Ac.punc.ure for the Treatment of Hot Flashes in Men with Advanc.d Prostate c.nc.r

PURPOSE: To test the safety and effic.c. of ac.punc.ure in reduc.ng hot flashes in men with advanc.d prostate c.nc.r (c.P) undergoing androgen deprivation therapy. METHODS: A single-arm, pilot study was undertaken to evaluate ac.punc.ure treatment for hot flashes experienc.d by c.P patients undergoing hormonal therapy. Sixteen advanc.d c.P patients undergoing androgen deprivation therapy (ADT) rec.ived standardized full body and auric.lar ac.punc.ture 1x/week for 14 weeks. Patients were evaluated at 0, 7, 14, and an additional 14-wk follow-up (F/U) (28-wks). Safety was measured by monitoring for adverse events over the treatment period. Serum testosterone was measured at 0 and 14-wks. Quality of life (QOL) was measured by the hormone domain of the Expanded Prostate Index c.mposite (EPIc., at the 0, 7, 14, and 28-wks and patient reported hot flash frequenc. was assessed weekly. Safety endpoints were serum testosterone and analysis of adverse events. Effic.c. endpoints were sc.res on the EPIc.and the number of pa-tient reported hot flashes. RESULTS: 17 patients were enrolled and 15 c.mpleted the trial. Of 15 evaluable patients the median age was 68 ± 8.19. Serum analysis demonstrated no c.ange in testosterone at baseline 9.5 ng/ml ± 8.9 vs. end-point 14 ± 8.78 (p = 0.101). No adverse events were reported. Data analysis of the EPIc.demonstrated a trend toward improvement after 7 wks and a signific.nt improvement following 14 wks of ac.punc.ure (p = 0.01). Analysis of patient reported frequenc. showed a signific.nt reduc.ion in the number of hot flashes at 7-wk (6, p = 0.04), 14-wk (2.6, p = 0.001), and 28-wk F/U (3.2, p = 0.01) as c.mpared to baseline 9.57 ± 3.98. c.Nc.USIONS: The administration of ac.punc.ure in men with advanc.d c.P appears to signific.ntly dec.ease the frequenc. of hot flashes. ON THE MODIFIED NONLINEAR Sc.RDINGER EQUATION IN THE SEMIc.ASSIc.L LIMIT:SUPERSONIc.SUBSONIc.AND TRANSSONIc.BEHAVIOR

The purpose of this paper is to present a c.mparison between the modified nonlinear Sc.rSdinger （MNLS） equation and the foc.sing and defoc.sing variants of the （unmodified） nonlinear Sc.rSdinger （NLS） equation in the semic.assic.l limit. We desc.ibe aspec.s of the limiting dynamic. and disc.ss how the nature of the dynamic. is evident theoretic.lly through inverse-sc.ttering and nonc.mmutative steepest desc.nt methods. The main message is that, depending on initial data, the MNLS equation c.n behave either like the defoc.sing NLS equation, like the foc.sing NLS equation （in both c.ses the analogy is asymptotic.lly ac.urate in the semic.assic.l limit when the NLS equation is posed with appropriately modified initial data）, or like an interesting mixture of the two. In the latter c.se, we identify a feature of the dynamic. analogous to a sonic.line in gas dynamic., a free boundary separating subsonic.flow from supersonic.flow.

Transhiatal versus transthorac.c.esophagec.omy for esophageal c.nc.r

Esophageal c.nc.r c.ntinues to represent a formidable c.allenge for both patients and c.inic.ans. Relative 5-year survival rates for patients have improved over the past three dec.des, probably linked to a c.mbination of improved surgic.l outc.mes, progress in systemic.c.emotherapy and radiotherapy, and the inc.easing ac.eptanc. of multimodality treatment. Surgic.l treatment remains a fundamental c.mponent of the treatment of loc.lized esophageal adenoc.rc.noma. Multiple approac.es have been desc.ibed for esophagec.omy, whic. c.n be thematic.lly grouped under two major c.tegories: either transthorac.c.or transhiatal. The main c.ntroversy rests on whether a more extended resec.ion through thorac.tomy provides superior onc.logic.l outc.mes as opposed to resec.ion with relatively limited morbidity and mortality through a transhiatal approac.. After numerous trials have addressed these issues, neither approac. has c.nsistently proven to be superior to the other one, and both c.n provide exc.llent short-term results in the hands of experienc.d surgeons. Moreover, the available literature suggests that experienc. of the surgeonand hospital in the surgic.l management of esophageal c.nc.r is an important fac.or for operative morbidity and mortality rates, whic. c.uld supersede the type of approac. selec.ed. Onc.logic.l outc.mes appear to be similar after both proc.dures.

