一项甘露醇治疗脑出血的随机对照研究

Objective: To study the usefulness of mannitol in spontaneous intracerebral hemorrhage (ICH) patients. Method: 128 CT proven supratentorial ICH patients within 6 days of ictus were randomized into study and control groups. The study group received mannitol 20%, 100 ml every 4 h for 5 days, tapered in the next 2 days. The control group received sham infusion. Primary endpoint was 1-month mortality and secondary endpoint functional disability at 3 months assessed by Barthel index score. Results: There were 65 patients in study and 63 in control groups. The study and control groups were evenly matched regarding age, Glasgow coma scale (GCS) score, Canadian Neurological Scale (CNS) score, pupillary asymmetry, pyramidal signs on non-hemiplegic side, and location, midline shift and ventricular extension of hematoma. At 1 month, 16 patients died in each group. The primary and secondary endpoints were not significantly different between the two groups. Conclusion: Low dose mannitol does not seem to be beneficial in patients with ICH.

The electrochemical behavior of Ti-6Al-4V with 1 mm and 16 mm thickness prepared by electron beam powder bed fusion(EB-PBF)was investigated in phosphate buffered *** results showed that EB-PBF Ti-6Al-4V with a larger component size was more resistant to corrosion compared to the smaller component,because of less acicularαʹphase content and moreβphase *** a non-equilibrium phase in the“high-energy state”,αʹphase has a greater susceptibility to corrode and reduces the corrosion resistance of the material,whileβphase improves corrosion resistance of titanium *** results show that the phase composition has a more significant effect on the corrosion performance than the grain size.

In situ tensile testing in a scanning electron microscope(SEM)in conjunction with high-resolution electron backscatter diffraction(HR-EBSD)under load was used to characterize the evolution of geometrically necessary dislocation(GND)densities at individual grain boundaries as a function of applied strain in a polycrystalline Mg-4Al *** increase in GND density was investigated at plastic strains of 0%,0.6%,2.2%,3.3% from the area including 76 grains and correlated with(i)geometric compatibility between slip systems across grain boundaries,and(ii)plastic *** develop expressions for the grain boundary GND density evolution as a function of plastic strain and plastic incompatibility,from which uniaxial tensile stress-strain response of polycrystalline Mg-4Al are computed and compared with experimental *** findings in this study contribute to understanding the mechanisms governing the strain hardening response of single-phase polycrystalline alloys and more reliable prediction of mechanical behaviors in diverse microstructures.

A Comprehensive Analysis of Efficacy and Safety of Probiotics in Inflammatory Bowel Disease

Introduction: Inflammatory bowel disease (IBD) affects approximately 7 million people worldwide. In the u.S. alone, per the CDC, 1.3% of adults, which is approximately 3,000,000 people, are diagnosed with inflammatory bowel disease-either, Crohn’s disease, or ulcerative colitis. The estimated cost of treatment can be close to $23,000 annually, with treatment regimens comprising biologic agents and anti-inflammatory therapies. Probiotics have recently gathered interest as a low-cost additional therapy option that, in addition to the current regimen of IBD management, allows for reductions in rates of IBD flare-ups by significantly reducing the number of emergency room visits and avoiding the need to constantly escalate treatment by addition of biologic agents in achieving remission. The Analysis Goal: Our research project aimed to see if there was a significant difference in the addition of probiotics to standard therapy in inflammatory bowel disease by comparing existing research studies and trials. We analyzed RCTs published in PubMed to assess the efficacy and safety of probiotics in patients with IBD in preventing frequent disease flare-ups and reducing the cost of care. Research Methods: We did a comparative analysis of available RCTs using a PubMed search and included studies that researched the addition of probiotic strains in patients with IBD (ulcerative colitis and Crohn’s disease). After reviewing the inclusion and exclusion criteria, the trials selected for analysis were reviewed by two independent reviewers. Results: We analyzed 21 RCTs, and 16 RCTs (76.2%) showed that probiotics are an effective therapy for IBD, inducing remission and reducing flare-up rates in patients on a standard treatment regimen. Conclusion: Probiotics given in combination with standard therapy in IBD are effective in decreasing disease activity rates and reducing remission rates. No significant adverse reactions to probiotics were noticed.

Aim: Due of paucity of studies on stress ulcer prophylaxis in intracerebral hemorrhage (ICH), we have evaluated the usefulness of ranitidine and sucralfate in preventing gastric hemorrhage (GH) in patients with ICH. Subjects and methods: In a hospital-based randomized placebo-controlled study, patients with CT-proven ICH within 7 days of ictus were randomized into ranitidine 50 mg i.v. eight hourly, sucralfate 1 g six hourly and placebo groups. Patients were conservatively managed. Primary endpoint was occurrence of GH within 15 days of ictus and secondary endpoint 1-month mortality. Results: The mean age of the patients was 57.2 (range 25-90) years and 40 were females. There were 45 patients in ranitidine, 49 in sucralfate and 47 in placebo group. Demographic, clinical and radiological features were not significantly different in 3 groups. GH occurred in 11 (23.4%) patients in placebo, 5 (11.1%) in ranitidine and 7 (14.3%) in sucralfate group, which was not significant. Only one female had GH. There were 13 (27.7%) deaths in placebo, 5 (11.1%) in ranitidine and 12 (24.5%) in sucralfate group. Pneumonia occurred in placebo group in 5 (10.6%), ranitidine in 2 (4.4%) and sucralfate in 5 (10.2%) patients, which was not significantly different. Co nclusion: Ranitidine and sucralfate do not seem to significantly prevent GH or r educe 1-month mortality.

