Reliability of ultrasound ovarian-adnexal reporting and data system amongst less experienced readers before and after training

BACKGROUND The 2018 ovarian-adnexal reporting and data system(O-RADS)guidelines are aimed at providing a system for consistent reports and risk stratification for ovarian lesions found on *** provides key characteristics and findings for lesions,a lexicon of descriptors to communicate findings,and risk characterization and associated follow-up recommendation ***,the ORADS guidelines have not been validated in North American institutions or amongst less experienced *** To evaluate the diagnostic accuracy and inter-reader reliability of ultrasound ORADS risk stratification amongst less experienced readers in a North American institution with and without pre-test *** A single-center retrospective study was performed using 100 ovarian/adnexal lesions of varying O-RADS *** these cases,50 were allotted to a training cohort and 50 to a testing cohort via a non-randomized group selection process in order to approximately equal distribution of O-RADS categories both within and between *** standard O-RADS scores were established through consensus of three fellowship-trained body imaging *** pGY-4 residents were independently evaluated for diagnostic accuracy and inter-reader reliability with and without pre-test O-RADS ***,specificity,positive predictive value,negative predictive value(NpV),and area under the curve(AUC)were used to measure *** kappa and weighted quadratic(pairwise)kappa values were used to measure inter-reader *** significance was ppatient age was 40±16 years with lesions ranging from 1.2 to 22.5 *** demonstrated excellent specificities(85%-100%pre-training and 91%-100%post-training)and NpVs(89%-100%pre-training and 91-100%post-training)across the O-RADS *** were variable(55%-100%pre-training and 64%-100%post-training)with malignant ORADS 4 and 5 Lesions pre-training and post-training A

Effect of training on resident inter-reader agreement with American College of Radiology Thyroid Imaging Reporting and Data System

Two-point Dixon and six-point Dixon magnetic resonance techniques in the detection,quantification and grading of hepatic steatosis

BACKGROUND Hepatic steatosis is a very common problem *** To assess the performance of two-and six-point Dixon magnetic resonance(MR)techniques in the detection,quantification and grading of hepatic *** A single-center retrospective study was performed in 62 patients with suspected parenchymal liver *** sequences included two-point Dixon,six-point Dixon,MR spectroscopy(MRS)and MR *** fraction(FF)estimates on the Dixon techniques were compared to the MRS-proton density FF(pDFF).Statistical tests used included pearson’s correlation and receiver operating *** FF estimates on the Dixon techniques showed excellent correlation(≥0.95)with MRS-pDFF,and excellent accuracy[area under the receiver operating characteristic(AUROC)≥0.95]in:(1)Detecting steatosis;and(2)Grading severe steatosis,(ppoint Dixon was not evaluable due to confounding T2**** estimates on six-point Dixon vs MRS-pDFF showed a moderate correlation(0.82)in iron overload vs an excellent correlation(0.97)without iron overload,(ppoint Dixon in grading mild steatosis improved(AUROC:0.59 to 0.99)when iron overload cases were *** excellent correlation(>0.9)between the Dixon techniques vs MRSpDFF did not change in the presence of liver fibrosis(pperformed satisfactorily for the evaluation of hepatic steatosis but with exceptions.

Fhotocatalytic degradation of polybrominated diphenyl ethers in pure water system

Due to the low water solubility of polybromi- nated diphenyl ethers, organic solvent is usually added into the oxidation system to enhance the removal efficiency. In this study the photocatalytic degradation of decabromodi- phenyl ether （BDE209）, a type ofpolybrominated diphenyl ether used throughout the world, in pure water without the addition of organic solvent was investigated. In the pure water system, BDE209 was not dissolved but dispersed as nano-scale particles with a mean diameter of 166 nm. Most of BDE209 （〉 98%） were removed within 4 h and the final debromination ratio was greater than 80%. Although the addition of organic solvent （tetrahydrofuran, THF） could lead to a relatively high BDE209 degradation rate, the final debromination ratio （〈 50%） was much lower than that in pure water system. Major oxidation inter- mediates of tetrahydrofuran, including tetrahydro-2-fura- nol and y-butyrolactone, were detected indicating the engagement of THF in the BDE209 degradation process. The photocatalytic degradation of BDE209 in the pure water system followed first-order kinetics. The BDE209 degradation rate constant increased from 0.0011 to 0.0023 min-1 as the pH increased from 3 to 9.

Implementation and application of pyNE sub-voxel R2S for shutdown dose rate analysis

pyNE R2S is a mesh-based R2S implementation with the capability of performing shutdown dose rate(SDR)analysis directly on CAD geometry with Cartesian or tetrahedral *** supports advanced variance reduction for fusion energy ***,the assumption of homogenized materials of pyNE R2S with a Cartesian mesh throughout a mesh voxel introduces an approximation in the case where a voxel covers multiple non-void *** work implements a sub-voxel method to add fldelity to pyNE R2S with a Cartesian mesh during the process of activation and photon source sampling by performing independent inventory calculations for each cell within a mesh voxel and using the results of those independent calculations to sample the photon source more *** sub-voxel R2S has been verifled with the Frascati Neutron Generator(FNG)-ITER and ITER computational shutdown dose rate benchmark *** results for sub-voxel R2S show satisfactory agreement with the experimental values or reference *** sub-voxel R2S has been applied to the shutdown dose rate calculation of the Chinese Fusion Engineering Testing Reactor(CFETR).In conclusion,sub-voxel R2S is a reliable tool for SDR calculation and obtains more accurate results with the same voxel size than voxel R2S.

Trace analysis of off-flavor/odor compounds in water using liquid-liquid microextraction coupled with gas chromatography--positive chemical ionization-tandem mass spectrometry

A rapid, inexpensive and laboratory friendly method was developed for analysis of off-flavor/odor compounds in fresh and salt water using gas chromatography with chemical ionization-tandem mass spectrometry. Off-flavor/odor compounds, included geosmin, 2- methylisobomeol （MIB）, 2-isobutyl-3-methyoxypyrazine （IBMp）, and 2-isopropyl-3-methoxypyrazine （IpMp）. Using this method, a single sample can be extracted within minutes using only 1 mL of organic solvent. The ion transitions for IpMp, IBMp, MIB, and geosmin were 153 〉 121, 167 〉 125, 152 〉 95, and 165 〉 109, respectively. The linearity of this method for analyzing MIB ranged from 4 to 200ng·L^-1, and from 0.8 to 200ng·L^-1 for the other analytes. Method recoveries ranged from 97% to 111% and percent relative standard deviations ranged from 3% to 9%, indicating that the method is accurate, precise, and reliable.

pancreatic cancer and its stroma: A conspiracy theory

pancreatic cancer is characterised by a prominent desmoplastic/stromal reaction that has received little attention until recent times. Given that treatments focusing on pancreatic cancer cells alone have failed to significantly improve patient outcome over many decades, research efforts have now moved to understanding the pathophysiology of the stromal reaction and its role in cancer progression. In this regard, our Group was the first to identify the cells(pancreatic stellate cells, pSCs) that produced the collagenous stroma of pancreatic cancer and to demonstrate that these cells interacted closely with cancer cells to facilitate local tumour growth and distant metastasis. Evidence is accumulating to indicate that stromal pSCs may also mediate angiogenesis, immune evasion and the well known resistance of pancreatic cancer to chemotherapy and radiotherapy. This review will summarise current knowledge regarding the critical role of pancreatic stellate cells and the stroma in pancreatic cancer biologyand the therapeutic approaches being developed to target the stroma in a bid to improve the outcome of this devastating disease.