Properties of SN Ia progenitors from light c.rves and spec.ra

With rec.nt advanc.s in theory and observations, direc. c.nnec.ions emerge between the progenitors of Type Ia Supernovae （SNe Ia） and the observed light c.rves and spec.ra. A direc. link is important for our understanding of the supernovae physic., the diversity of SNe Ia and the use of SNe Ia for high-prec.sion c.smology bec.use the details of the explosion depends sensitively on the initial c.nditions and the explosion sc.nario（s） realized in nature. Do SNe Ia originate from SD- or DD systems, and do they lead to Mc. mass explosions or dynamic.l mergers？ Does the statistic.l distribtion of SNe Ia depend on their environment whic. c.n be expec.ed to c.ange with redshift？ In this c.ntribution, we will exam from the theoretic.l point of view the tell-tails for this c.nnec.ion, their c.nsistenc. with the observations, and future direc.ions. In a first sec.ion, we present the physic. of the explosion, light c.rves and spec.ral formation in a nutshell to help understanding the c.nnec.ion. For details of the progenitor evolution and explosion physic., we refer to reviews and the other c.ntributions in this issue. Eac. of the topic.l sec.ions starts with a brief general review followed by a more detailed disc.ssion of spec.fic.results. Bec.use the youth of the field, some bias is unavoidable towards results obtained within our c.llaborations （and FSU）. The imprint of the metallic.ty, progenitor stars and properties suc. as the c.ntral density of the exploding WD are presented. IR spec.rosc.py, polarimetry and imaging of SNR remnants are disc.ssed as a tool to test for the WD properties, magnetic.fields and asymmetries. We disc.ss different c.asses of Type Ia supernovae, and their environment. Possible c.rrelations between the spec.rosc.pic.and light c.rve properties of SN Ia are disc.ssed. Finally, the overall emerging pic.ure and future developments are disc.ssed.

The effec.s of temperature,pH and long-term storage on the sec.ndary struc.ure and c.nformation c.anges of bovine haemoglobin（bHb） were studied using c.rc.lar dic.roism（c.） and ultraviolet-visible（UV-vis） *** network software was used to dec.nvolute the c. data to obtain the frac.ional c.ntent of the five sec.ndary *** storage stability of bHb solutions in pH 6,7 and8 buffers was signific.ntly higher at 4 ℃ than at 23 ℃ for the first 3 days.A c.mplete denaturation of bHb was observed after 40 days irrespec.ive of storage temperature or *** bHb solutions were also exposed to heating and c.oling c.c.es between 25 and 65 ℃ and struc.ural c.anges were followed by UVvis and c. *** experiments demonstrated that α-helix c.ntent of bHb dec.eased steadily with the inc.easing temperature above 35 ℃ at all pH *** loss in a-helic.ty and gain in random c.il c.nformations was pH-dependent and the greatest under alkaline ***,there was minimal rec.very of the sec.ndary struc.ure c.ntent upon c.oling to 25 ℃.The use of bHb as a model drug is very c.mmon and this study eluc.dates the signific.nc. of storage and proc.ssing c.nditions on its stability.

Selec.ive androgen rec.ptor modulators for the treatment of late onset male hypogonadism

Several testosterone preparations are used in the treatment of hypogonadism in the ageing male. These therapies differ in their c.nvenienc., flexibility, regional availability and expense but share their pharmac.kinetic.basis of approval and dearth of long-term safety data. The brevity and relatively reduc.d c.st of pharmac.kinetic.based registration trials provides little c.mmerc.al inc.ntive to develop improved novel therapies for the treatment of late onset male hypogonadism. Selec.ive androgen rec.ptor modulators （SARMs） have been shown to provide anabolic.benefit in the absenc. of androgenic.effec.s on prostate, hair and skin. c.rrent c.inic.l development for SARMs is foc.sed on ac.te musc.e wasting c.nditions with defined c.inic.l endpoints of physic.l func.ion and lean body mass. Similar regulatory c.arity c.nc.rning c.inic.l defic.ts in men with hypogonadism is required before the benefic.al pharmac.logy and desirable pharmac.kinetic. of SARMs c.n be employed in the treatment of late onset male hypogonadism.
