Clinicopathological characteristics of clinical early gastric cancer in the upper-third stomach

AIM: To elucidate the clinicopathological characteristics of clinically early gastric cancer in the upper-third stomach and to clarify treatment ***: A total of 683 patients with clinical early gastric cancer were enrolled in this retrospective study, 128 of whom had gastric cancer in the upper-third stomach(U group). All patients underwent a double contrast barium examination, endoscopy, and computed tomography(CT), and were diagnosed preoperatively based on the findings obtained. The clinicopathological features of these patients were compared with those of patients with gastric cancer in the middle- and lower-third stomach(ML group). We also compared clinicopathological factors between accurate-diagnosis and under-diagnosis groups in order to identify factors affecting the accuracy of a preoperative diagnosis of tumor ***: Patients in the U group were older(P = 0.029), had a higher ratio of males to females(P = 0.015), and had more histologically differentiated tumors(P = 0.007) than patients in the ML group. A clinical under-diagnosis occurred in 57 out of 683 patients(8.3%), and was more frequent in the U group than in the ML group(16.4% vs 6.3%, P < 0.0001). Therefore, the rates of lymph node metastasis and lymphatic invasion were slightly higher in the U group than in the ML group(P = 0.071 and 0.082, respectively). An under-diagnosis was more frequent in histologically undifferentiated tumors(P = 0.094) and in those larger than 4 cm(P = 0.024). The medianfollow-up period after surgery was 56 mo(range, 1-186 mo). Overall, survival and disease-specific survival rates were significantly lower in the U group than in the ML group(P = 0.016 and 0.020, respectively). AIM: To assess the efficacy of topical Semaphorin 3A(SEMA3A) in the treatment of allergic ***: Experimental allergic conjunctivitis(EAC)mice model induced by short ragweed pollen(SRW) in 4-week-old of BALB/c mice, mice were evaluated using haematoxylin and eosin(H&E) staining, immunofluor-escence and light microscope photographs. Early phase took the samples in 24 h after instillation and late phase took the samples between 4 to 14 d after the start of treatment. The study use of topical SEMA3A(10 U, 100 U,1000 U) eye drops and subconjunctival injection of SEMA3 A with same concentration. For comparison, five types of allergy eyedrops were quantified using clinical characteristics.· RESULTS: Clinical score of composite ocular symptoms of the mice treated with SEMA3 A were significantly decreased both in the immediate phase and the late phase compared to those treated with commercial ophthalmic formulations and non-treatment mice. SEMA3 A treatment attenuates infiltration of eosinophils entering into conjunctiva in EAC mice. The score of eosinophil infiltration in the conjunctiva of SEMA3 A 1000 U-treated group were significantly lower than low-concentration of SEMA3 A treated groups and non-treated group. SEMA3 A treatment also suppressed T-cell proliferation in vitro and decreased serum total Ig E levels in EAC mice. Moreover, treatment of SEMA3 A suppressed Th2-related cytokines(IL-5, IL-13 and IL-4)and pro-inflammatory cytokines(IFN-γ, IL-17 and TNF-α)release, but increased regulatory cytokine IL-10 concentration in the conjunctiva of EAC ***: SEMA3 A as a biological agent, showed the beneficial activity in ocular allergic processes with the less damage to the intraocular tissue. It is expected that SEMA3 A may be contributed in patients with a more severe spectrum of refractory ocular allergic diseases including allergic conjunctivitis in the near future.

