The Lower Peripheral Blood Lymphocyte to Monocyte Ratio Following Completion of First Line Chemotherapy Is a Risk Factor for Predicting Relapse in Patients with Diffuse Large B-Cell Lymphoma

Background and objective: During routine follow up, there is no specific predictor to ascertain relapse after standard first line chemotherapy in diffuse large cell lymphoma. Therefore, this study was designed to assess the prognostic significance of the ratio between absolute lymphocyte and monocyte counts (LMR) in the peripheral blood to verify relapse in diffuse large B cell lymphoma. Patients and methods: A total of 139 patients with newly diagnosed diffuse large B cell lymphoma (DLBCL) were evaluated and treated with CHOP or R-CHOP between the years 2009 and 2016. Three months following completion of first line therapy, Lymphocyte/monocyte ratio (LMR) was calculated from the routine automated complete blood cell count (CBC) attained a plateau after the bone marrow recovery after first line chemotherapy. The absolute lymphocyte count/absolute monocyte count ratio (LMR) was calculated by dividing the ALC by the AMC. Results: ROC curve analysis of 139 patients established 2.8 as cutoff point of LMR for relapse with AUC of 0.97 (95% CI 0.93 - 0.99, P ≤ 0.001). Cox regression analysis was performed to identify factors predicting relapse. In univariate regression analysis, ALC (95% CI 0.003 - 0.03, p ≤ 0.001), AMC (95% CI 15.4 - 128.8, p ≤ 0.001), LMR (95% CI 0.001 - 0.01, p ≤ 0.001), and LDH (95% CI 0.1 - 0.5, p ≤ 0.001) following completion of therapy are significant factors for relapse. Other significant factors for relapse are Ann Arbor stage (95% CI 1.1 - 6.9, P = 0.03), extranodal sites (95% CI 1.2 - 6.1, P = 0.01), age (95% CI 1.3 - 6.5, P = 0.01) and treatment of CHOP protocol (95% CI 0.05 - 0.6, P = 0.007). In a multivariate analysis LMR following completion of therapy was predictive for relapse (95% CI 0.001 - 0.2, P = 0.005). ALC was also significant in multivariate analysis (95% CI 0.01 - 0.8, P = 0.03). Local Consolidation Therapy versus Observation for Oligometastatic Non Small Cell Lung Cancer Patients: Phase II Randomized Trial

Background and Objective: Based on retrospective trials, most progression sites after first line systemic therapy for metastatic non small cell lung cancer (NSCLC) were the primary disease sites rather than new sites. Therefore we conducted phase II randomized study to determine whether oligometastatic NSCLC without disease progression after first line chemotherapy, have prolonged progression free survival when treated with local consolidation therapy of residual disease followed by surveillance compared with no local consolidation therapy (observation). Patients and Methods: Forty eight eligible patients were randomized to either immediate or no local consolidation radiotherapy. 26 patients of immediate local consolidation radiotherapy received 3 D-conformal radiation therapy to primary tumor site and metastatic sites of disease. 22 patients were followed up by observation. Results: Patients in local consolidation arm had significantly better progression free survival (PFS) compared with patients in observation group. Median PFS was 9.5 months (95% CI 7.8 - 11.08) in local consolidation arm and 4.5 months (95%CI 3.9 - 5.7) in observation arm. Patients in local consolidation arm had longer median time to appearance of new metastatic sites (10 months CI 9.3 - 12.6) than those patients in observation arm (4.5 months CI 4.2 - 6.9). Median overall survival (OS) of patients in local consolidation arm was 12 months (95% CI 12.1 - 18.01) and in observation arm 10 months (95% CI 8.7 - 13.8). One year OS rate was 42.3% in local consolidation arm and 31.8% in observation arm;2 year OS rate was 23.1% in local consolidation arm and only 4.5% in observation arm. Conclusion: Local consolidation radiotherapy is simple, safe, efficient, and not expensive treatment for oligometastatic non small cell lung cancer after upfront chemotherapy. In this study, the feasibility of using chitosan, a multifunctional environmentally friendly biopolymer, at different molecular weights to simultaneously adsorb various metal ions from aqueous solutions was evaluated. The experiments were carried out to estimate adsorptivity and selectivity of three molecular weights chitosan, 3.60 × 105 （Chl）, 5.97 × 105 （Ch2） and 9.47 × 105 （Ch3） g/mol, towards Cu（II）, Ni（II）, and Pb（II） ions in aqueous solutions. Screening for hepatocellular carcinoma by Egyptian physicians

