Assessment of cognitive function in patients with myasthenia gravis

Aim:During the past decade,there has been an increasing interest in the evaluation of cognitive function in myasthenia gravis(MG),neuromuscular transmission disorder caused by acetylcholine receptor ***,the results of previous studies on cognition and MG are inconsistent and *** study aimed to evaluate cognition in patients with mild/moderate grades of ***:This study included 20 patients with MG with a mean age of 28.45±8.89 years and duration of illness of 3.52±1.15 *** was tested using a sensitive battery of psychometric testing(Mini-mental State Examination[MMSE],Stanford-Binet Intelligence Scale 4^(th) edition[SBIS]and Wechsler Memory Scale-Revised[WMS-R])and by recording P300 component of event-related potentials(ERPs),a neurophysiological analog for cognitive ***:Compared with healthy subjects(n=20),patients had lower total scores of cognitive testing(MMSE,SBIS and WMS-R)(P=0.001),higher Beck Depression Inventory 2^(nd) edition scores(P=0.0001)and prolonged latencies(P=0.01)and reduced amplitudes(P=0.001)of P300 component of *** were identified between total scores of cognitive testing and age(r=-0.470,P=0.010),duration of illness(r=-0.788,P=0.001)and depression scores(r=-0.323,P=0.045).Using linear regression analysis and after controlling for age and depression scores,a significant correlation was identified between total scores of cognitive testing and duration of illness(β=-0.305,P=0.045).Conclusion:Patients with mild/moderate MG may have cognitive *** is important to determine prognosis and managing patients.

Effective damping of local low frequency oscillations in power systems integrated with bulk PV generation

High penetration of renewable sources into conventional power systems results in reduction of system inertia and noticeable low-frequency oscillations (LFOs) in the rotor speed of synchronous generators. In this paper, we propose effective damping of LFOs by incorporating a supplementary damping controller with a photovoltaic (PV) generating station, where the parameters of this controller are coordinated optimally with those of a power system stabilizer (PSS). The proposed method is applied to damp local electromechanical modes by studying a system comprising a synchronous generator and a PV station connected to an infinite bus. The PV station is modeled following the instructions of the Western Electricity Coordinating Council. The problem is modeled as an optimization problem, where the damping ratio of the electromechanical modes is designed as the objective function. Constraints including upper and lower limits of decision parameters and damping ratio of other modes are considered by imposing penalties on the objective function. Different optimization algorithms are used to pursue the optimal design, such as political, improved gray wolves and equilibrium optimizers. The results validate the effectiveness of the proposed controller with PSS in damping local modes of oscillations.

Spectral Solutions of Linear and Nonlinear BVPs Using Certain Jacobi Polynomials Generalizing Third- and Fourth-Kinds of Chebyshev Polynomials

This paper is dedicated to implementing and presenting numerical algorithms for solving some linear and nonlinear even-order two-point boundary value *** this purpose,we establish new explicit formulas for the high-order derivatives of certain two classes of Jacobi polynomials in terms of their corresponding Jacobi *** two classes generalize the two celebrated non-symmetric classes of polynomials,namely,Chebyshev polynomials of third-and *** idea of the derivation of such formulas is essentially based on making use of the power series representations and inversion formulas of these classes of *** derived formulas serve in converting the even-order linear differential equations with their boundary conditions into linear systems that can be efficiently ***,and based on the first-order derivatives formula of certain Jacobi polynomials,the operational matrix of derivatives is extracted and employed to present another algorithm to treat both linear and nonlinear two-point boundary value problems based on the application of the collocation *** analysis of the proposed expansions is *** numerical examples are included to demonstrate the validity and applicability of the proposed algorithms.

Explicit Shifted Second-kind Chebyshev Spectral Treatment for Fractional Riccati Differential Equation

This paper is confined to analyzing and implementing new spectral solutions of the fractional Riccati differential equation based on the application of the spectral tau method.A new explicit formula for approximating the fractional derivatives of shifted Chebyshev polynomials of the second kind in terms of their original polynomials is *** formula is expressed in terms of a certain terminating hypergeometric function of the type_(4)F_(3)(1).This hypergeometric function is reduced in case of the integer case into a certain terminating hypergeometric function of the type 3 F 2(1)which can be summed with the aid of Watson’s *** illustrative examples are presented to ensure the applicability and accuracy of the proposed algorithm.

ON THE COEFFICIENTS OF DIFFERENTIATED EXPANSIONS AND DERIVATIVES OF CHEBYSHEV POLYNOMIALS OF THE THIRD AND FOURTH KINDS

Two new analytical formulae expressing explicitly the derivatives of Chebyshev polynomials of the third and fourth kinds of any degree and of any order in terms of Chebyshev polynomials of the third and fourth kinds themselves are proved. Two other explicit formulae which express the third and fourth kinds Chebyshev expansion coefficients of a general-order derivative of an infinitely differentiable function in terms of their original expansion coefficients are also given. Two new reduction formulae for summing some terminating hypergeometric functions of unit argument are deduced. As an application of how to use Chebyshev polynomials of the third and fourth kinds for solving high-order boundary value problems, two spectral Galerkin numerical solutions of a special linear twelfth-order boundary value problem are given.