Pitting corrosion behavior and corrosion protection performance of cold sprayed double layered noble barrier coating on magnesium-based alloy in chloride containing solutions

Nitrogen processed, cold sprayed commercially pure(CP)-Al coatings on Mg-based alloys mostly lack acceptable hardness, wear resistance and most importantly are highly susceptible to localized corrosion in chloride containing solutions. In this research, commercially pure α-Ti top coating having good pitting potential(~1293 mV_(SCE)), high microhardness(HV_(0.025): 263.03) and low wear rate was applied on a CP-Al coated Mg-based alloy using high pressure cold spray technology. Potentiodynamic polarization(PDP) curves indicated that the probability of transition from metastable pits to the stable pits for cold spayed(CS) Al coating is considerably higher compared to that with the CS Ti top coating(for Ti/Al/Mg system). In addition, CS Ti top coating was in the passivation region in most pH ranges even after 48 h immersion in 3.5 wt% NaCl solution. The stored energy in the CS Ti top coating(as a passive metal) was presumed to be responsible for the easy passivation. Immersion tests indicated no obvious pits formation on the intact CS Ti top coating surface and revealed effective corrosion protection performance of the CS double layered noble barrier coatings on Mg alloys in 3.5 wt% NaCl solution even after 264 h.

An electron paramagnetic resonance(EPR)study on Mn^(2+)doped Cadmium formate dihydrate single crystals is carried *** EPR spectrum at room temperature exhibits only one out of five fine structural transitions which split into six hyperfine lines in all *** spectrum is simulated using the EasySpin program and evaluated spin Hamiltonian *** simulated EPR spectrum is in good agreement with the *** comparing direction cosines of spectroscopic splitting factor g and the direction cosines of different bonds determined by the crystal structure data it is found that Mn^(2+)enters the lattice substitutionally and only one Mn^(2+)site is *** obtained g and the hyperfine interaction constant A achieved are g=2.006±0.002,A=(98±2)×10^(-4)cm^(-1)and the second-order axial zero-field splitting parameter D=(60±2)×10^(-4)cm^(-1).

This paper describes results of seam welding of relatively high temperature melting materials, AISI 304, C-Mn steels, Ni-based alloys, CP Cu, CP Ni, Ti6Al4V and relatively low temperature melting material, AA6061. It describes the seam welding of multi-layered similar and dissimilar metallic sheets. The method described and involved advancing a rotating non-consumable rod(CP Mo or AISI 304) toward the upper sheet of a metallic stack clamped under pressure. As soon as the distal end of the rod touched the top portion of the upper metallic sheet, an axial force was applied. After an initial dwell time, the metallic stack moved horizontally relative to the stationery non-consumable rod by a desired length, thereby forming a metallurgical bond between the metallic sheets. Multi-track and multi-metal seam welds of high temperature metallic sheets, AISI 304, C-Mn steel,Nickel-based alloys, Cp Cu, Ti6Al4V and low temperature metallic sheets, AA6061 were obtained. Optical and scanning electron microscopy examination and 180 degree u-bend test indicated that defect free seam welds could be obtained with this method. Tensile- shear testing showed that the seam welds of AISI 304, C-Mn steel, Nickel-based alloy were stronger than the starting base metal counterparts while AA6061 was weaker due to softening. The metallurgical bonding at the interface between the metallic sheets was attributed to localized stick and slip at the interface, dynamic recrystallization and diffusion. The method developed can be used as a means of welding, cladding and additive manufacturing.

Significant effect of as-cast microstructure on texture evolution and magnetic properties of strip cast non-oriented silicon steel

In this study, two types of as-cast microstructure produced by strip casting were cold rolled and annealed to investigate the effect of initial microstructure on the textural evolution and magnetic properties of non-oriented silicon steel. The results indicated that the cold-rolled sheets of coarse-grained strip with pronounced {100} components exhibited stronger 入 fiber(//ND) and weaker γ fiber(//ND)texture as composed to the fine-grained strip with strong Goss({110}(001)) texture. After annealing, the former was dominated by η fiber(//RD) texture with a peak at {110}k {110}(001) texture. In addition, a number of precipitates of size ～30-150 nm restricted the grain growth during annealing, resulting in recrystallization of grain size of ～46 μm in the coarse-grained specimen and ～41μm in the fine-grained ***, higher magnetic induction(～1.72 T) and lower core loss(～4.04 W/kg) were obtained in the final annealed sheets of coarse-grained strip with strong {100} texture.
