A longitudinal study of PSA and its influential factors in a cohort of Chinese men with initial PSA levels less than 4 ng ml-1

To evaluate the longitudinal change in prostate-specific antigen （PSA） and the influence of initial PSA on the PSA change. We retrospectively analysed health examination data collected at Beijing Hospital from March 2007 to November 2011. Men with an initial PSA levels less than 4 ng ml- 1 and an annual PSA test for 5 years were enrolled into the study. The men were separated into four groups by the initial PSA level （0-0.99, 1-1.99, 2-2.99 and 3-3.99 ng ml- 1）, and the difference in PSA change among the four groups was analysed. A total of 1330 men were enrolled into the study. The mean age, initial PSA and PSA velocity （PSAV） were 58.17± 14.63 （range 24-91） years, 1.18±0.79 （range 0-4） ng m1-1 and 0.04±0.25 （range -1.34±2.02） ng m1-1 year-1, Pearson＇s correlation analysis showed no correlation between initial PSA and PSAV （r=-0.036, P=0. 189）. The PSAV of the 0-0.99, 1-1.99, 2-2.99 and 3-3.99 ng m1-1 initial PSA groups was 0.03±0.11, 0.07±0.32, 0.03±0.34 and -0.01±0.43 ng m1-1 year-1, respectively （P=0.06）. As the initial PSA increased, the percentage of having a PSAV over 0.75 ng m1-1 year-1 and a negative PSAV both significantly increased. Males with a baseline PSA of 0-0.99, 1-1.99, 2-2.99 and 3-3.99 ng ml- 1 had a 1.88%, 6.16%, 16.30% and 57.81% chance, respectively, that their PSA would increase above 4.0 ng ml- 1 over the following 4 years （P〈0.0001）. The PSAV has no correlation with the initial PSA level. However, as the initial PSA increases, the chance that males will have an abnormal PSA or PSAV in the future increases.

The Circular Electron Positron Collider(CEPC)is a large scientific project initiated and hosted by China,fostered through extensive collaboration with international *** complex comprises four accelerators:a 30 GeV Linac,a 1.1 GeV Damping Ring,a Booster capable of achieving energies up to 180 GeV,and a Collider operating at varying energy modes(Z,W,H,and tt).The Linac and Damping Ring are situated on the surface,while the subterranean Booster and Collider are housed in a 100 km circumference underground tunnel,strategically accommodating future expansion with provisions for a potential super Proton Proton Collider(SPPC).The CEPC primarily serves as a Higgs *** its baseline design with synchrotron radiation(SR)power of 30 MW per beam,it can achieve a luminosity of 5×10^(34)cm^(-2)s^(-1)per interaction point(IP),resulting in an integrated luminosity of 13 ab^(-1)for two IPs over a decade,producing 2.6 million Higgs *** the SR power to 50 MW per beam expands the CEPC's capability to generate 4.3 million Higgs bosons,facilitating precise measurements of Higgs coupling at sub-percent levels,exceeding the precision expected from the HL-LHC by an order of *** Technical Design Report(TDR)follows the Preliminary Conceptual Design Report(Pre-CDR,2015)and the Conceptual Design Report(CDR,2018),comprehensively detailing the machine's layout,performance metrics,physical design and analysis,technical systems design,R&D and prototyping efforts,and associated civil engineering ***,it includes a cost estimate and a preliminary construction timeline,establishing a framework for forthcoming engineering design phase and site selection *** is anticipated to begin around 2027-2028,pending government approval,with an estimated duration of 8 *** commencement of experiments and data collection could potentially be initiated in the mid-2030s.

