Internal fixator vs external fixator in the management of unstable pelvic ring injuries:A prospective comparative cohort study

BACKGROUND Reconstruction of the pelvic ring anatomy in unstable anterior pelvic ring injuries is a significant step to reduce the mortality rate associated with these injuries efficiently.There is a debate on using either an anterior subcutaneous pelvis internal fixator(INFIX)or an anterior supra-acetabular external fixator(EXFIX)to manage an unstable anterior pelvic ring fracture.AIM To compare the functional and radiological outcomes and complications of INFIX vs EXFIX in managing unstable pelvic ring injuries.METHODS A prospective cohort study included 54 patients with unstable pelvic ring fractures.The patients were divided into two groups;the INFIX group,in which 30 cases were fixed by INFIX,and the EXFIX group,in which 24 patients were treated by EXFIX.The average age in the EXFIX group was 31.17 years(16-57 years),while in the INFIX group,it was 34.5 years(17-53 years).The study included 20(66.7%)males and 10(33.3%)females in the INFIX group and 10(41.7%)males and 14(58.3%)females in the EXFIX group.The radiological outcomes were evaluated using Matta and Tornetta's score,and the functional outcomes using the Majeed score.RESULTS The results revealed a statistically significant difference between both groups(P=0.013)regarding radiological outcomes,according to Matta and Tornetta's score in favor of the INFIX group.Sitting,standing,and walking abilities were measured at a 3-mo follow-up visit using Majeed score modules.It was significantly better among the INFIX group than the EXFIX group in all three modules.At the final follow-up,both groups had no statistically significant difference according to the Majeed score;92.35 in the INFIX group and 90.99 in the EXFIX group(P=0.513).A lower surgical site infection rate was noticed in the INFIX group(P=0.007).CONCLUSION Anterior subcutaneous pelvis INFIX is associated with better radiological outcomes and a lower infection rate than anterior supra-acetabular EXFIX in managing patients with unstable anterior pelvic ring fractures.

The Generalized Search for a Randomly Moving Target

A target is assumed to move randomly on one of two disjoint lines L1 and L2 according to a stochastic process . We have two searchers start looking for the lost target from some points on the two lines separately. Each of the searchers moves continuously along his line in both directions of his starting point. When the target is valuable as a person lost on one of disjoint roads, or is serious as a car filled with explosives which moves randomly in one of disjoint roads, in these cases the search effort must be unrestricted and then we can use more than one searcher. In this paper we show the existence of a search plan such that the expected value of the first meeting time between the target and one of the two searchers is minimum.

Applying the Moisture Availability Index (NTDI) over Vegetated Land in Central Asia: Mongolian Steppe

The intensity of recent droughts and the uncertainty of moisture variability in the context of increasing temperatures and decreasing precipitation have affected the Mongolian grassland. Mongolia typifies the steppe terrain and semiarid to arid continental climate that extends across much of Central Asia. In semi-arid areas like Mongolian steppe, vegetation type and distribution are directly relate to the amount of water that plants can extract from the soil. An index for assessment of moisture availability (ma: defined as the ratio of actual to reference evapotranspiration) was developed, namely NTDI [1] 08D0C9EA79F9BACE118C8200AA004BA90B02000000080000000E0000005F005200650066003400300031003800340037003500380031000000 . NTDI (Normalized Day-Night Surface Temperature Difference Index) is defined as the ratio of the difference between the maximum daytime surface temperature and the minimum nighttime surface temperature, to the difference between the maximum and minimum surface temperatures estimated from meteorological data by applying energy balance equations. A verification study conducted at Liudaogou River Basin of the Loess Plateau, China, indicated the capability of NTDI to estimate ma accurately, (R2=0.97,p<0.001) [1] 08D0C9EA79F9BACE118C8200AA004BA90B02000000080000000E0000005F005200650066003400300031003800340037003500380031000000 . In Bayan Unjuul, Mongolia, application of NTDI during the growing season showed a significant inverse exponential correlation with ma (R2=0.86,p<0.001). This result indicates that the NTDI is potent to be used as a surrogate of moisture availability in steppe terrain of Central Asia.

Influence of the Annealing Temperature on the Thickness and Roughness of La2Ti2O7Thin Films

In this work, the impact of the substrate annealing temperature on the thickness and roughness of La2Ti2O7 thin films was verified. A group of LTO Thin films was grown on Si (100) substrates successfully via pulsed laser deposition technique (PLD) at various annealing temperatures with a constant numbers of pulses and energy per pulse. Scanning Electron Microscope (SEM) and Atomic Force Microscope (AFM) were used to investigate the thickness and roughness of the deposited La2Ti2O7 thin films. The average thickness of the thin films was decreased due to the increasing in the annealing temperature linearly;the maximum thickness was found (231 nm) when LTO thin film deposited at 500°C. The root mean square roughness was increased linearly with increasing the substrate Temperatures. The minimum roughness was found (0.254 nm) when LTO deposited at (500°C). From the obtained results, its clear evidence that the annealing temperature has an influence on the thickness and roughness of the LTO thin films.

Bias and Mean Square Error of Reliability Estimators under the One and Two Random Effects Models: The Effect of Non-Normality

The coefficient of reliability is often estimated from a sample that includes few subjects. It is therefore expected that the precision of this estimate would be low. Measures of precision such as bias and variance depend heavily on the assumption of normality, which may not be tenable in practice. Expressions for the bias and variance of the reliability coefficient in the one and two way random effects models using the multivariate Taylor’s expansion have been obtained under the assumption of normality of the score (Atenafu et al. [1]). In the present paper we derive analytic expressions for the bias and variance, hence the mean square error when the measured responses are not normal under the one-way data layout. Similar expressions are derived in the case of the two-way data layout. We assess the effect of departure from normality on the sample size requirements and on the power of Wald’s test on specified hypotheses. We analyze two data sets, and draw comparisons with results obtained via the Bootstrap methods. It was found that the estimated bias and variance based on the bootstrap method are quite close to those obtained by the first order approximation using the Taylor’s expansion. This is an indication that for the given data sets the approximations are quite adequate.

Analysis of Hospital Mortality Data: The Role of DRG’s

Background: Factors associated with hospital mortality are usually identified and their effects are quantified through statistical modeling. To guide the choice of the best statistical model, we first quantify the predictive ability of each model and then use the CIHI index to see if the hospital policy needs any change. Objectives: The main purpose of this study compared three statistical models in the evaluation of the association between hospital mortality and two risk factors, namely subject’s age at admission and the length of stay, adjusting for the effect of Diagnostic Related Groups (DRG). Methods: We use several SAS procedures to quantify the effect of DRG on the variability in hospital mortality. These procedures are the Logistic Regression model (ignoring the DRG effect), the Generalized Estimating Equation (GEE) that takes into account the within DRG clustering effect (but the within cluster correlation is treated as nuisance parameter), and the Generalized Linear Mixed Model (GLIMMIX). We showed that the GLIMMIX is superior to other models as it properly accounts for the clustering effect of “Diagnostic Related Groups” denoted by DRG. Results: The GLM procedure showed that the proportional contribution of DRG is 16%. All three models showed significant and increasing trend in mortality (P < 0.0001) with respect to the two risk factors (age at admission, and hospital length of stay). It was also clear that the CIHI index was not different under the three models. We re-estimated the models parameters after dichotomizing the risk factors at the optimal cut-off points, using the ROC curve. The parameters estimates and their significance did not change.
