Transient elastography compared to liver biopsy and morphometry for predicting fibrosis in pediatric chronic liver disease: Does etiology matter?

AIM: To evaluate transient elastography (Te) as a noninvasive tool in staging liver fibrosis compared with liver biopsy and morphometry in children with different chronic liver ***: A total of 90 children [50 with chronic hepatitis C virus (HCV), 20 with autoimmune hepatitis (AIH) and 20 with Wilson disease] were included in the study and underwent liver stiffness measurement (LSM) using Te. Liver biopsies were evaluated for fibrosis, qualitatively, by Ishak score and quantitatively by fibrosis area fraction (FAF) using digital image analysis (morphometry). LSM was correlated with fibrosis and other studied variables using spearman correlation. A stepwise multiple regression analysis was also performed to examine independent factors associated with LSM. Different cut-off values of LSM were calculated for predicting individual fibrosis stages using receiver-operating characteristic curve. Cut-off values with optimal clinical performance (optimal sensitivity and specificity simultaneously) were ***: The majority of HCV group had minimal activity (80%) and no/mild fibrosis (72%). On the other hand, the majority of AIH group had mild to moderate activity (70%) and moderate to severe fibrosis (95%) and all Wilson disease group had mild to moderate activity (100%) and moderate to severe fibrosis (100%). LSM correlated significantly with both FAF and Ishak scores and the correlation appeared better with the latter (r = 0.839 vs 0.879, P ed individual stages of fibrosis with high performance. Sensitivity ranged from 81.4% to 100% and specificity ranged from 75.0% to 97.2%. When we compared LSM values for the same stage of fibrosis, they varied according to the different etiologies. Higher values were in AIH (16.15 ± 7.23 kPa) compared to Wilson disease (8.30 ± 0.84 kPa) and HCV groups (7.43 ± 1.73 kPa). The feasibility of using N,N,N＇,N＇-tetra（2-ethylhexyl）diglycolamide （TeHDGA） in 75 vol.% n-dodecane-25 vol.% n-octanol as agents for the extraction and separation ofNd（III）, Sm（III）, Dy（llI）, Fe（III）, Ni（II）, and Cs（I） from concentrated chlo- ride solution was investigated. Different extraction behaviors were obtained towards rare earth elements （Ree） studied and Fe（IIl）, Ni（II） and Cs（I）. efficient separation of Nd（III）, Sm（III） and Dy（III） from Fe（III）, Ni（II）, and Cs（I） was achieved by TeHDGA, depending on the HCI, HNO3 or H2SO4 concentration. A systematic investigation was carried out on the detailed extraction prop- erties of Nd（III）, Sm（III）, and Dy（III） with TeHDGA from chloride media. The IR spectra of the extracted species were investi- gated.

BACKGROUND Reconstruction of the pelvic ring anatomy in unstable anterior pelvic ring injuries is a significant step to reduce the mortality rate associated with these injuries *** is a debate on using either an anterior subcutaneous pelvis internal fixator(INFIX)or an anterior supra-acetabular external fixator(eXFIX)to manage an unstable anterior pelvic ring *** To compare the functional and radiological outcomes and complications of INFIX vs eXFIX in managing unstable pelvic ring *** A prospective cohort study included 54 patients with unstable pelvic ring *** patients were divided into two groups;the INFIX group,in which 30 cases were fixed by INFIX,and the eXFIX group,in which 24 patients were treated by *** average age in the eXFIX group was 31.17 years(16-57 years),while in the INFIX group,it was 34.5 years(17-53 years).The study included 20(66.7%)males and 10(33.3%)females in the INFIX group and 10(41.7%)males and 14(58.3%)females in the eXFIX *** radiological outcomes were evaluated using Matta and Tornetta's score,and the functional outcomes using the Majeed *** The results revealed a statistically significant difference between both groups(P=0.013)regarding radiological outcomes,according to Matta and Tornetta's score in favor of the INFIX ***,standing,and walking abilities were measured at a 3-mo follow-up visit using Majeed score *** was significantly better among the INFIX group than the eXFIX group in all three *** the final follow-up,both groups had no statistically significant difference according to the Majeed score;92.35 in the INFIX group and 90.99 in the eXFIX group(P=0.513).A lower surgical site infection rate was noticed in the INFIX group(P=0.007).CONCLUSION Anterior subcutaneous pelvis INFIX is associated with better radiological outcomes and a lower infection rate than anterior supra-acetabular eXFIX in managing patients with unstable anterior pelvic

