Background Retinopathy of prematurity(ROP)is generally considered to be more frequent in males than in ***,it is not known whether sex differences in ROP affect all degrees of the condition,are global and have changed as neonatology has *** aimed to conduct a systematic review and meta-analysis of studies addressing sex differences in the risk of developing *** PubMed/MEDLINE and Embase databases were *** frequentist,random-effects risk ratio(RR)and 95%confidence interval(CI)were *** model averaged(BMA)meta-analysis was used to calculate the Bayes factors(Bfs).The Bf10 is the ratio of the probability of the data under the alternative hypothesis(H1)over the probability of the data under the null hypothesis(H0).Results We included 205 studies(867,252 infants).frequentist meta-analysis showed a positive association between male sex and severe ROP(113 studies,RR=1.14,95%CI=1.07–1.22)but no association with any ROP(144 studies,RR=1.00,95%CI=0.96–1.03).BMA showed extreme evidence in favor of H1 for severe ROP(Bf10=71,174)and strong evidence in favor of H0 for any ROP(Bf10=0.05).The association between male sex and severe ROP remained stable over time and was present only in cohorts from countries with a high or high-middle sociodemographic *** Our study confirms the presence of a male disadvantage in severe ROP but not in less severe forms of the *** are variations in the sex differences in ROP,depending on geographical location and sociodemographic level of the countries.

Thermodynamic Analysis and Optimization of flat Plate Solar Collector Using TiO_(2)/Water Nanofluid

To research solar energy's efficiency and environmental benefits,the thermal efficiency,exergy,and entropy of solar collectors were *** experiment involved two glass-topped collectors,fluid transfer tubes,and aluminum heat-absorbing *** wool insulation minimized heat loss.A 0.5% TiO_(2)/Water nanofluid was created using a mechanical and ultrasonic *** showed that solar radiation increased thermal efficiency until midday,reaching 48.48% for water and 51.23% for the *** increasing mass flow rates from 0.0045 kg/s to 0.02 kg/s,thermal efficiency improved from 16.26% to 47.37% for water and from 20.65% to 48.76% for the *** water provided 380 W and 395 W of energy in March and April,while the nanofluid increased it to 395 W and 415 W during these *** flow generated energy,and the Reynolds number raised *** noon exergy efficiency for nanofluids was 50%-55%,compared to 30% for *** noon,the broken exergy measured 877.53 W for the nanofluid and 880.12 W for *** Kirkuk,Iraq,the 0.5% TiO_(2)/Water nanofluid outperformed water in solar collectors.

Metamaterial Antenna is a special class of antennas that uses metamaterial to enhance their *** size affects the quality factor and the radiation loss of the *** antennas can overcome the limitation of bandwidth for small *** learning(ML)model is recently applied to predict antenna *** can be used as an alternative approach to the trial-and-error process of finding proper parameters of the simulated *** accuracy of the prediction depends mainly on the selected *** models combine two or more base models to produce a better-enhanced *** this paper,a weighted average ensemble model is proposed to predict the bandwidth of the Metamaterial *** base models are used namely:Multilayer Perceptron(MLP)and Support Vector Machines(SVM).To calculate the weights for each model,an optimization algorithm is used to find the optimal weights of the *** Group-Based Cooperative Optimizer(DGCO)is employed to search for optimal weight for the base *** proposed model is compared with three based models and the average ensemble *** results show that the proposed model is better than other models and can predict antenna bandwidth efficiently.

Pancreaticoduodenectomy in the Middle East: Achieving optimal results through specialization and standardization