Comparative Study between the Benefit of Pre-Emptive Pregabalin and Gabapentin on Acute Postoperative Pain for Elective Gynecological Surgery

Gabapentin, and pregabalin had been used in analgesic field some studies. This double blind randomized clinical trial was conducted to evaluate the pre-emptive use of gabapentin 900 mg and pregabalin 300 mg in reducing postoperative pain. Methods: A total number of 75 patients undergoing lower gynecological procedures were prospectively randomized, into three groups (group A, B and C), each group including 25 patients with total 75 patients. Pregabalin, gabapentin or placebo, the pain was assessed on a visual analogue scale (VAS) at 0, 6, 12, 18 & 24 hours postoperatively. Duration of effective analgesia was documented, and administration of extra analgesic doses of meperedine required in the first 24 hours. Results: Patients in the gabapentin or pregabalin had significantly lower VAS scores at 6, 12, 18 and 24 hours, than those in the placebo group. As for rescue analgesia with mepredine consumed in the gabapentin, and pregabalin were significantly less than in the placebo. As for the complications, both drugs had increased incidence of nausea, vomiting and dizziness postoperatively, while no significance was found between all groups as regard hypotension, bradycardia and shivering. Conclusion: Preoperative use of pregabalin or gabapentin provides comparable but significant prolonged postoperative analgesia, less nausea and vomiting compared to placebo after gynecological surgeries. However, it was associated with increased incidence of postoperative dizziness.

Characterization of Crude Oil, Asphaltenes Based on Bismuth (Bi), Thorium (Th) and Uranium (U) Levels: Potential Environmental Impact

The actinides (Th, U) and bismuth exist at ultra-trace levels in petroleum and have rarely been reported to characterize combustible fuels. However, high-performance mass spectrometry coupled with plasma technology has succeeded in reaching detectable limits in the ng/L range, and has thus made such characterization possible. Crude oil is used as sludge for certain agricultural purposes;and asphaltenes are converted to by-products for constructing car parks, pavements, roofing material and thoroughfares. Our study uncovers an area that has not been previously explored and we examine the potential ecotoxicology of bismuth, thorium and uranium in this context. Asphaltene derivatives were separated from crude oil samples via multiple-stage soxhlet extraction. The virgin crude oil and isolated asphaltenes were investigated for Bi, Th and U content using a high-powered hyphenated facility. A Perkin Elmer SCIEX DRC-e ICP coupled to a quadrupole mass spectrometer was employed to detect the elements of interest. The levels of these elements occurred in the mg/L (ppm) and μg/L (ppb) ranges and were compared for crude oil and asphaltenes. The data showed higher levels of the detected elements in the asphaltene phase. The feasibility of applying the experimental results as a fingerprinting tool for provenancing crude oil and asphaltenes was examined. The results are discussed from the perspective of bismuth and actinide pollution and its long-term impact on sustainable development.

Uneven distribution of minor metals and migration of isotopes in polymeric material （polypropylene） - originating under certain physical and chemical conditions-could possibly affect the stability and bio-compatibility of such material. Unusually high levels of embedded surface metal isotopes from migration effects could affect studies such as tissue engineering and biospecific adhesion of cells to polymeric surfaces. There is, therefore, a general need to know the distribution of metal isotopes in such polymeric materials. We have developed an ultrasensitive technique for assessing the isotopic distribution in polymer matrices, and studying migration of metal isotopes. The technique uses laser ablation linked to an ICP-MS instrument. It is semi-quantitative and capable of high-resolution detection over a wide range of elemental levels. Polymers usually contain catalytic residues and other minor metal impurities. Some of the isotopes of these metals migrate to the surface, while others remain embedded deep in the polymeric product. Such unwanted metallic residues and isotopes could be a potential hazard, and ablative laser technology has the ability to study homogeneity of such distributions in the polymer matrix. The aim of this paper, therefore, is to explore the potential of our method for studying isotope migration using suitable polypropylene samples.

Prevalence and Correlates of Overweight and Obesity among School Children in Sohag, Egypt

Background: Continually rising prevalence of obesity and overweight in children and adolescents is a major public health concern. This is due to its various and serious health hazards on one side and its preventable nature on the other side. This study aims at identification of prevalence of overweight and obesity and its risk factors among children between 6 - 14 years of age in Sohag, Egypt. Method: A cross sectional study was conducted on 711 school children between 6 - 14 years in four months’ period from February to May 2016. Weight and height of the studied children were measured to calculate BMI, which was compared with standard Egyptian growth charts for determining overweight and obesity. Then a questionnaire containing data about socio demographic, parental obesity, early life and dietary risk factors was completed by the children’s parents. Result: Out of 711 studied children, 117 (16.5%) were overweight and 104 (14.6%) were obese. Residing in urban area, having an obese parent or both of them, low level of maternal education, being on formula feeding during early life and lack of fresh vegetables and fruits in diets were significant risk factors for overweight and obesity in the studied children (P value < 0.05). Conclusion: Improperly educated mothers together with sedentary life style manifested in obesity among the parents and lack of sport practice among children constituted the majors risk factors for childhood obesity. This emphasizes the need for increased health awareness of the community about the importance of proper nutrition and physical activity.
