Supraceliac Aortic Clamping for Repair of Ruptured Abdominal Aortic Aneurysm in Patients with Short Aortic Neck Length

Objective: Prompt bleeding control with proximal aortic clamping and subsequent aortic repair are very important for ruptured abdominal aortic aneurysm. However, unsuitable anatomy, such as short aortic neck length, not only disturbs the means to an expeditious repair, but may also increase morbidity and mortality. In the present study, we aimed to evaluate the efficacy of supraceliac aortic clamping for improving surgical outcomes for patients with ruptured abdominal aortic aneurysm, who have a short aortic neck length. Method: Between April 2010 and September 2015, eighteen patients underwent emergent open surgical repair of ruptured abdominal aortic aneurysm. Eight patients with a short aortic neck length underwent supraceliac aortic clamping, and 10 underwent infrarenal aortic clamping. Results: The mean supraceliac aortic clamping time was 30 ± 7 minutes. There was 1 operative death in the infrarenal aortic clamping group due to respiratory failure, and the overall operative mortality was 6%. There were no significant differences between the 2 groups with respect to postoperative complication rates or mortality. Furthermore, there were no significant differences in variables of renal function between the 2 groups, through-out the study period. Conclusion: Supraceliac aortic clamping was associated with minimal mortality and morbidity, but not with harmful effects on postoperative renal function. Thus, supraceliac aortic clamping can be safely applied for ruptured abdominal aortic aneurysm with short aortic neck length.

The pectoralis major myocutaneous pedicle flap (PMMF) is still being used by many surgeons and plays an important role in head and neck reconstruction. The purpose of this series was to review our 10 years’ experience with the PMMF in head and neck reconstruction. One hundred and two patients who underwent the PMMF technique were reviewed on the clinical records. Postoperative complications were classified into flap loss, hemorrhage, infection, fistula formation, wound dehiscence and donor site complication. Eighty two patients (80.4%) demonstrated no complication. Six patients among 102 patients (5.9%) demonstrated total or partial skin necroses. Three female patients were completely dissatisfied with the cosmetic appearance after the PMMF. One of them required a reconstructive surgery with the latissimusdorsi flap. Without surgical expertise in plastic surgical field, an ear-nose-throat or an oral surgeon can performed the PMMF technique provided the operator is well aware of serious and frequent complications of this “workhorse” procedure.

This case report presents a 65-year-old man who developed early esophageal cancer with leiomyoma treated by endoscopic submucosal dissection(ESD).There have been several reports of co-existing superficial esophageal cancer and leiomyoma treated by endoscopic mucosal ***,there is no previous report describing the co-existing lesion treated by *** order to determine treatment strategies for esophageal cancer,accurate endoscopic evaluation of the cancerous depth is *** the present case,the combination of endoscopic ultrasonography and narrow-band imaging system with magnifying endoscopy was extremely useful to evaluate the superficial esophageal cancer with leiomyoma,which lead to the appropriate treatment,ESD.

目的评价高b值MR DWI及ADC值在乳腺良恶性病变诊断中的应用价值.方法 165例患者在行乳腺MR动态增强扫描前行不同b值（分别为500、1500 s/mm2）的DWI扫描,对171个怀疑或高度怀疑恶性病变者行回顾性分析.以正常乳腺组织为参考基准,选择增强图像中异常强化的高信号病变,同时在高b值（b= 1500 s/mm2）DWI中视觉判定是高信号的病变定义为恶性病变阳性结果,否则为良性病变阴性结果.对其中111个DWI视觉判定阳性结果的病变计算ADC值.依据全部病变穿刺活检病理诊断结果,应用Fisher精确检验和Wilcoxon秩和检验对比分析高b值DWI视觉评估中恶性和良性病变的阳性和阴性病灶数,以ADC值=1.13×10^-3 mm^2/s作为临界值,计算诊断的特异度和敏感度.结果乳腺病变穿刺活检病理证实的171个乳腺病变中,91个恶性病变,80个良性病变.高b值DWI视觉评估,139个阳性结果中,恶性病变83个,良性病变56个；32个阴性结果中,良性病变24个,恶性病变8个（非肿块性导管原位癌）,差异有统计学意义（P＜0.01）.所有浸润性癌和肿块样导管原位癌（DCIS）在DWI视觉判定中为阳性,8例非肿块性DCIS判定为假阴性,总体的敏感度为91.2％（83/91）,特异性为30.0％（24/80）.110个肿块样病变和1个局灶性病变DWI视觉评估阳性结果的病变中,63个恶性病变平均ADC值为（0.73±0.24）×10^-3 mm^2/s,48个良性病变平均ADC值为（1.19±0.42）×10^-3mm^2/s,差异有统计学意义（Z=5.818,P＜0.01）.以ADC值=1.13×10^-3mm^2/s作为临界值时,61个恶性病变为阳性结果,2个黏液癌为假阴性结果；27个良性病变为阴性结果,21个良性病变为假阳性,诊断敏感度是96.8％（61/63）,特异度为56.2％（27/48）.结论高b值DWI及ADC值对乳腺良恶性病变的鉴别诊断有一定的作用,但在诊断非肿块性乳腺病变时仍需慎重.

