Magnetic resonance imaging-based lymph node radiomics for predicting the metastasis of evaluable lymph nodes in rectal cancer

BACKGROUND Lymph node(LN)staging in rectal cancer(RC)affects treatment decisions and patient prognosis.For radiologists,the traditional preoperative assessment of LN metastasis(LNM)using magnetic resonance imaging(MRI)poses a challenge.AIM To explore the value of a nomogram model that combines Conventional MRI and radiomics features from the LNs of RC in assessing the preoperative metastasis of evaluable LNs.METHODS In this retrospective study,270 LNs(158 nonmetastatic,112 metastatic)were randomly split into training(n=189)and validation sets(n=81).LNs were classified based on pathology-MRI matching.Conventional MRI features[size,shape,margin,T2-weighted imaging(T2WI)appearance,and CE-T1-weighted imaging(T1WI)enhancement]were evaluated.Three radiomics models used 3D features from T1WI and T2WI images.Additionally,a nomogram model combining conventional MRI and radiomics features was developed.The model used univariate analysis and multivariable logistic regression.Evaluation employed the receiver operating characteristic curve,with DeLong test for comparing diagnostic performance.Nomogram performance was assessed using calibration and decision curve analysis.RESULTS The nomogram model outperformed conventional MRI and single radiomics models in evaluating LNM.In the training set,the nomogram model achieved an area under the curve(AUC)of 0.92,which was significantly higher than the AUCs of 0.82(P<0.001)and 0.89(P<0.001)of the conventional MRI and radiomics models,respectively.In the validation set,the nomogram model achieved an AUC of 0.91,significantly surpassing 0.80(P<0.001)and 0.86(P<0.001),respectively.CONCLUSION The nomogram model showed the best performance in predicting metastasis of evaluable LNs.

As one of the most widely used personal protective equipment(PPE),body armors play an important role in protecting the human body from the high-velocity impact of bullets or projectiles.The body torso and critical organs of the wear may suffer severe behind-armor blunt trauma(BABT)even though the impactor is stopped by the body armor.A type of novel composite material through incorporating shear stiffening gel(STG)into ethylene-vinyl acetate(EVA)foam is developed and used as buffer layers to reduce BABT.In this paper,the protective performance of body armors composed of fabric bulletproof layers and a buffer layer made of foam material is investigated both experimentally and numerically.The effectiveness of STG-modified EVA in damage relief is verified by ballistic tests.In parallel with the experimental study,numerical simulations are conducted by LS-DYNA®to investigate the dynamic response of each component and capture the key mechanical parameters,which are hardly obtained from field tests.To fully describe the material behavior under the transient impact,the selected constitutive models take the failure and strain rate effect into consideration.A good agreement between the experimental observations and numerical results is achieved to prove the validity of the modelling method.The tests and simulations show that the impact-induced deformation on the human body is significantly reduced by using STG-modified EVA as the buffering material.The improvement of protective performance is attributed to better dynamic properties and more outstanding energy absorption capability of the composite foam.

Postoperative abdominal herpes zoster complicated by intestinal obstruction:A case report

BACKGROUND Intestinal obstruction is a common occurrence in clinical practice.However,the occurrence of herpes zoster complicated by intestinal obstruction after abdominal surgery is exceedingly rare.In the diagnostic and treatment process,clinicians consider it crucial to identify the primary causes of its occurrence to ensure effective treatment and avoiding misdiagnosis.CASE SUMMARY Herein,we present the case of a 40-year-old female patient with intestinal obstruction who underwent laparoscopic appendectomy and developed herpes zoster after surgery.Combining the patient's clinical manifestations and relevant laboratory tests,it was suggested that the varicella zoster virus reactivated during the latent period after abdominal surgery,causing herpes zoster.Subsequently,the herpes virus invaded the visceral nerve fibers,causing gastrointestinal dysfunction and loss of intestinal peristalsis,which eventually led to intestinal obstruction.The patient was successfully treated through conservative treatment and antiviral therapy and subsequently discharged from the hospital.CONCLUSION Pseudo-intestinal obstruction secondary to herpes zoster infection is difficult to distinguish from mechanical intestinal obstruction owing to various causes.In cases of inexplicable intestinal obstructions,considering the possibility of a viral infection is essential to minimize misdiagnosis and missed diagnoses。

