Background: Efficient ampullary intervention is essential for endoscopic retrograde cholangiopancreatography(ERCP) in patients with a prior Billroth II gastrectomy. We retrospectively evaluated the safety and effectiveness of ampullary intervention using fully covered self-expandable metal stents(FCSEMSs) for the management of common bile duct(CBD) stones in a subset of patients with a history of Billroth II gastrectomy. Methods: This retrospective analysis involved patients with a prior Billroth II gastrectomy who underwent ampullary intervention with FCSEMSs for the management of CBD stones. The factors associated with FCSEMSs placement, treatment success, and procedural complications were analyzed. Results: A group of 15 patients(10 males; median age, 78 years) underwent biliary metal stent placement for high degree of CBD angulation(6), small or flat papilla with unclear margin(5), current use of double antiplatelet agents or an anticoagulant(2), unwanted instrumentation of the cystic duct(1), and insecure position of the scope(1). Ampullary intervention with FCSEMSs was successful in all patients. After dilating the ampulla of Vater and building a durable conduit with FCSEMSs immediately, CBD stones were removed successfully from all patients in a single session. A mild post-ERCP pancreatitis occurred in one patient, who recovered without complications. Conclusion: Ampullary intervention with FCSEMSs is safe and effective for the management of CBD stones in a subset of patients with a history of Billroth II gastrectomy.

Flexible copper clad laminates(FCCLs) were fabricated using the electro-plating process and the combined effect of the current density and plating time on their surface morphology,texture,hardness,electrical resistivity and folding behavior was *** achieve Cu layers with similar thicknesses,the current density was varied in the range of 0.2-3 A/dm2 and the plating time was controlled in the range of 0.5-7.5 h to compensate for the variation of the current *** surface morphology,hardness,and folding behavior were characterized by atomic force microscopy,nanoindentation technique and Massachusett Institute of Technology folding endurance test,*** X-ray diffraction patterns indicated that the Cu phase was formed without any secondary phases;however,the preferred orientation changed from(220) to(111) as the current density increased over 1 A/*** addition,it was observed that the root-mean-square and hardness values decreased when the current density increased and the plating time decreased *** electrical resistivity was as low as approximately 21 nΩ·m and the number of cycles without failure in the folding test was over 15 000,which were comparable to those of commercial FCCLs.

We develop a quantum key distribution (QKD) system with fast active optical path length compensation. A rapid and reliable active optical path length compensation scheme is proposed and applied to a plug-and-play QKD system. The system monitors changes in key rates and controls it is own operation automatically. The system achieves its optimal performance within three seconds of operation, which includes a sifted key rate of 5.5 kbps and a quantum bit error rate of less than 2% after an abrupt temperature variation along the 25 km quantum channel. The system also operates well over a 24 h period while completing more than 60 active optical path length compensations.

We report a case of Valsalva retinopathy associated with esophagogastroduodenoscopy (EGD) under propofol sedation. A 43-year-old woman who had no previous history of systemic or ocular disease presented with a complaint of decreased vision in her left eye, which developed one day after EGD under propofol sedation. According to the referring physician, the patient had experienced multiple sustained Valsalva maneuvers during EGD. The fundus examination of the left eye showed a large preretinal hemorrhage surrounded by multiple small retinal hemorrhages in the posterior pole. One month later, fundus examination revealed a floating organized vitreous hemorrhage. The pars plana vitrectomy was performed to treat persistent vitreous hemorrhage. One month after vitrectomy, fundus examination showed normal retina and the patient’s vision recovered to 20/20. Valsalva maneuver can occur during EGD under sedation, and Valsalva retinopathy should be considered as a possible cause. Valsalva retinopathy should be included in the differential diagnosis when a patient complains of blurred vision following EGD.

Background Advancements in neonatal care have increased preterm infant survival but paradoxically raised intraventricular hemorrhage(IVH)*** study explores IVH prevalence and long-term outcomes of very low birth weight(VLBW)infants in Korea over a *** Using Korean National Health Insurance data(NHIS,2010-2019),we identified 3372 VLBW infants with IVH among 4,129,808 live ***-related claims data,encompassing diagnostic codes,diagnostic test costs,and administered procedures were sourced from the NHIS *** results of the developmental assessments are categorized into four groups based on standard deviation(SD)*** characteristics and complications were compared among the *** regression models were employed to identify significant changes in the incidence of complications and to calculate odds ratios with corresponding 95%confidence intervals for each risk factor associated with mortality and morbidity in ***-term growth and development were compared between the two groups(years 2010-2013 and 2014-2017).Results IVH prevalence was 12%in VLBW and 16%in extremely low birth weight(ELBW)*** the past decade,IVH rates increased significantly in ELBW infants(P=0.0113),while mortality decreased(P=0.0225).Major improvements in certain neurodevelopmental outcomes and reductions in early morbidities have been observed among VLBW infants with *** percent of the population received surgical treatments such as external ventricular drainage(EVD)or a ventriculoperitoneal(VP)shunt,with the choice of treatment methods remaining consistent over *** IVH with surgical intervention group exhibited higher incidences of delayed development,cerebral palsy,seizure disorder,and growth failure(height,weight,and head circumference)up to 72 months of age(P<0.0001).Surgical treatments were also significantly associated with abnormal developmental screening test *** The neurodevelopmental outcomes of infa

As of 2020,the issue of user satisfaction has generated a significant amount of ***,we employ a big data approach for exploring user satisfaction among Uber *** develop a research model of user satisfaction by expanding the list of user experience(UX)elements(i.e.,pragmatic,expectation confirmation,hedonic,and burden)by including more elements,namely:risk,cost,promotion,anxiety,sadness,and ***,we collect 125,768 comments from online reviews of Uber services and perform a sentiment analysis to extract the UX *** results of a regression analysis reveal the following:hedonic,promotion,and pragmatic significantly and positively affect user satisfaction,while burden,cost,and risk have a substantial negative ***,the influence of expectation confirmation on user satisfaction is not ***,sadness,anxiety,and anger are positively related to the perceived risk of *** with sadness and anxiety,anger has a more important role in increasing the perceived burden of *** on these findings,we also provide some theoretical implications for future UX literature and some core suggestions related to establishing strategies for Uber and similar *** proposed big data approach may be utilized in other UX studies in the future.

This study aimed to investigate the effects of exercise-induced muscle fatigue in the unaffected knee joint on postural control and kinematic changes in stroke patients. Forty participants（20 stroke patients, 20 age-matched healthy participants） were recruited. To induce fatigue, maximum voluntary isometric contractions were performed in the unaffected knee joint in a Leg Extension Rehab exercise machine using the pneumatic resistance. We measured static and dynamic balance and lower-limb kinematics during gait. Changes in postural control parameters anteroposterior sway speed and total center of pressure distance differed significantly between the stroke and control groups. In addition, changes in gait kinematic parameters knee and ankle angles of initial contact differed significantly between stroke（paretic and non-paretic） and control groups. Muscle fatigue in the unaffected knee and ankle impaired postural control and debilitates kinematic movement of ipsilateral and contralateral lower limbs, and may place the fatigued stroke patients at greater risk for falls.
