The purpose of this study was to design a submicron-sized liposomal non-steroidal antiinflammatory drug(NSAID)preparation that targets the retina via topical instillation of eye drops.Bromfenac(BRF)-loaded liposomes were prepared using the calcium acetate gradient method.Liposome sizes and encapsulation efficiencies were optimized by screening several liposome formulations of lipid,drug concentration,and buffer solution.BRF entrapment efficiency was greater than 90%using this method,and was low using conventional hydration methods.High initial BRF loading using the pH gradient method caused aggregation of liposomes.To circumvent aggregation,the negatively charged lipid dicetylphosphate was incorporated into liposomes,which formed anion layer preventing coalescence.Release of BRF from liposomes was sustained for several hours depending on lipid concentration,inner water phase,initial drug amounts,and surface properties.Surface modification with chitosan(CS),a mucoadhesive cationic polymer,was achieved using electrostatic interactions of negatively charged liposomes.The optimal concentration of CS for evasion of liposome aggregation was 0.15%.

The Holocene alluvial fans and flood plains formed by the Sho and Oyabe Rivers spread out in the Tonami plain from the central through the northern parts in the Toyama Prefecture, central Japan. Along the foot of the surrounding mountains and hills, higher, middle and lower terraces of late Pleistocene-Holocene in age are distributed. These terraces have been displaced, by the reverse dip-slip activities of Tonami-heiya fault zone in a sense of upheaval in the mountains side, even during the Holocene time. We examined stratigraphic cross section utilizing borehole data and geomorphologically analyzed 5 m-DEM data in order to elucidate the fault trace of the Isurugi fault which Tonami-heiya fault zone. As the results, the northern segment of Isurugi fault seems to run along the northeastern foot of Hodatsu Hills and extends underground through the lower-most Oyabe River into the Toyama Bay. Consequently, its total length reaches about 30 km. In the southern segment, a continuous fault scarplet was recognized to cut across the lower dissected fans. The slip-rate of Isurugi fault is estimated to be 0.31 - 0.64 m/kyr. In the Hokuriku region, reverse faulting and related folding with strike in a NE-SW direction have occurred during the late Quaternary. The hinge line of block movement due to the activities of the Tonami-heiya fault zone is revealed to have shifted from the mountain side into the plain side within the Holocene time. In conclusion, the Quaternary folding and faulting associated with the crustal warping at a wavelength of about 20 km is currently in progress, causing both the subsidence of Tonami plain and the upheaval of surrounding mountains and hills.

Synchronous coexistence of liver metastases from cecal leiomyosarcoma and rectal adenocarcinoma: A case report

Multiple liver tumors represent a challenging condition for abdominal surgeons both in the selection of technique and the rarity of diagnosis. There are no case reports on co-existence of liver metastases from both intestinal leiomyosarcoma and adenocarcinoma. The patient described in this report successfully underwent resection of both primary lesions and liver metastases in combination with chemotherapy. As for the leiomyosarcoma, the primary cecal lesion was revealed more than three years after the patient's first visit. Peritoneal, lymph-node, and lung recurrences were observed afterward, and thus surgeries on those regions were performed. Pathologically, the peritoneal and lung recurrences comprised leiomyosarcoma and the lymphnode recurrence was diagnosed as adenocarcinoma. Despite newly discovered multiple lung recurrences and regional lymph-node metastases, the patient lived a normal life for 73 mo after the initial operation based on multidisciplinary therapy. He ultimately died of liver failure due to invasive lymph-node recurrence from the rectal adenocarcinoma, in addition to multiple lung recurrences from the leiomyosarcoma. Hepatic recurrence did not occur in this patient's case, which appears to be one reason for his long-term survival.

Laparoscopic gastrectomy is considered as an indispensable option between endoscopic resection and standard gastrectomy with open laparotomy for patients with early-stage gastric cancer.However,the extent of gastrectomy and remnant gastric function may affect patients’quality of life(QOL)after surgery.Therefore,function-preserving gastrectomy in addition to laparoscopic surgery could be considered in patients with early-stage gastric cancer.A prospective multicenter trial and meta-analyses of sentinel node(SN)mapping and biopsy for early-stage gastric cancer have demonstrated favorable SN detection rates and accuracy of nodal metastatic status.Although a combination of radioactive colloids with blue dyes as tracers is currently considered as the promising procedure of SN mapping in early-stage gastric cancer,several new technologies,such as indocyanine green fluorescence imaging,may markedly improve its accuracy.For early-stage gastric cancer,the development of laparoscopic personalized minimized gastrectomy with SN mapping may help retain patents’QOL after surgery.A recently developed full-thickness partial gastrectomy with SN mapping and basin dissection would become a reliable minimally invasive gastrectomy for treating patients with cN0 early-stage gastric cancer.

