Practices of Maternity’s Care Providers in the Municipality of Kozah 1 Regarding the Administration of Vitamin K1, Eye Care, and Umbilical Cord Care in Immediate Postpartum Newborns

Background: Neonatal mortality is a real public health problem in the world and particularly in countries with limited resources. Essential care remains an effective means of reducing this mortality. Objective: To evaluate the practice of administering of vitamin K1, ocular and cord care in maternity wards in the Kozah 1 municipality. Method: This is a cross-sectional study conducted from March 1st to June 30th, 2022 in all maternity wards in the Kozah 1 municipality. Socioprofessional aspects of providers and those related to the administration of vitamin K1, ocular and cord care were studied. Results: Eighty-five (85) maternity providers were included in this study. The average age of the providers was 36.2 years with extremes of 21 and 55 years. Providers were midwives (51.8%), childbirth attendants (35.3%), and matrons (12.9%). The average length of practice was 9.5 years with extremes of 1 and 28 years. For three out of four providers (75.3%), the postpartum period was defined as a period of 02 hours following delivery. The importance of cord, ocular care, and administration of vitamin K1 in postpartum was known respectively by 84.7%, 98.8%, and 92.9% of the providers. Chlorhexidine gel was used by 81.2% of providers for umbilical cord care. For ocular disinfection, more than half of the providers (52.9%) reported using half-diluted povidone iodine. No center had displayed care protocol. Conclusion: This study highlighted the level of knowledge and practice of three essential postpartum care practices. The products used for the different care practices are consistent with recommendations but vary according to the care providers.

Right Ulnar Reconstruction after Sequestrectomy by Non-Vascularized Fibular Transfer in a Girl of Three Years

Chronic osteomyelitis is serious because of the orthopedic sequels that they could cause. Extended diaphyseal sequestrations could cause bone loss and their management is delicate. Here we report a case of right ulnar diaphyseal reconstruction by non-vascularized fibula transfer. This was a three-year-old girl, non-sickle cell, who had chronic osteomyelitis of the right ulna. The evolution was towards an almost total ulnar diaphyseal sequestration with externalization of the distal extremity. The removal of this large sequestrum occurred almost spontaneously, leaving a significant bone loss over a length of about 6 cm. Secondarily, we reconstructed the right ulnar diaphysis by transfer of a free non-vascularized graft of the left fibula, maintained by a pin. The follow up was favorable with almost complete recovery of prono-supination. Fibular ossification has evolved as well and we did not notice any complications at the graft collection site. Non-vascularized fibula graft transfer is a useful therapeutic option in the management of significant bone defects secondary to chronic osteomyelitis of one of the two forearm bones.

Multifocal Tuberculosis Revealed by Facial Paralysis

Introduction: Tuberculosis is a chronic infectious disease that remains a global public health problem. Children, accounting for 10% of cases, are particularly vulnerable to this disease. Based on a case of multifocal tuberculosis in a 14-month-old infant, the authors conducted a literature review on this condition. Observation: The case involved a 14-month-old infant referred from a health center for persistent symptoms including anemia, fever, and lateral deviation of the mouth during crying. Clinical examination revealed an infectious and meningeal syndrome, as well as facial paralysis. The results of brain imaging, cerebrospinal fluid examination, fundus examination, and gastric lavage fluid examination led to the conclusion of multifocal tuberculosis in the infant without association with HIV. Improvement was observed one week after the start of antituberculosis treatment, with complete recovery without sequelae after 12 months. Conclusion: Although rare, multifocal tuberculosis does exists and its diagnosis remains challenging in infants. Early treatment generally leads to favorable outcomes, especially in the absence of HIV coinfection.

Epidemiological Aspects of Diseases Seen in Pediatric Surgery Consultation at the Kara Teaching Hospital

Background: Pediatric surgery in developing countries faces serious problems of inadequate infrastructures and equipment. Since July 2019, a new department was created in the north of Togo. It is in this context that this study is initiated, with the aim of taking stock of the epidemiological distribution of pediatric surgical diseases. Material and methods: This was a retrospective and descriptive study over a period of eighteen months, from October 30, 2019 to April 30, 2021. It took place in the pediatric surgery department of the Kara teaching hospital. It concerns pathologies seen at the consultation in the department collected in consultation register. Epidemiological parameters were studied. Results: In eighteen (18) months, two hundred and fifty-nine children, were seen in pediatric surgery consultation at the Kara Teaching Hospital. The annual frequency of consultations was 172.66. The monthly frequency was 14.39. The average age of the children was 4.56 years with extremes of 2 days and 15 years. There were 194 boys (74.90%) and 65 girls (25.10%). At the consultation, whatever the reason, the examination was normal in 17 children and 242 children had abnormalities. Two hundred and twelve children (81.85%) had each one abnormality and 30 children (11.58%) had each at least two. A total of 298 diseases were reported in the 242 children. These were 218 congenital diseases (73.15%) dominated by urological abnormalities with 37.16% of cases and 80 acquired diseases (26.85%). Peritoneovaginal Duct and Nück’s Duct Persistence in Children at Campus Teaching Hospital in Lomé(Togo)

Introduction: Peritoneovaginal duct (PVD) and Nück duct (ND) include several abnormalities with many clinical entities that are at the cause of a variety of symptoms. These are frequent in pediatric surgeon practice and could lead to serious complications. The aim of our work was to describe the epidemiological, diagnostic and therapeutic aspects of pathologies of PVD or ND in our conditions after the first five years of surgical activities of our pediatric department. Material and Methods: This was a retrospective and descriptive study, carried out on the files of children, aged zero to 15 years, presenting PVD or ND pathology, operated in the service, over a period of 4 years (2016 to 2019). Results were classified good, medium or bad. Results: During the study period, among the 865 children operated in pediatric surgery, 164 children were found for PVD and ND pathologies (18.95%). The average age was 5.3 years old. There were 152 boys (92.68%) and 12 girls (7.32%). The reason for consultation was inguinal or inguino-scrotal swelling in 99.3% of the children. The right side was affected in 58.5% of cases. In boys, there were inguinal hernias (27.5%), inguinoscrotal hernias (30.3%), hydroceles (36.9%) and cysts (5.3%). In girls, there were inguinal hernias (91.7%) and 1 ovarian hernia (8.3%). The umbilical hernia was associated with these pathologies in 39 cases. Treatment was conventional closure of the PVD or ND. With an average follow-up of 2.5 years (range 3 months and 3.5 years), the results were good in all cases. Conclusion: Pathologies of PVD or ND took an important place in the activities of the pediatric surgery department. Management by conventional surgery has given good results in all cases. Early treatment of these pathologies helps prevent complications.