Safety and utility of capsule endoscopy for infants and young children

Stress-Induced Flowering in Pharbitis—A Review

Many plant species are induced to flower by stress. Stress-induced flowering has been studied mostly in the short-day plant pharbitis (also called Japanese morning glory;Ipomoea nil, formerly Pharbitis nil). In this article, physiological characteristics, the regulation by salicylic acid (SA) and the expression of flowering-related genes in stress-induced flowering in pharbitis are reviewed. Pharbitis flowered under long-days in response to poor nutrition or low temperature. The pharbitis plants induced to flower by stress reached anthesis, fruited and produced fertile seeds. The progeny of the stressed plants developed normally. Grafting experiments indicated that a transmissible flowering stimulus is involved in poor nutrition stress-induced flowering. Aminooxyacetic acid (AOA), a phenylalanine ammonia-lyase (PAL) inhibitor, inhibited the stress-induced flowering, and this inhibition was overcome by SA. Stress induced PAL activity and SA biosynthesis. PnFT2, a pharbitis ortholog of the flowering gene FLOWERING LOCUS T of Arabidopsis thaliana, was expressed when the plants were induced to flower by stress. The overexpression of PnFT2 induced flowering, and PnFT2RNAi inhibited it. AOA inhibited PnFT2 expression induced by stress, and SA eliminated this inhibitory effect. SA enhanced PnFT2 expression under poor nutrition but not under non-stressful conditions. Therefore, stress may induce the production of SA and other unknown factor(s) that may work in combination to induce PnFT2 expression and flowering.

PROTEOME ANALYSIS OF RICE ROOT PROTEINS REGULATED BY GIBBERELLIN

To gain an enhanced understanding of the mechanism by which gibberellins （GAs） regulate the growth and development of plants, it is necessary to identify proteins regulated by GA. Proteome analysis techniques have been applied as a direct, effective, and reliable tool in differential protein expressions. In previous studies, sixteen proteins showed differences in accumulation levels as a result of treatment with GA3, uniconazole, or abscisic acid （ABA）, and/or the differences between the GA-deficient semi-dwarf mutant, Tan-ginbozu, and normal cultivars. Among these proteins, aldolase increased in roots treated with GA3, was present at low levels in Tan-ginbozu roots, and decreased in roots treated with uniconazole or ABA. In a root elongation assay, the growth of aldolase-antisense transgenic rice was half of that of vector control transgenic rice. These results indicate that increases in aldolase activity stimulate the glycolytic in the GA-induced growth of roots. In among GA, aldolase, and root growth. pathway and may play an important role this review, we discuss the relationship among GA, aldolase, and root growth.

Intervention Effects on the Willingness to Stop Smoking and Social Nicotine Dependence Based on Single-Session Group Therapy with Frequent Use of Role Play Targeting Smokers with Mental Disorders

Objective: We devised a “Smoking-Cessation Motivation Program” as a community welfare service aimed at making short-term improvements in social nicotine dependence and enhancing the willingness to stop smoking among people with mental disorders. Method: The study conducted a controlled trial to empirically examine the effects of the program on the willingness to stop smoking and social nicotine dependence among 26 smokers with mental disorders in the intervention group, who were compared with a control group of 16 individuals. Results: Results demonstrated the program’s effectiveness in reducing social nicotine dependence, and a variance analysis with age as the covariate evidenced a mutual interaction between social nicotine dependence and the willingness to stop smoking, confidence to stop smoking, and resisting the temptation to smoke. Conclusion: Future studies should examine longer-term program results, the impact of program duration, and other factors that influence the program’s effectiveness.

Clinical characteristics of hepatoduodenal lymph node metastasis in gastric cancer

AIM: To assess the clinical features of hepatoduodenal lymph node(HDLN) metastasis and to clarify the optimal indication of HDLN ***: We investigated a total of 276 patients who underwent gastrectomy with extended lymphadenectomy,including HDLN dissection,for gastric cancer between 1999 and 2012. Of these,26 patients(9.4%) had HDLN metastasis. First,we investigated the clinicopathological characteristics,their perioperative clinical outcomes,such as postoperative complications,and prognostic outcomes between patients with and without HDLN metastasis. Second,we detected the prognostic factors,particularly in patients with HDLN metastasis. Third,we assessed the therapeutic value of HDLN dissection to determine its optimal ***: The five-year overall survival rate of the patients with HDLN metastasis was 29%. Univariate and multivariate logistic regression analyses revealed that the tumour location(the middle or lower stomach [P = 0.005,OR = 5.88(95%CI: 1.61-38.1)] and p T category [T3 or T4,P = 0.017,OR = 4.45(95%CI: 1.28-21.3)] were independent risk factors for HDLNmetastasis. Cox proportional hazard analysis identified p N3 as an independent poor prognostic factor in the patients with HDLN metastasis [P = 0.021,HR = 5.17(95%CI: 1.8-292)]. For patients who underwent radical HDLN dissection,HDLN metastasis was a prognostic indicator in p N3 gastric cancer(P < 0.0001),but not p N1-2(P = 0.602). Furthermore,the index of therapeutic value of HDLN dissection for gastric cancer in the middle or lower stomach and the upper stomach was 3.4 and 0.0,***: We suggest that HDLN dissection should be indicated for p N1 or p N2 gastric cancers located at the middle or lower stomach.

