Although the administration of systemic steroid for nonsegmental vitiligo in the progressive stage is a recommended treatment according to guidelines, the clinical efficacy of this regimen has not been fully established. In this study, we evaluated the clinical efficacy of half-dose steroid treatment and stratified the evidence regarding its usefulness in progressive vitiligo patients. Half-dose steroid pulse therapy (500 mg/day of methylprednisolone for three sequential days) was administered intravenously three times monthly in five vitiligo patients. The visual changes in vitiligo lesions were evaluated on photographs and quantified using a spectrophotometer. As results, all patients completed three cycles of treatment without severe adverse events. Three of the five patients achieved disease arrest with decrease in white contrast. Therefore, short-term and halfdose steroid therapy is well tolerated and effective for achieving disease arrest in progressive nonsegmental vitiligo. The whiteness assessed by a spectrophotometer is possibly associated with therapeutic response to steroid therapy.

The accumulation of genetic alterations in driver genes is responsible for the development and malignant progression of colorectal cancer. Comprehensive genome analyses have revealed the driver genes, including APC, KRAS, TGFBR2, and TP53, whose mutations are frequently found in human colorectal cancers. Among them, the p53 mutation is found in ~60% of colorectal cancers, and a majority of mutations are missense-type at ‘hot spots’, suggesting an oncogenic role of mutant p53 by ‘gain-of-function’ mechanisms. Mouse model studies have shown that one of these missense-type mutations, p53 R270H (corresponding to human R273H), causes submucosal invasion of intestinal tumors, while the loss of wild-type p53 has a limited effect on the invasion process. Furthermore, the same mutant p53 promotes metastasis when combined with Kras activation and TGF-β suppression. Importantly, either missense-type p53 mutation or loss of wild-type p53 induces NF-κB activation by a variety of mechanisms, such as increasing promoter accessibility by chromatin remodeling, which may contribute to progression to epithelial–mesenchymal transition. These results indicate that missense-type p53 mutations together with loss of wild-type p53 accelerate the late stage of colorectal cancer progression through the activation of both oncogenic and inflammatory pathways. Accordingly, the suppression of the mutant p53 function via the inhibition of nuclear accumulation is expected to be an effective strategy against malignant progression of colorectal cancer.

Experimental Study of Heating Fluid between Two Concentric Cylinders with Cavities

In this paper, we are concerned with the heating fluid in the annular space between two concentric cylinders where the inner cylinder with cavities is rotated and the outer cylinder is at rest. The effects of cavities of the inner cylinder on the heating fluid and the cavitation behavior are examined. We measured the flow rate, rotating speed of rotor, friction torque, temperature rise and pressure drop between inlet and outlet of this device. The flow rate of water ranged from 0.42×10^-4 to 4.59×10^-4 ma/s at the inlet. The friction torque and the quantity of heat of the inner cylinder with cavities were larger than the case of no cavities, and they increased as the rotating speed of inner cylinder increased. The efficiency was close to about 1 as the rotating speed of inner cylinder increased. The behavior of cavitation in the cavity on the surface of inner cylinder was discussed.

The threat of biological and chemical terror acts remains a growing worldwide *** is therefore a need to develop appropriate technology for the detection of chemical and biological warfare agents,with early identification intended for use by first ***,we disclose the developed autonomous air sampling and detection system for evaluation of the presence of chemical and biological warfare agents that can be harmful to the *** current device utilizes the designed mist generator-assisted air collection system(338 l min^(−1))and biosensing chip technologies,such as electrochemical measurement,Au nanoparticle-based localized surface plasmon resonance,and rapid microfluidic chip PCR for detection of minute concentrations lower than the mean lethal dose(LD_(50))of nerve gases(sarin and VX),toxic proteins(BTX/A/Hc and ricin),and pathogens(anthrax simulant).An operation time of only 5–15 min is needed for the collection and detection;sample preparation is already integrated into the system without the need for direct human *** addition to the system’s sensitivity and ease of use,its portability makes it highly beneficial for first responders,which could aid in immediate risk assessment and mitigation of on-site events.

Noble metal-based-bimetallic catalysts have been highly investigated and applied in wide applications including biomass transformation via regioselective C−O hydrogenolysis while further modification especially with noble metal is highly promising yet still under ***,Ru was found as an effective modifier among the screened noble metals(Ru,Pt,Rh,Pd,Au,and Ag)for Ir-Fe/BN(Ir=5 wt%,Fe/Ir=0.25)catalyst in terminal C−O hydrogenolysis of 1,2-butanediol(1,2-BuD)to 2-butanol(2-BuOH).Only trace amount of Ru(up to 0.5 wt%)was effective in terms of high 2-BuOH selectivity(>60%)and activity(about twice).Larger amount of Ru species(3 wt%)highly enhanced the activity but gave low selectivity to 2-BuOH with by-products of terminal C−C bond *** catalyst(Ru(0.5)-Ir-Fe/BN)was reusable at least 4 times and gave moderate 2-BuOH yield(47%)in hydrogenolysis of 1,*** promoting effect of Ru addition(0.5 wt%)to Ir-Fe/BN on hydrogenolysis of various alcohols was also *** catalytic tests with various characterizations,the promotion mechanism of Ru species in trimetallic catalysts was *** Ru species in Ru(0.5)-Ir-Fe/BN form alloy with Ir and are enriched at the interface with BN surface,and direct interaction between Ru and Fe was not necessary in Ru-Ir-Fe *** interface of Ir and Fe on the surface of Ir-Fe alloy may work as active sites for 1,2-diols to secondary alcohols via direct C−O hydrogenolysis,in which Ru-modified Ir activates H_(2) to form hydride-like *** activity of Ru species in C−C bond cleavage was highly suppressed due to the direct interaction with Ir species and less exposed to *** loading amount of Ru species(3 wt%)led to the formation Ru-rich trimetallic alloy,which further works as active sites for C−C bond scission.