An Efficient Optimization Design Framework for Low-Resistance Shape of Bionic Amphibious Robot

To realize the low-resistance shape optimization design of amphibious robots,an efficient optimization design framework is proposed to improve the geometric deformation flexibility and optimization efficiency.In the proposed framework,the free-form deformation parametric model of the flat slender body is established and an analytical calculation method for the height constraints is derived.CFD method is introduced to carry out the high-precision resistance calculation and a constrained Kriging-based optimization method is built to improve the optimization efficiency by circularly infilling the new sample points which satisfying the constraints.Finally,the shape of an amphibious robot example is optimized to get the low-resistance shape and the results demonstrate that the presented optimization design framework has the advantages of simplicity,flexibility and high efficiency.

Metabolic dysfunction-associated fatty liver disease(MAFLD)is the most prevalent chronic liver condition worldwide.Current liver enzyme-based screening methods have limitations that may missed diagnoses and treatment delays.Regarding Chen et al,the risk of developing MAFLD remains elevated even when alanine aminotransferase levels fall within the normal range.Therefore,there is an urgent need for advanced diagnostic techniques and updated algorithms to enhance the accuracy of MAFLD diagnosis and enable early intervention.This paper proposes two potential screening methods for identifying individuals who may be at risk of developing MAFLD:Lowering these thresholds and promoting the use of noninvasive liver fibrosis scores.

An online detection technology must be developed for realizing the real-time control of friction stir welding.In this study,the three-dimensional force exerted on a material during friction stir welding was collected synchronously and the relationship between the forces and welding quality was investigated.The results indicated that the fluctuation period of the traverse force was equal to that of the lateral force during the stable welding stage.The phase difference between two horizontal forces wasπ/2.The values of the horizontal forces increased with welding speed,whereas their amplitudes remained the same.The proposed force model showed that the traverse and lateral forces conformed to an elliptical curve,and this result was consistent with the behavior of the measured data.The variational mode decomposition was used to process the plunge force.The intrinsic mode function that represented the real fluctuation in the plunge force varied at the same frequency as the spindle rotational speed.When tunnel defects occurred,the fluctuation period features were consistent with those obtained during normal welding,whereas the ratio parameter defined in this study increased significantly.

Large-inductance superconducting microstrip photon detector enabling 10 photon-number resolution

Efficient and precise photon-number-resolving detectors are essential for optical quantum information science.Despite this,very few detectors have been able to distinguish photon numbers with both high fidelity and a large dynamic range,all while maintaining high speed and high timing precision.Superconducting nanostrip-based detectors excel at counting single photons efficiently and rapidly,but face challenges in balancing dynamic range and fidelity.Here,we have pioneered the demonstration of 10 true photon-number resolution using a superconducting microstrip detector,with readout fidelity reaching an impressive 98%and 90%for 4-photon and 6-photon events,respectively.Furthermore,our proposed dual-channel timing setup drastically reduces the amount of data acquisition by 3 orders of magnitude,allowing for real-time photon-number readout.We then demonstrate the utility of our scheme by implementing a quantum random-number generator based on sampling the parity of a coherent state,which guarantees inherent unbiasedness,robustness against experimental imperfections and environmental noise,as well as invulnerability to eavesdropping.Our solution boasts high fidelity,a large dynamic range,and real-time characterization for photon-number resolution and simplicity with respect to device structure,fabrication,and readout,which may provide a promising avenue towards optical quantum information science.

