The Extension Rate of the Medial Collateral Ligament of the Knee Joint during the Valgus Stress Test: Two Case Reports

This study aimed to evaluate the influence of measuring the length of the medial collateral ligament (MCL) to compare the MCL burden when the knee joint is placed under valgus stress in the open and closed and closed kinetic chain. Two examiners conducted the examination. The MCL length was measured using ultrasonography. Two subjects were measured in unload bearing and load-bearing positions, with and without valgus stress test at the knee joint extension and 30° flexion, under eight different measurement conditions. The MCL of the subject was delineated in the longitudinal direction using an ultrasound system. The attachment points of the medial femoral and tibial condyle of the MCL were identified, and the ligament length was measured. The MCL rate before and after the valgus stress test in the loading and unloading positions was calculated. The MCL length increased by an average of 8.9% when the external stress test was performed in the non-weight bearing and knee extension positions and by an average of 17.0% when external stress was applied in the non-weight bearing and knee flexion positions. The MCL length increased by an average of 12.2% when the external stress test was performed in the load-bearing and knee extension positions and an average of 8.9% when the valgus stress test was applied in load-bearing and knee flexion positions. In conclusion, the effect of valgus stress on the MCL differs between load-bearing and non-load-bearing positions. It is considered that the dynamic stabilization mechanism works in the knee joint flexion position in the load position and works simultaneously as the static stabilization mechanism, which limits the knee joint valgus and reduces the extension rate of MCL. Effective Atomic Number Measurement with Energy-Resolved Computed Tomography Using Two-Dimensional “transXend” Detector

Introduction: We have previously developed an effective atomic number (Zeff) measurement method using linear attenuation coefficients (LACs) obtained by energy-resolved computed tomography (CT) with one-dimensional (1D) detector. The energy-resolved CT was performed with a “transXend” detector, which measured X-rays as electric current and then gave X-ray energy distribution with unfolding analysis using pre-estimated response function (RF). The purpose of this study is to measure Zeff by the energy-resolved CT using a flat panel detector (FPD). Methods: To demonstrate a 2D transXend detector, we developed the stripe absorbers for the FPD. Eleven human tissue-equivalent material rods which were grouped into four material categories were measured by X-rays with 120 kVp tube voltage, 2.3 mA tube current, and 1.0 s exposure time. Zeff is measured by the ratio of LACs with two different pseudo-monochromatic X-ray energies. RFs of each rod material were estimated by numerical calculation. First, we employed the RF estimated for the same rod material (self-RF scenario). Second, we employed the RF estimated for the different rod materials in the same material category (cross-RF scenario). The purpose of the cross-RF scenario was to find representative rod materials in each material category. Results: Upon the self-RF scenario, measured Zeffs were systematically underestimated. Median relative error to theoretical Zeff was -6.92% (range: -7.89% - -4.60%). After normalizing measured Zeffs to the theoretical one for Breast, median relative error improved to -0.75% (range: -1.79% - +1.73%). Upon the cross-RF scenario, the representative rod materials were found in two material categories. Conclusion: Zeff measurements were performed by energy-resolved CT using 2D transXend detector with numerically-estimated RF data. Normalized Zeffs for all rod materials in the self-RF scenario were in good agreement with the theoretical ones.

Redox Responsive Transcription Factor1 （RRTF1） in Arabidopsis is rapidly and transiently upregulated by H202, as well as biotic- and abiotic-induced redox signals. RRTF1 is highly conserved in angio- sperms, but its physiological role remains elusive. Here we show that inactivation of RRTF1 restricts and overexpression promotes reactive oxygen species （ROS） accumulation in response to stress. Transgenic lines overexpressing RRTF1 are impaired in root and shoot development, light sensitive, and susceptible to Alternaria brassicae infection. These symptoms are diminished by the beneficial root endophyte Piriformospora indica, which reduces ROS accumulation locally in roots and systemi- cally in shoots, and by antioxidants and ROS inhibitors that scavenge ROS. More than 800 genes were detected in mature leaves and seedlings of transgenic lines overexpressing RRTF1; ∽40% of them have stress-, redox-, ROS-regulated-, ROS-scavenging-, defense-, cell death- and related functions. Bioinformatic analyses and in vitro DNA binding assays demonstrate that RRTF1 binds to GCC-box-like sequences in the promoter of RRTFl-responsive genes. Upregulation of RRTF1 by stress stimuli and H202 requires WRKY18/40/60. RRTF1 is co-regulated with the phylogenet- ically related RAP2.6, which contains a GCC-box-like sequence in its promoter, but transgenic lines overexpressing RAP2.6 do not accumulate higher ROS levels. RRTF1 also stimulates systemic ROS accumulation in distal non-stressed leaves. We conclude that the elevated levels of the highly conserved RRTF1 induce ROS accumulation in response to ROS and ROS-producing abiotic and biotic stress signals.

Gas chromatography (GC) is one of the most versatile separation methods for volatile compounds, and a variety of polymeric materials as the stationary phases have been developed for various applications. In contrast to the successful applications and commercialization of the polymer-coated columns, the reports for the fibrous polymer-packed stationary phases, however, were quite limited except for the surface characterization of the fibers in the inverse GC (IGC) method. The IGC technique is basically based on the observation of a specific interaction between the fiber surface and the standard solute molecules injected as the sample probes.

