AIM： Endoscopic sphincterotomy （ES） is a well-established therapeutic modality for the removal of common bile duct （CBD） stones. Alter ES there are still around 10% of patients that experience recurrent CBD stones. The aim of this study is to investigate the composition of CBD stones before and alter ES and its clinical significance in Chinese ***： From January 1996 to December 2003, 735 patients with CBD stones received ES at Kaohsiung Veterans General Hospital and stone specimens from 266 patients were sent for analysis. Seventy-five patients had recurrent CBD stones and stone specimens from 44 patients were sent for analysis. The composition of the stones was analyzed by infrared （IR） spectrometry and they were classified as cholesterol or bilirubinate stones according to the predominant composition. Clinical data were ***： In the initial 266 stone samples, 217 （82%） were bilirubinate stones, 42 （16%） were cholesterol stones,3 were calcium carbonate stones, 4 were mixed cholesterol and bilirubinate stones. Patients with bilirubinate stones were significantly older than patients with cholesterol stones （66＋13 years vs 56＋17 years, P = 0.001）. In the 44 recurrent stone samples, 38 （86%） were bilirubinate stones, 3 （7%） were cholesterol stones, and 3 were mixed cholesterol and bilirubinate stones. In 27 patients, both initial and recurrent stone specimens can be obtained,23 patients had bilirubinate stones initially and 2 became cholesterol stones in the recurrent attack. Background:The current World Health Organization(WHO) classification of nasopharyngeal carcinoma(NPC) con?veys little prognostic *** study aimed to propose an NPC histopathologic classification that can poten?tially be used to predict prognosis and treatment ***:We initially developed a histopathologic classification based on the morphologic traits and cell differentia?tion of tumors of 2716 NPC patients who were identified at Sun Yat?sen University Cancer Center(SYSUCC)(training cohort).Then,the proposed classification was applied to 1702 patients(retrospective validation cohort) from hospitals outside SYSUCC and 1613 patients(prospective validation cohort) from *** efficacy of radiochemotherapy and radiotherapy modalities was compared between the proposed *** used Cox proportional hazards models to estimate hazard ratios(HRs) with 95% confidence intervals(CI) for overall survival(OS).Results:The 5?year OS rates for all NPC patients who were diagnosed with epithelial carcinoma(EC;3708 patients),mixed sarcomatoid?epithelial carcinoma(MSEC;1247 patients),sarcomatoid carcinoma(SC;823 patients),and squamous cell carcinoma(SCC;253 patients) were 79.4%,70.5%,59.6%,and 42.6%,respectively(P lone for patients with EC(HR 49–0.75),and possibly for those with SCC(HR = 0.67,95% CI = 0.56–0.80),MSEC(HR = 0.58,95% CI = 0..74–1.28).= 0.63;95% CI = 0.40–0.98),but not for patients with SC(HR = 0.97,95% CI = 0Conclusions:The proposed classification offers more information for the prediction of NPC prognosis compared with the WHO classification and might be a valuable tool to guide treatment decisions for subtypes that are associ?ated with a poor prognosis.

Laparoscopic surgery has become increasingly popular in the management of gastric gastrointestinal stromal tumors (GISTs) in recent years. One hundred and forty-five patients underwent curative resections of primary gastric stromal tumors between September 2002 and March 2012 were assigned to either an open surgery group (n = 99) or a laparoscopic surgery group (n = 46). In the open surgery group, there was a significantly higher number of samples with a mitotic index ≥ 10 (16.1% vs. 0%), more tumors located in the gastric cardia (13.1% vs. 6.5%), greater operative blood loss (80 mL vs. 50 mL) and a longer postoperative hospital stay (10 days vs. 6 days) than in the laparoscopic group. The surgical morbidity and mortality were 6.1% and 0% in the open group, whereas no complication or mortality in the laparoscopic group. Ten patients in the open group had tumor recurrences and no recurrence in the laparoscopic group. Multivariate analysis showed that tumor size and mitotic index were two independent risk factors associated with tumor recurrence. The 3-year disease-free survival rates and 5-year overall survival rates were similar between the two groups. The laparoscopic approach is a safe alternative procedure for gastric GISTs.

