AIM:To examine the influence of ghrelin on the regenerative potential of gastrointestinal(GI)***:Damage to GI epithelium was induced in mice by two intravenous injections of doxorubicin(10 and 6 mg/kg).Some of the doxorubicin-treated mice received a continuous subcutaneous infusion of ghrelin(1.25μg/h)for 10 d via implanted mini-osmotic *** label dividing stem cells in the S-phase of the cell cycle,all mice received a single intraperitoneal injection of 5'-bromo-2'-deoxyuridine(BrdU)one hour before *** stomach along with the duodenum were then removed and processed for histological examination and immunohistochemistry using anti-BrdU ***:The results showed dramatic damage to the GI epithelium 3 d after administration of chemotherapy which began to recover by day *** ghrelintreated mice,attenuation of GI mucosal damage was evident in the tissues examined *** analysis showed an increase in the number of BrdUlabeled cells and an alteration in their distribution along the epithelial lining in response to damage by *** mice treated with both doxorubicin and ghrelin,the number of BrdUlabeled cells was reduced when compared with mice treated with doxorubicin ***:The present study suggests that ghrelin enhances the regenerative potential of the GI epithelium in doxorubicintreated mice,at least in part,by modulating cell proliferation.

Atrial Myocardial Deformation Changes in Patients with Non-Valvular Atrial Fibrillation

Background: Atrial fibrillation (AF) is the most common cardiac arrhythmia. It increases cardiovascular morbidity, especially embolic stroke and mortality. Two-dimensional speckle tracking echocardiography (2D STE) is a useful method that has been used to detect changes in atrial myocardial deformation in AF patients. Objectives: To study atrial myocardial deformation changes in patient with non-valvular AF using 2D STE. Patients and Methods: This study included 25 patients with non-valvular AF and 25 normal healthy controls. 2D STE was used for assessment of strain and strain rate of septal and free walls of both right atrium (RA) and LA and left ventricle (LV). Results: Mean LA septal and lateral strain and strain rate were significantly reduced in the AF group compared to the control group (-7.2% ± 5.2% vs. -20.4% ± 3.9%, -8.7% ± 8.8% vs. -21.7% ± 3.4%) and (-0.9 ± 0.5 S-1 vs. -1.9 ± 0.4 S-1, –1.1 ± 0.6 S-1 vs. -2.04 ± 0.3 S-1) respectively. Mean RA septal and lateral strain and strain rate were significantly reduced in the AF group compared to the control group (-5.9% ± 6.1% vs. -23.4% ± 4.5%, -8.9% ± 9.3% vs. -21.7% ± 3.4%) and (-0.98 ± 0.6 S-1 vs. -1.9 ± 0.3 S-1, -1.3 ± 0.9 S-1 vs. -2.1 ± 0.5 S-1) respectively. Scalable Varied Density Clustering Algorithm for Large Datasets

Finding clusters in data is a challenging problem especially when the clusters are being of widely varied shapes, sizes, and densities. Herein a new scalable clustering technique which addresses all these issues is proposed. In data mining, the purpose of data clustering is to identify useful patterns in the underlying dataset. Within the last several years, many clustering algorithms have been proposed in this area of research. Among all these proposed methods, density clustering methods are the most important due to their high ability to detect arbitrary shaped clusters. Moreover these methods often show good noise-handling capabilities, where clusters are defined as regions of typical densities separated by low or no density regions. In this paper, we aim at enhancing the well-known algorithm DBSCAN, to make it scalable and able to discover clusters from uneven datasets in which clusters are regions of homogenous densities. We achieved the scalability of the proposed algorithm by using the k-means algorithm to get initial partition of the dataset, applying the enhanced DBSCAN on each partition, and then using a merging process to get the actual natural number of clusters in the underlying dataset. This means the proposed algorithm consists of three stages. Experimental results using synthetic datasets show that the proposed clustering algorithm is faster and more scalable than the enhanced DBSCAN counterpart.

Impact of Left Bundle Branch Block on Left Ventricular Mechanics in Patients with Idiopathic Dilated Cardiomyopathy

Objectives: Left bundle branch block (LBBB) is commonly associated with heart failure. We evaluated the prevalence and impact of LBBB on left ventricular mechanics using 2D strain imaging in patients with idiopathic dilated cardiomyopathy (IDCM). Methods: We included 101 patients with IDCM with mean age 38 ± 18 years: 74% were males and 13.9% of them were in NYHA Class III-IV. LBBB was present in 26 (37%) of included patients. Myocardial mechanics including longitudinal, circumferential strain and rotation and LV synchronization were assessed using two-dimensional strain imaging. Results: LBBB group had higher LV volumes, and PAP compared with non LBBB. Peak LV longitudinal systolic strain (εsys) of the septum and global LV SRsys were significantly lower in LBBB compared to non LBBB group (P 0.01, 0.03). TTP-d was greater in LBBB in comparison to non LBBB group (274.5 ± 116 versus 209.4 ± 139, P 0.02). The electromechanical delay between septal segments was 35 ± 18 ms and between lateral wall segments: 48 ± 24 ms, between anterior wall segments: 21 ± 11 and between inferior wall segments: 41 ± 12. Consequently, LV mechanical dyssynchrony was more evident in IDCM patients with LBBB. QRS width was correlated inversely with LV longitudinal strain and strain rate and electromechanical delay (P 0.0001) in non LBBB group. In LBBB QRS width was not related to cardiac mechanics. Using univariate analysis and after a multiple covariate adjustment, the baseline LBBB was associated with a significantly increased LV dysfunction. Conclusion:After correcting for potential confounders, LBBB was found to be associated with more deterioration of LV mechanics and exaggerated LV dyssynchrony in patients with IDCM.

