Outcomes of Hemodialysis Patients in Public Centres in Abidjan from September 2018 to October 2021: Data from the Renal Registry of Cote d’Ivoire

Background: Hemodialysis is the most widely used renal replacement therapy in developing countries such as Cote d’Ivoire. Objective: To study the outcome of chronic hemodialysis patients in public centres in Abidjan using data from the Renal Registry of Cote d’Ivoire (2RCI). Method: This was a prospective analytical cohort study of End Stage Kidney Disease (ESKD) patients undergoing iterative hemodialysis in the network of public hemodialysis centres in Abidjan and who had been registered in the 2RCI register by September 2018. Results: Our study involved 363 cases out of 379 patients registered in the 2RCI registry database in 2018. The mean age of the patients was 47.3 ± 12.1 years, with a male predominance (sex ratio 1.97). In 70.24% of cases, they had no health insurance. The average duration of dialysis for our patients was 6.94 ± 4.1 years, with extremes of 2 and 28 years. This duration was less than 5 years in 44.3% of cases, between 6 and 10 years in 42.9% and more than 10 years in 12.6% of cases. The comorbidities found were hypertension in 95.6% of cases, diabetes in 7.8% and HIV in 4.6%. Anemia was found in 68% of cases and stroke in 8.6%. During the study period, 161 deaths were observed, representing a mortality rate of 44.35%. Acute lung oedema (29.8%), stroke (6.8%) and other cardiovascular diseases (19.3%) were the main causes of death. In multivariate analysis, factors such as age ≥ 65 years (HR = 3.66;CI 95% = 1.55 - 8.67;p = 0.003), “married/coupled” status (HR = 2.02;CI 95% = 1.24 - 3.31;p = 0.005) and normal weight at the start of dialysis (OR = 9.59, CI95% = 4.19 - 21.95;p = 0.001) were associated with the risk of death. Conclusion: Hemodialysis is performed in Abidjan on young patients. The mortality rate after three years of dialysis is very high. Hence the need to pursue the policy of decentralizing public centres and optimizing access to dialysis in terms of quality and quantity in order to improve patient survival.

COVID-19 Infection and Acute Kidney Injury: About 43 Cases Report Collected at the Nephrology Department of the Farah Polyclinic in Abidjan

Background: Acute kidney injury (AKI) is one of the increasingly described complications of coronavirus infection. Objectives: To identify factors associated with death in patients with acute kidney injury (AKI) during Coronavirus disease (COVID-19) in Abidjan, C?te d’Ivoire. Material and Method: This was a monocentric retrospective analytical study of all patients over 18 years of age with AKI during COVID-19 at the Farah Polyclinic in Abidjan, C?te d’Ivoire. AKI was defined and ranked according to Kidney Disease Improving Global Outcomes (KDIGO) 2012. The data were collected from the medical record and processed using RStudio. Results: Forty-three cases were collected. The average age was 58.5 12 years. The sex ratio (M/F) was 4.4. The main comorbidities were high blood pressure (60.4%) and diabetes (37.2%). AKI was at KDIGO stage 3 in 58%, KDIGO 2 in 21% and KDIGO 1 in 21%. The diagnosis of acute tubular necrosis was retained in 44.2% of patients followed by acute functional kidney injury in 32.6%. Hemodialysis was initiated in 48.8% of cases. The main indication of dialysis was anuria (46.6%). In total, 55.8% of patients died. Background: Kidney Transplantation is the best treatment for patients in end stage renal disease. It’s a new therapeutic approach for such patients in Cote d’Ivoire which is expected to develop. Aim: Determine the adherence factors affecting kidney transplant recipient among patients on maintenance haemodialysis in Cote d’Ivoire and point out possible obstacles to the development of this new practice in the country. Patients and Methods: This was a cross-sectional study carried out from May to June 2016 in the Haemodialysis Centres of Abidjan. Any patient aged ≥18 years, on haemodialysis for at least 6 months, who signed the inform consent were subjected to a questionnaire. None of the respondents had been transplanted. The subjects addressed in the document were sociocultural status, opinion related to kidney transplantation and willingness to be transplanted or not and the reasons. A statistical analysis was performed to determine factors associated with kidney transplantation desire. Results: We included 295 (71.53% males) patients, with a mean age of 44.53 ± 12.09 years. Among this population, 36.61% had a higher level of education, 70.85% were Christians, 66.44% lived with partners, 56.61% had no income and 74.92% were treated in Public Health Centres. The median duration on dialysis was 34 months. A total of 287 (97.29%) patients had already heard of kidney transplantation among which 149 (51.94%) for the first time after initiation of haemodialysis. There was 231 (78.31%) patients willing to be transplanted with only 91 (39.39%) of them having a potential living donors. The main motivations were the desire to stop dialysis (52.38%) and the search for a better quality of life (41.13%). Among the 64 (21.69%) patients unfavourable to renal transplantation, 45.31% raised its higher cost compared to haemodialysis. Poor stability and dispersibility,as well as aggregation are considered as major challenges in clinical application of iron oxide nanoparticles(IONPs).Several studies have shown that the synthesis parameters and post-synthesis treatments e.g.,drying methods,have the capability to improve the particles'***,we investigate the combined effect of synthesis and post-treatment parameters on the particle size,stability and magnetism of ***(Fe_(3)O_(4))NPs were prepared via co-precipitation and post-treated using different methods,i.e.(i)freeze dried at-53℃,0.133 mbar for 48 h(liquid nitrogen frozen(LFD)and freezer frozen(FFD)),(ii)vacuum oven dried(VOD)at 60℃for 24 h,and(iii)kept wet colloidal(WET),dispersed in deionized *** Fe_(3)O_(4)NPs’chemical functional groups,size,shape,crystallinity,stability,aggregation,porosity,and magnetic properties were further analysed using different characterisation *** results showed that,while the WET sample had the best stability and significantly less aggregation at different temperatures,amongst post-treated Fe_(3)O_(4)NPs,LFD sample exhibited the best stability(up to 37℃),dispersion and smallest polydispersity ***,all dried NPs had superparamagnetic characteristics,while,LFD Fe_(3)O_(4)NPs had better magnetic properties and stability than other drying methods.