The diagnostic value of tenascin-C in acute aortic syndrome

OBJECTIVES Misdiagnosis of acute aortic syndrome(AAS)significantly increases mortality.Tenascin-C(TN-C)is an extracellular matrix glycoprotein related to cardiovascular injury.The elevation of TN-C in AAS and whether it can discriminate suddenonset of acute chest pain in Chinese remains unclear.METHODS We measured the plasma concentration of TN-C by ELISA in a cohort of 376 patients with chest or back pain.Measures to discriminate AAS from acute coronary syndrome(ACS)were compared and calculated.RESULTS From October 2016 to September 2021,376 undiagnosed patients with chest or back pain were enrolled.166 of them were finally diagnosed as AAS,100 were ACS and 110 without cardiovascular diseases(NCV).TN-C was significantly elevated in AAS at 18.18 ng/mL(IQR:13.10–27.68)compared with 7.51 ng/mL(IQR:5.67–11.38)in ACS(P<0.001)and 3.68 ng/mL(IQR:2.50–5.29)in NCV(P<0.001).There was no significant difference in TN-C level among the subtypes of AAS.Of the 166 AAS patients,the peaked level of TN-C was at acute stage(P=0.012),then a slight of decrease was observed at subacute stage.The area under receiver operating characteristic curve for AAS patients versus NCV was 0.979(95%CI:0.964-0.994)for TN-C.At a cutoff level of 11.474 ng/mL,TN-C has a sensitivity of 76.0%,specificity of 85.5%,accuracy of 82.0%,positive predictive value(PPV)of 76.0%,negative predictive value(NPV)of 85.5%.Diagnostic performance of TN-C was superior to D-dimer and hs-cTnT.CONCLUSIONS The concentration of serum TN-C in AAS patients was significantly higher than that in ACS patients and NCV.TN-C could be a new biomarker to distinguish AAS patients in the early stage after symptoms onset from other pain diseases.

Main controlling factor and mechanism of gas-in-place content of the Lower Cambrian shale from different sedimentary facies in the western Hubei area, South China

The Lower Cambrian shale gas in the western Hubei area, South China has a great resource prospect, but the gas-in-place（GIP） content in different sedimentary facies varies widely, and the relevant mechanism has been not well understood. In the present study, two sets of the Lower Cambrian shale samples from the Wells YD4 and YD5 in the western Hubei area, representing the deep-water shelf facies and shallowwater platform facies, respectively, were investigated on the differences of pore types, pore structure and methane adsorption capacity between them, and the main controlling factor and mechanism of their methane adsorption capacities and GIP contents were discussed. The results show that the organic matter（OM） pores in the YD4 shale samples are dominant, while the inorganic mineral（IM） pores in the YD5 shale samples are primary, with underdeveloped OM pores. The pore specific surface area（SSA） and pore volume（PV） of the YD4 shale samples are mainly from micropores and mesopores, respectively,while those of the YD5 shale samples are mainly from micropores and macropores, respectively. The methane adsorption capacity of the YD4 shale samples is significantly higher than that of the YD5 shale samples, with a maximum absolute adsorption capacity of 3.13 cm3/g and 1.31 cm3/g in average,respectively. Compared with the shallow-water platform shale, the deep-water shelf shale has a higher TOC content, a better kerogen type and more developed OM pores, which is the main mechanism for its higher adsorption capacity. The GIP content models based on two samples with a similar TOC content selected respectively from the Wells YD4 and YD5 further indicate that the GIP content of the deep-water shelf shale is mainly 3 4 m3/t within a depth range of 1000—4000 m, with shale gas exploration and development potential, while the shallow-water platform shale has normally a GIP content of <1 m3/t,with little shale gas potential. Low-noise and low-power pixel sensor chip for gas pixel detectors

Topmetal-M2 is a large-area pixel sensor chip fabricated using the GSMC 130 nm CMOS process in 2021.The pixel array of Topmetal-M2 consists of pixels of 400 rows×512 columns with a pixel pitch of 45μm×45μm.The array is divided into 16 subarrays,with pixels of 400 rows×32 columns per subarray.Each pixel incorporates two charge sensors:a diode sensor and a Topmetal sensor.The in-pixel circuit primarily consists of a charge-sensitive amplifier for energy measurements,a discriminator with a peak-holding circuit,and a time-to-amplitude converter for time-of-arrival measurements.The pixel of Topmetal-M2 has a charge input range of~0-3 k e-,a voltage output range of~0-180 mV,and a charge-voltage conversion gain of~59.56μV∕e-.The average equivalent noise charge of Topmetal-M2,which includes the readout electronic system noise,is~43.45 e-.In the scanning mode,the time resolution of Topmetal-M2 is 1 LSB=1.25μs,and the precision is^()7.41μs.At an operating voltage of 1.5 V,Topmetal-M2 has a power consumption of~49 mW∕cm~2.In this article,we provide a comprehensive overview of the chip architecture,pixel working principles,and functional behavior of Topmetal-M2.Furthermore,we present the results of preliminary tests conducted on Topmetal-M2,namely,alpha-particle and soft X-ray tests.
