Objective: To report a.ca.e of Ka.a.a.i disea.e (KD) with unusua. a.e a. presenta.ion. Ca.e Report: a.26-da.-old Sa.di boy presented with fever, irrita.ility a.d poor feeding for 3 da.s. There wa. right cervica. lymph node enla.gement. There wa. mild throa. congestion a.d bila.era. congested tympa.ic membra.e. The liver wa. pa.pa.le. Otherwise, there were no more a.norma. physica. findings. La.ora.ory findings revea.ed high erythrocyte sedimenta.ion ra.e a.d positive C-rea.tive protein. The pa.ient wa. a.mitted into a. isola.ion room a.d a.tibiotics were sta.ted. On the 5th da., the condition of the pa.ient deteriora.ed. a.picillin wa. discontinued a.d va.comycin wa. sta.ted till getting the results of the culture sensitivity. On the 7th da., he developed dia.er ra.h a.d fever spikes continued. On the 8th da., the pa.ient developed swelling, redness a.d hotness of the right ha.d. Possibility of sepsis wa. considered a.d the a.ministered a.tibiotics were imipenem a.d va.comycin, while cefota.ime wa. discontinued. On the 12th da., the pa.ient developed a.emia.a.d leukocytosis. Ga.ma.gluta.yl tra.speptida.e wa. high. On the 15th da., the pa.ient sta.ted to develop ma.ulopa.ula. ra.h on the trunk a.d desqua.a.ion of both ha.ds a.d feet with swelling, cra.ked lips, bila.era. conjunctiva. injection a.d unila.era. neck lymph node swelling. So, the infa.t’s condition wa. clinica.ly dia.nosed a. KD. a.pirin wa. sta.ted a.d 2 doses of intra.enous immunoglobulins were given. On the 18th da., the condition of pa.ient improved a.d the fever subsided. Conclusions: The dia.nosis of KD a.ong neona.es is a.clinica. cha.lenge. Dia.nosis is ba.ed on clinica. criteria.a.ter the exclusion of other disea.es presenting with high persistent fever. Ea.ly trea.ment by a.ministering intra.enous immunoglobulins a.d a.pirin ca. help prevent ca.dia. complica.ions. There is a.pressing need to ra.se a.a.eness a.ong pedia.ricia.s a.out this disea.e.

Optimized system for combined production of electricity/green hydrogen for multiple energy pa.hwa.s:a.ca.e study of Egypt

By optima. sizing of a.wind/photovolta.c hybrid renewa.le-energy(RE)system,trimming the surplus ca.a.ity to reduce the fluctua.ions in the electricity supplied to the grid,a.d using it to produce green hydrogen through electrolysis,a.sta.le output with ma.imum possible ca.a.ity fa.tor(CF)is genera.ed to ma.nta.n the electricity grid ***,the trimmed energy is used in a.seconda.y conversion pa.h tha. minimizes the weighted a.era.e cost of the energy genera.ed from the entire *** surplus power-to-ga. conversion a.lows the use of green hydrogen to produce electricity,metha.ol,or a.monia.subject to the resource a.a.la.ility,site cha.a.teristics,a.d fina.cia. *** on robust site selection criteria.the best performa.ce is obta.ned a. two sites:Ra. Gha.eb a.d Minya.a.hieving the lowest energy cost with some va.ia.ce in their *** the Ra. Gha.eb site,the optima.ly sized RE pla.t provided the grid with a.qua.i-stea.y ca.a.ity of 423 MW with a.CF of 80.04%a.d wa. ca.a.le of injecting 2965.8648 GWh throughout the yea. with the lowest cost of 2.4355¢/kWh.a.surplus of 3.9%of the tota. energy produced from the pla.t wa. directed to produce 1922-ton H_(2)/yea.,a.hieving the lowest cost of hydrogen production of$1.9745/kg H_(2).For the other selected site,Minya.the clipped energy is used to produce 3330.47-ton H_(2)/yea. with a. optimized lowest cost of$3.5268/kg H_(2).The difference in hydrogen costs wa. a.tributed to the number of full opera.ing hours of the electrolyser in both *** cost is ma.nly a.fected by the electricity price a.d the electrolyser *** both tending to decrea.e,future foreca.ts show hydrogen cost reductions.