The Large sky Area Multi-Object Fiber Spectroscopic Telescope（LAMOST） general survey is a spectroscopic survey that will eventually cover approximately half of the celestial sphere and collect 10 million spectra of stars, galaxies and QSOs. Objects in both the pilot survey and the first year regular survey are included in the LAMOST DR1. The pilot survey started in October 2011 and ended in June 2012, and the data have been released to the public as the LAMOST Pilot Data Release in August 2012. The regular survey started in September 2012, and completed its first year of operation in June 2013. The LAMOST DR1 includes a total of 1202 plates containing 2 955 336 spectra, of which 1 790 879 spectra have observed signalto-noise ratio（SNR） ≥ 10. All data with SNR ≥ 2 are formally released as LAMOST DR1 under the LAMOST data policy. This data release contains a total of 2 204 696 spectra, of which 1 944 329 are stellar spectra, 12 082 are galaxy spectra and 5017 are quasars. The DR1 not only includes spectra, but also three stellar catalogs with measured parameters： late A,FGK-type stars with high quality spectra（1 061 918 entries）, A-type stars（100 073 entries）, and M-type stars（121 522 entries）. This paper introduces the survey design, the observational and instrumental limitations, data reduction and analysis, and some caveats. A description of the FITS structure of spectral files and parameter catalogs is also provided.

Mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

目的探讨糖化血红蛋白（HbA1c）与糖尿病视网膜病变（DR）的关系及其诊断阈值。设计以社区人群为基础的横断面研究。研究对象北京社区糖尿病研究中2642例20-80岁已确诊的2型糖尿病患者，其中2007例（76.0％）受试者（1199例女性）参与研究，平均年龄（64.1±9.0）岁。方法全体参与医师经过专门培训，按照统一要求调查并填写调查表，同时对患者进行体格检查及相关的实验室检查，HbA1c用美国伯乐公司高压液相仪测定，并对所有患者用45°免散瞳彩色眼底照相机留取后极部眼底照片一张。DR的严重程度参照Airlie House分级法。采用 ROC 曲线分析HbA1c诊断DR的最佳阈值。主要指标 HbA1c在DR中的诊断阈值（敏感性、特异性、AUC）。结果本人群中DR患病率为（24.7±1.0）％（95％CI，22.8-26.6）。在二元逻辑回归分析中，DR的出现与较高的HbA1c浓度（OR=1.23；95％CI，1.14-1.33）有关，随着HbA1c增高，DR患病率呈上升趋势。当HbA1c≥6.5%时，诊断DR的ROC曲线下面积最大（59.2%），且敏感性（75.1%）和特异性（43.4%）高。在二元逻辑回归分析中，DR还与其他方面有关：较轻的年龄（OR=0.97；95％CI，0.95-0.98）、较长的糖尿病病程（OR=1.10；95％CI，1.08-1.12）、较高的收缩压（OR=1.01；95％CI，1.01-1.02）、较低的体重指数（OR=0.95；95％CI，0.92-0.98）和升高的血尿素氮浓度（OR=1.01；95％CI，1.00-1.01）。结论当糖化血红蛋白≥6.5%时与糖尿病视网膜病变显著相关，提示糖化血红蛋白为6.5%或许可以作为筛查糖尿病视网膜病变的阈值。（眼科，2016，25：321-325）

目的了解北京市社区2型糖尿病患者高血压的患病率、血压控制达标率.方法 2008年8月至2009年7月采用多极抽样方法与患者自愿原则,纳入北京市4个城区中15个社区卫生服务中心的2849例2型糖尿病患者为研究对象,行体格检查及相关的实验室检查.结果 2型糖尿病患者高血压的患病率为70.8％（2016/2849）.入组前已合并高血压者1966例,其中血压＜140/90 mm Hg（1mm Hg=0.133 kPa）者占54.8％（1077/1966）,血压达标（＜130/80 mm Hg）率17.8％（350/1966）.血压达标与未达标者间TG、空腹血糖、腰围、糖化血红蛋白水平差异有统计学意义（F值分别为23.51、23.26、39.00和22.91,均P＜0.01）.1198例行尿微量白蛋白检测,糖尿病合并高血压者较单纯糖尿病者尿微量白蛋白升高更多（P＜0.01）.结论北京社区2型糖尿病患者高血压的患病率较高,血压达标率低；应加强糖尿病合并高血压患者多重危险因素控制和有效的社区管理.