A 46-year-old man was referred to our hospital due to severe ischemic mitral regurgitation with severe bileaflet tethering and a dilated left ventricle. We performed left ventricular reconstruction and mitral valve annuloplasty combined with papillary muscle relocation. Although left ventricular continued remodeling occurred during follow-up, left ventricular reconstruction and mitral valve annuloplasty combined with papillary muscle relocation provided durable mitral repair, and his functional status was good. Thus, this combined surgical treatment may reduce mitral regurgitation recurrence after mitral valve annuloplasty.

Arsenic Polluted Groundwater and Its Countermeasures in the Middle Basin of the Ganges, Uttar Pradesh State, India

The arsenic contamination of groundwater in Uttar Pradesh State was first recognized in 2003 and is now seen at 20 Districts out of 70 Districts. University of Miyazaki has performed the arsenic mitigation project in Bahraich District, severest arsenic-affected one in the 20 Districts, from June 2008 until now, with JICA (Japan International Cooperation Agency). The integrated mitigation, such as the raising awareness of villager, installing of alternative water supply units and healthcare of arsenocosis patients, have been executed at the 2 villages. The symptom of the arsenocosis patients was not so severe, which will be, therefore, improved by drinking arsenic-safe water supplied through arsenic removal units, installed by this project. In this paper, following results is discussed for the situation and mechanism of arsenic contamination of groundwater, objected in connection with the installation of arsenic removal units: 1) Groundwater is almost contaminated with arsenic in deep tubewell (depth: about 30 m), but scarcely in shallow tubewell (depth: about 10 m);2) Arsenic contaminated groundwater is under the reduced condition with the oxidized condition for no-arsenic contaminated groundwater;3) Arsenic concentration shows almost linear correlation with concentrations of Fe2+ and -N;4) Ground is composed of sand with high arsenic content at around 25 m depth;5) Arsenic exists mainly in the phase of reducible fraction or weak acid soluble fraction but no oxidizable fraction in the ground.

Distribution of Chitinolytic Enzyme in the Organs and Molecular Cloning of a Novel Chitinase Gene from the Kidney of Marbled Rockfish <i>Sebastiscus marmoratus</i>