AIM: To assess the practice of Egyptian physicians in screening patients for hepatocellular carcinoma(HCC). METHODS: The study included 154 physicians from all over Egypt caring for patients at risk for HCC. The study was based on a questionnaire with 20 items. Each questionnaire consisted of two parts:(1) personal information regarding the physician(name, age, specialty and type of health care setting); and(2) professional experience in the care of patients at risk for HCC development(screening, knowledge about the cause and natural course of liver diseases and HCC risk). RESULTS: Sixty-eight percent of doctors with an MD degree, 48% of doctors with a master degree or a diploma and 40% of doctors with a Bachelor of Medicine, Bachelor of Surgery certificate considered the hepatitis C virus(HCV) genotype as risk factor for HCC development(P ell as liver cirrhosis(P < 0.05). Eighty-six percent of doctors in University Hospitals and 69% of Ministry of Health(MOH) doctors consider HCV infection as the leading cause of HCC in Egypt(P < 0.05). Seventy-two percent of doctors with an MD degree, 55% of doctors with a master degree or a diploma, 56% of doctors with an MBBCH certificate, 74% of doctors in University Hospitals and 46% of MOH hospital doctors consider abdominal ultrasonography as the most important investigation in HCC screening(P < 0.05). Renal transplant patients receive several immunosuppressive drug regimens that are potentially nephrotoxic for *** creatinine is the standard for monitoring kidney function;however,cystatin C(Cys C)and kidney injury molecule-1(KIM-1)have been found to indicate kidney injury earlier than serum creatinine and provide a better reflection of kidney ***,we assessed Cys C and KIM-1 serum levels in renal transplant patients receiving mycophenolate mofetil,tacrolimus,sirolimus,everolimus,or cyclosporine to evaluate kidney *** used both the Chronic Kidney Disease Epidemiology Collaboration(CKD-EPI)2021 equation,which is based on creatinine and combined creatinine with Cys C,and the CKD-EPI 2012 equation,which is based on Cys C alone,to estimate glomerular filtration rate(GFR).Then,we assessed the association between serum KIM-1 and GFRels in patients with the elevated serum creatinine,compared with those with normal serum *** estimated GFRs based on creatinine were significantly higher than those based on the other equations,while a significant positive correlation was observed among all *** KIM-1 levels were negatively correlated with the estimated GFRs by the CKD-EPI Cys C and the combined creatinine with Cys C equations.A serum KIM-1 level above 0.71 ng/mL is likely to indicate GFRelation between serum creatinine and Cys C in our renal transplant ***,serum KIM-1 may be used to monitor renal function when using potentially nephrotoxic drugs in renal transplants.

Numerical Investigation of Rotating Stall in Centrifugal Compressor with Vaned and Vaneless Diffuser

This study presents a numerical simulation of the stall and surge in a centrifugal compressor and presents a description of the stall development in two different *** first case is for a compressor with vaneless diffuser and the second is for a compressor with vaned diffuser of the vane island *** main aim of this study is to compare the flow characteristics and behavior for the two compressors near the surge operating condition and provide further understanding of the diffuser role when back flow occurs at *** showed that for a location near the diffuser entrance,the amplitude of the static pressure fluctuations for the vaneless diffuser case is higher than that for the vaned diffuser case near surge *** pressure fluctuations in the case of the vaneless diffuser appear with a gradual decrease of the mean pressure value as a part of the surge *** for the case of the vaned diffuser,the pressure drop during surge occurs faster than the case of the vaneless ***,results indicated that during surge in the case of vaneless diffuser,there is a region with low velocity and back flow that appears as a layer connecting all impeller passages near shroud surface and this layer develops in size with *** the other hand,for the case of vaned diffuser during surge,the low velocity regions appear in random locations in some passages and these regions expand with time towards the shroud *** showed that during stall,the impeller passages are exposed to identical impact from stall cells in the case of vaneless diffuser while the stall effect varies from passage to another in the case of the vaned diffuser.

