Mechanical Complications after Limb Osteosynthesis: Analysis of Etiologic Factors in 42 Cases

The purpose of this study is to identify the causes of mechanical complications of osteosynthesis of members to prevent and improve the results of our surgical treatments. Patients and Method: Retrospective study including all patients diagnosed with mechanical complications after osteosynthesis of the limbs and who received medical follow-up. 42 patients with an average age of 42.2 years, with a male predominance (36 cases) were selected. These complications predominated in the femur with 26 cases. The implants involved were predominantly screwed plates with 25 cases. We investigated and analyzed the different types of dismantling of material as well as the etiological factors that may be at the origin of these disassembly. Results: After a mean follow-up of 15.47 months, four types of complications were identified: material expulsion (20 cases), migration (3 cases), incurvation (7 cases), fracture of material (12 cases). 4 types of factors causing dismantling have been identified: the factors related to the patient, factors related to fracture, the factors related to the implant and factors related to the surgeon. Conclusion: Disassembly of orthopedic implants is most often of multifactorial origin. These factors may act in isolation but are most often entangled in the patient. Proper preoperative planning can help prevent them. However, unexpected stresses of the implant and certain factors related to the ground remain difficult to control.

Reconstruction of Post-Traumatic Bone Loss (BL) Members: Our Experience about 27 Cases

The purpose of this study is to evaluate our results of treatment of post-traumatic bone defect of members through various bone reconstruction techniques. Patients and method: It is a retrospective study continues from January 2008 to December 2012. It involved 27 patients who had bone loss following trauma members. The average age of patients was 36.2 years. We noted a male predominance (22 men). The causes of injuries were dominated by accidents of public roads (12 cases) and ballistic trauma (14 cases). We noted one case of work accident. The BL was localized in 10 cases the tibia, femur in 4 cases, 8 cases in the humerus, 2 cases in the fibula, 1 case was located in the olecranon, 1 case at the distal radius and 1 case to the ball. All lesions were open fractures. After a mean of support for 3 days, we performed emergency around 19 associated with internal fixation (10 cases external fixation, internal fixation 9 cases), 8 single around. Bone reconstruction was performed secondarily in the majority of cases (24 cases). Only 4 reconstructions were performed in emergency. Reconstruction techniques were dominated by conventional grafts (24 cases) associated or not with the technique of induced membrane. Bone reconstruction techniques we used were as follows: 24 cases of traditional autografts and 3 cases of intertibio-fibular graft. After an average decline of 43.6 months, our patients were evaluated clinically (axis and limb length) and radiological (bone healing). Results: bone healing was achieved in 22 cases after a mean of 3.8 surgeries and an average of 14.8 months. Conclusion: the main principles governing the reconstruction of a defect are pre healing associated infection and repair soft tissue whenever it can do. The current trend is to hybridization of the reconstruction process of the great losses of substance by developing strategies based on both the deliberate shortening.

We report an unusual manifestation of nontuberculous mycobacterial infection characterized by a giant bursitis on wrist and multiple tenosynovitis with many rice bodies formations. The clinical and radiological examinations are neither rather sensitive nor rather specific. The nuclear imagery of rice bodies formations provides elements of guidance. Cause of absence of the germ isolation, diagnosis was retained on probability items based on a suspicion of arguments beam: clinical, biological, bacteriological and histological. The patient was treated with medical and surgical procedure and provided a satisfactory evolution. At follow-up of 15 months, there were no clinical signs of local recurrence. Through a literature review, the problem of diagnosis of certainty will be discussed.

Objective:To evaluate the attitudes and practices of oncosexology in the management of breast and prostate cancer in Côte d’***:A cross-sectional,multicenter,descriptive and analytical survey carried out over 02 months from 1 November to 31 December 2022,among doctors involved in the management of breast and prostate cancer in Côte d’***:78 physicians on 114 participated in the study,with a participation rate of 79.5%.Only one doctor discussed the sexual risks associated with cancer with all his patients and 7.7%of doctors said they never broached questions of sexuality with their *** approach to sexuality was strongly associated with occupation(p=0.002).A request for oncosexological care was initiated by the patient and the partner respectively in 3.8%and 25.6%of *** 5.1%of physicians claimed to have received training in *** 92.3%of cases,doctors would like to have training on this ***:On the questions relating to sexuality,Physicians are not addressing sexuality with their patients diagnosed with breast or prostate *** lack of training in this area appears to be the main reason for this lack of communication and onco-sexological care.

