Evolution of incidental branch-duct intraductal papillary mucinous neoplasms of the pancreas: A study with magnetic resonance imaging cholangiopancreatography

AIM To investigate the type and timing of evolution of incidentally found branch-duct intraductal papillary mucinous neoplasms(bd-IPMN) of the pancreas addressed to magnetic resonance imaging cholangiopancreatography(MRCP) *** We retrospectively evaluated 72 patients who underwent, over the period 2006-2016, a total of 318 MRCPs(mean 4.4) to follow-up incidental, presumed bdIPMN without signs of malignancy, found or confirmedat a baseline MRCP examination. Median follow-up time was 48.5 mo(range 13-95 mo). MRCPs were acquired on 1.5T and/or 3.0T systems using 2D and/or 3D technique. Image analysis assessed the rates of occurrence over the follow-up of the following outcomes:(1) imaging evolution, defined as any change in cysts number and/or size and/or appearance; and(2) alert findings, defined as worrisome features and/or high risk stigmata(e.g., thick septa, parietal thickening, mural nodules and involvement of the main pancreatic duct). Time to outcomes was described with the Kaplan-Meir approach. Cox regression model was used to investigate clinical or initial MRCP findings predicting cysts *** We found a total of 343 cysts(per-patient mean 5.1) with average size of 8.5 mm(range 5-25 mm). Imaging evolution was observed in 32/72 patients(44.4%; 95%CI: 32-9-56.6), involving 47/343 cysts(13.7%). There was a main trend towards small( 0.01).CONCLUSION Changes in MRCP appearance of incidental bd-IPNM were frequent over the follow-up(44.4%), with relatively rare(8.3%) occurrence of non-malignant alert findings that prompted further diagnostic steps. Current trends in management of hepatitis B virus reactivation in the biologic therapy era

Hepatitis B virus (HBV) reactivation represents an emerging cause of liver disease in patients undergoing treatment with biologic agents. In particular, the risk ofHBV reactivation is heightened by the use monoclonalantibodies, such as rituximab (anti-CD20) and alemtuzumab (anti-CD52) that cause profound and longlasting immunosuppression. Emerging data indicatethat HBV reactivation could also develop following theuse of other biologic agents, such as tumor necrosis factor (TNF)-α inhibitors. When HBV reactivation is di-agnosed, it is mandatory to suspend biologic treatmentand start antiviral agents immediately. However, preemptive antiviral therapy prior to monoclonal antibodyadministration is crucial in preventing HBV reactivationand its clinical consequences. Several lines of evidencehave shown that risk of HBV reactivation is greatlyreduced by the identifi cation of high-risk patients andthe use of prophylactic antiviral therapy. In this article, we discuss current trends in the management of HBV reactivation in immunosuppressed patients receiving biologic therapy, such as rituximab, alemtuzumab and TNF-α antagonists.

A Field Experiment on Dust Emission by Wind Erosion in the Taklimakan Desert

Dust emission by wind erosion in surface is a serious problem in many arid regions around the world,and it is harmful to the ecological environment,human health,and social *** monitor the characteristics of saltation activity and to calculate the threshold wind velocity and sediment discharge under field conditions have significance on the research of dust emission by wind ***,a field experiment was conducted over the flat sand in the hinterland of the Taklimakan *** sampling system was installed on the flat sand surface at Tazhong,consisting of a meteorological tower with a height of 2 m,a piezoelectric saltation sensor（Sensit）,and a Big Spring Number Eight（BSNE） sampler *** of saltation activity was recorded every second using the *** BSNE station consisted of five BSNE samplers with the lowest sampler at 0.05 m and the highest sampler at 1.0 m above the soil *** was collected from the samplers every 24 *** is found that saltation activity was detected for only 21.5% of the hours measured,and the longest period of saltation activity occurring continuously was not longer than 5 min under the field *** threshold wind velocity was variable,its minimum value was 4.9 m s 1,the maximum value was 9.2 m s 1,and the average value was 7.0 m s *** threshold wind velocity presented a positive linear increase during the measurement *** observation site had a sediment discharge of 82.1 kg m 1 over a period of 24 *** on hourly saltation counts,hourly sediment discharge was ***,there was no obvious linear or other functional relationship between the hourly sediment discharge and wind *** results show that the changes of sediment discharge do not quite depend on wind velocity.