Many cases of toxicity and mortality either due to accidental or deliberate ingestion of hair dyes were reported in Egypt, Sudan, Israel, Morocco, Saudi Arabia, India and Tunisia. In the field of forensic analytical toxicology, the identification and quantification of chemicals are essential in deaths related to intoxication. The aim of the present study is to conduct chemico-analytical characterization of para-phenylenediamine (PPD) based hair dye of (Black stone hair dye, SHD) currently used for dying hear in Egypt, to confirm the identification of its components, that may be potentially toxic on oral or dermal exposure, this study uses comparison between different analytical methods. The applied analytical methods were inductively coupled plasma/MS (ICP/MS), witch used to determine the metal components of the dye, high performance liquid chromatography/MS (HPLC/MS) and gas chromatography/mass spectroscopy (GC/MS) were used for qualitative and quantitative analysis of PPD in the dye. A derivatization method was applied by transforming PPD (aromatic amines) into corresponding imine derivatives. This compound was prepared by condensation of 4-methylbenzeneamine with benzaldehyed to give N-benzylidene-4-methylbenzene-amine, which is a very stable internal standard. ICP/MS analysis revealed that most metal components of hair dye are Be, Na, Al, K, Ca, Sc, Ti, V, Cr, Mn, Fe, Cu, and Zr, where HPLC/MS analysis revealed that PPD is the main ingredient of SHD. Derivatization is necessary for accurate determination of PPD in hair dye by GC/MS method and this results could not be obtained by direct injection of dye (without derivatization).

Para-phenylenediamine （PPD）, tile main toxic ingredients of hair dyes, have been used by millions of consumers to improve their appearance. Stone Hair Dye （SHD） mainly contain PPD. SHD and PPD were evaluated by 3-（4, 5-dimethylthiazol-2-yl）-2, 5-diphenyltetrazdium bromide or MTT assay, which measures mitochondria metabolism in the entire cell culture and provides information about the percentage of cell survival. Utilizing the MTT assay, the cytotoxicity of SHD and PPD was determining on SH Sy5y culture from nervous system of rat. The short term exposure SH Sy5y culture were incubated with various aqueous solutions of different concentrations of SHD and PPD, and the LC50 of SHD and para-phenylenediamine was found to be 9.15 and 12.4 mg/ml. With increasing the concentration, cytotoxicity effect increase in PPD and SHD. There is a significant differences （P〈0.01） of cytotoxicity among concentrations used. It concluded that in vitro assay could give important information of the mechanism of toxicity at low dosages.

Effect of the Illuminating Point Spread Function and Roughness on the Speckle Correlation Field

Under conditions differing from those subjected for central limit theorem, the spatial autocorrelation function of speckle pattern resulting from illuminated rough surface is investigated. Its dependence on different illuminating apertures and the average of the roughness heights is presented theoretically and experimentally. The experiments were carried out using a set of circular and square apertures having different sizes. The results indicate that, increasing the size of the illuminating aperture leads to a decrease in the width of the main lobe of the spatial autocorrelation function.

Spinal accessory neuropathy in patients with chronic neck pain

AIM: To assess the presence of spinal accessory neuropathy in patients with chronic neck ***: Patients with pain either regional or focal in the neck or shoulders for at least 6 mo(chronic neck pain) were recruited randomly from the Rheumatology and Rehabilitation Outpatient Clinic at the Faculty of Medicine-Suez Canal University. Two groups were compared: 30 patients with chronic neck pain with mean age(36.97 ± 12.45 years) and 10 apparently healthy controls. Trapezius muscle examination including inspection and range of motion both active and passive was performed. A full clinical neurological examination was carried out to exclude peripheral neuropathy and motor neuron disease. According to the subject's type of work, cases were categorized into labor-intensive and non-labor intensive tasks. A nerve conduction study(NCS) was performed on spinal accessory nerves at both sides for all patients and controls. Parameters including latencies and amplitudes of compound motor action potential(CMAP) were compared with the chronicity of neck pain using the neck disability score. This cross sectional study was carried in the Rheumatology and Rehabilitation Department, at Suez Canal University Hospital, Ismailia, ***: Physical examination revealed that 80% of cases had spinal trapezius muscle spasm. Restricted neck motion was present in 16.6% of cases. No one suffered from muscle wasting or weakness. Pain was bilateral in 18 patients(60%), localized to the right side in six patients(20%) and localized to the left side in six patients(20%). The causes of neck pain in the patients studied were nonspecific, due to physical stresses, cervical spondylosis and mild cervical disc herniation. Mean disease duration in patients with labor-intensive tasks was(3.9 ± 2.1 years), which was longer than that in patients with non-labor intensive tasks(3.1 ± 1.9 years); however, this difference was statistically insignificant.
