult pati.nts could reflect an abnormal ci.cadi.n blood pressure rhythm. Ambulatory blood pressure moni.ori.g could be an essenti.l i.vesti.ati.n i. young adult MCi.pati.nts.

Objecti.e: The ai. of thi. work i. to develop and vali.ate a rapi., si.ple and sensi.i.e LC/MS/MS assay method for the determi.ati.n of Clopi.ogrel carboxyli. aci. i. human plasma usi.g Clopi.ogrel-D4-carboxyli. aci. as i.ternal standard. Methods: Analytes was extracted from 200 μl of plasma by a si.ple li.ui.-li.ui. extracti.n usi.g di.thyl ether – n-hexane (80:20, v/v). The chromatographi. separati.ns were achi.ved on a C18 column usi.g Methanol, de-i.ni.ed water and formi. aci. as a mobi.e phase at flow rate of 0.5 ml/mi.ute. Analysi. was moni.ored by multi.le reacti.ns moni.ori.g mode based on m/z transi.i.n of 308.10→113 for Clopi.ogrel carboxyli. aci. and 312.10→129 for i.ternal standard. Result: The method had a total run ti.e of about 4 mi.utes. The lower li.i. of quanti.i.ati.n (LLOQ) was 25 ng/ml showi.g good li.eari.y over the worki.g range of 25 – 3000 ng/ml (r ≥ 0.999). The i.tra- and i.ter day accuraci.s were 90% - 98% and 92.138% - 96.889% respecti.ely (devi.ti.n wi.hi. acceptable range ≤ 10%).Conclusi.n: i. was shown that thi. method i. sui.able for pharmacoki.eti. study followi.g oral admi.i.trati.n of Clopi.ogrel and can be successfully appli.d to the therapeuti. drug moni.ori.g of Clopi.ogrel i. Clopi.ogrel-treated pati.nts.

Starti.g from 4,6-di.ethyl-2-oxo-(1H)-3-pyri.i.ecarboni.ri.e 1 and 3-ami.opyrazolopyri.i.e 4, a seri.s of cyanopyri. di.e deri.ati.es 3a-i. Schi.f bases 5a-f, urea and thi.urea deri.ati.es 6a-b, ami.e deri.ati.es 7a-h, pyri.opyra- zolopy-ri.i.i.e 8a-b and pyri.opyrazolotri.zi.e 10a-b were synthesi.ed. Acti.i.i.s of eleven representati.e compounds were evaluated agai.st A-549 (lung), HEPG2 (li.er) and HCT-116 (colon) cancer cell li.es. The fi.di.gs revealed that some of the synthesi.ed compounds showed remarkable anti.ancer acti.i.i.s, especi.lly 8b whi.h di.played the hi.hest acti.i.y among the tested compounds wi.h i.50 equal to 2.9, 2.6 and 2.3 μmol. i. addi.i.n to synthesi. and bi.logi.al acti.i.i.s, we present di.cussi.n about the rati.nale of the desi.n and acti.i.y of the potent compound 8b usi.g struc- ture-based modeli.g tools.

The prevalence of enterotoxi.eni. Staphylococcus aureus was i.vesti.ated among 200 parti.i.ants worki.g i. three di.ferent food processi.g plants i. Egypt. Usi.g ski. swabs, 75 (38%) of the 200 tested persons were posi.i.e for the presence of S. aureus. Of the S. aureus posi.i.e persons, 28 (14%) harboured S. aureus produced staphylococcal enterotoxi.s. The serotypes of these enterotoxi.s were enterotoxi. A (68%), enterotoxi. B (36%), enterotoxi. C (46%) and enterotoxi. D (18%). Some of these i.olates produced more than one type of enterotoxi.s namely AB, AC, BC, BD, ABC and ACD. Analysi. of ri.k factors i.pli.ated i. ski. carri.ge of S. aureus as age, gender, mari.al status, educati.n, durati.n i. employment, frequency and method of hand wash and i.ci.ence of chroni. ski. i.fecti.n revealed i.si.ni.i.ant associ.ti.n wi.h staphylococcal ski. carri.ge. The obtai.ed results put forth the ri.k of food contracti.g contami.ati.n wi.h enterotoxi.eni. strai.s of S. aureus owi.g to ski. coloni.ati.n of S. aureus among food handlers.

The constructi.n of gas and oi. plants i.clude many i.ems such as steel structures, equi.ments, pi.i.g, tanks and vessels at di.ferent condi.i.ns such as offshore, onshore, above and underground shall be protected agai.st corrosi.n by coati.g. The protecti.e value of a coati.g system i. proporti.nal to the surface preparati.n;the objecti.es of the surface preparati.n are the surface cleanli.ess and the surface profi.e of the substrate. The surface preparati.n i. usually achi.ved by blast cleani.g usi.g di.ferent types of abrasi.e. Due to i.s hi.h hardness and low cost, si.i.a sand has been used extensi.ely for i.pact abrasi.n. i. i., however, very bri.tle wi.h most grai. di.i.tegrati.g on i.pact, causi.g excessi.e levels of si.i.eous dust whi.h i. a major health hazard and causes seri.us envi.onmental polluti.n. Consequently, i. has been banned as i.pact abrasi.e materi.l i. most countri.s around the world. Consi.erable research has been done i. the last twenty years to fi.d an i.pact abrasi.e materi.l as hard as si.i.a and that respects. So i. was i.portant to search for sand substi.uti.n among the local abundant mi.erals i. Egypt. Waste product obtai.ed from i.meni.e ore extracted from the mi.e was found to be the most sui.able alternati.e abrasi.e. Thi. work shows the abrasi.e types, the occurrence of the i.meni.e ore. The chemi.al properti.s, physi.al properti.s and mi.roscopi. exami.ati.n of the waste were i.vesti.ated. The obtai.ed result i.di.ates that, the waste product obtai.ed from i.meni.e ore can be uti.i.ed for abrasi.n as sand substi.uti.n.

The physi.o-chemi.al and bi.logi.al （algal and bacteri.l） quali.y of tap water i. Tanta Ci.y were eluci.ated. Samples were collected duri.g December 2011, coveri.g all the water plants and the areal extensi.n of the pi.e li.e network i. Tanta. Total di.solved soli.s content classi.i.s water ori.i. i.to surface water, shallow and deep groundwater. Fe, Mn and hardness i. some samples of groundwater ori.i. are obvi.usly exceedi.g the WHO （World Health Organi.ati.n） li.i.. Mostly, green algae are found （31 speci.s）, followed by di.toms （19 speci.s）, then blue-green algae （13 speci.s）. CCA （Canoni.al Correspondi.g Analysi.） i.di.ates that green algae are strongly correlated wi.h pH, NH4, alkali.i.y, Mn and Si. di.toms wi.h EC （electri. conducti.i.y）, turbi.i.y, Zn and Si.and blue green algae wi.h EC and alkali.i.y. 50% of samples of surface ori.i., 80% of samples of both deep and shallow groundwater ori.i.s crossed WHO and Egypti.n gui.eli.e i. thei. content of mean TCC （Total Coli.orm Count）. CCA esti.ates strong correlati.ns of TCC wi.h temperature, NH4 and PO4; E. coli.wi.h EC; heterotrophi. bacteri. wi.h turbi.i.y and NO3; Salmonella sp. wi.h Si.and SO4 and Pseudomonas aerugi.osa wi.h Mn. Fi.ally, the water plants treatment may be not effecti.e and pi.es serve as a reservoi. for pathogeni. mi.roorgani.ms.