CT diagnosis of recurrence after pancreatic cancer:Is there a pattern?

AIM:To investigate predilection sites of recurrence of pancreatic cancer by computed tomography(CT)in follow-up after surgery. METHODS:Seventy seven patients with recurrence after pancreatic cancer surgery were retrospectively *** operative technique,R-status,T-stage and development of tumor markers were evaluated. Two radiologists analyzed CT scans with consensus *** of local recurrence,lymph node recurrence and organ metastases were *** and progression of findings on follow-up CT were con-sidered as reference standard. RESULTS:The mean follow-up interval was 3.9± 1.8 mo,with a mean relapse-free interval of 12.9± 10.4 *** predominant site of recurrence was local (65%),followed by lymph node(17%),liver metastasis (11%)and peritoneal carcinosis(7%).Local recurrence emerged at the superior mesenteric artery(n=28),the hepatic artery(n=8),in an area defined by the surrounding vessels:celiac trunk,portal vein,inferior vena cava(n=22),and in a space limited by the mesenteric artery,portal vein and inferior vena cava(n=17). Lymph node recurrence occurred in the mesenteric root and left lateral to the *** was confirmed by surgery(n=22)and follow-up CT(n=55).Tumor markers[carbohydrate antigen 19-9(CA19-9),carcinoembryonic antigen(CEA)]increased in accordance with signs of recurrence in most cases(86%CA19-9;79.2% CEA). CONCLUSION:Specific changes of local and lymph node recurrence can be found in the course of the cardinal peripancreatic *** superior mesenteric artery is the leading structure for recurrence.

Hepatitis C (HCV) infection has an estimated global prevalence of 2.5%,causing chronic liver disease in 170 million people *** data has identified HCV infection as a risk factor for subclinical and clinical cardiovascular disease (CVD),but these data have been mixed and whether HCV is an independent risk factor for development of CVD remains *** this review,we present the literature regarding the association of HCV with subclinical and clinical CVD and the possible underlying mechanisms leading to increased CVD among those infected with *** infection leads to increased CVD via direct and indirect mechanisms with chronic inflammation,endothelial dysfunction and direct invasion of the arterial wall cited as possible *** review showed that HCV infection,particularly chronic HCV infection,appears to lead to increased subclinical CVD most consistently and potentially also to increased clinical CVD outcomes,leading to increased morbidity and ***,the majority of studies evaluating the impact of HCV therapy on CVD morbidity and mortality showed an improvement in subclinical and clinical CVD endpoints in patients who were successfully treated and achieved sustained viral *** results are of particular interest following the development of new direct antiviral agents which have made HCV eradication simple and feasible for many more patients globally,and in doing so may possibly reduce CVD morbidity and mortality in those with chronic HCV infection.

Analysis of Surface and Ground Water Exchange in Two Different Watersheds

Water regime of Albanian rivers is a Mediterranean typical one. During the wet period, it flows 85%-90% of the annual flow and the dry period represents only 10%-15% of the annual amount. In this paper the water regime of the Semani and Vjosa River is analyzed. These are two rivers with total different hydro-geological characteristics. Vjosa river watershed is mainly composed of massif calcareous rocks that are streaky and karstifled. A totally different view is in the other river, in Seman where the impermeable rocks dominate. Even the distribution of the precipitation is quite different. In the Vjosa River the amount of the precipitation varies from 1,500 mm to 2,500 mm per year and in Semani River only 1,100 mm per year. The flow in the wet period is mainly a result of the precipitation and the minimum discharge occurs during the dry period representing the base flow that is the contribution from the groundwater. In this point of view the flow in the dry period is also an indicator of the underground water resource. During the dry period the watershed gives what it received and what has cumulated during the wet period. This is more evident in the case of a karstic watershed. The recession curves were analysed for all the hydrometric stations in both river basins and the parameters of these curves are evaluated. These results are analyzed and compared between the two different watersheds reflecting the differences on water exchange of surface and ground water. Finally, an assessment of groundwater resources in both hydro geological basins is worked out.

