Prostate Cancer in the Thies Region, Senegal: Epidemiological, Diagnostic and Therapeutic Aspects

Introduction: Prostate cancer is the leading urological cancer. It is the most common cancer in men over 50. Objective: To determine the epidemiological, diagnostic and therapeutic characteristics of prostate cancer in hospitals in the Thiès region. Patients and Methods: We conducted a descriptive study from January 1st, 2015 to December 31st, 2020. We included all cases of primary prostate cancer confirmed on histology. Results: We collected data on 318 cases of primary prostate cancer during the study period. Mean patient age was 72.7 years (Range: 49;94 years). Family history of prostate cancer was found in 22 patients (6.91%). The average consultation time was 18.6 months. The circumstances of discovery were dominated by obstructive voiding disorders (97.16%). Digital Rectal examination was suggestive in 55.40% of patients. PSA level was above 20 ng/ml in 76.7% of patients. Prostatic adenocarcinoma was the only histological type (100%). Localized cancer represented 7.2% and locally advanced cancer occurred in 36.5% of cases, while metastatic cancer accounted for 56.3%. Radical prostatectomy was performed in 3.18% of cases. Mortality rate was estimated at 8.50% after 1 year. Conclusion: Prostate cancer is the leading urological cancer in the Thies region. It is characterized by the predominance of locally advanced and metastatic forms.

Humoral Response and Tolerance of Vaccination against SARS-CoV-2 in Adults Senegalese Patients Undergoing Hemodialysis: A Multicenter Prospective Study

Introduction: Following the COVID-19 pandemic, vaccination has been proposed in several countries as the main preventive measure despite very limited data, particularly in dialysis patients. We conducted this study to assess the immunological response to vaccination in Senegalese hemodialysis patients. Patients and Methods: We conducted a prospective study, in two dialysis centers in Dakar from March 30th to August 30th, 2021 including patients on hemodialysis for >6 months, vaccinated against SARS-CoV-2 according to the vaccination schedule recommended by WHO. A vaccine response was considered positive when seroconversion was observed after one dose of vaccine. The clinical efficacy of immunization was defined as the absence of new COVID-19 infection in patients who received a complete vaccination. Results: Among the 81 patients included in the study, 7.4% had anti-Spike IgM antibodies before their first vaccination. Seroprevalence of IgM antibodies was 38.3% one month after the first vaccine dose (at M1) and 8.6% one month after the second dose (at M4). Anti-Spike IgG antibodies were present in 40.3% of patients before vaccination, in 90.1% at M1, and in 59.7% at M4. Among patients previously infected with SARS-CoV-2, 10.2% had IgM antibodies at M0, 31.6% at M1, and 10.5% at M4 post-vaccination. Similarly, seroprevalences of IgG antibodies in this subgroup were 31.5%, 61.3%, and 50.0% respectively at M0, M1, and M4 post-vaccination. A comparison of seroconversion rates between M0 and M4 showed significant differences only for IgG in COVID-19 naive patients. Mean duration in dialysis and the existence of previous COVID-19 infection were associated with patients’ vaccinal response after the two doses. Age, gender and the use of immunosuppressive treatment did not influence post-vaccinal antibody production. Conclusion: Vaccination against COVID-19 in Senegalese hemodialysis patients induced a low seroconversion rate but it was well tolerated. M

Male Infertility as a Circumstance of Discovery of Adult Cryptorchidism: A Retrospective Analysis from the Region of Thies

Introduction: The aim of this work is to study the clinical aspects and the impact of cryptorchidism on male infertility in the region of Thies. Patients and methods: This is a retrospective study involving 28 patients followed up at the Saint Jean de Dieu, Barthimée and Tivaouane hospitals of the region of Thies between January 2007 and December 2016. Results: Among the 223 patients followed up for the undescended testicle, 13% (n = 28) were adults or at least 15 years old. 71% (n = 20) were married. Fourteen patients were followed up for primary infertility lasting for a period of time averaging 7 years. The mean age of the patients was 28 years (range: 17 years and 45 years). Cryptorchidism was unilateral in 82% (n = 23) and bilateral in 18% (n = 5). The mean delay of consultation varied from 1 to 13 years. The main reason for consultation was couple infertility (50%). The semen analysis was requested in all our patients. The levels of FSH, LH and testosterone were measured by immunoassay for the patients with azoospermia (n = 9). We noticed 2 cases of hypotestosteronemia and 3 cases of increased FSH serum level. Computed Tomography scans (CT) were performed in 28.5% of patients (n = 8) and ectopic testicles were found very high in the inguinal area in 5 cases. The open inguinal approach was used. In perioperative period, the testicle was found, high in the inguinal region in 5 patients (17.8% of the cases) and low in inguinal region in 19 patients (67.8%). In 4 cases the testicle was in abdominal position. The testicle was atrophied in 53.5% of the patients (n = 15) and 8 patients presented testicular hypotrophy. An orchidectomy was performed in 1 patient in whom it was impossible to lower the atrophied testicle. The postoperative period was uneventful and simple. The mean follow-up duration after the operation was 36 months (12 - 60 months).

