Impact of Left Bundle Branch Block on Left Ventricular Mechanics in Patients with Idiopathic Dilated Cardiomyopathy

Objectives: Left bundle branch block (LBBB) is commonly associated with heart failure. We evaluated the prevalence and impact of LBBB on left ventricular mechanics using 2D strain imaging in patients with idiopathic dilated cardiomyopathy (IDCM). Methods: We included 101 patients with IDCM with mean age 38 ± 18 years: 74% were males and 13.9% of them were in NYHA Class III-IV. LBBB was present in 26 (37%) of included patients. Myocardial mechanics including longitudinal, circumferential strain and rotation and LV synchronization were assessed using two-dimensional strain imaging. Results: LBBB group had higher LV volumes, and PAP compared with non LBBB. Peak LV longitudinal systolic strain (εsys) of the septum and global LV SRsys were significantly lower in LBBB compared to non LBBB group (P 0.01, 0.03). TTP-d was greater in LBBB in comparison to non LBBB group (274.5 ± 116 versus 209.4 ± 139, P 0.02). The electromechanical delay between septal segments was 35 ± 18 ms and between lateral wall segments: 48 ± 24 ms, between anterior wall segments: 21 ± 11 and between inferior wall segments: 41 ± 12. Consequently, LV mechanical dyssynchrony was more evident in IDCM patients with LBBB. QRS width was correlated inversely with LV longitudinal strain and strain rate and electromechanical delay (P 0.0001) in non LBBB group. In LBBB QRS width was not related to cardiac mechanics. Using univariate analysis and after a multiple covariate adjustment, the baseline LBBB was associated with a significantly increased LV dysfunction. Conclusion:After correcting for potential confounders, LBBB was found to be associated with more deterioration of LV mechanics and exaggerated LV dyssynchrony in patients with IDCM.

Variations of Cup-to-Disc Ratio in Children

Purpose: The aim of this study is early detection of increased cup to disc ratio and a survey study of cup-disc ratio in Egyptian children age group less than 15 years. Methods: Three hundred and eighty-two (382) children were enrolled in this study. They were classified into three age groups;infants’ Group (1) from 0-2 years, preschool Group (2) from 2-6 years and school children Group (3) from 6-15 years old. C/D ratio was measured for each child. Results: In Group (1), normal C/D ratio ranged from 0 to 0.1. The C/D ratio > 0.3 or a binocular difference of >0.2 could be regarded as one of the criteria of newborn glaucoma. Group (2): the normal ratio ranged from 0.1-0.2. Group (3): the normal ratio ranged from 0.2-0.3. The highest ratio 0.7 was found in two eyes. An asymmetry of 0.2 or more in the C/D ratios of the eyes was found in 4.8% of the children. Conclusion: C/D ratio > 0.3 and asymmetry of 0.2 is suggestive of glaucoma especially in infants of lower age group.

Moroccan Taxi Drivers Fatigue Using Pichot Questionnaire： A Cross-Sectional Survey

Studies conducted in many countries the world has shown that fatigued driving contributes to approximately 3%-30% of road accidents. We conducted this study to estimate the prevalence of fatigue among taxi drivers and to assess the determinant of fatigue among taxi drivers. In this cross-sectional study, 300 urban and rural taxi drivers from Fez, Morocco were enrolled from January to May 2013. The mean score of Pichot scale was 13.59 ± 8.23. The mean duration of sleep was 6.78 ±1.47 h per day. The mean rate of accident was 2.78 ± 1.12 per year. At the final model of the multiple linear regression we found an association statistically significant between Pichot scale and income, cardiovascular disorders, tobacco use, problems with colleagues, regularity of sleep and sleeping at the wheel. This study opened the door to many discussions about the professional drivers＇ conditions of work and we can argue that we should ameliorate the professional drivers conditions of work in Morocco.

Fetal Cerebellar Vermis Assessment by MRI. What Does It Add?

Background: Posterior fossa anomalies (PFAs) are considered one of the commonest brain anomalies identified by the fetal imaging techniques. The current study aims to estimate the significance of cerebellar vermis assessment by fetal MRI in fetal brain malformations. Patients and Methods: The study was conducted at a tertiary University hospital in Upper Egypt. Data were gathered prospectively from sixty pregnant women with sonographically diagnosed fetal brain malformations referred for Magnetic Resonance Imaging (MRI) between April 2015 and May 2019 followed by postpartum neonatal MRI brain examination for non-terminated and live birth cases. Results: Of the sixty fetuses with brain malformations, additional cerebellar vermis findings were seen on MRI in 10 cases (16.6%). Fetal MRI sensitivity, specificity, positive and negative predictive values were 100% that were in correlation with those of prenatal ultrasound turned out to be significantly higher. Conclusion: This study recommends fetal MRI for cerebellar vermis assessment in fetal brain malformations to guide the clinical management.

Isolation, Identification and Screening of Carotenoid-Producing Strains of <i>Rhodotorula glutinis</i>

An attempt was made to isolate yeast strains from different sources (flowers, trees exudates, fruits and cheese) which have a fast growth rate and high content of carotenoids. Seventy cultures of pigmented yeast were isolated. Forty six yeast isolates were selected and identified by simplified identification method which showed that all isolates belong to Rhodotorula glutinis. These isolates were screened in two steps. Seventeen promising isolates of Rhodotorula glutinis were selected after the primary screening. Then 6 yeast isolates were chosen and passed through the second screening. The most promising isolate (isolated from pin cushion flower, Scabiosa atropurpurea) was selected according to its total carotenoids content expressed as torulene (μg?g–1 and μg?L–1). This isolate was reclassified using a number of morphological, biochemical, and physiological characteristics which revealed that the yeast isolate agree well with those of Rhodotorula glutinis Var. glutinis. Dry biomass, amount and proportion of individual carotenoids and lipid content of the selected identified yeast were determined.

