Thiswork investigates an oblique stagnation point flowof hybrid nanofluid over a rigid surface with power lawfluidas lubricated layers. Copper (Cu) and Silver (Ag) solid particles are used as hybrid particles acting in water H2O asa base fluid. The mathematical formulation of flow configuration is presented in terms of differential systemthat isnonlinear in nature. The thermal aspects of the flow field are also investigated by assuming the surface is a heatedsurface with a constant temperature T. Numerical solutions to the governing mathematical model are calculatedby the RK45 algorithm. The results based on the numerical solution against various flow and thermal controllingparameters are presented in terms of line graphs. The specific results depict that the heat flux increases over thelubricated-indexed parameter.

Traumatic brain injury (TBI), an unmet need: TBI is an alteration in brain function caused by an external force with evidence of brain pathology. It could be from a bump, blow, blast or jolt to the head including penetrating the cranium. TBI is a public health concern worldwide due to its economic impact. Most TBIs are survivable, do not need hospitalization but may influence productivity. A smaller percentage of TBI due to falls or penetrating TBI (PTBI) needs hospitalization and accounts for largest fraction of TBI care costs.

Introduction:Tonsillectomy is one of the most common procedures performed in otolaryngology.There are various methods to perform tonsillectomies(cold and hot).Thermal damage and inflammation of the surrounding tissue using monopolar cautery is said to point to increased pain whereas immediate cooling of the burnt area is thought to reduce pain owing to heat dissipation.Our objective was to determine the difference in mean post‐tonsillectomy pain scores among patients irrigated with cold water(4℃)and in patients not irrigated with cold water.Material and Methods:A quasi‐experimental trial was conducted from January 2016 to December 2017 at a tertiary care hospital.All tonsillectomies were carried out with monopolar cautery at a power of 20 W.Patients either received cold water irrigation post tonsillectomy of the tonsillar bed(intervention arm)or no irrigation(control arm).The pain score was measured on Days 0,1,3,5,and 7.Pain scores were charted on a visual analog scale on the respective days.Results:Seventy‐eight patients were included in the study.The mean age of our patients was 10.26(4.24)years old in the intervention arm,and 11.95(4.19)years old in the control arm.It was observed that the pain was significantly lower in patients with cold water irrigation of the tonsillar fossa on Days 1,3,5,and 7(p=0.001).There were no readmissions postprocedure and none of our patients developed any complications postsurgery.Conclusions:In this trial,we report a reduced pain score at all days of observation in the intervention group.Irrigation of the tonsillar fossa is a safe,cost-effective,and less technically demanding technique that could be employed to reduce postoperative pain.Further studies with randomization,blinding and a larger sample size could further improve on our results.

Artificial Intelligence(AI)is being increasingly used for diagnosing Vision-Threatening Diabetic Retinopathy(VTDR),which is a leading cause of visual impairment and blindness worldwide.However,previous automated VTDR detection methods have mainly relied on manual feature extraction and classification,leading to errors.This paper proposes a novel VTDR detection and classification model that combines different models through majority voting.Our proposed methodology involves preprocessing,data augmentation,feature extraction,and classification stages.We use a hybrid convolutional neural network-singular value decomposition(CNN-SVD)model for feature extraction and selection and an improved SVM-RBF with a Decision Tree(DT)and K-Nearest Neighbor(KNN)for classification.We tested our model on the IDRiD dataset and achieved an accuracy of 98.06%,a sensitivity of 83.67%,and a specificity of 100%for DR detection and evaluation tests,respectively.Our proposed approach outperforms baseline techniques and provides a more robust and accurate method for VTDR detection.

Complete surgical resection(R0)of liver tumors,primary or metastatic,remains the main objective in management of primary and metastatic liver tumors[1].Tumors invading to the proximity of the hepatic venous outflow pose a peculiar challenge to achieve R0 resections,because this location may render a tumor unresectable.The median survival of patients with liver tumor without surgery is less than 12 months[2].Even with surgery,post-hepatectomy liver failure and subsequently increased mortality are the main problems associated with complex resection[3].Moreover,when the vein is affected,vein resection and subsequent reconstruction are necessary.Vascular resection is a standard practice in liver resection and transplantation[4],for example,hepatic vein reconstruction during a living-donor liver transplant,porto-mesenteric axis reconstruction during resection of advanced pancreatic cancer and caval reconstruction during resection of retroperitoneal tumors.Thus,novel techniques like total hepatic vascular exclusion(HVE)[5],veno-venous bypass[6]and ex vivo hepatic resection[7,8]have facilitated curative resections of tumors close to one or more major hepatic veins.

This paper analyzes the key indicators of the impact assessment the teleworking as practice of performing regular work from home or from a location close to home,on reducing the number of travel and reduced carbon emissions transportation activities.Models and methodologies described in the literature are a good basis for a better and more comprehensive and understanding of the positives and negative effects teleworking and real opportunities of its wider application,however there is a lack of systemic analysis.Development of a set of indicators for assessing emission reduction induced by the measures to promote sustainable transport.Due to the complexity of the transport sector,a change in any indicator models have side effects on other indicators(unwanted and multiplier effects)and what is the theme of our research work.

BACKGROUND Glomerulopathy with fibrillary deposits is not uncommon in routine nephropathology practice,with amyloidosis and fibrillary glomerulonephritis being the two most frequently encountered entities.Renal amyloid heavy and light chain(AHL)is relatively uncommon and its biopsy diagnosis is usually limited to cases that show strong equivalent staining for a single immunoglobulin(Ig)heavy chain and a single light chain,further supported by mass spectrometry(MS)and serum studies for monoclonal protein.But polyclonal light chain staining can pose a challenge.CASE SUMMARY Herein we present a challenging case of renal AHL with polyclonal and polytypic Ig gamma(IgG)staining pattern by immunofluorescence.The patient is a 62-yearold Caucasian male who presented to an outside institution with a serum creatinine of up to 8.1 mg/dL and nephrotic range proteinuria.Despite the finding of a polyclonal and polytypic staining pattern on immunofluorescence,ultrastructural study of the renal biopsy demonstrated the presence of fibrils with a mean diameter of 10 nm.Congo red was positive while DNAJB9 was negative.MS suggested a diagnosis of amyloid AHL type with IgG and lambda,but kappa light chains were also present supporting the immunofluorescence staining results.Serum immunofixation studies demonstrated IgG lambda monoclonal spike.The patient was started on chemotherapy.The chronic renal injury however was quite advanced and he ended up needing dialysis shortly after.CONCLUSION Tissue diagnosis of AHL amyloid can be tricky.Thorough confirmation using other available diagnostic techniques is recommended in such cases.