Objective: To evaluate the clinical and therapeutic aspects of male subfertility in the Region of Thies. Patients and methods: This is a retrospective and analytical study involving patients followed for subfertility over a period of 4 years from January 2013 to November 2017 at the level of 3 health structures in the region of Thies. Results: During the period, we collected 201 patients. The average age was 38 ± 8.4 years with a greater distribution in the age group 30-39 years. Primary subfertility was predominant with 81.1% of cases. The average duration was 5 years. We found a history of urethritis (4%) and orchiepididymitis (2.5%). Thirty-three percent of patients presented a varicocele (67 cases). Cryptorchidism was recorded in 2% of cases, testicular hypotrophy in 18.4% and testicular atrophy in 1.5%. The spermogram identified oligospermia-like abnormalities in 40.8% of cases, azoospermia 22.4%, and hypospermia 4%. For the qualitative abnormalities, we recorded cases of asthenospermia in 60.2% of the cases or 121 patients, a necrospermia 58.2% and a teratospermia 20.4%. A combination of these abnormalities was also reported as astheno-necrospermia in 19.4% of cases, oligo-astheno-necrospermia 14.4%, oligo-asthenotera-necrospermia 10% and oligo astero-teratospermia in 2.5%. An assessment of FSH, LH and testosterone was performed in 5.9% of the cases, or 12 patients. Varicocelectomy was performed in 68.4% of cases 19 patients, and (medically) assisted procreation in 2 patients. We recorded 13 cases of pregnancy. Conclusion: Male hypofertility is a real problem of management, and requires a particular approach.

Intra-Dialytique Hypotension: Prevalence and Associated Factors in 2 Haemodialysis Centres in Senegal

Introduction: This study aimed to assess the prevalence of Intra-dialytic hypotension (IDH) according to the European Best Practice Guidelines (EBPG) definition in relation to the number of haemodialysis sessions and the number of chronic haemodialysis patients and to identify its associated factors. Patients and Methods: We conducted an observational, multicentre and looking-forward study of descriptive and analytical purposes over a 4-week period. The study included all patients with regular chronic haemodialysis with at least two sessions per week and a duration of 180 minutes, who consented to participate in the study and were over 15 years of age. Data collection was done with pre-established forms. The following data were collected: blood pressure before each session, at 30, 60, 120, 180 and 240 minutes of each session;socio-demographic data;dialysis data;clinical, paraclinical, therapeutic data and nursing interventions. Results: The mean age of the patients was 43.84 ± 12.10 years. Among 568 haemodialysis sessions recorded in 50 patients, IDH was noted in 12 haemodialysis sessions, representing a prevalence of 2.11%. Fatigue was found in 5 sessions with IDH episodes (41.66%) followed by yawning (25%), nausea ± vomiting (16.66%) and cramps (16.66%). As nursing interventions, Trendelenburg position and normal saline administration were performed in all IDH episodes. High blood pressure, inter-dialytic weight gain greater than 3 kg, Uf/H > 10 ml/kg/H, anaemia and hypoalbuminaemia were associated with the occurrence of IDH. Conclusion: The prevalence of IDH according to the EBPG definition is low. However, it is an important cause of morbidity and mortality, especially cardiovascular involvement, and the factors associated with its presence have been clearly identified.

Statural and Weight Growth of Low Birth Weight at 9 Months

Introduction: Low birth weight (LBW) is defined as a birth weight 2500 g at birth, regardless of the term of pregnancy. The objective of this study is to evaluate their stature and weight growth from 0 to 9 months of corrected age (CA) in Senegal. Methodology: This is a prospective, descriptive, and analytical cohort follow-up up to 9 months of CA including all live newborns of LBW hospitalized and followed up from 01 August 2019 to 31 May 2020. World Health Organization growth charts were used to assess stature and weight growth. Results: During the study 136 LBW newborns were included. The mean gestational age was 32 weeks of amenorrhea. At discharge, 46 children (33.82%) were exclusively breastfed. At birth, the mean weight was 1487 g (3rd-10th percentile) and the mean height was 41.52 cm (10th-25th percentile). At 9 months of CA, the mean weight was 8119 g (median) and the mean height was 74 cm (median). The children had achieved satisfactory growth in weight (84%) and height (89%). At 9 months of CA, 27% of the children were behind in one of the four areas of psychomotor development. Conclusion: At the end of 9 months of CA, stature and weighted growth was normal.