Classification of Emphysema Subtypes: Comparative Assessment of Local Binary Patterns and Related Texture Features

The purpose of this study was to assess usefulness of local binary patterns (LBP) and related texture features, namely completed local binary patterns (CLBP) and local ternary patterns (LTP), for the classification of emphysema subtypes on low-dose CT images. Fifty patients (34 men and 16 women;age, 67.5 ± 10.1 years) who underwent low-dose CT (60 mAs) were included. They were comprised of 17 never smokers, 13 smokers without COPD, and 20 smokers with COPD. By consensus reading of low-dose CT images from these patients, two radiologists selected 3681 nonoverlapping regions of interest (ROIs) and annotated them as one of the following three classes: normal tissue, centrilobular emphysema, and paraseptal emphysema. From these ROIs, histogram of CT densities, LBP, CLBP, and LTP were calculated, and the 3 types of texture histograms were concatenated with the CT density histogram. These 3 types of histograms (referred to as combined LBP, combined CLBP, and combined LTP) were used to classify ROI using linear support vector machine. For each type of the combined histogram, the accuracy of classification was determined by patient-based 10-fold cross validation. The best accuracy of combined LBP, combined CLBP, and combined LTP were 81.36%, 82.99%, and 83.29%, respectively. Compared to the classification accuracies obtained with combined LBP, those with combined LTP or combined CLBP were consistently improved. In conclusion, the results of this study suggest that, on low-dose CT, LTP and CLBP were more useful for the classification of emphysema subtypes than LBP.

The Direction of Research on Active Aging and Healthy Life Expectancy in Japan

In Japan, 18.1% of the population known as baby-boomers will become the late-stage elderly in 2025, thereby needing a foundation to support this change. The Japanese Ministry of Health, Labour and Welfare is promoting the development of a regional comprehensive system allowing the elderly to continue living in their familiar surroundings. However, a care shortage is inevitable unless elders are able to age in good health, regardless of the system’s level of enhancement. This study aims to review the literature on active aging, clarify trends in clinical operations undertakings and research in Japan, and consider relevant research issues. After combining the search results of “active aging” and “healthy life expectancy,” we used a text mining technique to analyze the abstracts of 120 original articles and 213 reviews, commentaries, and features. Eight categories were extracted from the original articles: health statistics, gender, age, etc. From the reviews, commentaries, and features, 16 categories were extracted: orientation, disease, and living, etc. Cerebrovascular disease and osteoporosis were the most common diseases covered in the original articles;there has been a substantial amount of research on “active aging” and “healthy life expectancy” because they can easily lead to being bedridden and to a decrease in QOL. In the reviews, commentaries, and features, lifestyle-related diseases and menopause rather than cerebrovascular disease and osteoporosis, were extracted. The categorical differences found in the original articles may be due to the possibility that Japanese researchers are publishing their research abroad rather than in Japan or they submit research on topics that are guaranteed to be published at home or abroad. Little research has been conducted using the terms, “active aging” and “healthy life expectancy,” evidenced by the small number of studies generated. Endoscopic occlusion with silicone spigots for the closure of refractory esophago-bronchiole fistula after esophagectomy

A 65-year-old man with cT1bN0M0 stage I middle thoracic esophageal cancer underwent subtotal esophagectomy and gastric tube reconstruction through the posterior mediastinal route after preoperative carbon-ion radiotherapy and chemotherapy in a clinical trial. Anastomotic leakage occurred, but it spontaneously improved. At six months after the operation, he was rehospitalized with a cough and dysphagia. An esophago-bronchiole fistula and stenosis of the gastric tube were observed. He first underwent stent placement in the gastric tube. Two weeks later, the syringeal epithelium was burned by argon plasma coagulation after stent removal. Endoscopic occlusion was then performed for the fistula with two guidewire-assisted silicone spigots. Two weeks later, he was discharged on an oral diet, and he has not developed recurrence of the fistula or cancer for three years. This is the first report of endoscopic occlusion with a guidewire-assisted silicone spigot through the esophagus.

Evaluation of the Palatability of Dry Syrups and the Bitterness Intensities of Dry Syrups Mixed with Various Beverages

The purpose of this study was to evaluate the palatability of dry syrups for pediatric use by human gustatory sensation testing and the artificial taste sensor. The bitterness intensities of dry syrups mixed with various beverages were also evaluated using the taste sensor. Twenty dry-syrup formulations of antibacterial and anti-allergic drugs containing bitter active ingredients, frequently used in pediatric medicine in Japan, were selected for testing. The main factors influencing palatability were determined by factor analysis of data from human gustatory sensation testing using the semantic differential method. It was shown that the bitterness intensity of dry syrups in water could be predicted by the artificial taste sensor. The influence of different beverages (orange juice, milk or cocoa) mixed with the dry syrups was evaluated using the artificial taste sensor. Taste and texture were found to be the principal factors influencing the palatability of dry syrups. While the bitterness intensities of some dry syrups were increased by mixing with orange juice, the bitterness intensities of most dry syrups were decreased by mixing with milk or cocoa. This suggests that one or more constituents of milk or cocoa may reduce the bitterness intensities of dry syrups.
