Do statins reduce the mortality rate in stroke patients treated with systemic thrombolysis in a 5-year single-center study?

The present study investigated the association between pre-treatment with a cholesterol-lowering drug(statin) or new setting hereon and the effect on the mortality rate in patients with acute ischemic stroke who received intravenous systemic thrombolysis. During a 5-year period(starting in October 2008), 542 consecutive stroke patients who received intravenous systemic thrombolysis with recombinant tissue plasminogen activator(rt-PA) at the Department of Neurology, University Hospital Schleswig-Holstein, Campus Lübeck, Germany, were included. Patients were characterized according to statins. The primary endpoint was mortality;it was assessed twice: in hospital and 3 months after discharge. The secondary outcome was the rate of symptomatic intracerebral hemorrhage. Of the 542 stroke patients examined(mean age 72 ± 13 years;51% women, mean National Institutes of Health Stroke Scale(NIHSS) score 11), 138 patients(25.5%) had been pretreated with statin, while in 190 patients(35.1%) statin therapy was initiated during their stay in hospital, whereas 193(35.6%) never received statins. Patients pre-treated with statin were older and more frequently had previous illnesses(arterial hypertension, diabetes mellitus and previous cerebral infarctions), but were comparably similarly affected by the stroke(NIHSS 11 vs. 11;P = 0.76) compared to patients who were not on statin treatment at the time of cerebral infarction. Patients pretreated with statin did not differ in 3-month mortality from those newly treated to a statin(7.6% vs. 8%;P = 0.9). Interestingly, the group of patients pretreated with statin showed a lower rate of in hospital mortality(6.6% vs. 17.0;P = 0.005) and 3-month mortality(10.7% vs. 23.7%;P = 0.005) than the group of patients who had no statin treatment at all. The Toarcian Marrat Formation is exposed in outcrops in central Saudi Arabia and displays a variety of clastic and carbonate facies associated with well-preserved depositional geometries. It is unconformably overlies the Triassic Minjur Formation and it in turn is overlaid by the Middle Jurassic Dhruma Formation. Thirteen lithofacies types can be identified that permit the recognition of five lithofacies associations in a mixed clastic/carbonate platform. These lithofacies range from low-energy peritidal, intertidal, and back-shoal to moderate-and high-energy shoal and foreshoal lithofacies associations. The Marrat Formation exhibits three depositional sequences, each sequence is grouped into a transgressive systems tract(TST) and a highstand systems tract(HST) and then bounded by sequence boundary surfaces(SBSs). The TSTs are generally identified in clastic tidal-flat beds and back-shoal wackestones, while the HST is generally recorded in the carbonate tidal-flat and shoal. The vertical succession of facies associations from peritidal to foreshoal depositional environments is indicative of a deepening upward and retrogradational systems tract, from Lower to Upper Toarcian. The correlation between the studied sections reveals a general shallowing towards the south and the similarities between the studied sequences and others in the Arabian Gulf, the northern Neo-Tethys Plate, and Gondwanaland countries.

Effect of initial interference fit on pull-out strength in cementless fixation between bovine tibia and smooth stainless steel post was investigated in this study. Compressive behavior of bovine spongious bone was studied using mechanical testing in order to evaluate the elastic-plastic properties in different regions of the proximal tibia. Friction tests were carried out in the aim to evaluate the friction behavior of the contact between bovine spongious bone and stainless steel. A cylindrical stainless steel post inserted in a pre-drilled bovine tibia with an initial interference fit was taken as an in vitro model to assess the contribution of post fixation to the initial stability of the Total Knee Arthroplasty （TKA） tibial component. Pull-out experiments were carried out for different initial interference fits. Finite Element Models （FEM） using local elastic-plastic properties of the bovine bone were developed for the analysis of the experimental ultimate pull-out force results. At the post/bone interface, Coulomb friction was considered in the FEM calculations with pressure-dependent friction coefficient. It was found that the FEM results of the ultimate force are in good agreement with the experimental results. The analysis of the FEM interfacial stresses indicates that the micro-slip initiation depends on the local bone properties.