Performance Analysis of a Vertical Axis Tidal Turbine With Flexible Blades

In this study, a vertical axis tidal turbine with flexible blades is investigated. The focus is on analyzing the effect of flexible airfoils types and blade flexibility on turbine net output power. To this end, five different flexible airfoils （Symmetric and Non-symmetric） are employed. The results show that the use of a thick flexible symmetric airfoil can effectively increase output power compared to that achievable with a conventional rigid blade. Moreover, the use of highly flexible blades, as opposed to less flexible or rigid blades, is not recommended.

Automated diagnosis and staging of Fuchs’endothelial cell corneal dystrophy using deep learning

Background:To describe the diagnostic performance of a deep learning algorithm in discriminating early-stage Fuchs’endothelial corneal dystrophy(FECD)without clinically evident corneal edema from healthy and late-stage FECD eyes using high-definition optical coherence tomography(HD-OCT).Methods:In this observational case-control study,104 eyes(53 FECD eyes and 51 healthy controls)received HDOCT imaging(Envisu R2210,Bioptigen,Buffalo Grove,IL,USA)using a 6 mm radial scan pattern centered on the corneal *** was clinically categorized into early(without corneal edema)and late-stage(with corneal edema).A total of 18,720 anterior segment optical coherence tomography(AS-OCT)images(9180 healthy;5400 early-stage FECD;4140 late-stage FECD)of 104 eyes(81 patients)were used to develop and validate a deep learning classification network to differentiate early-stage FECD eyes from healthy eyes and those with clinical *** 5-fold cross-validation on the dataset containing 11,340 OCT images(63 eyes),the network was trained with 80%of these images(3420 healthy;3060 early-stage FECD;2700 late-stage FECD),then tested with 20%(720 healthy;720 early-stage FECD;720 late-stage FECD).Thereafter,a final model was trained with the entire dataset consisting the 11,340 images and validated with a remaining 7380 images of unseen AS-OCT scans of 41 eyes(5040 healthy;1620 early-stage FECD 720 late-stage FECD).Visualization of learned features was done,and area under curve(AUC),specificity,and sensitivity of the prediction outputs for healthy,early and late-stage FECD were ***:The final model achieved an AUC of 0.997±0.005 with 91%sensitivity and 97%specificity in detecting early-FECD;an AUC of 0.974±0.005 with a specificity of 92%and a sensitivity up to 100%in detecting late-stage FECD;and an AUC of 0.998±0.001 with a specificity 98%and a sensitivity of 99%in discriminating healthy corneas from all ***:Deep learning algorithm is an accurate autonomous nove

Cardiac diseases are one of the greatest global health *** to the high annual mortality rates,cardiac diseases have attracted the attention of numerous researchers in recent *** article proposes a hybrid fuzzy fusion classification model for cardiac arrhythmia *** fusion model is utilized to optimally select the highest-ranked features generated by a variety of well-known feature-selection *** ensemble of classifiers is then applied to the fusion’s *** proposed model classifies the arrhythmia dataset from the University of California,Irvine into normal/abnormal classes as well as 16 classes of ***,at the preprocessing steps,for the miss-valued attributes,we used the average value in the linear attributes group by the same class and the most frequent value for nominal ***,in order to ensure the model optimality,we eliminated all attributes which have zero or constant values that might bias the results of utilized *** preprocessing step led to 161 out of 279 attributes(features).Thereafter,a fuzzy-based feature-selection fusion method is applied to fuse high-ranked features obtained from different heuristic feature-selection *** short,our study comprises three main blocks:(1)sensing data and preprocessing;(2)feature queuing,selection,and extraction;and(3)the predictive *** proposed method improves classification performance in terms of accuracy,F1measure,recall,and precision when compared to state-of-the-art *** achieves 98.5%accuracy for binary class mode and 98.9%accuracy for categorized class mode.
