Degree of Hyper-Dynamic Circulation Correlates with the Severity of Liver Disease and Predicts Poor Outcome in Spontaneous Bacterial Peritonitis Patients in Intensive Care Unit

Background: Circulatory dysfunction is known in spontaneous bacterial peritonitis (SBP) patients. We aimed to determine whether degree of hyper-dynamic circulation is significantly correlated with severity of liver disease and poor outcome in these patients or not. Methods: 61 patients diagnosed to have SBP were enrolled. In addition to routine laboratory investigation, Child Pugh and APACHE II scores were calculated for all patients. Degree of renal impairment was defined using Acute Kidney Injury Network (AKIN) criteria. Stroke volume (SV) was measured by M mode and 2-dimensional Doppler echocardiography. Cardiac output (CO) and Systemic vascular resistance (SVR) were also calculated. All data were statistically analyzed. Results: SV, CO, SVR were significantly correlated with Child Pugh score, p value 0.001, respectively. In 46 (75.4%) patients who had AKI, degree of kidney injury using AKIN criteria is significantly correlated with SVR, SV, and CO. Inpatient mortality occurred in 10 (16.4%) patients. Non-survivors had lower SVR and higher SV and CO compared to survivors (1805 ± 105 dyn·sec/cm2, 80.3 ± 11.2 ml, and 6.7 ± 1.1 L/min vs 1936 ± 504, 67.6 ± 12.7, and 5.7 ± 0.9, p value 0.029, 0.005, 0.007 respectively). Using the Receiver Operating Characteristics (ROC) curve, the cut-off value of CO that predicts mortality was 5.89 L/min with sensitivity and specificity measuring 80%, 70.6% respectively. The area under the curve (AUC) is 0.767 with 95% confidence interval (CI) is 0.566 to 0.968, p value 0.008. At this value, the odds ratio (OR) is 7.33 with p value 0.012. Conclusion: Degree of hyper-dynamic circulation is significantly correlated with the severity of liver disease and predicts poor outcome in SBP patients.

The conversion of the cartesian coordinates of a point to its geodetic equivalent coordinates in reference to the geodetic ellipsoid is one of the main challenges in *** ellipse equation in the meridian plane significantly influences the value of the geodetic *** research analyzes this influence and how it can contribute to their *** study investigates the mathematical relation between them and presents an exact formula relating to the geodetic height and the ellipse *** addition,a heuristic formula for the relation between the geodetic height and the ellipse equation is proposed,which is independent of the geodetic latitude and has a relative accuracy better than 99.9 %.The calculation is stable,and the cost is low.

Antioxidant Effect of Sepia Ink Extract on Extrahepatic Cholestasis Induced by Bile Duct Ligation in Rats

Objective The aim of our study was to assess the complications of hepatic fibrosis associated with bile duct ligation and the potential curative role of sepia ink extract in hepatic damage induced by bile duct ligation. Methods Rattus norvegicus rats were divided into 3 groups: Sham-operated group, model rats that underwent common bile duct ligation (BDL), and BDL rats treated orally with sepia ink extract (200 mg/kg body weight) for 7, 14, and 28 d after BDL. Results There was a significant reduction in hepatic enzymes, ALP, GGT, bilirubin levels, and oxidative stress in the BDL group after treatment with sepia ink extract. Collagen deposition reduced after sepia ink extract treatment as compared to BDL groups, suggesting that the liver was repaired. Histopathological examination of liver treated with sepia ink extract showed moderate degeneration in the hepatic architecture and mild degeneration in hepatocytes as compared to BDL groups. Conclusion Sepia ink extract provides a curative effect and an antioxidant capacity on BDL rats and could ameliorate the complications of liver cholestasis.