BACKGROUND Zinc-α2-glycoprotein 1 (AZGP1) plays important roles in metabolism-related diseases. The underlying molecular mechanisms and therapeutic effects of AZGP1 remain unknown in non-alcoholic fatty liver disease (NAFLD). AIM To explore the effects and potential mechanism of AZGP1 on NAFLD in vivo and in vitro. METHODS The expression of AZGP1 and its effects on hepatocytes were examined in NAFLD patients, CCl4-treated mice fed a high fat diet (HFD), and human lo2 cells. RESULTS AZGP1 levels were significantly decreased in liver tissues of NAFLD patients and mice. AZGP1 knockdown was found to activate inflammation;enhance steatogenesis, including promoting lipogenesis [sterol regulatory elementbinding protein (SREBP)-1c, liver X receptor (LXR), fatty acid synthase (FAS), acetyl-CoA carboxylase (ACC), and stearoyl CoA desaturase 1 (SCD)-1], increasing lipid transport and accumulation [fatty acid transport protein (FATP), carnitine palmitoyl transferase (CPT)-1A, and adiponectin], and reducing fatty acid β-oxidation [farnesoid X receptor (FXR) and peroxisome proliferator-activated receptor (PPAR)-α];accelerate proliferation;and reverse apoptosis in lo2 cells. AZGP1 overexpression (OV-AZGP1) had the opposite effects. Furthermore, AZGP1 alleviated NAFLD by blocking TNF-α-mediated inflammation and intracellular lipid deposition, promoting proliferation, and inhibiting apoptosis in lo2 cells. Finally, treatment with OV-AZGP1 plasmid dramatically improved liver injury and eliminated liver fat in NAFLD mice. CONCLUSION AZGP1 attenuates NAFLD with regard to ameliorating inflammation, accelerating lipolysis, promoting proliferation, and reducing apoptosis by negatively regulating TNF-α. AZGP1 is suggested to be a novel promising therapeutic target for NAFLD.

By modeling the broadband spectral energy distributions （SEDs） of a typical flat spectrum radio quasar （FSRQ, 3C 279） and two GeV narrow-line Seyfert 1 galaxies （NLSls, PMN J0948＋0022 and 1H 0323＋342） in different flux stages with one-zone leptonic models, we find a universal correlation between their Doppler factors （δ） and peak luminosities （Lc） of external Compton scattering bumps. Compiling a combined sample of FSRQs and GeV NLSls, it is found that both FSRQs and GeV NLSls in different stages and in different sources follow the same δ-Lc correlation well. This indicates that the variations of observed luminosities may be essentially due to the Doppler boosting effect. The universal δ-lo relation between FSRQs and GeV NLS 1 s in different stages may be further evidence that the particle acceleration and radiation mechanisms for the two kinds of sources are similar. In addition, by replacing Lc with the observed luminosity in the Fermi/LAT band （LLAT）, this correlation holds and it may serve as an empirical indicator of δ. We estimate the δ values with LLAT for 484 FSRQs in the Fermi/LAT Catalog and they range from 3 to 41, with a median of 16, which are statistically consistent with the values derived by other methods.

In the recent decades,obesity rates among children and adolescents,especially males,have increased *** worldwide phenomenon is thought to significantly affect the levels of sex ***,the association between waist circumference(a marker of abdominal obesity)and sex hormone levels in children and adolescents is *** this study,4031 participants aged 6–19 years from the United States National Health and Nutrition Examination Survey(NHANES)in the USA were enrolled in this *** common confounders of age,race,body mass index,educational level,family income,diabetes,and time of sample collection were also *** participants missing any of the above information were excluded from the *** used multiple linear regression and other multiple statistics to assess the associations between waist circumference and serum testosterone,estradiol,sex hormone-binding globulin(SHBG),free androgen index(fai),and testosterone/estradiol ratio(T/E2).Waist circumference remained associated with sex hormone levels in children and adolescents after controlling for *** waist circumference increases,testosterone levels in children and adolescents show an overall decline after a brief increase,with the inflection point for waist circumference of 65–66 *** addition,waist circumference positively correlates with estradiol levels in male children(β=0.007,95%confidence interval:0.004–0.009).Moreover,circulating SHBG decreases in children and adolescents as waist circumference *** conclusion,this study highlighted waist circumference as a vital indicator affecting sex hormone levels in children and adolescents.
