Metabolic Emergencies in Newborns in a Subsaharian Neonatology Department: Evaluation of Glucose, Sodium and Potassium Disorders

Introduction: Metabolic neonatal adaptation is a complex phenomenon and metabolic disorders can be frequent in immature newborns or in life-threatening situations. In Low and Middle income countries (LMIC) the difficult access to some diagnostic tests makes the management of the metabolic emergencies challenging. The main objectives of this study were to assess the frequency and circumstances of occurrence and to describe the clinical picture associated with glucose, sodium and potassium disorders in neonates. Patients and Methods: Our study was a retrospective and descriptive study conducted in the neonatology unit of National Children Hospital Albert Royer in Dakar (Senegal) from January 1 to December 31, 2014. Results: The prevalence of the studied metabolic disorders was 46.7%. The most common metabolic disorder noted was Hyperglycemia followed by Hyponatremia. Thermoregulation disturbances were found particularly in newborns with serum sodium disorders (hyponatremia 33.5% and hypernatremia 59.7%). Neurological signs were noted in case of blood sugar abnormalities (hypoglycemia 26.1% and hyperglycemia 29.8%). Half of the newborns with hyperglycemia (82 cases/50%) had blood sugar levels greater than or equal to 2 g/l. Hypernatremia was severe (Serum sodium> 180 mmol/l) in 12 neonates (16.7%). The main diagnoses retained were sepsis (159 cases/45.4%), prematurity (96 cases/27.4%), intrauterine growth retardation (66 cases/18.9%), malformations (63 cases/18%), perinatal asphyxia (44 cases/12.6%) and malnutrition (36 cases/10.3%). For most metabolic disorders, the correction was late and was done beyond 48 hours. On average, the correction time varied between 3 hours and 6 days. The most frequent complications were cerebral edema (12 cases), brain death (8 cases) and increased intracranial pressure (3 cases). 为探究地层损失引起的地层变形在地层内部的传递规律,基于排液法模拟隧道施工引起的直接地层损失,进行不同地层损失率下砂土地层变形的物理模型试验研究.结果表明:在不同地层损失率下,地表及地层内部的沉降曲线均成高斯正态分布,地表沉降影响宽度约为5倍硐径,地表沉降槽宽度系数在13.5~15范围之内,约为2倍硐径;地表最大沉降随着地层损失率的增加而增大,大致呈二次函数关系;地层内部中心最大沉降与深度呈线性关系,最大沉降量随着深度的增加而增大,而地层沉降槽宽度随着深度的增加呈现减少的趋势,地层沉降槽宽度系数i与距离隧道横轴线的深度h呈线性关系;不同深度处的沉降槽体积自下而上有增大的趋势,沉降槽体积传递率的范围为60%~90%.

Physicochemical and Microbiological Quality of Shallow Groundwater in Lomé, Togo

The present study aimed to assess the physicochemical and microbiological quality of shallow groundwater tapped by private boreholes for water sale in Togo’s most urbanized coastal areas. Ninety-six (96) groundwater samples were collected at the water sale points for chemical and microbiological analyses using standard methods. The results showed that groundwater is predominantly acidic with fresh and brackish water of Na-Cl type. High concentrations of dissolved inorganic nitrogen (NO3- , NH4+and NO2-) and permanganate indices indicating potential organic matter were found. Among the major ions, Na+ (46.9%), Cl- (51.0%) and NO3- (50.0%) present the highest percentages of unsuitable concentrations compared to WHO guidelines. These findings indicate the control of the natural impact of seawater and lagoon system and anthropogenic pollution from domestic and urban wastes on groundwater’s physicochemical quality. About 65% of groundwater samples did not comply with the drinking water guidelines for microbial indicators, including total mesophilic flora, thermotolerant coliforms, and sulfite-reducing anaerobes. The results showed the failure of sanitation and hygiene conditions around sampling points and fecal contamination from sewage, pit latrines, septic tanks, and refuse and waste disposal. The degradation of physicochemical quality is higher in the old and high-density built-up areas. Simultaneously, microbial contamination represents a high risk of contracting waterborne or hygiene-sanitation-related diseases in the whole study area. Lipomas are the most common fatty mesenchymal tumors in the human body. They are very rare in the oral cavity. With a predilection, they have a rate of 1% to 4%. We report the case of a 74-year-old woman with a large painless lump on the left cheek of the mouth. This mass invading the deep spaces of the face induced a more difficult surgical excision.

Profile of Patients with Breast Cancer Brain Metastasis in Abidjan

Patients with breast cancer will develop brain metastases in 15% - 25% of cases. Objective: To identify the profile of patients with breast cancer brain metastasis. Method: Retrospective study over 4 years in the oncology department of Treichville Hospital. All patients with breast carcinoma who had a brain metastasis during their cancer management were included. Results: 41 cases of breast carcinoma brain metastasis were collected. Patients were older than 40 years in 78% of cases. Family history of breast cancer was found in 7% of patients. More than 80% of patients had consulted with T3 and T4 tumor masses, with 100% of cases of node involvement. The brain was the second site of metastasis. The tumors were Triple negative in 48%, Her2 positive in 36% and luminal in 16%. Brain metastases were of late onset in the majority of cases (68%). They were clinically characterized by headaches (100%), most often included in an intracranial hypertension syndrome (52%). They were multiple (60%). Therapeutically, no surgery was performed on the metastases. Three patients received whole brain radiation therapy. All patients received systemic therapy. In terms of response, 30 cases of progression were observed after treatment and 2 cases of complete response were obtained. Survival from onset of brain metastasis was of 75% at 3 months and 2% at 12 months. Patients with longer survival rates were over 40 years old with triple negative breast cancer. Conclusion: Brain metastasis from breast cancer most often occurs in a particular clinical and pathological context in our countries.