Metastatic(Advanced)Prostate Cancer Resistant to Castration:The Time before Resistance Development and Therapeutic Measures Undertaken in Cote d’Ivoire

Objective: Contribute to improving care for prostate cancer patients in Cote d’Ivoire by identifying the average time before the development of the hormone-resistance and inherent therapeutic strategies. Patients and Method: It was a retro and prospective study over three years performed at the University Teaching Hospital of Treichville in Abidjan. 84 patients with metastatic prostate adenocarcinoma and resistant to hormone therapy were involved in the study. The analysis of clinical records enabled us to integrate different parameters in our study. Results: Our patients were relatively young (an average age of 61 years) and most of them belonged to the middle socio-economic class (50%). The resistance occurred within an average period of 9 months. The mixing of Docetaxel and Prednisone chemotherapy was the most used method of treatment (70% of cases). However, few patients (9.5%) received Abiraterone Acetate. Digestive and hematologic toxicities were noticed but they could be controlled by inherent treatment or disappear spontaneously. After nine months of treatment, 54.24% of patients who received specific treatment were alive, but high rates of mortality were observed as for patients who were only treated symptomatically. Conclusion: The relatively short average time before the development of hormone-resistance shows the aggressiveness of prostate adenocarcinoma from black Africans. The main therapeutic standard in our context remains Taxane-based chemotherapy with a good and fair tolerance.

The minority carrier’s recombination velocity at the junction and at the back surface is used for the modeling and determination of the optimum thickness of the base of a silicon solar cell in the static regime, under magnetic field and temperature influence. This study takes into account the Umklapp process and the Lorentz effect on the minority carriers photogenerated in the base.

Introduction: Low birth weight is responsible for 9.1 million child deaths each year worldwide. It is the leading cause of perinatal and infant mortality. The objective of this work was to determine the epidemio-clinical profile as well as the future of low birth weight at the Reference Health Center of commune VI in the district of Bamako, Mali. Methods: Our study was descriptive and prospective over a year from April 1, 2018 to March 31, 2019. Data were taken from hospital records and newborn referral/evacuation forms. Data processing was performed using Epi Info software version 3.5.4 and Word. Results: The frequency of low birth weight was 34.94%. Multiparity accounted for 47.84%, sex ratio was 0.93, maternal arterial hypertension was present in 41.66%, genitourinary infection was in 58.37% and delivery was by low way in 86.12%. The majority of newborns had a gestational age between 28SA-33SA (56.52%) and a weight between 1501-1999 grams (47.36%). Mortality accounted for 18.66%. Conclusion: Low birth weight is common in our settings with modifiable risk factors. Practitioners must play on them to minimize its importance.

Endocrine and Cytogenetic Profile of Variations in Genital Development: Series of 9 Cases at the Mali Hospital

We carried out a retrospective and descriptive study in the Medicine and Endocrinology service of the Mali Hospital from January 2015 to December 2018. Nine cases of anomalies in sexual differentiation were referred for pubertal delay, growth retardation, gynecomastia, micropenis or infertility. Outcomes: The average age was 14 years;5 patients were declared female at birth;2 were married without children;7 patients came from the region of Kayes;8 patients consulted for pubertal delay;1 patient presented with association of a blind vagina and 2 bilateral inguinal gonads. 3 of 4 patients declared male at birth had a micropenis;2 patients had gynecomastia with a history of hypospadias. Ultrasound and biological data showed a clinical-hormonal picture of hypergonadotropic hypogonadism in 6 patients, 1 case of hypogonadotropic hypogonadism with hypercorticism and 2 cases with normal gonadotropic axis;two cases of azoospermia. Karyotypic analysis showed: 3 cases of SK, 2 cases of Klinefelter Syndrome, 2 cases of Androgen Insensitivity Syndrome and 2 cases of male 46 XX syndrome including one with sry negative. Conclusion: The precise diagnosis of anomalies in sexual differentiation remains complicated in Mali, due to the inadequacy of the technical platform. Diagnosis must be made at an early stage to allow normal growth, puberty and satisfactory fertility. Taking charge of the medical treatment possibly associated with plastic surgery could give good results.
