Nonalcoholic fatty liver disease(NAFLD)is the most common liver disease worldwide,with an estimated prevalence of 25% *** is closely associated with metabolic syndrome,which are both becoming increasingly more common with increasing rates of insulin resistance,dyslipidemia,and *** NAFLD is strongly associated with obesity,lean or nonobese NAFLD is a relatively new phenotype and occurs in patients without increased waist circumference and with or without visceral ***,there is limited literature comparing and illustrating the differences between lean/nonobese and obese NAFLD patients with regard to risk factors,pathophysiology,and clinical *** this review,we aim to define and further delineate different phenotypes of NAFLD and present a comprehensive review on the prevalence,incidence,risk factors,genetic predisposition,and ***,we discuss and compare the clinical outcomes,such as insulin resistance,dyslipidemia,hypertension,coronary artery disease,mortality,and progression to nonalcoholic steatohepatitis,among lean/nonobese and obese NAFLD ***,we summarize the most up to date current management of NAFLD,including lifestyle interventions,pharmacologic therapies,and surgical options.

Role of FOXP3 in Immunohistochemical Expression in Preeclampsia

Background: FoxP3 gene variants have been linked to endometriosis, infertility, and autoimmune illnesses, according to numerous researches. Maternal sensitivity to the PE gene and the genetic variations of FoxP3 has not been thoroughly investigated. Objective: Investigation of the immune-histochemical expression of FoxP3 in placental tissue of PE patients. Methods: A total of 26 pre-eclamptic women as a case and 26 ethnically matched healthy pregnant women as a control group aged between 18 and 40 years old of different gravidity and parity referred to the labor ward for delivery either by vaginal delivery or cesarean section was enrolled to investigate the immunohistochemical expression of FOXP3 in placental tissue of PE patients. Results: Lower expression of FOXP3 IHC was statistically significant and noted in the group of preeclampsia compared to the healthy control group. Lower gestational age at delivery and a higher percentage of cesarean section were statistically significant and noted in the group of preeclampsia compared to the healthy control group. Conclusion: In comparison to the healthy control group, preeclampsia patients had statistically significantly lower FOXP3 IHC expression, and FOXP3 polymorphism was associated with the development of PE. Our findings can serve as a guide for statistical analyses and functional investigations that are more in-depth.

The role of graft reperfusion sequence in the development of non-anastomotic biliary strictures following orthotopic liver transplantation: A meta-analysis

Background: Liver transplant is a potential cure for liver failure and hepatic malignancy but there are many techniques which have been described for vascular reconstruction. This study was to compare the prevalence of non-anastomotic biliary stricture and other surgical complications based on Clavien-Dindo scoring system, in initial portal reperfusion(sequential) versus simultaneous or initial artery reperfusion. Data sources: Meta-analysis of published studies comparing the outcomes of both techniques was carried out. Data search was conducted across the major databases and studies were selected under the guidance of the Cochrane guidelines for systematic reviews and meta-analysis. Results: Seven studies were included to address the primary and the secondary outcomes. No statistical difference was found in the incidence of non-anastomotic biliary strictures(OR = 0.40; P = 0.14), regardless of reperfusion technique. The pooled estimate of the Clavien-Dindo grading of complications was not significantly different between the techniques, though Clavien-Dindo II complications were higher in the simultaneous or initial artery reperfusion group than the initial portal reperfusion group(OR = 2.73; P = 0.01). Similarly, there was no difference in the operative time, hospital stay and other outcomes addressed in this report. Conclusions: The available evidence suggests that there is no significant difference demonstrated in the rate of non-anastomotic biliary strictures or other complications, between the two techniques, except for Clavien-Dindo II complications.

Reperfusion of Delayed Acute Occlusive Limb Ischemia: Is It Worthwhile?

Delayed reperfusion of acute occlusive limb ischemia causes local and systemic serious consequences and is the main cause of morbidity and mortality in these patients. The aim of this study was to examine the outcome and risk factors of reperfusion injury in such cases. Patients and Methods: Retrospective review of all cases presented, to King Fahd Hospital of University, with acute occlusive limb ischemia more than 12 hours was performed between June 2004 and November 2012. Grades of ischemia, extremities, comorbidities, morbidities and mortality were recorded. Results: During the study period, 92 patients were included, 47 (51%) were embolic and the rest was thrombotic. On admission, 15 patients had grade III ischemia, 68 had grade IIb, 8 had grade IIa and 1 had grade I. Four patients died (4.3%) and 15 (16%) patients had amputation. The risk factors of amputation were age (p = 0.031), extremity (lower limb 21% vs. Upper limb 0%, p = 0.019), cause of ischemia (thrombotic 24% vs. embolic 8.5%, p = 0.049) and grade of ischemia (p = 0.001). Conclusion: Delayed reperfusion of acute occlusive ischemia carries acceptable morbidity and mortality and could be performed even in irreversible ischemia. The risk factors of amputation are age, lower limb ischemia, thrombosis and grade III ischemia.