A New Species of the Genus Rhabdophis Fitzinger,1843(Squamata:Colubridae)in Southwestern Sichuan,China

The genus Rhabdophis is a group of widely distributed snakes with more than 20 species.Recent field surveys uncovered a species in southwestern China,which has long been considered as R.pentasupralabialis.Combined molecular and morphological analyses revealed it as a new species Rhabdophis chiwen sp.nov.Based on 12 specimens,this new species is distinguished by the following characters:1)dorsal body saddlebrown,dorsal scales typically with black margins forming spots and stripes,the margin of the outer row forming two faint dorsolateral black cross-bars alongside body;2)ventral scales 151–159,the outer margin of ventral scales and several lateral rows of dorsal scales forming ventrolateral longitudinal brownish-red coloration,with faint black spots in the middle of ventral scales;3)a black oblique stripe present below eyes,often with a black spot between the 2 nd and 3 rd supralabial and a black stripe on the 5 th supralabial;4)eyes dark khaki,pupils black;5)infralabials usually 7,the first four in contact with anterior chin-shields;6)temporal scales 1+1;7)dorsal scales in 15 rows,feebly keeled except the outer 1–2 rows;8)anal scale divided;subcaudals 45–59;9)preocular 1 and postoculars 3(occasionally 2);10)body medium-sized(snout-vent length:adult males 404–431 mm,adult females 409–476 mm);11)tail moderate(tail length/total length in adult males 0.205–0.238,in adult females0.172–0.193).With the discovery of this new species,the total number of species in genus Rhabdophis is 28 with 12 th species known to occur in China.

Case of severe alcoholic hepatitis treated with granulocytapheresis

Severe alcoholic hepatitis(AH) has a high mortality,and it is associated with encephalopathy,acute renal failure,sepsis,gastrointestinal bleeding,and endotoxemia.The 28-d mortality remains poor(34%-40%),because no effective treatment has been established.Recently,corticosteroids(CS) have been considered effective for significantly improving the prognosis of those with AH,as it prevents the production of pro-inflammatory cytokines.However,CS are not always appropriate as an initial therapeutic option,such as in cases with an infection or resistance to CS.We describe a patient with severe AH complicated by a severe infection caused by the multidrug resistance bacteria(Pseudomonas aeruginosa),and was successfully treated with granulocytapheresis monotherapy without using CS.The experience of this case will provide understanding of the disease and information treating cases without using CS.

Protective effect of Irsogladine on monochloramineinduced gastric mucosallesions in rats:a comparative study with rebamipide

AIM To examine the effect of irsogladine, a novel antiulcer drug, on the mucosal ulcerogenic response to monochloramine (NH 2Cl) in rat stomach, in comparison with rebamipide, another antiulcer drug with cytoprotective activity. METHODS AND RESULTS Oral administration of NH 2Cl (120*!mM) produced severe hemorrhagic lesions in unanesthetized rat stomachs. Both irsogladine ( 1*!mg/*!kg - 10*!mg/*!kg , po ) and rebamipide ( 30*!mg/*!kg - 100*!mg/*!kg , po ) dose dependently prevented the development of these lesions in response to NH 2Cl, the effect of irsogladine was significant at 3*!mg/*!kg or greater, and that of rebamipide only at 100*!mg/*!kg . The protective effect of irsogladine on NH 2Cl induced gastric lesions was significantly reduced by N G nitro L arginine methyl ester (L NAME) but not by indomethacin, while that of rebamipide was significantly mitigated by indomethacin but not by L NAME. Topical application of NH 2Cl (20*!mM) caused a marked reduction of potential difference (PD) in ex vivo stomachs. This PD reduction was not affected by mucosal application of irsogladine, but significantly prevented by rebamipide. The mucosal exposure to NH 4OH (120*!mM) also caused a marked PD reduction in the ischemic stomach (bleeding from the carotid artery), resulting in gastric lesions. These ulcerogenic and PD responses caused by NH 4OH plus ischemia were also significantly mitigated by rebamipide, in an indomethacin sensitive manner, while irsogladine potently prevented such lesions without affecting the PD response, in a L NAME sensitive manner. CONCLUSION These results suggest that ① NH 2Cl generated either exogenously or endogenously damages the gastric mucosa, ② both irsogladine and rebamipide protect the stomach against injury caused by NH 2Cl, and ③ the mechanism underlying the protective action of irsogladine is partly mediated by endogenous nitric oxide, while that of rebamipide is in part mediated by endogenous prostaglandins.

By means of an ultrafast optical technique,we track focused gigahertz coherent phonon pulses in objects down to sub-micron in size.Infrared light pulses illuminating the surface of a single metal-coated silica fibre generate longitudinal-phonon wave packets.Reflection of visible probe light pulses from the fibre surface allows the vibrational modes of the fibre to be detected,and Brillouin optical scattering of partially transmitted light pulses allows the acoustic wavefronts inside the transparent fibre to be continuously monitored.We thereby probe acoustic focusing in the time domain resulting from generation at the curved fibre surface.An analytical model,supported by three-dimensional simulations,suggests that we have followed the focusing of the acoustic beam down to a~150-nm diameter waist inside the fibre.This work significantly narrows the lateral resolution for focusing of picosecond acoustic pulses,normally limited by the diffraction limit of focused optical pulses to~1μm,and thereby opens up a new range of possibilities including nanoscale acoustic microscopy and nanoscale computed tomography.
