Congophilic fibrils in the glomeruli with polyclonal immunoglobulin gamma staining-another cause for diagnostic overlap:A case report

BACKGROUND Glomerulopathy with fibrillary deposits is not uncommon in routine nephropathology practice,with amyloidosis and fibrillary glomerulonephritis being the two most frequently encountered *** amyloid heavy and light chain(AHL)is relatively uncommon and its biopsy diagnosis is usually limited to cases that show strong equivalent staining for a single immunoglobulin(Ig)heavy chain and a single light chain,further supported by mass spectrometry(MS)and serum studies for monoclonal *** polyclonal light chain staining can pose a *** SUMMARY Herein we present a challenging case of renal AHL with polyclonal and polytypic Ig gamma(IgG)staining pattern by *** patient is a 62-yearold Caucasian male who presented to an outside institution with a serum creatinine of up to 8.1 mg/dL and nephrotic range *** the finding of a polyclonal and polytypic staining pattern on immunofluorescence,ultrastructural study of the renal biopsy demonstrated the presence of fibrils with a mean diameter of 10 *** red was positive while DNAJB9 was *** suggested a diagnosis of amyloid AHL type with IgG and lambda,but kappa light chains were also present supporting the immunofluorescence staining *** immunofixation studies demonstrated IgG lambda monoclonal *** patient was started on *** chronic renal injury however was quite advanced and he ended up needing dialysis shortly *** Tissue diagnosis of AHL amyloid can be *** confirmation using other available diagnostic techniques is recommended in such cases.

Introduction:Chronic diseases are becoming more prevalent *** effects of chronic illnesses are disastrous not only for the diagnosed person but also for their entire *** study explores chronic diseases’social and economic impact on low-income *** study aims to determine the economic and social implications of various chronic diseases and the loss of income due to these conditions among low-income individuals in Southern Punjab,***:A sample of 424 patients was selected from different areas of Southern *** were collected using a questionnaire that included questions about economic status,self-reported health status,social status,management strategies,and health insurance,among other ***:The mean monthly income of the respondents was found to be 57,097.6 Pakistani rupee(PKR),and the mean monthly expenses for treatment were 8,256.1 *** loss of income was calculated at 15%.Additionally,62%of patients spent more than 10%of their monthly income on managing their *** 85%of the respondents reported that chronic diseases affected their social ***,80%of patients lacked health ***:Chronic diseases impose significant economic and social burdens on patients and their families in Southern *** reduce the burden of chronic diseases,the government should enhance healthcare services in this region and provide health insurance to low-income families.

Effective Action for Noncommutative U(1) Gauge Theory with Higher Dimensional Terms

In this paper we apply the assumption of our recent work in noncommutative scalar models to the noncommutative U（1） gauge theories. This assumption is that the noneommutative effects start to be visible continuously from a scale ANC and that below this scale the theory is a commutative one. Based on this assumption and using background field method and loop calculations, an effective action is derived for noncommutative U（1） gauge theory. It will be shown that the corresponding low energy effective theory is asymptotically free and that under this condition the noncommutative quadratic IR divergences will not appear. The effective theory contains higher dimensional terms, which become more important at high energies. These terms predict an elastic photon-photon scattering due to the noncommutativity of space. The coefficients of these higher dimensional terms also satisfy a positivity constraint indicating that in this theory the related diseases of superluminal signal propagating and bad analytic properties of S-matrix do not exist. In the last section, we will apply our method to the noncommutative extra dimension theories.

Recently emerged SARS-CoV-2 caused a major outbreak of coronavirus disease 2019(COVID-19)and instigated a widespread fear,threatening global health *** date,no licensed antiviral drugs or vaccines are available against COVID-19 although several clinical trials are under way to test possible *** this urgent situation,computational drug discovery methods provide an alternative to tiresome high-throughput screening,particularly in the hit-to-lead-optimization *** of small molecules that specifically target viral replication apparatus has indicated the highest potential towards antiviral drug *** this work,we present potential compounds that specifically target SARS-CoV-2 vital proteins,including the main protease,Nsp12 RNA polymerase and Nsp13 *** integrative virtual screening and molecular dynamics simulations approach has facilitated the identification of potential binding modes and favourable molecular interaction profile of corresponding ***,the identification of structurally important binding site residues in conserved motifs located inside the active site highlights relative importance of ligand binding based on residual energy decomposition *** the current study lacks experimental validation,the structural information obtained from this computational study has paved way for the design of targeted inhibitors to combat COVID-19 outbreak.

Appendiceal inflammation affects the length of stay following appendicectomy amongst children:a myth or reality?