Biometric security is a growing trend,as it supports the authentication of persons using conﬁdential biometric *** of the transmitted data in multi-media systems are susceptible to attacks,which affect the security of these *** systems provide sufﬁcient protection and privacy for *** recently-introduced cancellable biometric recognition systems have not been investigated in the presence of different types of *** addition,they have not been studied on different and large biometric *** point that deserves consideration is the hardware implementation of cancellable biometric recognition *** paper presents a suggested hybrid cancellable biometric recognition system based on a 3D chaotic *** rationale behind the utilization of the 3D chaotic cryptosystem is to guarantee strong encryption of biometric templates,and hence enhance the security and privacy of *** suggested cryptosystem adds signiﬁcant permutation and diffusion to the encrypted biometric *** introduce some sort of attack analysis in this paper to prove the robustness of the proposed cryptosystem against *** addition,a Field Programmable Gate Array(FPGA)implementation of the pro-posed system is *** obtained results with the proposed cryptosystem are compared with those of the traditional encryption schemes,such as Double Random Phase Encoding(DRPE)to reveal superiority,and hence high recogni-tion performance of the proposed cancellable biometric recognition *** obtained results prove that the proposed cryptosystem enhances the security and leads to better efﬁciency of the cancellable biometric recognition system in the presence of different types of attacks.

Impact of ginger aqueous extract on carbimazole induced testicular degenerative alterations and oxidative stress in albino rats

Objective: To evaluate the effect of ginger (Zingiber officinale) aqueous extract, a natural herb, with antioxidant properties, on testicular toxicity and oxidative stress induced by the antithyroid drug carbimazole in albino ***: Four groups of male albino rats were used. Group I served as control. Group II rats were treated with ginger aqueous extract (24 mg/mL). Group III rats were given orally carbimazole (1.35 mg/kg bw). Group IV rats were given carbimazole and ginger extract. Animals were sacrificed and their testes were removed and stained with H&E for histological examination. Sperms were collected from epididymis for detection of sperm head abnormalities. Immunohistochemical expression of PCNA and Bax was detected in the ***, CAT and GSH were measured in the ***: Treating rats with carbimazole revealed significant alterations in the tissue of testis including decreased seminiferous epithelium height, decreased diameter of seminiferous tubule and changes in the spermatogenic layers arrangement. Intertubular hemorrhage and congested blood vessels were noted. An increase in sperm head abnormalities was *** cell proliferation was reflected by a decrease in PCNA expression, while the increase in apoptotic rate was accompanied with an increase in Bax expression. Oxidative stress was demonstrated by an increase in malondialdehyde and decrease in activity of catalase and glutathione. Combined treatment of carbimazole and aqueous ginger extract led to an improvement in histological, morphometrical, immunohistochemical changes and oxidative stress induced by ***: The ameliorative effects of ginger extract could be due to its antioxidant properties.

Derivation the Schemes of Lateral and Vertical Dispersion Parameters: Application in Gaussian Plume Model

The main objective of this paper is to estimate the plume dispersion parameters in lateral direction (σy) and vertical direction (σz) by using power law wind speed and the scheme of eddy diffusivity in unstable condition. Comparison among our model and algebraic [1] and integral [2] formulations were held. We find that our model and two other models are in agreement with observed data.

Effect of alternative antibiotics in treatment of cefotaxime resistant spontaneous bacterial peritonitis

AIM:To evaluate effective alternative antibiotics in treatment of cefotaxime-resistant spontaneous bacterial ***:One hundred cirrhotic patients with spontaneous bacterial peritonitis [ascitic fluid polymorphonuclear cell count(PMNLs) ≥ 250 cells/mm 3 at admission] were empirically treated with cefotaxime sodium 2 g/12 h and volume expansion by intravenous human *** patients were subjected to history taking,complete examination,laboratory tests(including a complete blood cell count,prothrombin time,biochemical tests of liver and kidney function,and fresh urine sediment),chest X-ray,a diagnostic abdominal paracentesis,and the sample subjected to total and differential cell count,chemical examination,aerobic and anaerobic *** were divided after 2 d by a second ascitic PMNL count into group Ⅰ;patients sensitive to cefotaxime(n = 81),group Ⅱ(n = 19);cases resistant to cefotaxime(less than 25% decrease in ascitic PMNL count).Patients of group Ⅱ were randomly assigned into meropenem(n = 11) or levofloxacin(n = 8) *** patients performed an end of treatment ascitic PMNL *** were considered improved when:PMNLs decreased to < 250 cells/mm 3,no growth in previously positive culture cases,and improved clinical manifestations with at least 5 d of antibiotic ***:Age,sex,and Child classes showed no significant difference between group Ⅰ and group Ⅱ.Fever and abdominal pain were the most frequent manifestations and were reported in 82.7% and 80.2% of patients in group Ⅰ and in 94.7% and 84.2% of patients in group Ⅱ,*** in group Ⅱ had a more severe ascitic inflammatory response than group Ⅰ and this was demonstrated by more ascitic lactate dehydrogenase(LDH) [median:540 IU/L(range:150-1200 IU/L) vs median:240 IU/L(range:180-500 IU/L),P = 0.000] and PMNL [median:15 000 cell/mm 3(range:957-23 822 cell/mm 3) vs 3400 cell/mm 3(range:695-26 400 cell/mm 3),P = 0.000] *** fluid culture was po
