Hyperprolactinemia in Hospitals in Lomé: Diagnostic and Therapeutic Aspects

Introduction. Hyperprolactinemia represents a supraphysiological secretion of prolactin. In clinical practice, it is the most frequently encountered anterior pituitary disorder. However, its real prevalence is little known in Africa. The purpose of this study is to list all cases of hyperprolactinemia over the past four years and to make an inventory of the various etiologies found and their management. methodology. This is a cross-sectional descriptive study on the files of patients who came for consultation in the internal medicine and endocrinology department with hyperprolactinemia retrospectively collected from January 2017 to December 2020. Included were patients followed or whose the diagnosis of hyperprolactinemia was established in endocrinological consultation during the study period. Results. This study recorded 26 cases of hyperprolactinemia. The female sex represented 73.08% (sex ratio m/F 0.37) and the average age was 33.92 years. The 40 - 50 age group was the most represented with 30.77%. In women, galactorrhea accounted for 73.07%, amenorrhea 57.69% of clinical pictures and infertility 23.08%. In men, infertility accounted for 7.69% of presentations, gynecomastia 11.54% and erectile dysfunction 15.38%. The duration of evolution of the signs was 4 months in 23.08% of the patients. The mean prolactinemia was 702.58 ng/ml. CT was performed in 80.85% of patients. The main etiologies were pituitary adenomas (77%). All our patients were treated with dopaminergic agonists. Cabergoline was prescribed in 77% of patients. The evolution was favorable in 96.25% of patients. Conclusion. Hyperprolactinemia is a pathology that exists in our regions. The main etiology remains pituitary adenomas and treatment with cabergoline.

Compliance with Oral Anti-Diabetic Treatment in Lome

Objective: The objective of this study was to assess adherence to antidiabetic therapy in type II diabetic patients and factors associated with Lomé. method: This was a descriptive observational study conducted among diabetics in diabetes care centers from may 1, 2020 to October 1, 2020, for a period of 6 months. Results: A total of 300 patients were questioned. The mean age was 56.61 years (standard deviation = 11.7). The sex ratio was 0.6. All the patients were on drug treatment, 83.4% of whom were on metformin and 37.5% on Glibenclamide. In 58.3% of the cases, the patients were on dual therapy and 96.2% had an abovenormal glycated hemoglobin value. There was a medication adherence problem with poor adherence in 30.7%. Being aged 50 and over (ORC = 1.78, p = 0.048), playing sports (ORR = 1.87, p = 0.046), have reaching a higher level of education (ORR = 5.78, p = 0.005) significantly increased the chances of patients having a good/minimal problem of adherence. Conclusion: Diabetes affects in Togo, more women than men with a predominance of those aged 50 and over. Adherence to therapy remains a challenge.

To the editor:Allogeneic hemopoietic stem cell transplantation(allo-HSCT)represents the only long-term survival treatment choice for patients with myelodysplastic syndromes(mDS)and mDS/myeloproliferative neoplasm(mPN).Unfortunately,post-hemopoietic stem cell transplantation(HSCT)relapse remains a cause of treatment failure and the results of salvage treatments are *** better conditioning regimens is urgently *** studies have shown synergistic antileukemic effects between decitabine(DEC)and idarubicin(IDA).[1]In an attempt to design a conditioning strategy with very low toxicity but considerable myelosuppressive activity and potential immune-enhancing effects for patients with high-risk mDS and mDS/mPN,we combined DEC and IDA with busulfan,cyclophosphamide,andudarabine for a modied myeloablative regimen in this prospective,multicenter cohort study(hereafter referred to as the“DEC/IDA study”).

目的观察膜转运体megalin、Cubilin在发生发育小鼠肾内的分布,从而揭示它们在肾发生发育中相应功能分化的时空规律。方法C57BL/6小鼠按雌雄1∶1合笼饲养,并于胚(胎)龄(ed)E10、12、14、16、18 d,及生后日龄(Pd)P1、3、7、14、28、42 d,分离胎鼠、仔鼠或成鼠的肾脏。应用改良的块染技术对不同发育阶段的肾脏组织进行整体固定及染色,分别进行常规石蜡包埋,制成连续的石蜡切片(5μm)。选取连续切片中目标切片,采用免疫组织化学技术和免疫荧光技术对不同发育阶段的小鼠肾脏膜转运体megalin和Cubilin进行定位,观察其在不同发育阶段的表达情况。结果megalin和Cubilin在E10 d的中肾管上皮细胞开始有表达,E12 d时表达于肾脏输尿管芽上皮细胞的近腔面游离缘,二者主要以共表达模式存在。E14 d以后在发生发育小鼠肾脏的“逗号”小体、“S”型小体上皮细胞的游离缘呈弱表达,于肾脏近端小管的上皮细胞游离缘表达明显,偶尔可见在未发育成熟的血管球有弱表达。megalin和Cubilin在细段、远端小管、集合管系统以及间质部分都未见表达。结论膜转运体megalin和Cubilin在中肾管、输尿管芽、“逗号”小体、“S”型小体,以及近端小管的依次表达规律提示小鼠肾脏在中肾形成时期就开始有蛋白或多肽的转运功能。此外,二者稳定地出现在较为成熟的近端小管,较弱地出现在未成熟近端小管,表明其重吸收蛋白质的功能与近端小管结构的成熟度有关。