The investigations of the crystal and magnetic structures of the BaFe12_xAlxO19 （x = 0.1-1.2） solid solutions have been performed with powder neutron diffractometry. Magnetic properties of the BaFe12_xAlxOt9 （x = 0.1-1.2） solid solutions have been measured by vibration sample magnetometry at different temperatures under different magnetic fields. The atomic coordinates and lattice parameters have been Rietveld refined. The invar effect is observed in low temperature range （from 4.2 K to 150 K）. It is explained by the thermal oscillation anharmonicity of atoms. The increase of microstress with decreasing temperature is found from Rietveld refinement. The Curie temperature and the change of total magnetic moment per formula unit are found for all compositions of the BaFet2_xAlxO19 （x = 0.1-1.2） solid solutions. The magnetic structure model is proposed. The most likely reasons and the mechanism of magnetic structure formation are discussed.

This study aims to examine the effect of replacing vanadium by niobium and iron on the tribological behavior of hot-isostatic-pressed titanium alloy （Ti-6Al-4V） biomaterial, using a ball-on-disk-type oscillating tribometer, under wet conditions using physiological solution in accordance with the ISO7148 standards. The tests were carried out under a normal load of 6 N, with an AISI 52100 grade steel ball as a counter face. The morphological changes and structural evolution of the nanoparticle powders using different milling times （2, 6, 12 and 18 h） were studied. The morphological characterization indicated that the particle and crystallite size continuously decrease with increasing milling time to reach the lowest value of 4 nm at 18-h milling. The friction coefficient and wear rate were lower in the samples milled at 18 h （0.226, 0.297 and 0.423; and 0.66 × 10-2, 0.87 × 10-2 and 1.51 × 10-2 μm3 N-1 i, tm-1） for Ti-6Al-4Fe, Ti-6A1-7Nb and Ti-6Al-4V, respectively. This improvement in friction and wear resistance is attributed to the grain refinement at 18-h milling. The Ti-6Al-4Fe samples showed good tribological performance for all milling times.

Neuroendocrine neoplasms of liver-A 5-year retrospective clinico-pathological study applying World Health Organization 2010 classification

AIM To study the clinicopathological characteristics of neuroendocrine neoplasms(NEN) on liver samples and apply World Health Organization(WHO) 2010 grading of gastroenteropancreatic(GEP) *** Clinicopathological features of 79 cases of NEN of the liver diagnosed between January 2011 to December 2015 were analyzed. WHO 2010 classification of GEP NEN was applied and the tumors were graded as G1, G2 or G3. Two more categories, D1/2(discordant 1/2) and D2/3(discordant 2/3) were also applied. The D1/2 grade tumors had a mitotic count of G1 and Ki-67 index of G2. The D2/3 tumors had a mitotic count of G2 and Ki-67 index of G3. The follow up details which were available till the end of the study period(December 2015) were *** Of the 79 tumors, 16 each were G1 and G2, and 18 were G3 tumors. Of the remaining 29 tumors, 13 were assigned to D1/2 and 16 were D2/3 grade. Male preponderance was noted in all tumors except for G2 neoplasms, which showed a slight female predilection. The median age at presentation was 47 years(range 10-82 years). The most common presentation was abdominal pain(81%). Pancreas(49%) was the most common site of primary followed by gastrointestinal tract(24.4%) and lungs(18%). Radiologically, 87% of the patients had multiple liver lesions. Histopathologically, necrosis was seen in only D2/3 and G3 tumors. Microvascular invasion was seen in all grades. Metastasis occurred in all grades of primary NEN and the grades of the metastatic tumors and their corresponding primary tumors were similar in 67% of the cases. Of the 79 patients, 36 had at least one follow up visit with a median duration of follow up of 8.5 mo(range: 1-50 mo). Secondary sex ratio of assisted reproductive technology babies