Risk factors to predict severe postoperative pancreatic fistula following gastrectomy for gastric cancer

AIM:To allow the identification of high-risk postoperative pancreatic fistula(POPF)patients with special reference to the International Study Group on Pancreatic Fistula(ISGPF)***:Between 1997 and 2010,1341 consecutive patients underwent gastrectomy for gastric cancer at the Department of Digestive Surgery,Kyoto Prefectural University of Medicine,*** on the preoperative diagnosis,total or distal gastrectomy and sufficient lymphadenectomy was performed,mainly according to the Japanese guidelines for the treatment of gastric *** these,35 patients(2.6%)were diagnosed with Grade B or C POPF according to the ISGPF classification and were treated *** hospital records of these patients were reviewed ***:Of 35 patients with severe POPF,17(49%)and 18(51%)patients were classified as Grade B and C POPF,*** several clinical factors,the severity of POPF according to the ISGPF classification was significantly correlated with the duration of intensive POPF treatments(P=0.035).Regarding the clinical factors to distinguish extremely severe POPF,older patients(P=0.035,65 years≤vs<65 years old)and those with lower lymphocyte counts at the diagnosis of POPF(P=0.007,<1400/mm3vs 1400/mm3≤)were significantly correlated with Grade C POPF,and a low lymphocyte count was an independent risk factor by multivariate analysis[P=0.045,OR=10.45(95%CI:1.050-104.1)].CONCLUSION:Caution and intensive care are required for older POPF patients and those with lower lymphocyte counts at the diagnosis of POPF.

A Loop Thermosyphon Type Cooling System for High Heat Flux

With rapid development of the semiconductor technology, more efficient cooling systems for electronic devices are needed. In this situation, in the present study, a loop thermosyphon type cooling system, which is composed mainly of a heating block, an evaporator and an air-cooled condenser, is investigated experimentally in order to evaluate the cooling performance. At first, it is examined that the optimum volume filling rate of this cooling system is approximately 40%. Next, four kinds of working fluids, R1234ze(E), R1234ze(Z), R134a and ethanol, are tested using a blasted heat transfer surface of the evaporator. In cases of R1234ze(E), R1234ze(Z), R134a and ethanol, the effective heat flux, at which the heating block surface temperature reaches 70°C, is 116 W/cm2, 106 W/cm2, 104 W/cm2 and 60 W/cm2, respectively. This result indicates that R1234ze(E) is the most suitable for the present cooling system. The minimum boiling thermal resistance of R1234ze(E) is 0.05 (cm2·K)/W around the effective heat flux of 100 W/cm2. Finally, four kinds of heat transfer surfaces of the evaporator, smooth, blasted, copper-plated and finned surfaces, are tested using R1234ze(E) as working fluid. The boiling thermal resistance of the blasted surface is the smallest among tested heat transfer surfaces up to 116 W/cm2 in effective heat flux. However, it increases drastically due to the appearance of dry-patch if the effective heat flux exceeds 116 W/cm2. On the other hand, in cases of copper-plated and finned surfaces, the dry-patch does not appear up to 150 W/cm2 in effective heat flux, and the boiling thermal resistances of those surfaces keep 0.1 (cm2·K)/W.