M2BPGi for assessing liver fibrosis in patients with hepatitis C treated with direct-acting antivirals

BACKGROUND Assessing liver fibrosis is important for predicting the efficacy of direct-acting antivirals(DAAs)and patient ***-invasive techniques to assess liver fibrosis are becoming ***,serum Mac-2 binding protein glycosylation isomer(M2BPGi)was identified as a non-invasive marker of liver *** To investigate the diagnostic accuracy of M2BPGi in assessing liver fibrosis in patients with chronic hepatitis C(CHC)treated with *** From December 2017 to August 2018,80 treatment-naive adult patients with CHC who were eligible for DAAs therapy were consecutively enrolled in this observational cohort *** 12 weeks,65 patients were treated with sofosbuvir/daclatasvir,and 15 patients were treated with sofosbuvir/daclatasvir and a weight-based dose of ribavirin at knowledge and technology association for hepatitis C management clinic,Cairo,*** measured serum M2BPGi levels,PAPAS index,fibrosis-4(FIB-4)score and liver stiffness measurements(LSM)at baseline and 12 weeks after the end of *** M2BPGi levels were measured using enzyme-linked immunosorbent *** All patients achieved sustained virologic response(SVR12)(100%).Serum M2BPGi levels,LSM,FIB-4 score and PAPAS index decreased significantly at SVR12(P<0.05).Serum M2BPGi levels correlated positively with LSM at baseline and SVR12(P<0.001).At baseline,compared with the FIB-4 score and PAPAS index,M2BPGi was the best marker to distinguish patients with grade F4 fibrosis(AUC=0.801,P<0.001),patients with grade F2 from grade F0-1 fibrosis(AUC=0.713,P=0.012),patients with grade F3-4 from grade F0-2 fibrosis(AUC=0.730,P<0.001),and patients with grade F2-4 from grade F0-1 fibrosis(AUC=0.763,P<0.001).At SVR12,M2BPGi had the greatest AUCs for differentiating patients with grade F4 fibrosis(AUC=0.844,P<0.001),patients with grade F3 from grade F0-2 fibrosis(AUC=0.893,P=0.002),patients with grade F3-4 from grade F0-2 fibrosis(AUC=0.891,P<0.001),and patients with grad

With the rapid development in business transactions,especially in recent years,it has become necessary to develop different mechanisms to trace business user records in web server log in an efficient *** business transactions have increased,especially when the user or customer cannot obtain the required *** example,with the spread of the epidemic Coronavirus(COVID-19)throughout the world,there is a dire need to rely more on online business *** order to improve the efficiency and performance of E-business structure,a web server log must be well utilized to have the ability to trace and record infinite user *** paper proposes an event stream mechanism based on formula patterns to enhance business processes and record all user activities in a structured log *** user activity is recorded with a set of tracing parameters that can predict the behavior of the user in business *** experimental results are conducted by applying clustering-based classification algorithms on two different datasets;namely,Online Shoppers Purchasing Intention and Instacart Market Basket *** clustering process is used to group related objects into the same cluster,then the classification process measures the predicted classes of clustered *** experimental results record provable accuracy in predicting user preferences on both datasets.

The present study was to investigate the copepodid species infecting rosy goatfish Parupeneus rubescens,one of the most economically important fishes in the Arabian Gulf.A copepodid species identified from the examined fish specimens belongs to the Taeniacanthidae family and is labeled as Taeniacanthus yamagutii Shiino,1957,mainly depending on its morphological,morphometric,and ultrastructural characteristics,in particular the presence of maxilliped claw with a conspicuous digitiform process at the base,the terminal process of the second maxilla stout,and a setiform element at the tip of each exopod spine of legs *** order to ensure the accurate identification and exact taxonomic characterization of this species,the 28S rRNA gene sequence was *** result revealed that the present copepodid species belong to the Taenicanthidae family and has a close relationship with Taeniacanthus yamagutii(gb|KR048852.1)in the same *** present study demonstrated that the rosy goatfish is a host for Taeniacanthus species,which will be helpful to prevent this parasitic infection.