Objective: To assess the secondary sex ratio (SSR) of assisted reproduction technology babies within a fertility clinic in Melbourne, Australia. Methods: A retrospective cohort study was conducted on 3369 babies who were born following single embryo transfer between 2011 and 2016. Variables examined included embryo creation date, maternal and paternal ages, maternal body mass index (BMI), type of infertility, sperm parameters, fertilization method, type of embryo and stage of transfer. Multivariate regression analysis was performed on the data set using STRATA V9.2. Results: More males were found to be born to embryos created in summer (adjusted odds ratio=1.46, 95% confidence interval: 1.08-1.95;P=0.01). Lower BMI of mother (≤30 kg/m 2) and younger age of parents (≤35 years) were associated with an increase in SSR (50.1% vs. 47.1%;50.6% vs. 48.8%;50.0% vs. 48.9%) respectively. Decreased SSR value was associated with primary subfertility, using in-vitro fertilization procedure and fresh embryo compared to those with secondary subfertility, using intracytoplasmic sperm injection procedure and thawed embryo (49.3% vs. 50.6%;47.1% vs. 50.8%;49.4% vs. 50.1%) respectively. Conclusions: The fertilization methods, type of embryo, stage of embryo transfer, parameters of the sperm and status of subfertility do not significantly affect the SSR. SSR is affected by the seasonality, maternal age, and BMI.

Decrease of Alpha-fetoprotein in Patients with Cirrhosis Treated with Direct-acting Antivirals

Background and Aims:The lack of specificity has limited the role of serum alpha-fetoprotein(AFP)for hepatocellular carcinoma(HCC)screening among patients with cirrhosis related to hepatitis C virus(HCV)*** sought to examine whether AFP may decrease after achieving a sustained virological response(SVR)in patients with HCV-related ***:We performed a retrospective study of patients with HCV-related cirrhosis who were cured with direct-acting antiviral(DAA)therapy at the University of California,Los *** values,including serum AFP,were measured before and after completing the DAA ***:Fifty-six patients met the inclusion criteria,with median(interquartile range[IQR])age of 67(58-69)years and with 51.8%being *** patients received DAA therapy without *** decreased from median(IQR)7.2(4.2-13.4)ng/mL before DAAs to 4.2(2.7-6.3)ng/mL at the end of treatment and 4.2(2.9-6.8)ng/mL at 12 weeks after treatment(P<0.001).Model for end-stage liver disease(MELD),fibrosis-4(FIB4),and aspartate transaminase(AST)to platelet ratio index(APRI)scores at baseline were not significantly associated with AFP *** multivariate analysis,platelet count,AST and total bilirubin at baseline were significantly correlated to AFP reduction(P=0.04,0.009 and 0.04,respectively).The higher the baseline AFP,the greater the reduction in *** was no statistically significant correlation between baseline AFP and MELD,FIB4 or APRI ***:There was a significant decrease in AFP in patients with cirrhosis who achieved a SVR with *** a reduction in AFP after DAA treatment,AFP should be further studied as a screening modality for HCC in patients with cirrhosis.

To improve the performance of solid composite propellants(SCPs)supplemented with ammonium nitrate(AN)as an oxidizer,the incorporation of energetic ingredients such as explosives,energetic binders or catalysts is a common effective *** this purpose,polyurethane(PU),a typical inert binder,was mixed with nitrocellulose(NC)as an energetic *** composite solid propellant compositions based on AN and NC-modified polyurethane binder with different NC ratios were *** prepared formulations were characterized using Fourier transform infrared spectroscopy(FTIR),RAMAN spectroscopy,X-ray diffraction(XRD),electron densimetry,thermogravimetric(TG)analysis,and differential scanning calorimetry(DSC).A kinetic study was then performed using the iterative KissingerAkahira-Sunose(It-KAS),Flynn-Wall-Ozawa(It-FWO),and non-linear Vyazovkin integral with compensation effect(VYA/CE)*** theoretical performances,such as theoretical specific impulse,adiabatic flame temperature,and ideal exhaust gaseous species,were also determined using the NASA Lewis Code,Chemical Equilibrium with Application(CEA).Spectroscopic examinations revealed the existence of NC and full polymerization of PU in the prepared *** to density tests,the density of the propellant increases as the nitrocellulose component *** to the thermal analysis and kinetics study,the increase in NC content catalyzed the thermal decomposition of the AN-based composite solid *** on the theoretical study,increasing the amount of NC in the propellant increased the specific impulse and,as a result,the overall performance.