Non-Small Cell Lung Cancer:Therapeutic and Evolutionary Aspects in Cote d’Ivoire

Non-small cell lung cancer (NSCLC) is a heterogeneous group of malignancies including squamous cell carcinoma, adenocarcinoma, bronchioloalveolar carcinoma and large cell carcinoma. They are known to be of poor prognosis, despite a better understanding of bronchial oncogenesis. This descriptive retrospective study carried out over three years (January 2013 to December 31st, 2016) at the Oncology Department of the Treichville University Hospital in Abidjan was aimed to assess the efficacy of new treatments for NSCLC in Cote d’Ivoire and to specify their benefit in terms of quality of life, comfort and overall survival. This 77 patients study, found that NSCLC, the most frequent of which is squamous cell carcinoma, occurs in relatively young subjects, smokers (91%) diagnosed at a locally advanced or metastatic stage (89%). Cisplatin base multi-drug chemotherapy was mostly used (71%). Combined all treatments, there was a significant clinical gain. However, no complete response was observed. Hematological and digestive toxicities were seen in more than 30% of cases. Overall survival was of 9 months on average. This study is an advocate to optimize primary prevention policies for a cancer with poor prognosis despite the development of new treatments such as target therapies.

Endocrine and Cytogenetic Profile of Variations in Genital Development: Series of 9 Cases at the Mali Hospital

We carried out a retrospective and descriptive study in the Medicine and Endocrinology service of the Mali Hospital from January 2015 to December 2018. Nine cases of anomalies in sexual differentiation were referred for pubertal delay, growth retardation, gynecomastia, micropenis or infertility. Outcomes: The average age was 14 years;5 patients were declared female at birth;2 were married without children;7 patients came from the region of Kayes;8 patients consulted for pubertal delay;1 patient presented with association of a blind vagina and 2 bilateral inguinal gonads. 3 of 4 patients declared male at birth had a micropenis;2 patients had gynecomastia with a history of hypospadias. Ultrasound and biological data showed a clinical-hormonal picture of hypergonadotropic hypogonadism in 6 patients, 1 case of hypogonadotropic hypogonadism with hypercorticism and 2 cases with normal gonadotropic axis;two cases of azoospermia. Karyotypic analysis showed: 3 cases of SK, 2 cases of Klinefelter Syndrome, 2 cases of Androgen Insensitivity Syndrome and 2 cases of male 46 XX syndrome including one with sry negative. Conclusion: The precise diagnosis of anomalies in sexual differentiation remains complicated in Mali, due to the inadequacy of the technical platform. Diagnosis must be made at an early stage to allow normal growth, puberty and satisfactory fertility. Taking charge of the medical treatment possibly associated with plastic surgery could give good results.

Ischemic Stroke Complicating Neuromeningeal and Pulmonary Tuberculosis in Children: About a Case in Senegal

Tuberculosis is a major public health problem, especially in developing countries. Neuro-meningeal involvement is rare but represents one of the most severe forms with high morbidity and mortality. We report the case of an ischemic stroke complicating neuromeningeal and pulmonary tuberculosis in a four-month-old infant at the National Hospital Center of children Albert Royer (Senegal). The case was a four-month-old, male infant with no history. His vaccination was up to date according to Senegal’s expanded vaccination program. There was no notion of tuberculosis contagion. He was seen for a prolonged fever for more than a month and chronic cough. He had moderate malnutrition. Neurological examination revealed coma, meningeal syndrome, generalized tonic convulsions and hypertonia. The pupils were in bilateral miosis. Osteotendinous reflexes were abolished with a bilateral Babinski sign. He also had severe respiratory distress, bilateral pulmonary condensation syndrome. Brain computed tomography was in favor of an ischemic stroke. The test GeneXpert MTB/RIF was positive on cerebrospinal fluid and gastric tubing fluid. The curative treatment was based on a quadruple therapy based on rifampicin, isoniazid, pirazinamide and ethambutol. The evolution was unfavorable with the death of the patient.

Metastatic(Advanced)Prostate Cancer Resistant to Castration:The Time before Resistance Development and Therapeutic Measures Undertaken in Cote d’Ivoire

Objective: Contribute to improving care for prostate cancer patients in Cote d’Ivoire by identifying the average time before the development of the hormone-resistance and inherent therapeutic strategies. Patients and Method: It was a retro and prospective study over three years performed at the University Teaching Hospital of Treichville in Abidjan. 84 patients with metastatic prostate adenocarcinoma and resistant to hormone therapy were involved in the study. The analysis of clinical records enabled us to integrate different parameters in our study. Results: Our patients were relatively young (an average age of 61 years) and most of them belonged to the middle socio-economic class (50%). The resistance occurred within an average period of 9 months. The mixing of Docetaxel and Prednisone chemotherapy was the most used method of treatment (70% of cases). However, few patients (9.5%) received Abiraterone Acetate. Digestive and hematologic toxicities were noticed but they could be controlled by inherent treatment or disappear spontaneously. After nine months of treatment, 54.24% of patients who received specific treatment were alive, but high rates of mortality were observed as for patients who were only treated symptomatically. Conclusion: The relatively short average time before the development of hormone-resistance shows the aggressiveness of prostate adenocarcinoma from black Africans. The main therapeutic standard in our context remains Taxane-based chemotherapy with a good and fair tolerance.