Actinopterygii express two types of chitinase (acidic fish chitinase-1 (AFCase-1) and acidic fish chitinase-2 (AFCase-2)) that are active at acidic pHs and involved in digestion in the stomach. We proposed the existence of a new fish chitinase that has a non-digestive function. In this study, we used Sebastiscus marmoratus, for which characteristics and cDNA cloning of chitinase isozymes (SmChi-1, SmChi-2) in the stomach have been reported. Initially, we examined the distribution of chitinase and β-N-acetylhexosaminidase (Hex) in the body and then we tried to clone novel chitinase cDNA from the kidney. Chitinase and Hex activities were measured using pNP-(GlcNAc)n, (n = 2, 3) and pNP-GlcNAc as substrates, respectively. Total RNA was extracted from the kidney. RT-PCR was performed to obtain chitinase cDNA fragments using reverse transcriptase with an oligo dT primer. The RACE method was used to obtain sequences of the upstream and downstream regions of cDNA. The full-length chitinase cDNA was determined using PrimeSTAR®?Max DNA polymerase with proofreading activity. High chitinase activity was observed in the stomach, as previously reported. In addition, relatively high activity was observed in the liver, spleen, kidney, and heart. In contrast, Hex activity was detected in all organs. This result is consistent with the report that Hex is related to body-wide metabolism. Full-length cDNA (SmChi-3) of the novel chitinase was obtained from the kidney, which contained 1440 bp open reading frames. The domain structure of SmChi-3 was assumed to be similar to those of SmChi-1 and SmChi-2. SmChi-1 and SmChi-2 have a serine and glycine-rich linker region, which is characteristic of AMCase. In contrast, SmChi-3 contained no apparent sequence in the linker region. Phylogenetic analysis revealed the existence of a new chitinase group, which was named fish chitinase-3 (FCase-3) and differed from AFCase-1 and AFCase-2.

AIM To investigated the association between adenoma detection rate(ADR) and sessile serrated ADR(SSADR) and significant predictors for sessile serrated adenomas(SSA) *** This study is a retrospective, single-center analysis. Total colonoscopies performed by the gastroenterologists at the University of Tokyo Hospital between January and December 2014 were retrospectively identified. Polyps were classified as low-grade or high-grade adenoma, cancer, SSA, or SSA with cytological dysplasia, and the prevalence of each type of polyp was investigated. Predictors of adenoma and SSA detection were examined using logistic generalized estimating equation models. The association between ADR and SSADR for each gastroenterologist was investigated by calculating a correlation coefficient weighted by the number of each gastroenterologist's *** A total of 3691 colonoscopies performed by 35 gastroenterologists were assessed. Overall, 978 (26.5%) low-and 84 (2.2%) high-grade adenomas, 81 (2.2%) cancers, 66 (1.8%) SSAs, and 2 (0.1%) SSAs with cytological dysplasia were detected. Overall ADR was 29.5%(men 33.2%, women 23.8%) and overall SSADR was 1.8%(men 1.7%, women 2.1%). In addition, 672 low-grade adenomas (68.8% of all the detected lowgrade adenomas), 58 (69.9%) high-grade adenomas, 29 (34.5%) cancers, 52 (78.8%) SSAs, and 2 (100%) SSAs with cytological dysplasia were found in the proximal colon. Adenoma detection was the only significant predictor of SSA detection (adjusted OR: 2.53, 95%CI: 1.53-4.20; P < 0.001). The correlation coefficient between ADR and SSADR weighted by the number of each gastroenterologist's examinations was 0.606(P < 0.001).CONCLUSION Our results demonstrated that ADR is correlated to SSADR. In addition, patients with adenomas had a higher prevalence of SSAs than those without adenomas.

Study of the Incorporation Behavior of L-Leu and D-Leu in L-Val Crystallization

Branched-Chain Amino Acids (BCAAs) are essential amino acids mainly produced through fermentation. With respect to BCAA crystallization, the incorporation behavior of L-form BCAAs as a guest amino acid in the crystallization of L-form BCAAs is well known. However, the incorporation behavior of D-form BCAAs as a guest amino acid in the crystallization of L-form BCAAs is not clear. In this study, we focused on the crystal conformation and incorporation behavior of D-Leucine (D-Leu) and L-leucine (L-Leu) in the crystallization of L-valine (L-Val). Moreover, the difference in the incorporation behavior was investigated on the basis of the distribution ratio of crystals to mother liquor, and the crystal structure and interaction energy were calculated using a molecular dynamics method. The crystal composed of L-Leu in L-Val formed as a solid solution and L-Leu was substituted into the crystal lattice of L-Val. In the case where D-Leu was added as a guest amino acid during L-Val crystallization, D-Leu was not incorporated into the L-Vallattice because the interaction energy between a D-Leu molecule and the L-Val crystal lattice was substantially greater than that between an L-Leu molecule and the L-Val crystal lattice.