Background: Pancreaticoduodenectomy is a challenging surgical intervention that remains the cornerstone in the treatment of localized peri-ampullary pathologies. The concept of treatment standardization has been well-established in many high-volume centers in the world. Here, we present our experience in pancreaticoduodenectomy from 1994 to 2015. Methods: We performed a retrospective review of the medical charts of patients who underwent pancreaticoduodenectomy at our institution. Data was entered to SPSS statistical software and analyzed. The Mann-Whitney U and fisher’s exact tests were used to report statistical differences between groups. Results: Of the 370 patients who underwent pacreaticoduodenectomy, 300 were analyzed. The 1-, 3-, 5- and 10-year survival rates were 85%, 35%, 15%, and 7%, respectively with a 30-day mortality rate of 5.0%(15 patients). The median age of the patients was 61 (13-84) years, with 193 (64.3%) males and 107 (35.7%) females. The median operative time was 300 (130-570) min. The median postoperative length of hospital stay was 12 (5-76) days. Thirty-two patients required re-laparotomies;10 for pancreatic leak, 7 for biliary leak and 15 for control of bleeding. Seventy-five (25.0%) patients developed pancreatic fistulae. Delayed gastric emptying was present in 31 (10.3%) patients. A significant improvement in surgical outcome was observed in cases done after 2008 which indicates the important role of specialized team in surgical management. Conclusions: The number of patients undergoing pancreaticoduodenectomy has been increasing annually over the past twenty-two years in our institution with results comparable to published series from high-volume centers. Through standardization of surgical techniques and perioperative management carried out by a specialist team, our results continue to improve despite the increasing complexity of cases referred to our unit.

Time-to-enrollment in clinical trials investigating neurological recovery in chronic spinal cord injury:observations from a systematic review and Clinical *** database

Currently,large numbers of clinical trials are performed to investigate different forms of experimental therapy for patients suffering from chronic spinal cord injury(SCI).However,for the enrollment process,there are different views on how the time period between injury and interventions should be ***,we sought to evaluate the impact of time-to-enrollment in chronic SCI clinical trials.A data set comprising 957 clinical studies from clinical *** was downloaded and analyzed focusing on the eligibility criteria for post-injury *** also aggregated individual patient data from nine clinical trials of regenerative interventions for chronic SCI selected by a systematic literature search from 1990 to *** of the studies were assessed and compared by dividing into three groups based on time-to-enrollment(group 1≤12 months,group 2=12-23 months and group 3≥24 months).In Clinical *** registry,445 trials were identified for chronic SCI where 87%(385)were unrestricted in the maximum post-injury time for trial *** systematic literature search,nine studies and 156 patients(group 1=30,group 2=55 and group 3=71)were *** range of time-to-enrollment was 0.5 to 321 months in those *** also observed various degrees of motor and sensory improvement in between three time-to-enrollment *** results indicate that enrolling wide ranges of time-to-enrollment in a group may present imprecise *** trial designs should consider appropriate postinjury time frames to evaluate therapeutic benefit.

The Beneficial Effect of 3-Month-Induction Therapy with Corticosteroids and Mycophenolate Mofetil followed by Maintenance Therapy with Yearly Rituximab Infusions as Sole Maintenance Therapy in Cryptogenic Chronic Hypersensitivity Pneumonitis

Background: The available data on cryptogenic chronic hypersensitivity pneumonitis (ccHP) indicate an inherited predisposition to disease with triggering autoimmune phenomena. Hence, we evaluated prospectively the role of a new autoimmune regimen in treatment of its severe and progressive disease. Patients and Methods: A total of 9 patients were included in the study. They had criteria for ccHP viz. 1) clinical features of cryptogenic progressive restrictive lung disease, 2) high-resolution computed tomographic pulmonary abnormalities, and 3) bronchoalveolar lavage lymphocytosis (>30%). The regimen consisted of an initial induction phase of 3-month Solumedrol 1 g IV daily for 3 days followed by 1 month of Prednisone (P) 60 mg/day to tapered down to discontinuation by 3rd month. They also had received Mycophenolate mofetil (MMf) 1 g twice daily for 3 months. This stage was followed by a maintenance phase of yearly Rituximab infusions (1 g followed by 1 g 2 weeks later). Results: compared to their previous 6 months deterioration;all patients showed significant improvement in their forced vital volume, diffusion capacity for carbon monoxide, 6-minutes-walk after the induction phase (at 3 months) which improved further at 15 months with Rituximab therapy. Conclusion: After 3-month induction therapy with P and MMf;yearly R treatment is a safe, practical and effective long-term therapy for ccHP.