The effect of the severity of appendiceal inflammation on post-operative stay in children following appendicectomy has shown conflicting *** study was conducted to determine the association between the severity of appendiceal inflammation and post-operative stay amongst children undergoing open appendicectomy.A retrospective cohort study was conducted at a District General Hospital for two years.A total of 204 patients were included in the study with an age range between 3 and 16 *** were 54.9%while the rest were *** age was 12.5±3 *** association of the severity of appendiceal inflammation and post-operative stay was assessed by multivariable Cox Proportional hazards *** post-operative stay was 2.32 days(95%CI 2.14–2.51).Macroscopically perforated appendix,histological inflammation and post-operative complications were significantly associated with post-operative stay on univariable analysis(P<0.05).Whereas,the multivariable analysis showed that the post-operative stay was significantly prolonged only in case of either perforated appendix or post-operative complications while it remained unaffected by the histological inflammation.

The Role of Synapsins in Neurological Disorders

Synapsins serve as flagships among the presynaptic proteins due to their abundance on synaptic vesicles and contribution to synaptic communication. Several studies have emphasized the importance of this multi-gene family of neuron-specific phosphoproteins in maintaining brain physiology. In the recent times, increasing evidence has established the relevance of alterations in synapsins as a major determinant in many neurological disorders. Here,we give a comprehensive description of the diverse roles of the synapsin family and the underlying molecular mechanisms that contribute to several neurological *** physiologically important roles of synapsins associated with neurological disorders are just beginning to be understood. A detailed understanding of the diversified expression of synapsins may serve to strategize novel therapeutic approaches for these debilitating neurological disorders.

Fractional order modeling and control of dissimilar redundant actuating system used in large passenger aircraft

In this paper, a methodology has been developed to address the issue of force fighting and to achieve precise position tracking of control surface driven by two dissimilar *** nonlinear dynamics of both actuators are first approximated as fractional order models. Based on the identified models, three fractional order controllers are proposed for the whole system. Two Fractional Order PID（FOPID） controllers are dedicated to improving transient response and are designed in a position feedback configuration. In order to synchronize the actuator dynamics, a third fractional order PI controller is designed, which feeds the force compensation signal in position feedback loop of both actuators. Nelder-Mead（N-M） optimization technique is employed in order to optimally tune controller parameters based on the proposed performance criteria. To test the proposed controllers according to real flight condition, an external disturbance of higher amplitude that acts as airload is applied directly on the control surface. In addition, a disturbance signal function of system states is applied to check the robustness of proposed controller. Simulation results on nonlinear system model validated the performance of the proposed scheme as compared to optimal PID and high gain PID controllers.

Background: Acute Kidney Injury (AKI) stands as a prominent postoperative complication in on-pump cardiac surgery, with repercussions on morbidity, mortality, and hospitalization duration. Current diagnostic criteria relying on serum creatinine levels exhibit a delayed identification of AKI, prompting an exploration of alternative biomarkers. Aims and Objectives: This study is designed to overcome diagnostic constraints and explore the viability of serum Cystatin C as an early predictor of Acute Kidney Injury (AKI) in individuals undergoing on-pump cardiac surgery. The investigation aims to establish the relationship between serum Cystatin C levels and the onset of AKI in patients subjected to on-pump cardiac surgery. Primary objectives involve the assessment of the diagnostic effectiveness of serum Cystatin C, its comparison with serum creatinine, and the exploration of its potential for the early identification and treatment of AKI. Methodology: Conducted as a single-center study at the cardiac surgery department of BSMMU in Bangladesh from September 2020 to August 2022, a comparative cross-sectional analysis involved 31 participants categorized into No AKI and AKI groups based on Kidney Disease: Improving Global Outcomes (KDIGO) criteria. Data collection encompassed preoperative, post-CBP (cardiopulmonary bypass) conclusion at 2 hours, postoperative day 1, and postoperative day 2 intervals. Statistical analyses included Chi-squared tests, independent Student’s t-tests, and one-sample t-tests. Significance was set at P Results: The study revealed no significant differences in baseline characteristics between the No AKI and AKI groups, except for CPB time and cross-clamp time. Serum Cystatin C levels in the AKI group exhibited statistical significance at various time points, highlighting its potential as an early detector. Spatially Adaptive Image Restoration Using Fuzzy Punctual Kriging

We present a general formulation based on punctual kriging and fuzzy concepts for image restoration in spatial domain. Gray-level images degraded with Gaussian white noise have been considered. Based on the pixel local neighborhood, fuzzy logic has been employed intelligently to avoid unnecessary estimation of a pixel. The intensity estimation of the selected pixels is then carried out by employing punctual kriging in conjunction with the method of Lagrange multipliers and estimates of local semi-variances. Application of such a hybrid technique performing both selection and intensity estimation of a pixel demonstrates substantial improvement in the image quality as compared to the adaptive Wiener filter and existing fuzzykriging approaches. It has been found that these filters achieve noise reduction without loss of structural detail information, as indicated by their higher structure similarity indices, peak signal to noise ratios and the new variogram based quality measures.