Abrikossof’s Tumor: Report of a Case

The granular cell tumor of the breast is a rare tumor and a usually benign disease that appears clinically and radiologically like a malign tumor. We report a case of GCT in a woman of 41 years, who present a clinically and radiologically suspect later. The anatomopathological examination showed the benign tumor proliferation and a large tumorectomy was realized. The immunostaining with PS100 and NSE protein confirm the nature of the tumor.

Urban areas house vegetation cover in several forms, fulfilling several ecological functions like thermal regulator, biodiversity, air quality, etc. However, their extent is often not very well known, especially in African cities, making it sometimes difficult to assess their real impact on the urban ecosystem functioning. This work aims to analyse the capacity of satellite sensors for mapping vegetation and wetlands in urban areas. The data produced by the MSI sensors of Sentinel 2 and OLI of Landsat-8 are used to identify and map the vegetation cover in the Dakar region through a supervised classification with the Support Vector Machine (SVM) algorithm. The results show that it is sometimes not very easy to analyse urban vegetation with high spatial resolution images (HRS) resulting from the configuration of the vegetation in an urban environment, sometimes characterized by isolated trees or small green spaces. This explains why Sentinel-2 data which spatial resolution of 10 meters gives a better result compared to Landsat-8 data which is 30 meters. However, a good rendering is noted for the vegetation around the wetlands area for the two sensors resulting from the high density and the size of the vegetated perimeters in this part of the capital. Overall, there is an underestimation of urban vegetation cover, particularly for Landsat-8. The use of very high spatial resolution images could be necessary to better assess the potential of satellite data for monitoring urban vegetation in Sahelian context.

Isolation of Iron Bacteria from Washing Sludge Filters Kolda (Senegal) Water Treatment Station and Study of the Kinetics of Biological Degradation of the Iron (II)

In the present work, the treatment of synthetic waters doped with iron (II) has been studied. The treatment mechanism used in this study is the biological oxidation which consists, in test tubes, of bringing bacteria isolated on Petri dishes into contact with water containing divalent iron (II). These de-ironing bacteria (non-specific bacteria) are used to carry out laboratory biological oxidation experiments on iron (II) under different pH conditions (pH = 3.4 - 3.6, pH = 7.3 - 7.5 and pH = 9.8 - 10) and variable concentration of iron (II). Biological treatment trials included different concentrations of iron during time intervals of a day. Examination of the elimination kinetics of Iron (II) indicates a removal rate of 59.453% for an initial iron concentration in the synthetic solution of 1 mg·L−1 at basic pH (pH = 9.8 - 10). Therefore, the degradation of divalent iron by this method seems to be quite effective, but it should be noted that biological nitrification is impaired by the presence of high iron concentrations above 5 mg·L−1.

Comparative Effects of Natural and Synthetic Fungicides on the Pink Root Disease of Onion (<i>Allium cepa </i>L.), in Nursery

Onion is an important vegetable in Senegal horticulture, where it occupies the first place. However in the greatest areas production like the Gandiolais, in the North of Senegal, Pink root disease caused by a soil born fungus Pyrenochaeta terrestris (Hansen) Gorentz, Walker and Larson affects its productivity especially in nursery. On soil sterilized and inoculated by the fungus, we tested 5 natural fungicides and 2 synthetic fungicides, to measure their effectiveness on the Pink root disease on onion, in nursery. The device used is a split plot with 4 repetitions and 8 treatments. The measured variables are the emergence rates, the survival rates and the infestation rates. Our experimentations have shown that, for the emergence rates, there is no significant difference between thiophanate-methyl (Pelt 44), Carbendazim (Bavistine), Azadirachta indica A. Juss and Carica papaya L. It is the same between C. papaya and Moringa oleifera Lam, between M. oleifera and Cassia alata L. It is also the same between C. alata and Crotolaria retusa L. Regarding the survival rates, there is no significant difference between thiophanate-methyl, Azadirachta indica, Carbendazim, C. papaya and M. oleifera. The same is true between M. oleifera and Cassia alata, and between C. alata and C. retusa. Concerning the infestation rates, apart from the control check, all other treatments show no difference.