An overview of the marine natural products in clinical trials and on the market

The first marine natural products that served as leads or scaffolds for medicines were discovered in the middle of last century:the arabinosyl glycosides from the marine sponge Tectitethya *** and modifications of the natural molecules generated antiviral and antileukemic drugs developed in the 1970’s and in the following decades,including the first effective treatment against HIV *** the improvement of techniques for the elucidation of chemical structure of the molecules,as well as chemical synthesis,especially from the 1990’s,there was an increase in the number of bioactive natural products characterized from marine *** chemical structures with high specificity towards molecular targets in cells allowed the development of new drugs with indication for the treatment of several illnesses,from cancer to new antibiotics,and even neurological *** there are at least 13 molecules derived from marine natural products on advanced clinical trials,and nine were approved to be used as *** that in the past eight years,more than 1000 new compounds from marine organisms were described,per year,the expectation is that many more drugs will be derived from marine natural products in a near future.

In the present work, the synthesis of micro- and nano-sized spheres of metallic bismuth by microwave-assisted solvothermal method is reported. The synthesis method was carried out at different power levels and at a unique frequency of microwave irradiation. The sphere sizes were controlled by the microwave power level and the concentration of dissolved precursor. Structural and morphological characterization was performed by SEM, HRTEM, EELS and XRD. The results demonstrated that rhombohedral zero valent Bi spheres were synthesized after microwave radiation at 600 and 1200 W. However, if the power level is decreased to 120 W, a monoclinic phase of Bi203 is obtained with a flake-like morphology. In comparison with a conventional hydrothermal process, the microwave-assisted solvothermal approach provides many advantages such as shorter reaction time, optimum manipulation of morphologies and provides a specific chemical phase and avoids the mixture of structural phases and morphologies which is essential for further applications such as drug delivery or functionalization with organic materials, thanks to its biocompatibility.

Satisfaction, Professional Mobility and Leadership in Academic-Scientific Organizations

The research work has been done in the field of social and organizational psychology. It is aim to analyze the factors which influence the levels of satisfaction and achievement reached by those working for scientific organizations and their relationship with professional mobility （Andrews, Aichholzer, Cole, Mittermeir, Stole-Heiskanen, UNESCO--United Nations Educational, Scientific and Cultural Organization, 1971） [1]. A stratified sample was taken from universities and different disciplines, based on a population of teachers from the Cuyo region （N = 355 R ＋ D-Research ＆ Development Units）（5% error margin）. At this first stage, the research teachers were from Universidad Nacional de Cuyo （N = 53 Research Units）： one chief or director and members. Quantitative techniques were used （two questionnaires）. The results show that researchers＇ satisfaction at different levels is connected with professional mobility and disciplinary fields. Regarding leadership, and considering professional mobility, a general feeling of satisfaction emerges among researchers, regardless of their disciplinary field.

Multiple Redox and Non-Redox Interactions Define 2-Cys Peroxiredoxin as a Regulatory Hub in the Chloroplast

In plants, the highly abundant 2-cysteine peroxiredoxin （2-CysPrx） is associated with the chloroplast and involved in protecting photosynthesis. This work addresses the multiple interactions of the 2-CysPrx in the chloroplast, which depend on its redox state. Transcript co-regulation analysis showed a strong linkage to the peptidyl-prolyl-cis/trans isomerase Cyclophilin 20-3 （Cyp20-3） and other components of the photosynthetic apparatus. Co-expression in protoplasts and quantification of fluorescence resonance energy transfer （FRET） efficiency in vivo confirmed protein interactions of 2-CysPrx with Cyp20-3 as well as NADPH-dependent thioredoxin reductase C （NTRC）, while thioredoxin x （Trx-x） did not form complexes that could enable FRET. Likewise, changes in FRET of fluorescently labeled 2-CysPrx in vitro and in vivo proved redox dependent dynamics of 2-CysPrx. Addition of Cyp20-3 to an in vitro peroxidase assay with 2-CysPrx had no significant effect on peroxide reduction. Also, in the presence of NTRC, addition of Cyp20-3 did not further enhance peroxide reduction. In addition, 2-CysPrx functioned as chaperone and inhibited aggregation of citrate synthase during heat treatment. This activity was partly inhibited by Cyp20-3. As a new interaction partner of decameric 2-CysPrx, photosystem Ⅱ could be identified after chloroplast fractionation and in pull-down assays after reconstitution. In summary, the data indicate a dynamic function of plant 2-CysPrx as redox sensor, chaperone, and regulator in the chloroplast with diverse functions beyond its role as thiol peroxidase.