The possibility of utilizing thorium as a fuel in a pressurized water reactor(PWR)has been proven from the neutronic perspective in our previously published work without assessing the thermal hydraulic(TH)and solid structure ***,the TH and solid structure performances must be studied to confirm these results and ensure the possibility of using a thorium-based fuel as an excellent accident-tolerant *** TH and solid structure performances of thorium-based fuels were investigated and compared with those of UO_(2).The radial and axial power peaking factors(PPFs)for UO_(2),(^(232)Th,^(235)U)O_(2),and(^(232)Th,^(233)U)O_(2)were examined with a PWR assembly to determine the total PPF of each *** Gd_(2)O_(3)and er_(2)O_(3)were tested as burnable absorbers(BAs)to manage the excess reactivity at the beginning of the fuel cycle(BOC)and reduce the total ***_(2)O_(3)resulted in a more significant reduction to the total PPF and,therefore,a greater reduction to the temperature distribution compared to Gd_(2)O_(3).Given these results,we analyzed the effects of adding er_(2)O_(3)to thorium-based fuels on their TH and solid structure performances.

A dynamic analysis of both twisting and regular towers is carried out to determine the results of considering soil-structure interaction(SSI)on high-rise *** addition,the difference between the seismic performance of using twisting towers over regular ones is *** twisting tower is a simulation of the evolution Tower(Moscow).The towers’skeletons consist of RC elements and rest on a reinforced concrete piled-raft *** soil model is considered as multi-layered with the same soil properties as the zone chosen for the analysis(New Mansoura City,egypt).The only difference between both towers is their shape in *** whole system is modelled and analyzed in a single step as one full 3D model,which is known as the direct approach in *** analyses are carried out using finite-element software(Midas GTS NX).Dynamic output responses due to three records of seismic loads are proposed and presented in some *** on the results,it is concluded that SSI has a considerable effect on the dynamic response of tall buildings mainly because of the foundation flexibility,as it leads to lengthening the vibration period,increasing the story drift and the base shear for both cases.

Objectives:This study aimed to assess the nurses’knowledge and compliance with infection control standard precautions and evaluate the impact of the designed infection control educational program on nurses’knowledge and compliance with standard precautions at the maternity *** and Methods:A quasi‑experimental one‑group pretest‑posttest design was used on convenient sample of 60 nurses working at Obstetrics and Gynecological hospital in Cairo University Hospital,Kasr el Ainy,who received“designed infection control educational program”for 6 *** data were collected through the questionnaire comprising demographics,knowledge,and compliance with standard precautions *** nurses’knowledge and compliance score were compared before and after ***:The studied nurses had higher level of knowledge(85.3%)and compliance(92.8%)regarding infection control standard precautions after the educational program compared to before the *** results revealed statistically significant difference between before and after the program regarding nurses’knowledge score(15.5±2.9 vs.17.1±1.6,Pe score(58.5±13.2 vs.74.3±5.1,Pere was insignificant relation between nurses’knowledge and compliance with standard precautions both before(r=0.952,P=0.474)and after educational program(r=0.164,P=0.223).Conclusion:The nurses’level of knowledge and compliance regarding infection control standard precautions was significantly improved after the ***,it is recommended that periodical educational programs regarding the standard precautions of infection control are essential for nurses at maternity hospital.

Nuclear fuel performance modeling and simulation are critical tasks for nuclear fuel design optimization and safety analysis under normal and transient *** performance is a complicated phenomenon that involves thermal,mechanical,and irradiation mechanisms and requires special multiphysics *** this study,a fuel performance model was developed using the COMSOL Multiphysics *** modeling was performed for a 2D axis-symmetric geometry of a UO2fuel pellet in the e110 clad for VVeR-1200 *** modeling considers all relevant phenomena,including heat generation and conduction,gap heat transfer,elastic strain,mechanical contact,thermal expansion,grain growth,densification,fission gas generation and release,fission product swelling,gap/plenum pressure,and cladding thermal and irradiation *** model was validated using a code-to-code evaluation of the fuel pellet centerline and surface temperatures in the case of constant power,in addition to validation of fission gas release(FGR)*** prediction proved that the model could perform according to previously published VVeR nuclear fuel performance parameters.A sensitivity study was also conducted to assess the effects of uncertainty on some of the model *** model was then used to predict the VVeR-1200 fuel performance parameters as a function of burnup,including the temperature profiles,gap width,fission gas release,and plenum pressure.A compilation of related material and thermomechanical models was conducted and included in the modeling to allow the user to investigate different material/performance *** the model was developed for normal operating conditions,it can be modified to include off-normal operating conditions.