Impact of ginger aqueous extract on carbimazole induced testicular degenerative alterations and oxidative stress in albino rats

Objective: To evaluate the effect of ginger (Zingiber officinale) aqueous extract, a natural herb, with antioxidant properties, on testicular toxicity and oxidative stress induced by the antithyroid drug carbimazole in albino ***: Four groups of male albino rats were used. Group I served as control. Group II rats were treated with ginger aqueous extract (24 mg/mL). Group III rats were given orally carbimazole (1.35 mg/kg bw). Group IV rats were given carbimazole and ginger extract. Animals were sacrificed and their testes were removed and stained with H&E for histological examination. Sperms were collected from epididymis for detection of sperm head abnormalities. Immunohistochemical expression of PCNA and Bax was detected in the ***, CAT and GSH were measured in the ***: Treating rats with carbimazole revealed significant alterations in the tissue of testis including decreased seminiferous epithelium height, decreased diameter of seminiferous tubule and changes in the spermatogenic layers arrangement. Intertubular hemorrhage and congested blood vessels were noted. An increase in sperm head abnormalities was *** cell proliferation was reflected by a decrease in PCNA expression, while the increase in apoptotic rate was accompanied with an increase in Bax expression. Oxidative stress was demonstrated by an increase in malondialdehyde and decrease in activity of catalase and glutathione. Combined treatment of carbimazole and aqueous ginger extract led to an improvement in histological, morphometrical, immunohistochemical changes and oxidative stress induced by ***: The ameliorative effects of ginger extract could be due to its antioxidant properties.

Kinetic spectrophotometric determination of certain cephalosporins using iodate/iodide mixture

A simple, precise and accurate kinetic spectro-photometric method for determination of ce-fradine anhydrous, cefaclor monohydrate, ce-fadroxil monohydrate, cefalexin anhydrous and cefixime in bulk and in pharmaceutical formula-tions has been developed. The method based on a kinetic investigation of the reaction of the free carboxylic acid group of the drug with a mixture of potassium iodate and potassium iodide at room temperature to form yellow coloured triiodide ions. The reaction was followed up spectrophotometrically by measuring the increase in absorbance at 352 nm as a function of time. The initial rate, fixed time, variable time and rate-constant methods were adopted for constructing the calibration curves but fixed time method has been found to be more applicable. The analytical performance of the method, in terms of accuracy and precision, was statistically validated;the results were satisfactory. The method has been successfully applied to the determination of the studied drugs in commercial pharmaceutical formulations. Statistical comparison of the results with a well established reported method showed excellent ag- reement and proved that there is no significant difference in the accuracy and precision.

Predictive Value of Neutrophil-to-Lymphocyte Ratio in Outcomes of Patients with Acute Coronary Syndrome

Objectives: Previous studies have demonstrated the role of inflammation in acute coronary syndrome (ACS). The neutrophil-to-lymphocyte ratio (NLR) was found to be a useful inflammatory marker for predicting adverse outcomes. We hypothesized that an elevated neutrophil-to-lymphocyte ratio would be associated with increased mortality in patients with acute coronary syndrome. Methods: The study consisted of 40 patients with acute coronary syndrome who were admitted to Cardiology Department—Menoufia University Hospitals. The primary endpoint was all-cause in-hospital as well as 30-day mortality, and the patients were divided into three tertiles according to their admission NLR results. Results: All-cause 30-day mortality in the three groups based on NLR was 0.0%, 7.7% and 28.6%, in the low-, middle- and high-NLR groups, respectively (P χ2 test). In a logistic regression analysis, including baseline demographic, clinical, and biochemical covariates, the neutrophil-to-lymphocyte ratio was an independent predictor of mortality (OR = 2.44, 95% CI = 1.185 - 5.007, P Conclusion: An elevated neutrophil-to-lymphocyte ratio (NLR), a simple, relatively inexpensive and universally available inflammatory marker, seems to be a predictor of 30-day mortality in patients with acute coronary syndrome.

Study of Correlation Coefficient for Breast Tumor Detection in Microwave Tomography

In microwave tomography, it is necessary to increase the amount of diverse observation data for accurate image reconstruction of the dielectric properties of the imaging area. The multi-polarization method has been proposed as a suitable technique for the acquisition of a variety of observation data. While the effectiveness of employing multi-polarization to reconstruct images has been confirmed, the physical considerations related to image reconstruction have not been investigated. In this paper, a compact-sized imaging sensor using multi-polarization for breast cancer detection is presented. An analysis of the correlation coefficient of the received data of adjacent antennas was performed to interpret the imaging results. Numerical simulation results demonstrated that multi-polarization can reconstruct images better compared to single polarizations owing to its low correlation coefficient and condition number.

A Fire Risk Modelling and Spatialization by GIS

The management of the forest fire risk starts with it assessment. This assessment made the object of several works of research and many models of fire risk have been related. The model that interests us here is that established for Mediterranean forests. This last is conceived according to a sum weighted model integral three factors, where each is affected by a weight, function of his influence on the propagation of the fire. However, this model remains critically and deserves a development and an improvement. For it, and seen the importance and the influence of climatic condition in the departure and in the propagation of fire, we propose, in this paper, to improve this formula by the addition of another climatic factor (marked ICL), and to present it under a product shape while respecting the same definition of the risk. The application of the proposed model, suggested uses the technical geomatics to mapping the degree of the fire risk. In this setting, a SIG has been established and applied on a forest of Bouzareah clump in Algiers. Originality as it will allow the understanding of fire hazard and vulnerability of the environment for a better control of risk.
