We report a case of a patient in remission of acute lymphoblastic leukemia (ALL) with severe positional headaches that required an epidural blood patch (EBP) despite the higher risks of infection and introduction of blast cells to the epidural space. A 43-year-old male with a history of ALL presented with persistent positional headache after multiple intrathecal punctures. Despite initial improvement with medical treatment and bed rest, severe positional headache [consistent with post-dural puncture headache] constantly agonized the patient. EBP was performed after discussion of all of the medical teams involved. Following the procedure, the patient experienced immediate pain relief. EBP is very effective in the management of post-dural puncture headache (PDPH). Still there is risk of introducing infectious and/or malignant cells into the central nervous system. Alternatively, there are agents available that could be employed other than a patient’s own blood.

AIM: To explore the approaches exerted by mesenchymal stem cells (MSCs) to improve Parkinson’s disease (PD) ***: MSCs were harvested from bone marrow of femoral bones of male rats, grown and propagated in culture. Twenty four ovariectomized animals were classified into 3 groups: Group (1) was control, Groups (2) and (3) were subcutaneously administered with rotenone for 14 d after one month of ovariectomy for induction of PD. Then, Group (2) was left untreated, while Group (3) was treated with single intravenous dose of bone marrow derived MSCs (BM-MSCs). SRY gene was assessed by PCR in brain tissue of the female rats. Serum transforming growth factor beta-1 (TGF-β1), monocyte chemoattractant protein-1 (MCP-1) and brain derived neurotrophic factor (BDNF) levels were assayed by ELISA. Brain dopamine DA level was assayed fluorometrically, while brain tyrosine hydroxylase (TH) and nestin gene expression were detected by semi-quantitative real time PCR. Brain survivin expression was determined by immunohistochemical procedure. 2-(2-Oxo-2-(piperidin-1-yl)ethylidene)thiazolidin-4-one (1) was used as a key intermediate for the synthesis of many thiazolo[3,2-a] pyridine derivatives. Thus, 4-thiazolidinine (1) was refluxed with aromatic aldehydes where the corresponding 4-thiazolidinone derivatives (2a, b) were afforded. Cyclization of (2a, b) with malononitrile and aldehydes gave the corresponding thiazolo[3,2-a] pyridines (5a, b). Thiazolo[3,2-a] naphthyridine derivatives (6a, b) were obtained from the reaction of compounds (5a, b) with formic acid. Acylation of (5a, b) with acetic anhydride or benzoyl chloride for 2 h produced the corresponding amide derivatives (7,8). While, upon treatment of (5b) with acetic anhydride for 5 h furnished tricyclic structure (9). Furthermore, compounds (5a, b) were refluxed with either chloroacetonitrile to give thiazolo[3,2-a] pyrrolo [2,3-c] pyridines (10a, b) or with malononitrile to afford the corresponding thiazolo[3,2-a] naphthyridine derivatives (14a, b). In addition, the corresponding thiourea and Pyrazolo [3,4:4",5] thiazolo derivatives (15) and (16) were produced via the reaction of 5 ethyl with ethylisothiocyanate and phenyl hydrazine. Molecular docking simulations into the active site of ALR2 were performed, and showed that, some of the synthesized compounds more suitable inhibitor against ALR2 with farther modification in future.

Pharmacist-Managed Anticoagulation Services for Warfarin Management in Tertiary Hospitals:The Egyptian Experience

Background:Warfarin is widely regarded as the main anticoagulant in lowering the risk of *** study used indicators to compare pharmacist-managed anticoagulation services,using a well-prepared protocol,with physician-managed anticoagulation services・Methods:A retrospective prospective pilot study was conducted to compare patient outcomes before and after transitioning patients to phannacist-managed anticoagulation services,comparing the proportion of those with therapeutic international normalized ratio(INK),subtherapeutic INK,and supratherapeutic INR,as well as their bleeding occurrences as indicators of assuring quality ***:A significant improvement in anticoagulation management was noted in the transition to pharmacist-managed anticoagulation *** proportion of those with subtherapeutic INR decreased from 61.8%to 11.8%(p<0.001),those with supratherapeutic INR decreased from 20.6%to 2.9%(p<0.001),those with therapeutic INR increased from 17.6%to 85.3%(p<0.001),and the occurrence of bleeding decreased from 11.8%to 0.0%,without significant difference in warfarin doses(median from 4 before the transition to 5 after);in addition,the time to reach therapeutic INR decreased from 12-24 weeks to 2-8 weeks after transitioning to pharmacist-managed anticoagulation ***:Pharmacist-managed anticoagulation services are considered safer and more effective than physician-managed anticoagulation services alone in terms of patientsJ adherence and satisfaction,which provide an excellent opportunity for quality assurance care.
