After HAART (highly active antiretroviral therapy), which is a powerful antiviral drug regimen, was introduced into HIV therapy for AIDS patients, AIDS mortality decreased dramatically. The incidence of encephalopathy among AIDS patients was reported to be 10% to 40% in the past, but the introduction of HAART resulted in a decrease in HIV encephalopathy and an increase in mild CNS-related deficits, which affect the quality of life of AIDS patients. We report the case of an AIDS patient who developed schizophrenia-like delusions associated with encephalopathy. Administration of an antipsychotic drug and hypnotics was effective in relieving the symptoms of patients with insomnia, depressed mood, and delusions. Further information on the neuronal mechanism underlying the mental symptoms observed in our patient will be necessary to understand the process of pathogenesis of the encephalopathy and to devise an adequate clinical strategy for treatment.

Usefulness of Newly Devised Clomiphene Citrate Administration Method Compared with the Conventional Method in Ovulation and Pregnancy

Objective: Since not all women wish to conceive a child through aggressive treatment, we investigated the usefulness of modified repeated intracyclic clomiphene citrate (CC) therapy (repeated CC therapy) as a newly devised administration method. Methods: We evaluated the effects of CC administration on menstrual cycle length and retrospectively compared ovulation and pregnancy in 220 women who received CC at our hospital. Patients in the conventional method group received 50 mg per day for five days, starting on the fifth day of menstruation (withdrawal bleeding). Groups with and without menstrual cycle shortening after conventional CC administration were compared. The repeated CC therapy group was also compared with the non-shortened group. Repeated CC therapy was administered for the first five days as in the conventional method, and a second five-day repeat treatment was administered after an interval of five to seven days. Pregnancy rates, including indirect pregnancies, were evaluated by three different methods. Results: Ovulation and pregnancy rates were significantly better in the shortened group than in the non-shortened group (P < 0.001 and P = 0.010, respectively). Even in the non-shortened group, ovulation and pregnancy rates including indirect pregnancies were significantly improved when ovulation was observed with repeated CC therapy (P < 0.001 and P = 0.022, respectively). Conclusions: For patients whose menstrual cycle was not improved or shortened, repeated CC therapy as the newly devised CC administration method is useful as the next step after the conventional CC administration method.

For recovery of metals from low-concentration sources, biosorption is one of promising technologies and poly-γ-glutamic acid（γ-PGA） has been known as a potential biosorbent for recovery of heavy metals from aqueous solutions. Effects of solution pH on recovery of rare-earth metal Nd are systematically examined to clarify mechanisms of Nd recovery by y-PGA and its sodium salt（y-PGANa）. The recovery efficiency of Nd by y-PGA increases from 2.4 to 19.6% as pH increases from 2 to 4. Subsequently the Nd recovery efficiencies for y-PGA and y-PGANa remain almost constant in the range of pH from 4 to 7. For pH 〉 7 the increase in Nd recovery is significant and 100% recovery of Nd is achieved at pH 9. The pH dependency on Nd recovery by y-PGANa is similar to that by y-PGA. Contributions of adsorption and precipitation/coagulation to Nd recovery process are quantified. Whereas the adsorption dominates Nd recovery at lower pH（〈～4）, the precipitation/coagulation controls Nd recovery process for pH 〉 7. At higher pH, purple gel precipitates are observed. The maximum adsorption capacities for γ-PGA and yPGANa are 215 mg-Nd/（g-γ-PGA） at pH 4 and 305 mg-Nd/（g-y-PGANa） at pH 3, respectively. From the spectra of FT-IR and XPS, the biosorption of Nd onto y-PGA and y-PGANa via electrostatic interaction with carboxylate anions at pH 3 is verified. The Nd complexation with amide and carboxylate anion groups on γ-PGA and γ-PGANa may also contribute to the Nd recovery. The biosorption isotherms for Nd recovery by γ-PGA and γ-PGANa can be satisfactorily fitted by the Langmuir model. The thermodynamic studies suggest that the biosorptions of Nd by γ-PGA and γ-PGANa are endothermic. The utilization of γ-PGA and γ-PGANa as potential and eco-friendly biosorbents for the highly effective recovery of Nd from aqueous solution is confirmed.

