Surgical Correction for Paralytic Strabismus Using T-Plate Anchor

External or internal ophthalmoloplegia will result in a complete or a partial ocular dysmotility leading to a debilitating and variable manifest binocular diplopia for a majority of the patients. Complete third, forth and sixth nerve cranial nerve palsies are among the many number of etiologies appearing as paralytic strabismus. Successful clinical management, elimination of symptomatic diplopia in the primary field of gaze and increased binocular field of motor and sensory fusion as a result of the oculomotor nerve (III) palsy are challenging tasks for physicians facing this difficult clinical entity. Here we report a novel surgical technique in the clinical management of this disease through suture-fixation of medial rectus muscle onto Titanium plate (T-plate) already anchored into the nasal orbital wall.

LEAST SQUARES TYPE ESTIMATION FOR DISCRETELY OBSERVED NON-ERGODIC GAUSSIAN ORNSTEIN-UHLENBECK PROCESSES

In this article, we consider the drift parameter estimation problem for the nonergodic Ornstein-Uhlenbeck process defined as dXt = OXtdt + dGt, i > 0 with an unknown parameter θ> 0, where G is a Gaussian process. We assume that the process {xt,t≥ 0} is observed at discrete time instants t1=△n,…, tn = n△n, and we construct two least squares type estimators θn and θn for θ on the basis of the discrete observations ,{xti,i= 1,…, n} as →∞. Then, we provide sufficient conditions, based on properties of G, which ensure that θn and θn are strongly consistent and the sequences √n△n(θn-θ) and √n△n(θn-θ) are tight. Our approach offers an elementary proof of [11], which studied the case when G is a fractional Brownian motion with Hurst parameter H∈(1/2, 1). As such, our results extend the recent findings by [11] to the case of general Hurst parameter H∈(0,1). We also apply our approach to study subfractional Ornstein-Uhlenbeck and bifractional Ornstein-Uhlenbeck processes.

Extracorporeal shock wave therapy for the treatment of coccydynia: a series of 23 cases

BACKGROUND AND OBJECTIVE Coccydynia can lead to significant functional disability and worsening of quality of life if not properly managed. In this study, we aim to assess the outcomes of extracorporeal shock wave therapy in patients with coccydynia. METHODS A prospective case series study was carried out from January to December 2015. Twenty-three patients, mean age of 38.3±12.1 (range 18-64), were included. The majority were females (13;56.5%), had pain for at least 6 weeks (17;73.9%) and had trauma to the sacrococcygeal region (17;73.9%). They had three sessions (one session per week for three consecutive weeks) of focused shock wave therapy directed to the maximal point of coccygeal tenderness. Numerical pain scale and Oswestry disability index were used to assess outcome. RESULTS Six (26.1%) patients did not complete the follow-up because of no, or minimal, improvement of their pain. After 6 months of follow-up, the median numerical pain scale significantly decreased from 7.0±4.0 to 2.0±2.0 among the 17 patients with coccydynia (P<0.001). The median Oswestry disability index improved from 24.0±9.0 before therapy to 8.0±9.0 at final follow-up (P<0.001). Before treatment, 12 (70.6%) patients had moderate-to-severe disability. In contrast, no patients had severe disability and only one (5.9%) patient had moderate disability at final follow-up (P<0.001). CONCLUSION Extracorporeal shock wave therapy had favorable outcomes in treating coccydynia. The majority of patients had partial relief of their pain and disability following this therapy.

Response surface methodology-based optimization of ultrasound-assisted extraction of β-sitosterol and lupeol from Astragalus atropilosus(roots) and validation by HPTLC method

Objective:To optimize the ultrasonication method for efficient extraction ofβ-sitosterol and lupeol from the roots of Astragalus atropilosus using Box-Behnken design of response surface methodology(RSM),and its validation by high performance thin layer chromatography(HPTLC)***:Ultrasonication method was used to extractβ-sitosterol and lupeol from Astragalus atropilosus(roots).RSM was used to optimize the different extraction parameters *** to solid ratio(10–14 m L/g),temperature(60-80℃)and time(40–60 min)to maximize the yield ofβ-sitosterol and *** quantitative estimation ofβ-sitosterol and lupeol was done in chloroform extract of Astragalus atropilosus by validated HPTLC method on 10 cm×20 cm glass-backed silica gel 60 F254 plate using hexane and ethyl acetate(8:2,v/v)as mobile ***:A quadratic polynomial model was found to be most appropriate with regard to R1(yield of total extraction;R2/%CV=0.9948/0.28),R2(β-sitosterol yield;R2/%CV=0.9923/0.39)and R3(lupeol yield;R2/%CV=0.9942/0.97).The values of adjusted R2/predicted R2/signal to noise ratio for R1,R2,and R3 were 0.9782/0.9551/48.77,0.9904/0.9110/31.33,and 0.9927/0.9401/36.08,respectively,indicating a high degree of correlation and adequate *** linear correlation plot between the predicted and experimental values for R1,R2,and R3 showed high values of R2 ranging from 0.9905-0.9973.β-sitosterol and lupeol in chloroform extract of Astragalus atropilosus were detected at Rf values of 0.22 and 0.34,respectively,atλmax=518 *** optimized ultrasonic extraction produced 8.462%w/w of R1,0.451%w/w of R2 and 0.172%w/w of R3 at 13.5 m L/g liquid to solid ratio,78℃of temperature and 60 min of ***:The experimental findings of RSM optimized extraction and HPTLC analysis can be further applied for the efficient extraction ofβ-sitosterol and lupeol in other species of Astragalus.