Effects of Carboxymethylcelluloses (CMC) on Some Hydraulic Properties of Sandy Soil

The property of hydrophilic polymers capable absorbing huge volumes of water led to many practical applications of these new materials in arid regions for improving the water retention in sandy soils. Effects of four carboxymethylcelluloses （CMC） mixed at various rates with the sandy soil, on the water-holding capacity and hydraulic conductivity （Ks） when leached with distilled water （simulating rain）, tap water, and saline water were evaluated. The maximum water absorption of CMCs ranged between 80 and 100 kg. kg^-1 of polymer; however, the absorbent swelling capacity decreased significantly with increasing the salt concentration in the solution. The water absorption capacity of CMCs decreased significantly when incorporated in the sandy soil compared to that of the absorbent alone. Application of CMC increased significantly the available water content up to 3 ± 0.5 times. All soils treated with CMCs showed a significant lower in Ks compared to the control soil. Meanwhile, Ks was found increased with increasing the salt concentration in the leaching solution. This understanding of characteristics of the absorbents and the interactions among absorbents, soil, and irrigation water quality would be of help in water management of sandy soil

Safety and efficacy of metallic stent for unresectable distal malignant biliary obstruction in elderly patients

AIM To study the safety of insertion of metallic stents in elderly patients with unresectable distal malignant biliary *** Of 272 patients with unresectable distal malignant biliary obstruction, 184 patients under the age of 80 were classified into Group A, and 88 subjects aged 80 years or more were classified into Group B. The safety of metallic stent insertion, metal stent patency period, and the obstruction rate were examined in each *** In Group B, patients had a significantly worse per-formance status, high blood pressure, heart disease, cerebrovascular disease, and dementia; besides the rate of patients orally administered antiplatelet drugs or anticoagulants tended to be higher(P < 0.05). Metallic stents were successfully inserted in all patients. The median patency period was 265.000 ± 26.779(1-965) d; 252.000 ± 35.998(1-618) d in Group A and 269.000 ± 47.885(1-965) d in Group B, with no significant difference between the two groups. Metallic stent obstruction occurred in 82 of the 272(30.15%) patients; in 53/184(28.80%) patients in Group A and in 29/88(32.95%) of those in Group B, showing no significant difference between the two groups. Procedural accidents due to metal stent insertion occurred in 24/272(8.8%) patients; in 17/184(9.2%) of patients in Group A and in 7/88(8.0%) of those in Group B, with no significant difference between the two groups, *** These results suggested that metallic stents can be safely inserted to treat unresectable distal malignant biliary obstruction even in elderly patients aged 80 years or more.

Generation of Arbitrary Pressure Pulsation of Wide Frequency Range for Flow Meter Testing in a Laminar Gas Pipeline

The present paper presents a device to test flow meters under an arbitrary pressure pulsation in a gas pipeline with a laminar flow containing frequency components up to 50 Hz, with the amplitude reaching hundreds of pascals. In order to reduce flow noise, the device has a strainer-like element connected to a pipeline under test and uses an open-loop control law based on the frequency response test. The control signal is calculated by adding the inputs to obtain each of the sinusoidal waves included in the original wave, which was decomposed by Fourier analysis. The validity of the developed method is demonstrated through the generation tests of superimposed pressure waves containing frequency components up to 50 Hz. Analysis of the relative uncertainty demonstrated the relative uncertainty to be less than 10% when the generated pressure is larger than 360 Pa.

Real-world local recurrence rate after cold polypectomy in colorectal polyps less than 10 mm using propensity score matching

BACKGROUND The impacts of chemotherapy on patients with malignant gastrointestinal obstructions remain unclear,and multicenter evidence is *** To evaluate the effectiveness and safety of chemotherapy in patients with unresectable malignant gastrointestinal *** We conducted a multicenter retrospective cohort study that compared the chemotherapy group who received any chemotherapeutics after interventions,including palliative surgery or selfexpandable metal stent placement,for unresectable malignant gastrointestinal obstruction vs the best supportive care(BSC)group between 2014 and 2019 in nine *** primary outcome was overall survival,and the secondary outcomes were patency duration and adverse events,including gastrointestinal perforation and gastrointestinal *** In total,470 patients in the chemotherapy group and 652 patients in the BSC group were *** the follow-up period of 54.1 mo,the median overall survival durations were 19.3 mo in the chemotherapy group and 5.4 mo in the BSC group(log-rank test,P<0.01).The median patency durations were 9.7 mo[95% confidence interval(CI):7.7-11.5 mo]in the chemotherapy group and 2.5 mo(95%CI:2.0-2.9 mo)in the BSC group(log-rank test,P<0.01).The perforation rate was 1.3%(6/470)in the chemotherapy group and 0.9%(6/652)in the BSC group(P=0.567).The gastrointestinal bleeding rate was 1.5%(7/470)in the chemotherapy group and 0.5%(3/652)in the BSC group(P=0.105).CONCLUSION Chemotherapy after interventions for unresectable malignant gastrointestinal obstruction was associated with increased overall survival and patency duration.