One new furanocembranoid diterpene,11-hydroxy-Δ^(12(13))-pukalide(1),along with six known secondary metabolites,11-acetoxy-Δ^(12(13))-pukalide(2),13α-acetoxypukalide(3),pukalide(4),3α-methoxyfuranocembranoid(5),Δ9(15)-africanene(6),and methyl(5′E)-5-(2′,6′-dimethylocta-5′,7′-dienyl)furan-3-carboxylate(7)were isolated from the Okinawan soft coral Sinularia *** chemical structures were elucidated based on spectroscopic analysis(FTIR,NMR,and HRESIMS),and the relative stereochemistry of 1 was determined by NOESY experiments and acetylation,which yielded derivative *** addition,compounds 1 and 7 exhibited toxicity in the brine shrimp lethality test.

Background In in vitro fertilization (IVF) treatment, preimplantation genetic diagnosis/screening (PGD/S) attempts to detect chromosomal abnormalities in embryos before implantation. Using the meta-analytic and qualitative review approaches, this study aims to evaluate the effect of PGD/S on clinical pregnancy, live births, and childhood outcomes. Methods We conducted a literature search using 1) PubMed and other search engines, and 2) an ancestry search by track-ing references cited in prior work. After screening the studies, we extracted information pertinent to the meta-analysis. We calculated the effect sizes for clinical pregnancy and live birth rates, and performed a moderation analysis by maternal age, type of genetic screening, and timing of the biopsy. For childhood outcomes, we conducted a systematic review of studies reporting the anthropometric, psychomotor, cognitive, behavioral, and family functioning of PGD/S children. Results We included 26 studies for clinical pregnancy and live births, and 18 studies for childhood outcomes. Results indi-cated that women who underwent comprehensive chromosome screening-based PGD/S had significantly higher clinical pregnancy rates (rr 1.207, 95% CI 1.017–1.431) and live birth rates (rr 1.362, 95% CI 1.057–1.755) than those whose IVF treatment did not include PGD/S. Early childhood outcomes of PGD/S children did not differ from those of non-PGD/S children. Conclusions Comprehensive chromosome screening-based PGD/S can improve clinical pregnancy and live birth rates without adversely affecting functioning in childhood at least up to age 9. Results are discussed in the context of bioethical, financial, legal, and psychological issues surrounding PGD/S.

Since future energy harvesting technologies require stable supply and high-efficiency energy conversion,there is an increasing demand for high-performance organic thermoelectric generators(TEGs)based on waterproof thermoelectric *** poor stability of n-type organic semiconductors in air and water has proved a roadblock in the development of reliable thermoelectric power *** developed a simple green route for preparing n-type carbon nanotubes(CNTs)by doping with cationic surfactants and fabricated films of the doped CNTs using only aqueous *** thermoelectric properties of the CNT films were investigated in *** nanotubes doped using a cationic surfactant(cetyltrimethylammonium chloride(CTAC))retained an n-doped state for at least 28 days when exposed to water and air,indicating higher stability than that for contemporary CNT-based thermoelectric *** wrapping of the surfactant molecules around the CNTs is responsible for blocking oxygen and water from attacking the CNT walls,thus,extending the lifetime of the n-doped state of the *** also fabricated thermoelectric power conversion modules comprising CTAC-doped(n-type)and sodium dodecylbenzenesulfonate-(SDBS-)doped(p-type)CNTs and tested their stabilities in *** modules retained 80±2:4%of their initial maximum output power(at a temperature difference of 75℃)after being submerged in water for 30 days,even without any sealing fills to prevent device *** remarkable stability of our CNT-based modules can enable the development of reliable soft electronics for underwater applications.

BACKGROUND Granulocyte colony-stimulating factor(G-CSF)is a cytokine produced in inflammatory environments that induces differentiation and proliferation of neutrophils in bone *** report a rare case of aggressive G-CSFproducing squamous cell carcinoma of the tongue exhibiting fluorine-18 deoxyglucose(FDG)accumulation in primary lesion,metastatic lymph nodes,spleen,and bone marrow on positron emission tomography–computed tomography(PET/CT).CASE SUMMARY We report a 58-year-old female with a rapid enlarged lingual mass with partial *** test results from the initial examination revealed a leukocyte count of 21380/μ*** PET/CT,extensive FDG accumulation was observed in the tongue and bilateral cervical lymph nodes,with elevated FDG accumulation in the spleen and bone marrow although no distant metastases were *** performed partial glossectomy and bilateral neck *** staining with G-CSF antibodies on biopsy specimen and resected samples revealed that both specimens were G-CSF *** is a rare case of G-CSF producing tongue carcinoma with elevated FDG accumulation in the spleen and bone *** In patients with the tongue cancer and hyperleukocytosis,where FDG accumulations in the spleen and bone marrow are observed using PET/CT and when these accumulations are not caused by metastasis,G-CSF-producing tumors,with associated poor prognosis,should be considered.