Vanadium pentoxide film for microsecond pulse generation in 1.5-μm region

Q-switched erbium doped fiber laser(EDfL)was passively realized using vanadium pentoxide(V_(2)O_(5))embedded into polyethylene glycol(PEG)film as saturable absorber(SA).The laser could successfully generate stable self-starting pulses when the V_(2)O_(5)film was placed in an EDfL *** operated at 1562.4 nm *** repetition rate can be varied from 91.7 kHz to 128.2 kHz while the pulse width shrank from 10.90μs to 7.81μs with rising pump power from 110.9 mW to 166.5 *** pulse energy recorded was 3.2 nJ at pump power of 166.5 *** results indicate that the saturable absorption of V_(2)O_(5)has promising nonlinear photonic applications especially in fiber laser development for 1.5-μm region.

On the Marchenko System and the Long-time Behavior of Multi-soliton Solutions of the One-dimensional Gross-Pitaevskii Equation

We establish a rigorous well-posedness results for the Marchenko system associated to the scattering theory of the one dimensional Gross-Pitaevskii equation （GP）. Under some assumptions on the scattering data, these well-posedness results provide regular solutions for （GP）. We also construct particular solutions, called N- soliton solutions as an approximate superposition of traveling waves. A study for the asymptotic behaviors of such solutions when t-→±∞ is also made.

Ketamine and midazolam sedation for pediatric gastrointestinal endoscopy in the Arab world

AIM:To evaluate the safety and effectiveness of intravenous ketamine-midazolam sedation during pediatric endoscopy in the Arab ***:A retrospective cohort study of all pediatric endoscopic procedures performed between 2002-2008 at the shared endoscopy suite of King Abdullah University Hospital,Jordan University of Science & Technology,Jordan was *** children were > 1 year old and weighed > 10 kg with American Society of Anesthesiologists class 1 or *** was performed in terms of sedation-related complications(desaturation,respiratory distress,apnea,bradycar-dia,cardiac arrest,emergence reactions),adequacy of sedation,need for sedation reversal,or failure to complete the ***:A total of 301 patients(including 160 males) with a mean age of 9.26 years(range,1-18 years) were *** were premedicated with atropine;and 79.4%(239/301) had effective and uneventful *** 248(82.4%) of the 301 patients received a mean dose of 0.16 mg/kg(range,0.07-0.39) midazolam and 1.06 mg/kg(range,0.31-2.67) ketamine,respectively within the recommended dosage *** maximum midazolam dose was exceeded in 17.6% patients [34 female(f):19 male(M),P = 0.003] and ketamine in 2.7%(3 M:5 f).Maximum midazolam dose was more likely to be exceeded than ketamine(P fficult to sedate and required a 3rd sedative;12(4%) required reversal and 7(2.3%) failed to complete the *** developed apnea,bradycardia,arrest,or emergence ***:Ketamine-midazolam sedation appears safe and effective for diagnostic pediatric gastrointestinal endoscopy in the Arab world for children aged > 1 year and weighing > 10 kg without co-morbidities.

Vertical Transmission of Schistosoma Japonicum in the Rabbit

The aim of the present study was to confirm observations on the vertical transmission of Schistosoma japonicum in the rabbit. S. japonicum- infected pregnant rabbits were used in this study. Perfusion of mother rabbits was done 9weeks after infection in order to obtain worm bur- dens in relation to their initial cercarial ***- schistosoma specific Ig M antibodies in serum samples collected from rabbit kittens were detected by EL *** results showed that gestation period lasted the norm al2 9- *** the exposed mother rabbits became infected with S. *** Ig M antibody OD values were detected in12 out of the6 0 kittens exam ined (2 0 .0 % ) . In group C and A,4 0 .0 % and 17.9% of the kitten were congenitally infected,re- spectively. 18.1% of the kittens born to mothers infected with a single dose of 2 0 0 cercariae per rabbit were positives;this is not significantly different from that obtained for the 6 0 0 dose group (2 2 .2 % ) .Three randomly selected Ig M+ kittens harbored between one and two adult worm s. The livers of these kittens displayed granulomatous lesions. It is concluded that congenital S. japonicum infection does occur in the rabbit and is affected by the m other stage of pregnancy and to a lesser extent by its infection load.