目的了解北京城区社区2型糖尿病患者管理3年降糖药物调整情况及影响血糖达标的相关因素。方法2008年8月至2011年7月对在北京城区社区就诊的3201例2型糖尿病患者定期随访，进行血糖、血压血脂综合达标管理。依据管理3年前后降糖药物调整情况分为4组：增加胰岛素组（255例）、增加口服降糖药组（641例）、降糖药不变组（1621例）及降糖药减少组（684例），观察各组血糖达标情况。结果管理3年后，①4组患者糖化血红蛋白（HbAle）分别从（8．3±1．9）％、（7．4±1．7）％、（6．8±1．3）％和（7．2±1．5）％，降至管理后的（7．5±1．5）％（t=5．38，P〈0．01）、（7．0±1．3）％（t=5．17，P〈0．01）、（6．7±1．2）％（t=2．62，P〈0．05）和（7．1±1．4）％（t=1．76，P〉0．05）；HbAle下降〉0．5％的患者比例分别为52．2％（133／255）、34．3％（220／641）、24．6％（399／1621）和31．9％（218／684），增加胰岛素组血糖下降幅度最大；②管理前血糖未达标（HbAle≥7％）的1280例患者中，管理3年后562例（43．9％）达标；③初等、中等、高等教育背景者管理后血糖达标率分别为61．5％（342／556）、64．7％（1174／1814）、74．5％（619／813）；人均月收入〈2000元、2000～4000元、〉4000元者管理后的血糖达标率分别为58．1％（971／1670）、77．3％（828／1071）和66．8％（137／205）；④多因素逐步回归分析显示，增加胰岛素、增加口服降糖药、人均月收入2000～4000元、管理后BMI下降幅度和年龄，与HbA1c下降独立相关，OR值分别为0．45、0．80、0．72、0．96和1．08（P〈0．05）。结论通过3年积极调整降糖药物，2型糖尿病患者血糖达标率明显提高，尤以增加胰岛素组血糖下降幅度最大。低收入、低教育程度者仍是血糖达标关注的重点。

目的了解北京社区2型糖尿病患者血脂异常状况。方法选择2008年8月至2009年7月北京三甲医院与15个城区社区联合开展糖尿病纵向管理项目基线数据库中的3316例2型糖尿病患者，分析其血脂情况。血脂异常的诊断根据2007年中国成人血脂异常防治指南。结果①3316例中男性1329例，女性1987例，平均年龄（65±10）岁。75．6％（2506／3316）患者至少有一种血脂异常，其中男性72．5％（964／1329），女性77．6％（1542／1987）；②高TG血症、高Tc血症的患病率分别为41．9％（1388／3316）、48．1％（1595／3316），31．5％（1043／3316）患者LDL－C水平升高，21．2％（703／3316）患者HDL－C水平降低；血脂异常患者中已接受降脂药治疗者575例（22．9％），有血脂异常史者1393例TG、总TC、LDL－C和HDL-C的达标率分别为48．0％（669／1393）、17．4％（242／1393）、30．9％（430／1393）和75．8％（1056／1393）；③血脂异常组患者BMI、腰围、血压、血糖、糖化血红蛋白及尿酸水平均较高；④超重、血糖控制未达标组有较高的血脂异常患病率，分别为79．0％（1678／2125）、78．9％（1756／2227）；⑤回归分析显示，血脂异常与性别、年龄、糖化血红蛋白、BMI均相关。结论北京社区2型糖尿病患者血脂异常患病率较高，血脂异常人群中接受降脂药物治疗率很低。血脂异常与年龄、BMI及糖化血红蛋白均有关。对社区2型糖尿病患者除控制血糖外，还应重视脂代谓十紊乱。