A multi-shot target assembly and automatic alignment procedure for laser–plasma proton acceleration at high repetition rate are *** assembly is based on a multi-target rotating wheel capable of hosting more than 5000 targets,mounted on a 3D motorized stage to allow rapid replenishment and alignment of the target material between laser *** automatic alignment procedure consists of a detailed mapping of the impact positions at the target surface prior to the irradiation that ensures stable operation of the target,which alongside the purpose-built design of the target wheel,enables operation at rates up to 10 *** and continuous laser-driven proton acceleration at 10 Hz is demonstrated,with observed cut-off energy stability about 15%.

microstructure and mechanical Properties of X80 Pipeline Steel Joints by Friction Stir Welding Under Various Cooling Conditions

X80 pipeline steel plates were friction stir welded(FSW)under air,water,liquid CO2+water,and liquid CO2 cooling conditions,producing defect-free *** microstructural evolution and mechanical properties of these FSW joints were *** granular bainite was observed in the nugget zone(NZ)under air cooling,and lath bainite and lath martensite increased signifi cantly as the cooling medium temperature *** particular,under the liquid CO2 cooling condition,a dual phase structure of lath martensite and fi ne ferrite appeared in the *** to the case under air cooling,a strong shear texture was identifi ed in the NZs under other rapid cooling conditions,because the partial deformation at elevated temperature was retained through higher cooling *** liquid CO2 cooling,the highest transverse tensile strength and elongation of the joint reached 92%and 82%of those of the basal metal(Bm),respectively,due to the weak tempering softening.A maximum impact energy of up to 93%of that of the Bm was obtained in the NZ under liquid CO2 cooling,which was attributed to the operation of the dual phase of lath martensite and fi ne ferrite.

metformin(met)is a drug developed for the treatment of patients with typeⅡ ***,met estimation in pharmaceutical formulations and human fluids has gained a growing *** extend requisite data that can be used to assessment of met quantitatively based on charge-transfer(CT)complexation,the present study describes the synthesis and characterization of CT complexes that formed between drug met and the organicπ-acceptors picric acid(PA),chloranilic acid(CLA),chloranil(CHL),7,7',8,8'-tetracyanoquinodimethane(TCNQ),and dichlorodicyanobenzoquinone(DDQ).The properties of the formed CT complexes were investigated by elemental,spectral(UV-visible,IR,and Raman spectroscopies),thermal(TG)and morphological(SEm)*** results indicated that the complexation of met with either PA or CLA acceptors occurs through proton transfer interaction,whereas its complexation with CHL,TCNQ,or DDQ acceptors occurs through n→π*interaction.

Phase Selection and microhardness of Directionally Solidified AlCoCrFeNi_(2.1) Eutectic High-Entropy Alloy

In the present work,the solidification behaviors and microhardness of directionally solidified AlCoCrFeNi_(2.1) eutectic highentropy alloy(EHEA)obtained at different growth velocities are *** microstructure of the as-cast AlCoCrFeNi_(2.1) EHEA is composed of bulky dendrites(NiAl phase)and lamellar eutectic structures,indicating that the actual composition of the alloy slightly deviates from the eutectic ***,it is interesting to observe that the full lamellar structure of this alloy is obtained through directional *** order to explain this phenomenon,the maximum interface temperature criterion and the interface response function(IRF)theory are applied to calculate the velocity range of the transition from the prima.y phase to the eutectic,which is 1.2–2×10^(4)μm/***,microhardness is one of the important parameters to measure the mechanical properties of ***,the microhardness test is performed,and the test result indicates that the microhardness(HV)increased with increasing growth velocity(V)or decreased with increasing lamellar spacing(λ).The dependences ofλand HV on V are determined by using a linear regression *** relationships between theλ,V and HV are given as:λ=11.62V^(-0.48),HV=305.5V 0.02 and HV=328.1λ^(0.04),*** microhardness of the AlCoCrFeNi_(2.1) EHEA increases from 312.38 HV to 329.54 HV with the increase in growth velocity(5–200μm/s).Thus,directional solidification is an effective method to improve the mechanical properties of alloys.

Cervical Thoracic Necrotizing Fasciitis with the mamma.y Gland Spread of Odontogenic Origin

Necrotizing fasciitis is an uncommon infection, but potentially lethal, especially when associated with systemic disorders such as diabetes. We report the case of a 35-year-old female with uncontrolled diabetes mellitus, presenting with edema of the neck, facial and left mamma.y gland, secondary to untreated dental infection, progressing to a full-blown necrotizing fasciitis in a short period of time with sepsis. The patient was managed with aggressive multidisciplinary medical and surgical treatment. Despite the technologic advances in diagnosis and treatment, complications still result with astounding high mortality. Clearly, the morbidity associated to this infection, even in diabetic patients, can be minimized if an early diagnosis and effective debridement are done.
